I'm so excited to have Dr. Bruce Lessey of ReceptivaDX join me on the podcast. Dr. Lessey has dedicated his research life to studying the lining of the uterus and more specifically, how to help embryos stick better. His research led to the discovery of the role of BCL-6 in the lining of the uterus as a diagnostic test patients can have done by their doctors to guide them toward the best protocol for transfer success. The test isn't for women just going through IVF either. This test can also be done to help women get an endometriosis diagnosis without going through a laparoscopy surgery. ⠀ ⠀ In addition to being a Fertility Doctor at The Fertility Center of the Carolinas, in Greenville SC, Dr. Lessey is also the scientific advisor for Cicero DX. Is your pregnancy test negative, again? Let's get real. It sucks when we do everything we can in our power to make something happen, and it just doesn't. If you've ever wanted to get pregnant, and have seen a negative result on the test, you know exactly what I'm talking about. It doesn't matter if it's one month of trying or 2 years…. "Trying to get pregnant" can be painful, and the phrase itself is defeatist. The mindset it establishes is one based on fear vs. love. I like to lead from a place of love, and encourage my patients to think positively. For that reason, I advocate for replacing it with, "preparing for pregnancy." Doesn't it just sound more peaceful, and open to the process? I think so. Regardless if you are on board with my line of thinking, I do want to share some helpful tips and suggestions so that your path to pregnancy is as informed and efficient as possible. Here are my top tips: My #1 tip for preparing for pregnancy is to NOT put blind faith in fertility apps. Tip #2: Develop a resilient, and positive mindset. Tip #3: Make use of science (diagnostic testing) to understand what could be preventing you from getting pregnant. Oh, and ask questions. Lots of them! It's a delight to have Dr. Geoffrey Sher back. He's been on the show several times, and our discussion on "How to Avoid Implantation Failure During IVF" is one of the most viewed on the channel! Today we're talking about treating Endometriosis - related infertility. Here are some facts about Endometriosis as shared by Dr. Sher: - Endometriosis is a condition where cells similar to the lining of the uterus (endometrium) grow outside of the uterus, in the pelvic cavity. - It occurs in about 10% of all women and can be symptom free or may cause a wide spectrum of symptoms. - It's estimated that 25% - 40% of all female infertility is linked to endometriosis in some way. - Endometriosis is a life-long condition. It cannot be completely removed or cured. While endometriosis can usually be controlled (medically or surgically), the condition itself is not curable. - Removing endometriomas from an ovary may help with getting pregnant. Dr. Sher is also sharing information about how the endometrium begins to grow outside the uterus, and why some women remain asymptomatic, and others develop symptoms. Egg freezing is IVF without the F. You go through a standard cycle of In Vitro Fertilization in which you take hormone shots that stimulate the growth of eggs. A doctor like me then extracts your eggs through a simple surgical procedure, and I work with an embryologist to preserve your eggs in liquid nitrogen until you're ready to use them. So the big question is what happens now that you're ready to use your eggs? In other words, what are the steps towards fertilization (or as I like to say, what the F?) Step 1. One of the most common questions I get asked is “Why didn't my IUI work?" It's a great question because there are many factors that come into play with intrauterine insemination. If you're my patient, we'd start by checking the Fallopian tubes, uterus, and sperm quality first.⠀ But let's say you've done the #TUSHYcheck, the tubes are clear and everything else looks good. So why doesn't IUI work? The answer I share with my patients is: because you're human. Yes, it's really as simple as that. ⠀ To put IUI into perspective, 90% of the time it doesn't work. ⠀ So if your IUI didn't work, I want you to know that there's nothing wrong with you, and you're normal, and human. Read the full show notes on Dr. Aimee's website. Dr. Geoffrey Sher is back, and we are going to be covering a very important topic: why do you need to do PGT testing if you're doing IVF (or do you?) Dr. Sher is the co-founder of Sher Fertility Solutions (SFS) and internationally renowned expert in the field of ART (Assisted Reproductive Technology) and has been influential in the births of over 17,000 IVF babies. Around 2005, Dr. Sher's team published the first paper that showed beyond a shadow of a doubt that if you were able to fully karyotype the embryo cells that a euploid embryo would have an excellent chance. Today there is hardly an IVF program in the world that is not doing PGS/PGT-A to assess embryo viability. Today, we are discussing if PGT is needed for all IVF cycles, the role DOR plays in assessing if PGT is useful, and other maternal chromosomal and genetic abnormalities that may mean you'd want to consider PGT. Plus, he's addressing the concern that some patients have around the possibility that PGT may harm an embryo (short answer: if done correctly it does not).
