I am so excited to share this conversation with you. In this episode, I sit down with Dr. Mark Ratner, Chief Science Officer at Theralogix, to talk about a topic I've become incredibly passionate about: NAD+ and its role in egg quality and fertility. If you've been wondering what NAD+ actually is, why your eggs need it, and whether supplementation could make a real difference in your IVF or egg freezing outcomes, this is a must-listen. We get into the science of how NAD+ and sirtuins work together in our cells, why their decline with age is such a big deal for egg health, and the emerging research — from mouse studies to a promising new human trial — suggesting that boosting NAD+ levels could improve fertilization and pregnancy rates. I also share a personal story about a patient whose results completely blew me away after she started NAD+ supplementation on her own. This is the kind of conversation that gets me fired up about the future of fertility medicine. In this episode, we cover: What NAD+ is and how it's present in every cell of your body The role of mitochondria in egg development and why eggs require more energy than any other cell What sirtuins are and how SIRT1 appears to regulate ovarian reserve How the age-related decline in NAD+ and sirtuins creates a "double whammy" for egg quality The connection between NAD+, chromosomal abnormalities (aneuploidy), and diminished ovarian reserve Key findings from mouse studies and a new human study showing doubled fertilization and clinical pregnancy rates with NAD+ boosting How to start supplementing and what to look for Resources: Theralogix website: theralogix.com — Use discount code EGG for ~15% off The IVF Class at The Egg Whisperer School  The Egg Whisperer Show podcast on Spotify and Apple Podcasts Dr. Aimee's Supplement Stack information Subscribe to my YouTube channel for more fertility tips! In this episode, I sit down with Dr. Ramy Ghayda, an experienced urologist and the Chief Medical Officer at Legacy, to dive deep into the world of male fertility. Dr. Ghayda brings his expertise in urology and male infertility to discuss why it's "always nice to have sperm on ice,"a phrase I say often because preserving male fertility is just as important as egg freezing. We explore Legacy's revolutionary approach to men's reproductive health, a digital fertility clinic that provides everything from at-home semen analysis and sperm freezing to DNA fragmentation testing, telehealth consultations, and nutritional support: all designed to fill the gap in fertility care that has traditionally focused only on women. Throughout our conversation, Dr. Ghayda and I discuss the alarming decline in male fertility rates over the past few decades and what's causing it; from environmental toxins and endocrine-disrupting chemicals to lifestyle factors like diet, stress, and sleep. We talk about why fertility should be considered the sixth vital sign and how infertile men face higher risks of chronic diseases later in life. Dr. Ghayda also shares his passion for breaking the taboos around male infertility, rebalancing the emotional workload in couples struggling to conceive, and helping men understand that they are 50% of the fertility equation. This episode is a must-listen for anyone interested in optimizing their reproductive health or supporting their partner through the fertility journey. In this episode, we cover: Legacy's comprehensive approach to male fertility, including at-home semen analysis, sperm freezing, DNA fragmentation testing, and telehealth support The science behind declining sperm counts and quality over the past 50 years and what environmental factors may be contributing How lifestyle choices (smoking, alcohol, marijuana, heat exposure, diet, exercise, and sleep) directly impact sperm production Why DNA fragmentation testing matters and how it can reveal correctable fertility problems The emotional toll of infertility on men and how to rebalance the conversation and workload between partners Why fertility is now being considered the sixth vital sign and its connection to overall health and longevity Practical steps men can take to be proactive about their reproductive health and support their partners Resources: Legacy's website: https://www.givelegacy.com Legacy's educational guides and resources on male fertility The Egg Whisperer Show podcast on Spotify and Apple Podcasts Dr. Aimee's Supplement Stack information Subscribe to my YouTube channel for more fertility tips! Does Sperm Really Matter?

