Dr. Jennifer Wargo is an Associate Professor in the Department of Surgical Oncology at The University of Texas MD Anderson Cancer Center and a Stand Up To Cancer researcher. Jennifer is a physician scientist, and this means she splits her time between providing care to patients and doing research to find better ways of treating disease. Specifically, Jennifer performs surgeries and treats patients one day each week. She spends the rest of her week studying how to better treat patients with cancer and how cancer may ultimately be prevented. When she's not doing research or treating patients, Jennifer enjoys spending quality time with her family. Some of their favorite activities include going for walks, biking, hiking, and visiting the beach. Jennifer also likes to explore her creative side through art and photography, as well as to be active through running, biking, yoga, and surfing. She received her A.S. degree in nursing and B.S. degree in biology from Gwynedd-Mercy College. Afterwards, Jennifer attended the Medical College of Pennsylvania where she earned her M.D. Jennifer completed her Clinical Internship and Residency in General Surgery at Massachusetts General Hospital. Next, Jennifer was a Research Fellow in Surgical Oncology at the University of California, Los Angeles. She then accepted a Clinical Residency in General Surgery at Massachusetts General Hospital. From 2006-2008, Jennifer was a Clinical Fellow in Surgical Oncology at the National Cancer Institute of the National Institutes of Health. She then served on the faculty at Massachusetts General Hospital and Harvard University. In 2012, Jennifer received her MMSc. degree in Medical Science from Harvard University. Jennifer joined the faculty at The University of Texas MD Anderson Cancer Center in 2013. She is Board Certified by the American Board of Surgery, and she has received numerous awards and honors throughout her career. These have included the R. Lee Clark Prize and Best Boss Award from the MD Anderson Cancer Center, the Rising STARS and The Regents' Health Research Scholars Awards from the University of Texas System, the Outstanding Young Investigator and Outstanding Investigator Awards from the Society for Melanoma Research, as well as a Stand Up To Cancer Innovative Research Grant for her microbiome work. She has also received other awards for excellence in teaching, research, and patient care. In our interview, Jennifer shares more about her life and science.

In this explainer episode, we've asked Katrina Stone, Clinical Genetics Doctor, and Clinical Fellow at Genomics England, to explain what happens when you go for whole genome sequencing for a rare condition. Florence: What happens when I go for whole genome sequencing? I'm joined by Katrina Stone, Clinical Genetics Doctor, to find out more. So, Katrina, first things first. What is the purpose of whole genome sequencing? Katrina: The purpose of whole genome sequencing is to try to make a precise genetic diagnosis for someone with a suspected or confirmed genetic condition. Florence: And why might someone get whole genome sequencing? Katrina: They might get whole genome sequencing because they are known to have a condition which is likely to be genetic, but the medical team wants to find out what the exact genetic cause is. In other cases, the diagnosis might not be known, and the reason for doing whole genome sequencing is to find out whether there is a genetic condition present. Some of the benefits of having the test is that. If a condition is identified, this can provide an explanation for the family about what's been going on, and it can also bring to an end further unnecessary investigations. Also, if a genetic diagnosis is confirmed, this can sometimes point towards other things which might need to be kept an eye on for the individual. In addition, once a diagnosis is confirmed, a doctor can advise the family on the likelihood of other members of the family or future children being affected with the same condition, and they can use this information to help with future family planning. Florence: So, then what happens when a person physically goes to get the test? Katrina: In most cases, an individual will see a specialist doctor. This might be a genetics doctor, but it could be a doctor specialising in another body system. They'll do a full assessment of the individual, including finding out lots of information about them and their family, and also examining them to look for any clues that might point towards a specific genetic diagnosis. Once the family have decided to go ahead with the test, their consent will be taken, where the test will be explained in more detail, including the pros and cons of going ahead with the test and after that samples can be taken. Usually this is a blood sample, but occasionally a saliva sample or cheek swab could be taken. The best way to perform whole genome sequencing is with a sample from the person being tested along with both of their parents. And the reason for this is that it makes it easier to separate out genetic changes that are more likely to be significant from those that just represent harmless genetic variation what makes us all unique. Florence: What happens to this sample after the test has taken place? Katrina: So, the blood samples will go to a genetics lab where the genetic material known as DNA is extracted. The DNA is then sequenced, so we get an electronic file of all their genetic information. This is then analysed firstly by a computer which picks out changes or variants in their DNA, which are more likely to be significant. After this, a trained clinical scientist analyses the data in detail. Sometimes there isn't a clear-cut result, and the scientists might need help from others and interpreting the result, but if there is, they can create a report which details the likely diagnosis. Florence: And finally, how will the patient get the result from their whole genome sequencing test? Katrina: Usually, the result is fed back to the patient and their family by the clinician who arranged their testing or one of their close colleagues. It's important to