They discuss Dr Minneci's recent article, “Cost-Effectiveness of Nonoperative Management vs Upfront Laparoscopic Appendectomy for Pediatric Uncomplicated Appendicitis Over 1 Year,” in which the authors found that cost-effectiveness of management of pediatric appendicitis is sensitive to changes in utilities achieved by nonoperative management. It ushered in the laparoscopic revolution but also increased the number of bile duct injuries. Through the dedicated efforts of many the rate of bile duct injury has been reduced, now mirroring open cholecystectomy. The robotic surgery revolution is well underway and unsurprisingly this technology has been applied to cholecystectomy. Ed graduated medical school in London in the late 1990s and spent his surgical career in middle England, where he obtained his surgical fellowship and research doctorate in bowel cancer. At the end of Ed's fellowship, Ed spent some time in Basingstoke, UK, with Professor Bill Heald and Professor Brendan Moran further subspecialising within the fields of pelvic cancer. Outside Ed's clinical duties, he is an active Surgical Oncology examiner for European Board of Surgery and a training convenor for the Royal College of Surgeons with now over 80 peer-reviewed publications. Ed has peer-reviewed over 300 medical journal manuscripts. Furthermore, Ed is also the International Ambassador for the British Association of Surgical Oncology, actively promoting better training, delivery of care and public awareness on cancer surgery. In this episode, host Dr. Amy Park interviews co-host Dr. Mark Hoffman about laparoscopic hysterectomies. --- SHOW NOTES The episode begins with Mark describing his journey to becoming a minimally invasive gynecologic surgeon, with more exposure to traditional laparoscopic surgery (“straight stick”) throughout his career, but with a recent revival of robotic surgery as well. Overall, Mark still prefers traditional laparoscopy. However, there are certain instances where robotics is especially helpful – namely in patients with a high BMI and in myomectomies where robots make the extensive suturing more manageable. Additionally, robotic surgery is easier to do in a situation where students and residents are not available to assist. Most importantly, he likes to get an MRI, look at the anatomy, and decide what the best approach is for each individual patient. And of course, the doctors emphasize the importance of having a strong team to operate with. Next, Mark discusses his tips and tricks to a successful laparoscopic hysterectomy. He likes to be in the operating room before the patient is there to ensure it is set up correctly. Once the patient arrives, he stresses the importance of proper patient positioning on the bed, with the arms always tucked at the patient's side, and then inserting the Foley catheter after draping. The doctors then discuss incision locations and sizes, with Mark preferring all 5 cm incisions. Mark continues with the steps of the procedure – he starts with the fallopian tubes, then gets the utero-ovarians and carries around the round ligament to move the ovaries laterally. He then emphasizes skeletonizing the uterines/posterior peritoneum. He saves the anterior incisions for last as they can get complicated with adhesions from prior C-sections, for example. Mark highlights the “critical view,” which is the anterior cup, posterior cup, ring, and vessels on the other side. The doctors go on to discuss colpotomy, barbed sutures, visualization, antibiotics, and more. Laparoscopic surgery currently starts with a digital interface in a video image. Asensus is adding augmented intelligence to the video image in real-time to support the surgeon and surgical team with additional information to improve outcomes and the experience of the clinicians. Ed explains, "We're starting with things already being done. We have a feature that is a digital measurement. This allows the surgeon, if measurement is critical to this portion of the procedure, for instance, in general surgery, we'd say five to six centimeters from the pylorus, to start a staple line in a sleeve gastrectomy. We either estimate that through different ways, or we'd have to put a tape measure inside the patient to measure that distance accurately to get that measurement. So we're doing that now digitally." "We have a telemanipulator. Our Asensus system is currently the second telemanipulator to be approved in the US. And so, we have it in a few centers already. And that integrates into the operating room as separate arms on separate pods or platforms that can be moved around, connected to a cockpit or a console, where the surgeon sits to do the manipulating." "Then, the video feeds feed into our Intelligent Surgical Unit or ISU, and those video images are processed and sent to the cloud. We'll store them, we could annotate them, and we can also use them to influence, in real-time, certain types of feedback based on what would be interpreted from the video image. Prostate cancer is the most common cancer and the second leading cause of cancer death among men in the United States. Prostate cancer usually grows very slowly, and finding and treating it before symptoms occur may not improve men's health or help them live longer.A surgical treatment for prostate cancer, the radical prostatectomy procedure removes the entire prostate gland. Radical prostatectomy can now be done by laparoscopic or robotic techniques. In open prostate surgery, the prostate gland is removed through a larger incision in the lower abdomen. Laparoscopic prostate surgery involves key-hole incisions which used for inserting a lighted viewing instrument (laparoscope) into the pelvic region and allows examination and removal of the prostate without a large abdominal incision.

