He obtained an MD degree at Rutgers New Jersey Medical School and did his undergraduate studies at the University of Pennsylvania. He has worked in a variety of treatment settings, from non-profit community mental health clinics to federally-sponsored multi-site medication development studies.In addition to his clinical work, Dr. Hrymoc continues to be active in the local addiction treatment community. In September 2008, he founded Perspectives on Addiction, an interdisciplinary educational group for addiction professionals in Los Angeles. Dr. Hrymoc has served as the representative of the states of California, Alaska, and Hawaii to the American Academy of Addiction Psychiatry and was a member of the education committee of the California Society of Addiction Medicine. Dr. Hrymoc co-authored a chapter in Principles of Addiction Medicine, the main textbook of the American Society of Addiction Medicine and has been published in several academic journals.

For many years opium and its derivatives were the mainstay of pain management, with local anesthetics making an appearance in this country in the past century or so. While scientists, physicians, researchers, and industry have sought to find better medications and approaches, the onset of the opioid crises in early 2000s really sped up that mandate. The FDA has committed resources and funding to help these new treatments and as a result there are now several new drugs on the market.Joining me today to explore these new modalities are Dr. Rahki Dayal who is a Professor at the University of California, Irvine, double board-certified in anesthesiology and pain medicine. She chairs the Pain Committee for the California Society of Anesthesiologists and is the Program Director of Pain Medicine at UCI. Dr. Amber Borucki is an Associate Professor of Anesthesiology at Stanford University and a pediatric anesthesiologist and pain specialist from Stanford School of Medicine. She is also the newly elected Sectrtary/Treasurer of the Society for Pediatric Pain Medicine.On January 30 the FDA approved a new non-opioid pan medication for moderate to severe pain. In the past few years several new classes and types of medications have also been approved for use. Jim also suggested that this data could be a deciding factor in economic development decisions for businesses.   Discussing Business Engagement and Tax Equity Andy and Jim discussed their engagement with the business community, focusing on education and ensuring tax equity. Andy explained that their teams help businesses understand and correctly fill out tax forms, particularly in states with complex rules. Jim agreed, emphasizing the importance of consistency and professionalism in government dealings. They both acknowledged the potential for tax inequities and the need to level the playing field for businesses.   Discussing Data Management and AI in Local Governments Jim and Andy discussed the rapid growth of data generation in local governments due to technological advancements. Andy, who is visiting Pittsburgh for the first time, expressed his excitement about the city's revitalization and planned to extend his stay for a bike tour along the rivers. Dr. Itai Danovitch, MD, MBA is a Professor and the Chair of the Department of Psychiatry and Behavioral Neurosciences at Cedars-Sinai Medical Center in Los Angeles. He earned his bachelor's degree from UC Berkeley and his medical doctorate from the UCLA School of Medicine. He completed psychiatry residency at Columbia University, an addiction psychiatry fellowship at Cedars-Sinai Medical Center and a Master of Business Administration at the UCLA Anderson School of Management. Dr. Danovitch's clinical practice and research is focused on the treatment of substance use disorders and the integration of medical and mental health services. His current research is funded by the National Center for Advancing Translational Sciences (NCATS), the National Institute on Drug Abuse (NIDA), and the National Institute of Arthritis and Musculoskeletal and Skin Diseases (NIAMS). He is the author of over 80 articles and book chapters and co-editor of two books on substance use disorders. Dr. Danovitch is a Distinguished Fellow of the American Society of Addiction Medicine, a Distinguished Fellow of the American Psychiatric Association and past president of the California Society of Addiction Medicine. He also serves as a Governor-appointed state Commissioner on the California Mental Health Services Commission. Greg Hammer, MD is a Professor at Stanford University School of Medicine, pediatric intensive care physician, pediatric anesthesiologist, mindfulness expert, and the author of GAIN without Pain: The Happiness Handbook for Health Care Professionals. A member of the Stanford WellMD initiative, Dr. Hammer is the former Chair of the Physician Wellness Task Force for the California Society of Anesthesiologists. He has been a visiting professor and lecturer on wellness at institutions worldwide and teaches GAIN to medical students, residents, and fellows at Stanford. Dr. Hammer's clinical focus is in pediatric cardiac anesthesia and pediatric critical care medicine. His research is in developmental pharmacology and immunology, and he has an active laboratory with multiple ongoing studies in these areas. RAPM Editor-in-Chief Brian Sites, MD, joins Lei Xu, MD, and Ed Mariano, MD, MAS, FASA, FASRA, two coauthors of “Rate of occurrence of respiratory complications in patients who undergo shoulder arthroplasty with a continuous interscalene brachial plexus block and associated risk factors,” first published in May 2023. Dr. Xu is a clinical assistant professor in the department of anesthesiology at Stanford. She