note that not everyone will get a genetic diagnosis from the test. This doesn't necessarily mean there isn't a genetic diagnosis present. There are several reasons why tests might be negative. One is that no test is perfect and something important might have been missed because of the way the test works. Or it may be that the person being tested has a change in a gene that hasn't been described as causing a disease before, so we wouldn't even know to look for it. There's also a possibility that there isn't a single genetic cause for their symptoms. Rather, lots of minor genetic factors are causing their condition. We're not very good at testing for these yet. Finally, there could be a non-genetic cause that just hasn't been identified yet. One of the benefits of having a whole genome sequencing test is that the data can be stored and looked at again in the future, either in light of new evidence or once our knowledge of genetics has improved. Alexey Tolchinsky, Psy.D. is a clinical psychologist in private practice in Maryland. He specializes in trauma work and anxiety disorders. Concurrently, she published research papers and contributed to undergraduate and postgraduate teaching programs. Sinéad's area of special interest lies in intravenous drug users living with HIV and/or Hepatitis, focusing on managing services to engage these patients in care. Her recent role has allowed her to work closely with this patient cohort, both clinically and in a research capacity, with notable presentations on anti-retroviral commissioning at BHIVA. Currently serving as the Chief Pharmaceutical Officer's Clinical Fellow at NICE, Sinéad collaborates closely with prominent leaders like David Webb, the Chief Pharmaceutical Officer, and Professor Jonathan Benger, Chief Medical Officer at NICE, on projects that continue to expand her professional and personal skillset. This is a very meaningful podcast discussion for me as I not only get to catch up with a dear college friend I also get to deeply explore the occupational impact of how one's life path can evolve after acquiring a profound neuro divergence early in life. Ashley and I's paths in our early twenties crossed quite a bit however I never would have guessed I would ever become a therapy colleague with her over a decade after moving away from Olympia, WA. When I last was actively connected to Ashley's life she was the most prolific fiction and play writer I had ever met. She was destined to become a professional editor and facilitator of creative wonder. Her momentum as a writer was however stunted as it is for many of us with the sudden acquisition of a disabling experience. At age 25 Ashley acquired a cerebral vascular accident impacting her left side. This occupational disruption ended up fueling Ashley's passion to go back to school. She not only regained her motor function, but she also regained her voice, and new gifts, however, she could not develop these ambitions on her own, it took a village including occupational therapy, physical therapy, speech therapy, brain injury support groups, vocational rehab, and the fight to self advocate for her educational accommodations. Connecting with Ashley I couldn't avoid the correlations between her life path and the challenges my friend Paul Johnson a leader in the WA disability rights movement in how hard he had to fight to access the ability to write in general and to access formal publishing in particular. His life story took over a decade to write and I was his support volunteer intern where it took over 2.5 years of active letter by letter word by word typing a revising. Because of Paul, I have learned to never take my voice and mobility for granted. I am so glad I was able to share Paul's work with Ashley with a new foundation of shared meaning as we all interconnected in our experiences of rare young adult neurodivergence and how that impacts and changes access to writing and our relationship to our own voice when you depend on supported communication to express yourself. Paul's family offered consent to continue sharing his story and our time together publically with the goal of inspiring others to tell their stories. This week's episode features collegial debate between two colleagues who are proponents of two schools of thought: Attachment or EFT Theory, and Differentiation or Crucible Theory. The two schools are substantially different in how they view aspects of adult development, and we have two guests who will walk us through the differences and similarities. We are joined this week by Adam Fisher and Nathan Hardy. In the IMG roadmap program, this has never been seen as an alternative path to residency since most international doctors choose residency as their first option. Let's explore how this young doctor has achieved this feat by choosing an alternative route! Dr. Siegried Chin, a Philippines' native, migrated to the United States in 2020 and became a permanent citizen in 2021 after her graduation from the De la Falle Medical and Health Science Institute in 2017. Like many of her fellow IMG colleagues, Dr. Chin admittedly encountered many challenges during the match season. She applied to 120 programs and scored 5 interviews, none of which were successful. Cognizant of her own weaknesses by attaining a low step one score, Dr. Chin, though devastated for not being matched, did not wallow in self-pity, but decided to utilize the available online resources for the sole purpose of networking on several social media platforms to boost her future chances. It was then that she was introduced to a doctor who had migrated from the Philippines and had graduated from the fellow program of which she ultimately became part. Dr. Chin is currently working as a Memory Disorder Pre-Clinical Fellow at the Augusta University in Georgia. She was fortunate to have received a five-year NIH grant for studies at this extraordinary program. She states that she is getting hands on knowledge when dealing with patients who have Alzheimers or Parkinson's Disease along with other related disorders. As a fellow, which is a paid position, she performs similar functions