Cochlear implants are neat little devices that drastically improve one's quality of life. However, only about 10% of those that qualify for cochlear implants have actually received one. When one thinks of cochlear implants, the first thing that might come to mind is surgery--invasive, lengthy, with a long time of recovery. However, at PNI, patients can expect a much faster, minimally invasive treatment option. Dr. Rebecca Lewis, the Audiology Director of the Adult & Pediatric Cochlear Implant Program, highlights the importance of getting an evaluation done, and early. Studies have shown that the earlier one gets treatment for their hearing issues, the better the outcome. Mathilde Tranberg, an outreach and patient support specialist at PNI, is a cochlear implant recipient herself. As she was implanted as a child, she knows firsthand how helpful it can be. Her goal is to spread the word about cochlear implants through community outreach and education. Listen in and learn about how these innovative devices are changing lives.

These put more focus on reviewing performance through self-reflection and peer feedback described in the RACP's MyCPD Framework as a Category 2 activity. To help synchronise CPD seamlessly with practice, the value of supervision as a reflective activity has been recognised in the 2024 framework. But sometimes hearing aids aren't enough, or a patient's hearing deteriorates beyond the devices' capabilities. Then, it's time to consider cochlear implants—small electronic devices that go beyond hearing aids. They bypass damaged hair cells in the ear and send electronic signals straight to the auditory nerve. Dr. Rebecca Lewis is an audiologist who specializes in evaluating patients who might be eligible for cochlear implants. Until recently, doctors installed the devices only after people had lost almost all of their hearing. But because hearing is crucial to social connection, and social connection is key to staving off dementia, the bar for cochlear implants is coming down. Anyone who has tried a few sets of hearing aids and still can't understand conversation should consider cochlear implants, Dr. Lewis says. Rebecca qualified from Guy's Medical School and initially pursued a career in Anaesthetics gaining FRCA in 1996. She then changed specialty to General Practice (MRCGP 2000) having been interested in women's health and gained DRCOG in 1998. During her time in General Practice, she became interested in menopause and by how dramatically it could affect individuals. In today's podcast we try and understand the impact that racial bias makes on variation in clinical care. For example, racialized patterns in the use of analgesia were brought to light over 20 years ago but are still occurring today. In research from the UK published in March it was found that women of African or South Asian extraction were significantly less likely to receive an epidural during vaginal birth, or instrumental assistance with the delivery. The direct reasons for this variation were not revealed by the study, and could simply reflect the preferences of different cultural groups. But if that's the case, it behoves us to address gaps in health education in a culturally sensitive way. This podcast highlights the subtle drivers of racialized disparity at different layers of service delivery. At an individual level implicit bias can affect clinical decision-making. At an institutional level there may be known resourcing issues not being addressed, like availability of translators to help diverse patients understand what they're consenting to. And all this takes place within the context of structural racism, the inequity that was long ago baked into society's power structures. Rebecca Lewis may look like a very unassuming Physical Therapist, but to me, she's a sustainability super star. She's trying sustainable actions, one by one, to see how she can be more mindful of her footprint on the earth, and responsible for what she uses and tends to. It started in 2021 soon after she moved into her house, and was diagnosed with Long Covid, and was bed bound for months; she needed something to feel productive. It started with a loaf of bread, and then yogurt, and knitting, etc. Rebecca teaches us that some changes can be  small or simple (cloth napkins!), and make a big impact. She also talks about courage to try new things. She also holds the position of Adjunct Professor at Mount Saint Mary University / John Tracey Deaf and Hard of Hearing program. She has provided rehabilitative services, including hearing aids, hearing protection, Osseo integrates devices, auditory brainstem implant, and cochlear implant (CI) services to adults and children. She serves on the California Newborn Hearing Screening Learning Committee to improve outcomes for children with hearing loss across the state. She is also a State Champion for the American Cochlear Implant Alliance, aiming to improve access to CIs nationwide. She is dedicated to serving the community using a whole patient approach with evidence-based and compassionate care. Before joining PNI, Dr. Lewis held a range of positions at the USC Caruso Family Center for Childhood Communication, UCLA Medical Center, The Learning Center School for the Deaf, and the world-renowned House Clinic. She developed a hearing wellness program for the USC Thornton School of Music and has published in the field of cochlear implantation. Dr. Lewis received her Doctorate in Audiology from Vanderbilt University. She completed externships at the Greater Los Angeles Veterans Administration and at the Care Center at the House Ear Institute. Personally, I walked away with so many points to think about it was difficult to narrow it down to just 3 takes.   TODD-TAKES To Rebecca's point on taking pride in your work, we as tech providers need to communicate better how technology actually enables field workers to regain pride in their work they feel may be threatened. It starts by taking the time to listen, engage and truly empathize. Then take the needed action to meet them where they are and make it easy to use. Training that resonates is a critical component as well. By making it more of a hands-on job simulation you increase the likelihood of retention. Take time to explain the why behind the what you are doing. Intentionality will always rule the day with everything from a new tool to leveraging BIM. Get everyone on the same page from the start. Construction is a relationship business. Do not discount any relationship along your path. The simple fact is that you have to get along with people in order to get any results.

