Our interview today is with Dr. Vyvyane Loh, a board-certified physician in obesity and internal medicine. She is the founder and leader of Transform Alliance for Health, a Boston preventive-care practice that  specializes in weight management and the treatment of chronic metabolic diseases such as diabetes, hypertension and dyslipidemia. She and her staff are known for helping people lose 50 pounds or more and getting their type-2 diabetic patients off their many medications. Vyvyane has spent her medical career developing expertise in immunology, metabolic syndrome, fat metabolism, clinical nutrition, and preventive medicine. In today's interview, we discuss how abdominal, or visceral, fat is linked to a wide range of metabolic disorders. Vyvyane goes on to explain how there's a clearcut association between obesity and decreased brain volume that rarely gets discussed. When her overweight patients complain about their behavior around food and how they consistently give in to snacks that patients know are bad for them, Vyvyane explains how the challenges they are facing is often a result of the brain struggling with decreased blood flow and the shrinkage of neurons. Vyvyane also shares how a patient asked Vyvyane if she knew anything about the Atkins diet, and although she didn't, Vyvyane ended up doing the diet along with her patient. This led Vyvyane to start seriously researching whether a ketogenic diet could help people not only lose weight, but also reverse chronic disease. Toward the end of today's interview, we explore Vyvyane's interest in macrophages, which are specialized cells involved in the detection and destruction of bacteria and other harmful organisms. We also have a nice discussion about how Vyvyane took some time off from practicing medicine to enroll in the writing program at Warren Wilson College in North Carolina in 1999. She spent the next two years writing a novel, “Breaking the Tongue.”  Set in Singapore during World War II, her book was nominated for the prestigious International IMPAC Award in fiction and was selected by the New York Public Library as one of its top 25 books of 2004. In this video Dr. Kirkland talks about the future of aging and how the biology of aging affects everyone and should not be thought of as only relevant to older people. He also discusses the path of bringing senolytics to wider use. Dr. James Kirkland is Director of the Robert and Arlene Kogod Center on Aging at Mayo Clinic and Noaber Foundation Professor of Aging Research. Dr. Kirkland's research focuses on cellular senescence, age-related adipose tissue and metabolic dysfunction, and development of agents and strategies for targeting fundamental aging mechanisms to treat age-related chronic diseases and disabilities and to extend healthspan. He published the first article about drugs that clear senescent cells, senolytic agents. A novel, mechanism-based, hypothesis-driven drug development paradigm was used to discover senolytic drugs. Based on the observation that senescent cells release factors that cause apoptosis of the cells around them, yet are themselves resistant to apoptosis, Dr. Kirkland hypothesized that senescent cells utilize senescent cell anti-apoptotic pathways (SCAPs) for protection from their own senescence-associated secretory phenotype (SASP). Using bioinformatics analyses of senescent vs. non-senescent cells and RNA interference, Dr. Kirkland identified these SCAPs and verified their importance for senescent cell survival. Dr. Kirkland used bioinformatics approaches to identify agents that target key nodes across the SCAP network and demonstrated these drugs are senolytic in rodent and human cultured cells and mice in vivo. These senolytic drugs include Dasatinib (D), Quercetin (Q), Fisetin, Navitoclax, and related compounds. Dr. Kirkland showed these agents delay, prevent, or alleviate multiple disorders in mouse models of human chronic diseases and aging phenotypes. Conditions alleviated in mouse models include frailty, diabetes, hepatic steatosis, cirrhosis, renal dysfunction, neuropsychiatric disorders, dementias, pulmonary fibrosis, osteoporosis, osteoarthritis, retinal degeneration, diastolic dysfunction, cardiac ischemia, vascular hyporeactivity, infertility, and skin disorders, among others. He demonstrated that intermittent, orally administered senolytics reduce senescent cell abundance in adipose tissue and blood markers of senescent cell burden in blood of patients with diabetic kidney disease. He and collaborators found that a brief course of senolytics enhances physical function and reduces frailty in patients with idiopathic pulmonary fibrosis, a fatal, cellular-senescence-driven disease for which available treatments have been unsatisfactory. Multiple clinical trials are currently underway of the senolytics that Dr. Kirkland discovered. He is a scientific advisory board member for several companies and academic organizations. In addition to being President-Elect of AFAR, he has been a member of the National Advisory Council on Aging of the National Institutes of Health, and past chair of the Biological Sciences Section of the Gerontological Society of America. He holds