as a PGY1, can see patients under the supervision of an attending, can prescribe medication and has a temporary license to practice. Dr. Chin acclaims her success to her tenacity and shares the following tips with future IMGs: Have a strong support system of family and friends Be steadfast in your capabilities, and be realistic Network, foster academic friendships by joining support groups on social media like Twitter, Facebook, Instagram and others Remember to give back to the community and to others who were once in your position. During the pandemic, an increasing number of children and adolescents have reported struggling with anxiety and depression. How can we help them process their emotions and get the support they need? So anything from having to remember, having to delegate, having to pay attention to dynamics, you know, noticing how people are feeling, if they're getting along, if they're happy in a situation, those are all things that are in the mental load. 3:41 The problem becomes when only one person is shouldering that mental load and they didn't fully agree to do that. 4:30 One thing when I'm working with premarital couples that we talk about a lot is for them to explore this idea of mental load, because it often doesn't really feel problematic until there's a lot of layered responsibilities. 7:16 If you are suffocating under the mental load, you have to be willing to let your partner do it in their way, as long as it gets done. And that might mean that you come home and you cringe because the way that the person they chose to do landscaping didn't do it the right way. But you have to take a deep breath and recognize that the bigger picture is that you're getting a break. 8:55 So my job in this situation is to keep expressing that I'm frustrated, let my partner know that I'm really looking forward to the way that they're resolving this and all of that kind of stuff, but like allowing him to truly deal with it. Which is really hard, and when I work with couples, a lot of people will be like, “but it means that our house isn't going to look right.” And I'm like, “yes, I totally get that.” And it takes a period of time to readjust, to giving some of this up and seeing that most of the time, it truly does work out.9:52 Couples get themselves in really messy spots when they start getting into this, it's almost like a stubbornness, where they're saying, I'm not going to talk about it because they should. I'm not going to bring that up because they should know better. I'm not going to go to their parents house anymore because their parents shouldn't act that way. I'm not going to let my partner know that they bummed me out because they should already know that; who wouldn't know that?10:18 The second you start to get to that place, you get into a place that I call willfulness. Where you have put your feet in the ground, you're digging your heels in and you're saying, I'm just going to sit still here until you please me. The problem is, if your partner truly knew, if they did, most of the time they would do the right thing. So if, if they really did know about the mental load, most of the time they'll want to talk to you about it. If they do know, and they still don't care to do it, then that's actually really important information for you to hear. 12:02 And when we start thinking in the shoulds, what we do is we really block out communication and we also tend to then shame the other person. Because if you should know something or you should do something, then it means that you've made a mistake, right? Because you didn't do it or you didn't know it. 13:19 So difficult conversations are going to be everywhere during the holiday season. I think they always are, but especially now, because people have to decide what they're thinking about their health, what they feel comfortable with, what they feel disappointed by and adding that on to the layers of disappointment, discomfort, sadness, grief, all of that, that has already happened over the past year and some, right? So it's a lot. Something that is really, really important for people to be able to do when they're approaching a difficult conversation is to know how to enter it gently. And to enter it from a space where you want your partner to be your ally. You're not entering it immediately seeing them as the opposition.14:09 What happens with really tough conversations sometimes is that we feel so anxious. We feel so overwhelmed, so worried that we're not going to get what we need, that we already address the other person like they're our enemy. 14:49 So if we start from an offensive position, we can not expect the other person to be in a neutral position. But we often act really surprised when they're defensive.15:58 Let the other person talk first, because as soon as you let the other person talk, their defenses are down.17:51 It takes the ability to have other awareness and self-awareness to be gentle and firm. If you don't have both of those awarenesses, what happens is you are too far on one side or the other. 18:42 But when you're having a tough conversation with your partner, you want to try to stay in this place of: I still see you as a human being with feelings, with needs, with thoughts. I'm still curious about you. I want to understand where you're coming from. And, I see myself as a person with thoughts and needs. And I want you to be curious about me.21:04 Human beings are wired to connect with other people, but we're also really wired for aggression too. So we are nuanced, we like to fight people and we like to love people. Neither one is actually the more dominant trait, they're both there. 24:10 So in our everyday interactions, we want to increase the amount that we're turning towards, because what research has shown us is that when people are turning towards their relationships, and I think this is pretty obvious, tend to be happier. 24:56 In the big picture with our communities, we want to create connection, not isolation and distance. In our intimate partnerships, obviously you want connection. So you want to really look for the ways in which you can turn towards.