Play Episode Listen Later Mar 23, 2026 13:57

Does sperm really matter? If you tuned into the conversation with Dr. Paul Turek and I earlier this week, you'll know that of course it matters!   It truly does take both sperm and an egg to make a baby. Today I'll review the sperm question a bit, and explain why it matters. AND, I'll give you a lot more info about how to improve sperm quality.   Here's the thing: I know that guys want to hear what they want to hear. They have selective listening. I'm not kidding when I tell you that I had someone email me recently and say "My husband's sperm tested out GREAT, and when we went over the results with the doctor we heard two different things. He thinks he heard our doctor say that it means he can just keep drinking, smoking, and enjoying marijuana and doesn't need to make any lifestyle changes. I heard something very different, which was that he should still cut back until we are pregnant. Can you help settle this for us?"   And while it's an unpopular answer (at least with the guys), my response is: Men should cut back or abstain from drinking, and smoking (marijuana too) if you're TTC.   Here are a few of the things I suggest to anyone bringing the sperm side of the equation to the baby making situation: - consider freezing your own sperm for future use when you are young. - get in the best shape of your life if you're trying to conceive. Sperm matters, and men's health is directly related to their sperm quality. - take some prenatal vitamins. Yes, really. Many of the same supplements that I recommend for egg quality also are great for sperm quality. - cut back on smoking, drinking, and drug use. Marijuana negatively impacts sperm. - don't cook your balls: avoid hot tubs, using your laptop on your lap, and other situations that may create high temps for the area. - find ways to cut back on stress, if you live a stressful life. Mindfulness and meditation are great for everyone. Read the full transcript and article about this at my website. Do you have questions about IVF?Click here to join Dr. Aimee for The IVF Class. Preparing for IVF can be an emotional and complex journey, and having the right information beforehand can make all the difference. In this episode, I'm joined by Dr. Max Ezzati, a highly respected reproductive endocrinologist, to discuss the most important thing to know before starting IVF. Dr. Ezzati shares key insights about how patients can optimize their chances of success, the factors that influence embryo quality, and why understanding ovarian reserve is crucial. We also dive into the role of lifestyle factors, genetic screening, and cutting-edge advancements in reproductive medicine. If you're considering IVF or supporting someone on their fertility journey, this conversation will empower you with the knowledge you need. One important concept we discuss is the IVF pyramid, which illustrates how the number of eggs retrieved translates to the number of viable embryos. Not every egg leads to a healthy embryo, and understanding this process can help patients set realistic expectations about their IVF journey. In this episode, we cover: How to assess your ovarian reserve before IVF Why embryo quality matters and how it's evaluated The importance of individualized treatment plans How lifestyle factors, such as diet and stress, impact IVF outcomes The role of genetic screening in IVF success Common misconceptions about IVF and what patients should really focus on Read the full show notes on my website. In this episode, I'm joined by Dr. Jeff Lysiak, PhD, co-founder and Chief Science Officer at PS Fertility, to discuss phosphatidylserine (PS) and how this critical molecule on sperm is revolutionizing how we diagnose male infertility. Jeff shares the fascinating story of how his research at the University of Virginia led to discovering that PS on the surface of sperm is essential for fertilization and how traditional semen analysis completely misses this vital piece of the puzzle. Read the full show notes on my website.  This episode addresses a frustration I've experienced for years in my practice: couples with "normal" semen analysis results who still have trouble when it comes to trying to conceive. Research shows that 20-30% of men with normal basic semen analysis could still be infertile, and we've had no way to identify them, until now.  In this episode, we cover: What phosphatidylserine (PS) is and why it's critical for sperm to fuse with and fertilize an egg The major limitations of standard semen analysis (count, motility, morphology) and why 20-30% of men with "normal" results may still be infertile How the PS Detect test works using flow cytometry to assess over 10,000 sperm per sample The connection between varicocele and low PS scores—and how varicocelectomy surgery can restore PS levels to normal ranges When the PS test should be ordered in a fertility workup and what to do if scores are low Early data showing over 30% of men seeking fertility treatment have low PS scores, including those with normal semen analysis How this test can prevent women from undergoing unnecessary treatments when the male factor is undiagnosed Resources: PS Fertility website: psfertility.com PS Detect at-home test kit Nature Communications publication on phosphatidylserine and fertilization Dr. Aimee's Supplement Stack The TUSHY Method OVANAD+ by Theralogix. Hosted by Dr. Ban, I'm sharing everything you need to know about getting precision fertility care.... in ten minutes. You'll hear all about the TUSHY Method, the specifics of the BALLS Method, along with the Egg Whisperer DIET and your fertility TEAM. There are many relatively easy and affordable ways to get smart about your fertility. We will never eliminate all of the unknowns, but we can take some simple steps to understand our body and how it may impact our ability to get pregnant. For example, there are sperm kits that you can get delivered to your home without even needing to see a doctor. There's so much technology available now to improve treatment outcomes. Yet, what I think is really important to note is that just because you see a fertility doctor it does not mean you want to do a treatment. I think that's a big barrier for people. What I tell people is to go see someone and get your TUSHY checked, get your sperm checked. Learn more about yourself so that you're not wasting time or money doing something that's not going to work. Read the full show notes on my website. Do you have questions about IVF and Egg Freezing? How Important is Embryo Quality?

