Discover all the answers to some of the most common questions you may have about your menstrual cycle, hormones in your menstrual cycle, ovulation, implantation, and the 2ww (two week wait). If you are trying to get pregnant, you may have heard the term—two week wait, the often nervous time of waiting between ovulation and taking a pregnancy test. For many, this is a dreaded time because it's hard to know exactly what is happening. But by learning more about your cycle, you can have more knowledge about what might happen. The implantation window is during the two week wait. Since progesterone is the hormone responsible for supporting implantation, it must be present during the entire implantation window. You can track your progesterone during this window by tracking PdG, progesterone in urine. Proov has unique testing capabilities to test PdG during the implantation window to confirm successful ovulation. Studies show PdG levels are incredibly important for implantation and pregnancy success, for example, women with high PdG during the implantation window were 75% more likely to get pregnant. I'm talking all about the benefits of known sperm donation with Danielle Winston of The Seed Scout @theseedscout on today's brand new episode of The Egg Whisperer Show. When you're doing IVF, it's important to be prepared. I want you to have the best outcome possible, and for me that includes making sure you have everything you need on hand to support your physical, mental, and emotional health as you go through the process. Today on the podcast, I'm sharing my top 10 things you need when you're embarking on IVF: 1. Get your electrolyte rich drinks ahead of time 2. Stock up on a protein powder+shake that you love 3. Consider scheduling a food delivery service 4. Prepare for a poop emergency: stool softeners are your friend 5. Purchase panty liners because you can spot and that's considered normal after an egg retrieval and waterproof underwear helps with the discharge too 6. Egg Whisperer fertility pants 7. Grab that Tylenol or Advil ahead of time and a heating pad too for aches and pains 8. Take precautions against Ovarian Hyperstimulation Syndrome (OHSS) 9. Build your fertility TEAM 10. Dr. Camran Nezhat is world renowned for his work in minimally invasive and robotic surgery, and he specializes in treating people with endometriosis. Today, I'm so honored to have Kristin McQuaid joining me to talk about a very touching and powerful topic: support for surrogate mothers and intended parents through infant loss. Kristin and her husband, Steve, started a non-profit organization in honor of their daughter, London Quinn, who was born still at 39 weeks in July, 2021. They named the organization "London is the Reason," and the non-profit focuses on supporting surrogates and their intended parents through infant loss. I feel quite privileged to have Kristin share her story, and the story of her daughter London with us. I know you will be moved by what she shares. In this episode, I'm joined by Dr. Geoffrey Sher of Sher Fertility Solutions. He is an internationally renowned fertility expert. He trained under “The Fathers of IVF, ” Drs. Patrick Steptoe and Robert Edwards in the UK. In 1982 he established the 1st private (non-university based) IVF program in the US. Over a career spanning almost 40 years Dr. Sher has been influential in the births of over 17000 IVF babies and has helped fashion the entire field of ART. He is the author of “ In Vitro Fertilization, the A.R.T. of Making Babies” and (more recently), “Unexplained IVF failure and Recurrent pregnancy loss: The Immunologic Link”." Today he's joining me to talk about Immunologic Implantation Dysfunction (IID), which may be a factor in unexplained IVF failure and recurrent pregnancy loss (RPL). When seeing patients, I always recommend people to gather their Fertility TEAM, and the T is for therapist. I'm so excited to have Dr. Loree Johnson share her valuable info with all of you today. Lucy Lines began her career as an embryologist, starting at Monash IVF in Melbourne in 2001. Does sperm really matter? If you've been tuning in to all of the posts this week for "Super BALLS Week" you'll know that of course it matters! It truly does take both sperm and an egg to make a baby. Today I'll review the sperm question a bit, and explain why it matters. AND, I'll give you a lot more info about how to improve sperm quality. Here's the thing: I know that guys want to hear what they want to hear. They have selective listening. I'm not kidding when I tell you that I had someone email me recently and say "My husband's sperm tested out GREAT, and when we went over the results with the doctor we heard two different things. He thinks he heard our doctor say that it means he can just keep drinking, smoking, and enjoying marijuana and doesn't need to make any lifestyle changes. I heard something very different, which was that he should still cut back until we are pregnant. Can you help settle this for us?" And while it's an unpopular answer (at least with the guys), my response is: Men should cut back or abstain from drinking, and smoking (marijuana too) if you're TTC. Here are a few of the things I suggest to anyone bringing the sperm side of the equation to the baby making situation: - consider freezing your own sperm for future use when you are young. - get in the best shape of your life if you're trying to conceive. Sperm matters, and men's health is directly related to their sperm quality. - take some prenatal vitamins. Yes, really. Many of the same supplements that I recommend for egg quality also are great for sperm quality. - cut back on smoking, drinking, and drug use. Marijuana negatively impacts sperm. - don't cook your balls: avoid hot tubs, using your laptop on your lap, and other situations that may create high temps for the area. - find ways to cut back on stress, if you live a stressful life. Talking about your fertility when you're over 40 is not easy, but I want women to stop apologizing for desiring a family. Times have changed. It's okay to want a baby over 40. It's not crazy, nor selfish. My approach is for all of my patients to be given their best chance at a healthy pregnancy in the shortest period of time. How do we do this? It starts by not believing everything you read. There's an international headline I saw recently on BBC news that fertility doctors are exploiting older women and “dealing false hope”. Do I believe that fertility doctors deliver false hope? No way! I do not. Do I believe that it's imperative to get a diagnosis before treatment, regardless of who you are and your age? Yes! Diagnosis before treatment. Please! I want my patients over 40 to understand their follicle count. Then I want them to decide if moving forward with treatment using their own eggs is the best path forward. There are so many ways to have a baby if your own body is not making it happen there are other creative paths to explore. Egg and embryo donation and adoption are a few examples. I have patients that come to me from all over the world. There's one question I always ask when reviewing their IVF cycle history. What was your embryo quality? I hear time and time again from patients that they don't know it because their doctor told them embryo quality doesn't matter with IVF. That's simply not true! Just like diamonds in a jewelry store. Quality matters. Just like there is a grading system for diamonds, there is also a grading system for embryos. It's important for you to know that this exists and to ask for your embryo grade. IVF is a very emotional journey. It can be riddled with fear, doubt, and is full of periods of waiting and wondering. For my patients, I want to reduce any emotional or physical burdens by creating an IVF checklist for them. It covers the core essentials of everything we need to do, so there are no conversations that begin with, “I wish I would have…” down the road. I don't want you to have to wonder if you've done everything you can to have a successful IVF outcome. This is true if you're my patient, or you live half-way across the world. One of the most common questions I get asked is “Why didn't my IUI work?" It's a great question because there are many factors that come into play with intrauterine insemination. If you're my patient, we'd start by checking the Fallopian tubes, uterus, and sperm quality first.⠀ But let's say you've done the #TUSHYcheck, the tubes are clear and everything else looks good. So why doesn't IUI work? The answer I share with my patients is: because you're human. Dr. Geoffrey Sher is back, and we are talking about Intrauterine Insemination, or IUI. In 1982, Dr. Sher and his colleagues were the first to wash sperm, and then put it through a centrifuge process. After an incubation period, they used a catheter to inject the sperm into the uterine cavity of a patient. He's a true pioneer, and this was the first time IUI had been done!.
