The first is the pharmacology around the choice and strength of local anaesthetics used in epidural analgesia - thanks Siv. The second part we discuss a very large pragmatic study in the New England Journal of Medicine of 11000 women studying the use of tranexamic acid as a prophylaxis during caesarean surgery (elective and emergency). Thanks for tuning into this week's episode of One For The Road. This week I am joined by doctor and menopause specialist Rebecca Lewis, as we discuss the links between alcohol consumption and the exacerbation of the symptoms of menopause. Rebecca qualified from Guy's Medical School and initially pursued a career in Anaesthetics gaining FRCA in 1996. She then changed specialty to General Practice (MRCGP 2000) having been interested in women's health and gained DRCOG in 1998. During her time in General Practice, she became interested in menopause and by how dramatically it could affect individuals. She found this the most rewarding area of medicine to be involved in as once a woman is correctly diagnosed and treated with the right type and dose of HRT their life can be transformed and needless suffering stopped. As a result, she is now passionate about improving education and knowledge about menopause, not only within medicine but in the workplace and society as a whole. In surgical care worldwide, there has been a growing trend towards enhanced recovery with the emphasis on early mobility, avoidance of unnecessary procedures and drug-induced side effects, and faster return of normal gastrointestinal function, with the end result of shorter hospital stay. It's natural to assume that regional anesthesiology and acute pain medicine would play a central role in enhanced recovery protocols for all of the well-described benefits. However, the base evidence is continually evolving as both anesthetic and surgical practices change. In this month's RAPM Focus, Associate Editor Edward Mariano, MD, joins Kariem El-Boghdadly, MBBS, BSc, FRCA, EDRA, lead author of an international collaboration in the regional anesthesiology and enhanced recovery world, “Role of regional anesthesia and analgesia in enhanced recovery after colorectal surgery: a systematic review of randomized controlled trials,” first published in March 2022 (https://rapm.bmj.com/content/47/5/282). Investigators carefully evaluated thirteen published randomized controlled trial studies involving 1,170 participants and concluded that regional analgesia may only have modest evidence to support its inclusion in enhanced recovery protocols involving colorectal surgery patients. Dr. Kariem El-Boghdadly is a consultant anesthetist at Guy's and St. Thomas' NHS Trust in London, and an editor of the journal Anesthesia. Dr. El-Boghdadly completed his undergraduate training at University College of London in 2007 and proceeded to undertake his anesthetic training in Southeast School of Anesthesia. He was awarded the Nuffield Prize for the fellowship of the Royal College of Anaesthetists and completed fellowships in prehospital medicine in Australia, followed by regional anesthesia in Toronto, Canada. Dr. El-Boghdadly is an honorary senior lecturer at King's College London, and his research interests include regional anesthesia, airway management, and research delivery. He is a brilliant researcher and writer and is well-known for his work in the COVIDSurg Collaborative during the ongoing pandemic, and the ASRA-ESRA regional anesthesia nomenclature project. *The purpose of this podcast is to educate and to inform. I previously believed that HRT (Hormonal Replacement Therapy) was off the table for me - don't ask me why. I guess I always thought it was for women that were really struggling with menopause symptoms and that being a qualified fitness educator, I should be able to battle on through with the knowledge that I had. SO glad I spoke with Dr Rebecca, if only to be able to make more informed decisions about how I intended to embrace longevity and stay feeling SEX-AY! Dr Rebecca Lewis (MBBS FRCA DRCOG MRCGP) qualified from Guy's Medical School in London in 1991 and initially pursued a career in Anaesthetics, obtaining FRCA in 1997. She then changed speciality and became a GP gaining MRCGP in 2000. She continues to practise as a GP and has developed an interest in the menopause, giving lectures and writing articles on the subject. She is determined to improve the understanding of the menopause in current and future generations through education and access to accurate evidence based medical information. She is passionate about making the menopause a much more positive experience and enabling women to get back to feeling themselves again. She has two young children and enjoys running and keeping fit.