received her medical degree from Stanford before completing her residency in anesthesiology at the University of California San Francisco, and then a fellowship in regional anesthesia and acute pain medicine at Stanford. She is interested in the application of regional anesthesia outside the operating room. Her research interests include studying the functional outcomes of clinical pathways that include regional anesthesia in the primary arthroplasty in acute trauma populations. Dr. Mariano is a professor and senior vice chair in the department of anesthesiology at Stanford. He has developed techniques and patient care pathways to improve post-operative pain, patient safety, and other outcomes, and has published over 250 peer-reviewed articles. He has held leadership positions in the California Society of Anesthesiologists, American Society of Anesthesiologists, American Society of Regional Anesthesia and Pain Medicine, and multiple journal editorial boards. In their retrospective observational study, over 1,000 patients were evaluated undergoing total shoulder replacement with a continuous interscalene block. Respiratory complications were categorized into four groups: none, mild, moderate, and severe. Interscalene blocks are important to the success of these surgeries, but there remain concerns about respiratory morbidity. *The purpose of this podcast is to educate and to inform. Adam J. Milam, MD, PhD is a Senior Associate Consultant in the Department of Anesthesiology and Perioperative Medicine at Mayo Clinic Arizona. Prior to starting this position, he completed his cardiothoracic anesthesiology fellowship at Cleveland Clinic and residency at Cedars-Sinai Medical Center. He graduated from Wayne State University School of Medicine. Prior to medical school, he attended Johns Hopkins University where he graduated with a Bachelor of Arts in Public Health. He also holds a Master of Health Science as well as a Doctor of Philosophy in Public Health from the Department of Mental Health at Johns Hopkins Bloomberg School of Public Health.Dr. Milam has been actively involved in several national and state societies including the American Society of Anesthesiologists, the Society of Cardiovascular Anesthesiologists, the National Medical Association, and the California Society of Anesthesiologists; he serves on committees for each of these organizations. He also is actively engaged in research; Dr. Milam's research is rooted in practices, policies, and interventions to address health disparities and health inequities. He has published over 80 peer-reviewed publications and has more than 75 national, international, and invited presentations. Then Peter Lurie, President of the Center for Science in the Public Interest (CSPI) joins us to talk about CSPI's work advocating for a safer, healthier food system, as well as their newsletter "NutritionAction." Plus, Ralph questions why the US still hasn't ratified the UN's Convention on the Rights of the Child— everyone else has done it!Steve McNamara is a newspaper publisher, editor, and reporter. He has previously written for and edited the Winston-Salem Journal, The Miami Herald, Car and Driver magazine, and the San Francisco Examiner. From 1966 to 2004 Steve and his wife, Kay McNamara, published the Pacific Sun—the country's second-oldest alternative weekly newspaper— and Steve has served as president of the California Society of Newspaper Editors and as founding president of the National Association of Alternative Media. In 2008 he helped revive the San Quentin News— a newspaper written and edited by incarcerated men at San Quentin Prison— and continues to work as a volunteer adviser at the paper. The recidivism rate among the inmates is zero… And actually, the problem that we have is the turnover is terrific because they keep getting paroled or serving their sentence. Steve McNamara, San Quentin NewsPrisons are little empires and the emperor is the warden, and what he decides goes. And many, if not most of the other wardens in California wanted no part of this damn newspaper. So we had a lot of trouble getting it distributed. But by now it's become very popular with the inmates throughout the state, and with many of the correctional officers as well. Because we've made some intelligent decisions. One was to write the personality profiles of some of the better correctional officers and of the programs that take place in the prison.Steve McNamara, San Quentin NewsMost people in the criminal justice system think that it goes this way: somebody breaks into a house, and then they get caught, and they go before a judge, and they get sentenced, and they disappear. And as far as many, many people in the criminal justice system are concerned, that's the end of it. These people have disappeared. We don't need to worry about them any more. But as everybody should know, 80-90% of them will be back on the streets…So do you want these guys to have the same sort of attitude about life as they did when they went into prison? If not, here's a chance maybe to move things in a better direction.Steve McNamara, San Quentin NewsI've credited the Center for Science in the Public Interest with transforming the nutritional habits of perhaps 40 million people. It generated front page news, it was on the evening television news, Congressional hearings. Recently, and this has happened to a lot of citizen groups, the media has not been covering what we're doing.Ralph NaderDr. Peter Lurie is President and Executive Director of the Center for Science in the Public Interest—an independent, science-based consumer advocacy organization that advocates for a safer, healthier food system. The CSPI also publishes