Proponents of fascial plane blocks argue that their value can be found in opioid reduction and improvement of pain scores. On the other hand, critics argue that the effect size tends to be very small, amounting to a placebo. To address these conflicting ideas, a recent study examined the role of quadratus blocks and analgesia for laparoscopic hemicolectomy. In this month's RAPM Focus, Editor-in-Chief Brian Sites, MD, joins Jens Borglum, MD, PhD, the senior author of “Anterior quadratus lumborum block does not reduce postoperative opioid consumption following laparoscopic hemicolectomy: a randomized, double-blind, controlled trial in an ERAS setting,” first published in September 2022 (https://rapm.bmj.com/content/48/1/7). This fascinating study had 69 patients randomized to receive either an anterior quadratus lumborum block or isotonic saline, a placebo. Dr. Jens Borglum is a consultant anesthetist, clinical research associate professor, and head of research at Zealand University Hospital, University of Copenhagen, Denmark. His main interests are ultrasound-guided nerve blocks and he is a prolific researcher and contributor to Regional Anesthesia & Pain Medicine. Endometriosis Awareness takes place across the globe during the month of March with a mission to raise awareness of a condition that affects an estimated 200 million people, about 1 in 10 women, worldwide. Endometriosis is an inflammatory condition where tissue, similar to the endometrial tissue that lines of the uterus, grows outside of the uterus.Laparoscopic surgery is the only definitive way to determine whether or not endometriosis is present in a person's body. A doctor might recommend surgery if a person has severe pain that medication does not relieve. With surgery, a skilled surgeon, specializing in endometriosis, can not only find the endometriotic tissue, but also take out all or some of the affected tissue; however this is not a cure because, eventually, endometriosis and pain will return.Doctors typically recommend trying conservative treatment approaches first, opting for surgery only if initial treatments fail. In any case, there are some things to take into consideration, including whether one wants to get pregnant later. You might not be able to have children after certain surgical endometriosis interventions. February 24, 2023Mark, Ray, and Scott discuss questions that came into the PRS Community.   I listened to the most recent podcast episode regarding the updated PCNL codes and I'm still confused on why we can't bill 50436 or 50437 with 50080/50081? Dilation was deleted from the code description and I attended an AUA webinar in Dec 2022 that said the dilation could be reported separately. What's changed?Good morning, Does 50548 include laparoscopic bladder cuff, or would the bladder cuff be separately billable with 51999? Bladder cuff is not mentioned in the code descriptor.It was discussed at New Orleans seminar that retrograde pyelogram (74420) was not appropriate to report with stent exchanges/ureteroscopic procedures (as it is considered included as part of the guidance for these procedures). The kids keep them pretty busy, but their favorite times are spent on warm sunny beaches.   Sam Eisenstein, MD La Jolla, CA   Sam Eisenstein is an Assistant Professor of Colon and rectal surgery and director of Inflammatory Bowel Disease surgery at UC San Diego Health.  He has worked there for the past 8 years after graduating both residency and fellowship at The Mount Sinai Medical Center in New York.  Sam is best known as the founder and organizer of the IBD-NSQIP collaborative, a large multi institutional data collaborative examining outcomes after IBD surgery, but he also is involved in several clinical trials for perianal Crohn's and has extensive experience with stem cell injections for anal fistulae. He is also on the scientific advisory board for the Crohn's and Colitis Foundation for his work on the next big IBD data collaborative, IBD-SIRQC (Surgical Innovation, Research and Quality Collaborative).  Sam has a Wife and 3 kids (6,8, and 3) and spends most of his free time running around after them these days, but also enjoys traveling and getting out into nature with his family.

Endometriosis is a complex hormonal-dependent disease that can wreak havoc on your fertility. It's typically associated with painful periods, but in some cases, it can spread quietly. The cause of endometriosis is still fairly unknown, which makes finding a concrete way to treat it difficult. What we do know is that hormones (estrogen) can make it spread, and inflammation is a key factor. Laparoscopic surgery is the only known tool to accurately diagnose endo but this procedure can be very invasive and can pose delays in fertility efforts.Since options are lacking on the western medicine side, it makes good sense to explore what alternative methods can offer. Research in the field of eastern medicine has shown that endometriosis, like many other gynecological disorders, responds very well to traditional Chinese medicine and herbs.