And this week also marks national Apple Day, Thursday the 21st of October is a very important day for celebrating orchards across the UK and to raise awareness of the environmental impact just to remind people of the importance of orchard fields, and nature to humanity in general. Hey there, my wise friends and welcome to Episode 34 of The Collective Wisdom Podcast. I've got a really special guest for you today. And this one's quite personal because Dr. Rebecca Lewis is actually someone who has helped me to navigate through the menopause. She has very kindly agreed to come and share the wealth of knowledge and wisdom that she has about the menopause, and managing the huge array of symptoms that can be part of it. I count myself as being very lucky that a few years ago, when I was talking to my friend Cat, I was just describing this kind of flatness and apathy and lack of get up and go, that I just thought that was part of life. And she said, have we had the menopause conversation yet? It was so far off my radar. And then she sent me a link to Liz Earle's Podcast, where Liz was interviewing Dr. Louise Newson, who was a GP at the time working in a clinic in Birmingham and specialising in the menopause. In the interview, she described the range of not only physical, but also mental symptoms that can be experienced by women as their oestrogen levels decrease. And it was the first time that I'd even considered that the way I was feeling might be hormonal rather than just the way life is. By bringing this conversation into the open, Louise and Rebecca and the team have ensured that as many women as possible will get the help that they need. In this week's episode, Dr Louise Newson chats to her good friend, Dr Rebecca Lewis, who is also a clinical director at Newson Health Menopause and Wellbeing Centre. Together they discuss surgical menopause. Surgical menopause can often be very different to a natural menopause because it happens very suddenly. Women who have their ovaries removed during an operation are plunged into the menopause straight away. In addition, women who have a hysterectomy without their ovaries being removed still have a higher risk of an early menopause. Dr Newson and Dr Lewis discuss the most effective ways of treating surgical menopause. Dr Lewis' Three Take Home Tips: Think about menopause before your surgery - be aware of the symptoms you may experience. Discuss HRT with your surgeon before surgery. Have a solution and treatment plan in place before the operation.

Symptoms of depression commonly occur during the perimenopause/menopause. Here, Dr Newson is joined by Dr Rebecca Lewis and together they discuss why symptoms can occur & ways of managing them with the right treatment. For many menopausal women, these symptoms are incorrectly diagnosed as depression; research shows around 70% of perimenopausal/menopausal women are inappropriately offered/given antidepressants. There is no evidence that antidepressants are beneficial for the low mood associated with changing hormone levels. Dr Rebecca Lewis' Thee Take Home Tips about Menopause and Depression: Low mood is a very common symptom during the menopause - you are not alone! There is a great treatment available and the right type and dose of oestrogen can really help these symptoms.
