Fertility Docs Uncensored is hosted by Dr. Carrie Bedient from the Fertility Center of Las Vegas, Dr. Susan Hudson from Texas Fertility Center, and Dr. Abby Eblen from Nashville Fertility Center. In this episode, the docs welcome visiting physician Dr. Shelley Dolitsky from Shady Grove Fertility in Towson, Maryland, for an in-depth conversation about recurrent pregnancy loss. Dr. Dolitsky begins by reviewing how different professional organizations define recurrent pregnancy loss. The American Society for Reproductive Medicine considers two or more losses—including very early biochemical losses—to be recurrent pregnancy loss, while the American College of OB/GYN defines it as two clinical losses under 20 weeks. The docs discuss how age dramatically affects miscarriage risk, with up to 75% of women over 40 experiencing miscarriages, compared with an overall rate of three to five percent. They walk through the full evaluation, which includes assessing the uterine cavity for abnormalities such as scar tissue, polyps, or congenital malformations; ensuring the fallopian tubes are normal and ruling out tubal damage; and performing chromosome analysis on both partners. Testing for antiphospholipid antibodies and lupus anticoagulant is also essential, as these can contribute to placental clotting issues. The conversation highlights the importance of screening for chronic medical issues that might be undiagnosed. About half of patients with recurrent pregnancy loss will have an identifiable and often treatable cause. Recurrent pregnancy loss (RPL) affects approximately 5% of couples and is an emotional burden on those affected. There is some evidence that vaginal progesterone supplementation may be considered in patients with recurrent pregnancy loss who are experiencing vaginal bleeding during the first trimester. October is Pregnancy, Infant and Child Loss Awareness Month. All month long we will honor our babies gone to heaven far too soon. Have you heard of recurrent pregnancy loss? Recurrent Pregnancy Loss is defined by two or more failed pregnancies and is considered distinct from infertility. When the cause is unknown, each pregnancy loss merits careful review to determine whether specific evaluation may be appropriate, and after two or more losses, a thorough evaluation is warranted. Today's guest experienced 5 miscarriages after having her baby girl, one being an ectopic pregnancy where she lost her fallopian tube, and then was diagnosed with secondary infertility. Anesha Stanley's first pregnancy ended with a miscarriage in 2014. Three months later she found out she was pregnant with her rainbow baby girl, Sinai. She waited two years before trying to conceive again. In 2017 we began experiencing recurrent pregnancy losses. Miscarriage #2 on January 14, 2017. An ectopic pregnancy #3, November 1, 2017 was treated with methotrexate. Anesha went onto experience two more losses and was diagnosed with secondary infertility and tubal failure. In this episode, Anesha takes us back to what it was like experiencing the highs of finding out she was pregnant and the lows and pain of miscarriage. In this episode, I'm joined by Dr. Geoffrey Sher of Sher Fertility Solutions. He is an internationally renowned fertility expert. He trained under “The Fathers of IVF, ” Drs. Patrick Steptoe and Robert Edwards in the UK. In 1982 he established the 1st private (non-university based) IVF program in the US. Over a career spanning almost 40 years Dr. Sher has been influential in the births of over 17000 IVF babies and has helped fashion the entire field of ART. He is the author of “ In Vitro Fertilization, the A.R.T. of Making Babies” and (more recently), “Unexplained IVF failure and Recurrent pregnancy loss: The Immunologic Link”." Today he's joining me to talk about Immunologic Implantation Dysfunction (IID), which may be a factor in unexplained IVF failure and recurrent pregnancy loss (RPL). He is an internationally renowned fertility expert. He trained under “The Fathers of IVF, ” Drs. Patrick Steptoe and Robert Edwards in the UK. In 1982 he established the 1st private (non-university based) IVF program in the US. Over a career spanning almost 40 years Dr. Sher has been influential in the births of over 17000 IVF babies and has helped fashion the entire field of ART. He is the author of “ In Vitro Fertilization, the A.R.T. of Making Babies” and (more recently), “Unexplained IVF failure and Recurrent pregnancy loss: The Immunologic Link”." Today he's joining me to talk about Immunologic Implantation Dysfunction (IID), which may be a factor in unexplained IVF failure and recurrent pregnancy loss (RPL).