I have patients that come to me from all over the world. There's one question I always ask when reviewing their IVF cycle history. What was your embryo quality? I hear time and time again from patients that they don't know it because their doctor told them embryo quality doesn't matter with IVF. That's simply not true! Just like diamonds in a jewelry store. Quality matters. Just like there is a grading system for diamonds, there is also a grading system for embryos. It's important for you to know that this exists and to ask for your embryo grade. When it comes to getting pregnant and IVF, the implantation rate is determined by the embryo quality and rating.  Read the full show notes at Dr. Aimee's website. IVF is a very emotional journey. It can be riddled with fear, doubt, and is full of periods of waiting and wondering. For my patients, I want to reduce any emotional or physical burdens by creating an IVF checklist for them. It covers the core essentials of everything we need to do, so there are no conversations that begin with, "I wish I would have…" down the road. I don't want you to have to wonder if you've done everything you can to have a successful IVF outcome. This is true if you're my patient, or you live half-way across the world. I want to equip you with a complete IVF checklist. I want you to feel prepared going into IVF so there is less chance of having any regrets if things don't pan out as you hope they will. Of course, this can't serve as your only guidance during IVF. Please discuss anything and everything with your doctor! The Top 10 Reasons You're Having a Hard Time Getting Pregnant

Play Episode Listen Later Feb 27, 2026 22:15

There are many things that can impact your ability to become pregnant. In today's episode, I'm reviewing the top 10 things to consider if you are TTC.    1. Environmental Toxins. Avoid microwaving food in plastic containers or bottles. Always store food and glass or stainless steel containers. Million Marker offers a test to find out if you have in-body toxicity. 2. Chemicals in food and everyday products. Verify that the products you are using are chemical free. You can do that on https://www.ewg.org/ 3. Being sedentary. The CDC and American College of OBGYN, both recommend exercise in amounts of 150 minutes per week. Avoid over exercising, as low BMI can also cause fertility issues. 4. Food allergies or intolerances. Work with a nutritionist to find out if you have a food allergy or other food intolerance. You may also want to consider working with a specialist if you have PCOS or working with a nutritionist like Rohini Bajekal to help you manage PCOS if you have it. 5. Blocked Fallopian Tubes. Check with your doctor to find out if this is what may be causing you to have a hard time TTC. 6. THC and Cannabinoids can affect the motility of the fallopian tube and the receptors in the lining of the uterus. This makes embryo implantation harder and it might not happen as quickly as you would want. 7. Nicotine. Nicotine can affect the DNA and the egg in the sperm cells, and it can also affect a woman's fertility such that she could go potentially into menopause earlier. 8. Blaming birth control pills. Many people think that birth control pills "cause" infertility, but the truth is that because they regulate your period they may be masking the indicators of infertility. Make sure you get your hormone levels checked with your doctor every year to confirm your levels.  9. Ignoring biology. Every woman is born with a finite number of eggs, and if you want to wait to have kids until later in life, be sure and freeze your eggs when you are in your 20s.  10. Not getting enough sleep.    Listen on Dr. Aimee's website.   Curious about IVF? Click here to join Dr. Aimee for The IVF Class. How to Deal with a Negative Pregnancy Test