NutritionAction, a healthy-living guide for consumers. Dr. Lurie previously worked with the Food and Drug Administration and Public Citizen's Health Research Group, where he co-authored their Worst Pills, Best Pills consumer guide to medications.We at CSPI try to educate consumers on the one hand. And on the other hand, we try to take care of the environment such that consumers in some ways don't even need to be as educated because the environment is different.Peter Lurie, President of Center for Science in the Public InterestI think that most people—including in all likelihood the Current Commissioner— understand that the food program within FDA has been the sort of “poor stepchild” of  the agency. People have focused more on drugs, they've focused more on medical devices, vaccines, even more recently on tobacco. And so food has been relatively neglected. And I think that we've at times paid the price for that.Peter Lurie, President of Center for Science in the Public InterestThere's no issue too trivial for the industry to show up in an obviously self-interested way and advocate on their own behalf.Peter Lurie, President of Center for Science in the Public InterestIn Case You Haven't Heard with Francesco DeSantis1. Late last week, the National Labor Relations Board issued a decision in the Cemex Construction Materials case, establishing a “new framework” that “when a union requests recognition on the basis that a majority of employees in an appropriate bargaining unit have designated the union as their representative, an employer must either recognize and bargain with the union or promptly file a... petition seeking an election.” Crucially though, this ruling also establishes that “if an employer who seeks an election commits any unfair labor practice that would require setting aside the election, the petition will be dismissed, and—rather than re-running the election—the Board will order the employer to recognize and bargain with the union.” This stunning decision is among the most important revisions to labor rules in decades and will apply retroactively.2. On a hot streak, Bloomberg Law reports that the NLRB also decided last week that Quickway Transportation “must reopen a terminal in Louisville, [Kentucky], that the company illegally shut down in 2020 after drivers there formed a union.” This sets a powerful new precedent for recourse against companies that have used the tactic of shutting down locations in order to stave off unionization – most notably Starbucks.3. The American Prospect reports that in Georgia, Governor Brian Kemp has aggressively courted EV manufacturers using Inflation Reduction Act tax incentives. Yet, Governor Kemp has awarded these “lucrative contracts for building out the factories to non-union construction firms.” These firms also happen to be major donors to Georgia Republicans, including Kemp himself, who formerly owned a non-union construction company. 4. Mondoweiss has published a new report on the founding of the Institute for the Critical Study of Zionism. This Institute “aims to support the delinking of the study of Zionism from Jewish Studies, and to reclaim academia and public discourse for the study of Zionism as a political, ideological, and racial and gendered knowledge project, intersecting with Palestine and decolonial studies, critical terrorism studies, settler colonial studies, and related scholarship and activism.”6. NBC News reports that a group of nearly 90 Democratic members of Congress sent a letter to President Biden last week calling on him to take further action to address the student debt crisis. These members ended the letter by writing “We urge you to continually find ways to use your authority to bring down student debt, address the rising cost of college, and make postsecondary education affordable for all students who choose that path. Borrowers have already waited nearly a year for the relief you announced in August 2022, and critics of your plan to help 43 million Americans are likely to renew their attacks with regard to your rulemaking announcement. We urge you to reject their bad-faith, partisan attempts to delay relief and carry out your efforts to help borrowers as quickly as possible.”7. According to the Baltimore Banner, Charm City may soon be facing its own version of the Cop City fight. Per the report, Baltimore officials are planning to construct a “tactical village” which will be used to train police. There are some differences between the projects however: whereas Atlanta's Cop City is slated to be built upon a razed section of forest, Baltimore's tactical village is proposed to be built on the campus of Coppin State University, a historically Black institution. Another key difference, while Cop City is estimated to cost $90 million, the tactical village is estimated at a whopping $330 million. The city has proposed a new “public safety income tax” to fund the project.8. The Washington Post has published a profile of Sarah Feinberg, an employee at the defense contractor Booz Allen Hamilton who blew the whistle on rampant over-billing of the government by the company. In July, Booz Allen agreed to pay $377 million to settle the case. Perhaps the most shocking portion of her complaint was when “a senior manager…called federal auditors “too stupid” to notice overcharging.”9. WTOP reports the fast-casual chain Chipotle has agreed to pay over $300,000 in a settlement with the District of Columbia regarding the company's alleged violations of child labor law. DC Attorney General Brian Schwalb's office identified more than 800 alleged violations in the District, including “requiring minors to work past 10 p.m., working more than eight hours a day, working more than six consecutive days, or working more than 48 hours in one workweek.” The settlement does not require Chipotle to admit wrongdoing.10. Finally, AP reports that, during an address to Jesuits in Lisbon,  Pope Francis “[said] ‘backward' U.S. conservatives have replaced faith with ideology.” So remember listeners, now when you say conservatives have replaced faith with ideology, you can cite the Pope.This has been Francesco DeSantis, with In Case You Haven't Heard. In Platemark s3e29, host Ann Shafer speaks with April Vollmer, an artist working in mokuhanga (Japanese color woodblock printing), who also wrote the indispensable guide to that form: Japanese Woodblock Print Workshop (Berkeley: Watson-Guptill Publications, 2015).   