honorary appointments at Boston University and the University of Groningen in the Netherlands. In this video Dr. Kirkland talks about how and why senescent cells are formed, how they can be removed and what the benefits of removing them is. Dr. James Kirkland is Director of the Robert and Arlene Kogod Center on Aging at Mayo Clinic and Noaber Foundation Professor of Aging Research. Dr. Kirkland's research focuses on cellular senescence, age-related adipose tissue and metabolic dysfunction, and development of agents and strategies for targeting fundamental aging mechanisms to treat age-related chronic diseases and disabilities and to extend healthspan. He published the first article about drugs that clear senescent cells, senolytic agents. A novel, mechanism-based, hypothesis-driven drug development paradigm was used to discover senolytic drugs. Based on the observation that senescent cells release factors that cause apoptosis of the cells around them, yet are themselves resistant to apoptosis, Dr. Kirkland hypothesized that senescent cells utilize senescent cell anti-apoptotic pathways (SCAPs) for protection from their own senescence-associated secretory phenotype (SASP). Using bioinformatics analyses of senescent vs. non-senescent cells and RNA interference, Dr. Kirkland identified these SCAPs and verified their importance for senescent cell survival. Dr. Kirkland used bioinformatics approaches to identify agents that target key nodes across the SCAP network and demonstrated these drugs are senolytic in rodent and human cultured cells and mice in vivo. These senolytic drugs include Dasatinib (D), Quercetin (Q), Fisetin, Navitoclax, and related compounds. Dr. Kirkland showed these agents delay, prevent, or alleviate multiple disorders in mouse models of human chronic diseases and aging phenotypes. Conditions alleviated in mouse models include frailty, diabetes, hepatic steatosis, cirrhosis, renal dysfunction, neuropsychiatric disorders, dementias, pulmonary fibrosis, osteoporosis, osteoarthritis, retinal degeneration, diastolic dysfunction, cardiac ischemia, vascular hyporeactivity, infertility, and skin disorders, among others. He demonstrated that intermittent, orally administered senolytics reduce senescent cell abundance in adipose tissue and blood markers of senescent cell burden in blood of patients with diabetic kidney disease. He and collaborators found that a brief course of senolytics enhances physical function and reduces frailty in patients with idiopathic pulmonary fibrosis, a fatal, cellular-senescence-driven disease for which available treatments have been unsatisfactory. Multiple clinical trials are currently underway of the senolytics that Dr. Kirkland discovered. He is a scientific advisory board member for several companies and academic organizations. She is an American former boxer who held world titles in three different weight classes simultaneously. Wolfe is regarded by many within the sport as one of the greatest female boxers of all time. Wolfe early Life was very rough, she was molested as a preteen,. She was a dropout unable to read or write when her mom died. That same year, her father was killed. Her brother was also killed in an attempted robbery. Another is serving a life sentence. Furthermore, She served 18 months in Prison on a Drug Charge. After that, living a straight life didn't come easy. With two young daughters, Wolfe tried to provide in any way she could. The result was a year spent working construction during the day and being homeless through the night. Wolfe and her daughters lived under a boat in a desolate parking lot. She also was a personal trainer and coach of several professional boxers, including former light middleweight contender James Kirkland. She appears in the 2017 film Wonder Woman in the supporting role of the Amazon Artemis.

In today’s episode, we talk about zombie cells, a term used to describe senescent cells because of their refusal to die. Our guest on this topic is Dr. James Kirkland, a geriatrics specialist and researcher at the Mayo Clinic in Rochester, Minnesota, who is known for his research into the role that senescence and senescent cells have on age-related dysfunction and chronic disease. As senescent cells build up in the body, they promote cellular aging and a host of chronic conditions related to aging, such as dementia, cancer, atherosclerosis, diabetes and arthritis. In today’s interview, we focus on Jim’s 2015 study where he and his colleagues at Mayo were the first to report on the potential of senolytic drugs to selectively kill zombie senescent cells. Jim’s paper in Aging Cell has been hailed as a major breakthrough in aging research. Jim is the director of the Robert and Arlene Kogod Center of Aging at Mayo and the president of the American Federation for Aging Research. The goal of James’ lab and research is to develop methods to remove senescent cells to delay, prevent, alleviate or partially reverse age-related chronic diseases.  Jim and his colleagues believe that doing this will help extend people’s health span, and, more important, prolong the period of life where people can live free of disabilities caused by chronic disease.