Is your pregnancy test negative, again? Let's get real. It sucks when we do everything we can in our power to make something happen, and it just doesn't. If you've ever wanted to get pregnant, and have seen a negative result on the test, you know exactly what I'm talking about. It doesn't matter if it's one month of trying or 2 years…. "Trying to get pregnant" can be painful, and the phrase itself is defeatist. The mindset it establishes is one based on fear vs. love. I like to lead from a place of love, and encourage my patients to think positively. For that reason, I advocate for replacing it with, "preparing for pregnancy." Doesn't it just sound more peaceful, and open to the process? I think so. Regardless if you are on board with my line of thinking, I do want to share some helpful tips and suggestions so that your path to pregnancy is as informed and efficient as possible. Here are my top tips: My #1 tip for preparing for pregnancy is to NOT put blind faith in fertility apps. Tip #2: Develop a resilient, and positive mindset. Tip #3: Make use of science (diagnostic testing) to understand what could be preventing you from getting pregnant. Oh, and ask questions. Lots of them! Tip #4: Talk to your doctor about things you can do that are in your control like lifestyle and diet. Read the full show notes on Dr. Aimee's website. In this episode, I'm thrilled to welcome Dr. Rahi Victory, a Canadian-trained OB-GYN and reproductive endocrinology specialist who is truly revolutionizing fertility care. Dr. Victory has built a practice that stands apart from traditional fertility clinics through its unwavering commitment to transparency, honesty, and patient-centered care. His approach treats patients as individuals with unique journeys rather than putting them through cookie-cutter protocols, and he serves international patients from across the globe. Read the full show notes on Dr. Aimee's website Throughout our conversation, Dr. Victory and I dive deep into what it really means to make your fertility journey YOUR priority, not your doctor's. We explore his comprehensive approach to patient care: from the initial deep-dive consultation that examines everything from medical history to lifestyle factors, to his philosophy on extensive testing that leaves no stone unturned. Dr. Victory shares his belief that infertility represents a loss of control for patients, and his mission is to restore that control by educating them thoroughly and empowering them to make informed decisions about their own care. In this episode, we cover: • Dr. Victory's holistic approach to initial patient consultations, including psychological, emotional, and lifestyle assessments • Comprehensive testing protocols for both male and female patients, including DNA fragmentation testing and natural cycle monitoring • The importance of transparent communication when delivering test results and educating patients about their diagnosis • How to counsel patients about ALL their options (from doing nothing to surgery to IVF) without pushing a particular treatment • The three components of fertility treatment success: the passage, the passenger, and the destination • What to do when treatment doesn't work and how to investigate every possible factor • How to find a physician who aligns their priorities with yours and advocates for you throughout your journey Dr. Victory's practice: www.drvictory.com Do you have questions about IVF? Click here to join Dr. Aimee for The IVF Class. Last year in the United States a baby was born every 8 seconds. That's roughly 3.8 Million babies born last year alone! You don't have to be a fertility doctor to know what that means. Millions of people just like you were trying to conceive. Research tells us that most couples conceive within six cycles of timed intercourse, but many others (roughly 10% of couples) are struggling with fertility. Here's the funny thing about what I do. I want to teach you how NOT to be my patient. Of course, there are exceptions to that statement. But in general, I'd much prefer to share with you what I've learned about preparing for pregnancy in hopes that it makes your journey to parenthood easier. Planning Your Fertility Future: Understanding Your Fertility (Plus Egg Freezing and IVF)

Play Episode Listen Later Jan 16, 2026 9:21

In this episode, I'm breaking down something that should be simple but often isn't: how to plan your fertility future. You don't need to be a fertility specialist or have a science degree to understand the basics of fertility planning. I want everyone to approach this the same way you'd plan a vacation: with intention, research, and realistic expectations. The most important thing I want you to know? Humans ovulate just one egg per month, and that egg only has about a 15% chance of being genetically healthy, even in our twenties. This fundamental truth changes everything about how we should approach family planning. Throughout this episode, I share my three-rule framework that I live by:  What do I want?  What is it going to take to get what I want?  And am I willing to do it?  When applied to fertility, these questions help you get concrete and honest about your timeline, your options, and the steps you need to take now to avoid struggle later. I explain why fertility treatment is really about "egg rescue" - saving eggs from being lost forever - and why planning your fertility deserves at least as much attention as planning that dream vacation to the Maldives. In this episode, we cover: Understanding that we ovulate only one egg per month with a 15% chance of genetic viability at best The reality of conception timelines: why it can take 6-12 months for healthy couples to conceive Miscarriage statistics and why 20-30% of pregnancies end in the first trimester through no fault of your own The three-question framework for planning your fertility future: what you want, what it takes, and your willingness to act How egg freezing works as "IVF without the F" and why it's about rescuing eggs, not depleting them Age-related fertility decline: why your chances at 41 (10% viable eggs) are dramatically different than in your twenties Preparing for pregnancy like you'd prepare for a vacation: getting medically ready, addressing health issues, and genetic carrier screening Do you have questions about IVF? Click here to join Dr. Aimee for The IVF Class.