What's mokuhanga, you ask? It's a method developed in Japan that was used to print images that are probably familiar to you, like Hokusai's Great Wave. It allows artists to work with water-based inks (more environmentally friendly) on multiple blocks to build up images, which are printed by hand using a flat paddle called a baren. Each color is carried on a separately carved block and is layered during printing. It means a lot of carving, but it also means one doesn't need a press, chemicals, or a ton of equipment. In other words, it can be done at home in your kitchen.   The range of work made possible in mokuhanga is impressive. Once used for ukiyo-e prints (pictures of the floating world) in the Edo period, now contemporary artists from the world over are experimenting with its possibilities. Including April Vollmer. Running a desk while growing and managing a team can be extremely challenging. Someone who has managed to pull it off is Allan Fisher, founder of Premier Financial Search.    Allan's got an amazing track record of personally billing over a million dollars for ten years straight. Then during Covid, Allan decided to turn a threat into an opportunity and grow his practice from a small, office-based team of 4 people to a remote team of 15 people based across the country.   According to Allan, the secret to his success involves excellent an operational support team, robust systems and processes, leveraging automation technology and commit to ongoing staff training & development.    In his long recruiting career, Allan has faced his share of adversity. So it is not gonna make us recession-proof, but it will insulate us.” This is how Allan summarized how they have dealt with economic downturns and recessions since he started his recruitment business.   Allan started his recruitment business just around 2001, prior to the 9/11 attack. However, he had been a recruiter since 1988. So you can just imagine how many downturns Allan had undergone as a recruiter and business owner - the dot com bubble, the 9/11 attack, the real-estate bubble leading to the 2008 recession, and COVID. Now that we are all facing a similar uncertainty it is invaluable to hear Allan's thought process and insight, and below are the takeaways:   Coming out of a recession, the market usually rebounds in a dramatic way and this period usually brings prosperity. You will only lose out if you quit. When the market changes for the better, clients will have more choices and they will revert to QUALITY. Be consistent with what you do and never change the quality that you present in any market. Never stop doing business development. When things are doing well, it may be easy to get clients but never stop doing business development.   Why Having an “Integrator” is Key to Operational Success    Being a successful business owner is not the same as being a successful recruiter. On the other hand, the best recruiters are not necessarily always the best business owners. The best business owners are actually the ones who are good at marketing, sales, and leadership. Hence, if you are a successful recruiter transitioning to becoming a business owner, hiring someone complementary to your technical skills is a key to success.    This is the case with Allan, and he shared his journey of discovering this logical approach with his team. By finding his and his team's strengths, he was able to find the best role for his members. From within his team, he was able to find an integrator who is very effective in operational execution.    As a recruitment business owner, things you may need to ask yourself: Do you have an integrator to help in the operational aspects of your business? Are you able to set up your team for success by having them work primarily on their strengths?   Growing Your Business by Building a Team in a Remote Working Environment   A key highlight when growing Allan's business is when they started to grow the team from 4 to 15 members in a couple of years by hiring people remotely. “A couple of years ago when we realize that location was not really a factor for our clients. Then I realized, maybe it should not be a factor for us,” he recalled. So Allan started to work on building his team, by first running a search. He looked for candidates that have been doing what they do for 3-4 years and ran a national search. He was able to find rockstar team members in doing so. Given that this is a remote setup, Allan also revealed how they adjusted their training methodology in order to accommodate their new team members. He went on to receive his medical doctorate from the Washington University School of Medicine in St. Louis, Missouri. He completed his residency in the Department of Anesthesiology at University of California Los Angeles, where he also pursued a fellowship in interventional pain management. Board-certified in both anesthesiology and pain management, Dr. Leitner is fluent in Spanish, Hebrew and Hungarian.
