懶人包部落格版整理 https://linshibi.com/?p=40124 隨著台灣疫情邁向高峰,要如何有效減少中重症,減少死亡是重中之重。多數人,特別是打滿三劑疫苗者,得病絕大多數都是輕症或無症狀。台灣到5月8日之前確診的24萬人中,有99.75%是輕症或是無症狀,也不需要接受抗病毒藥物的治療。但其中有些人有轉為重症的高風險因子,最重要的就是年齡(大於65歲)或是糖尿病癌症心血管疾病慢性肺病等等。這個適應症是這些藥物在臨床試驗中做出來的。美國,全世界都是這樣給藥的。需要在發病後五天內還是輕症的時候給藥,能有效預防後續進展到後續的住院或死亡。 台灣從5日開始已經不是住院才給藥。這幾天聽羅副報告就知道,藥物已經分派到地方各藥局,在居家照護的朋友也有機會可以拿到新冠口服抗病毒藥。當然可能還不夠,我們要盡量這些有重症高風險因子的染疫者,都能在有症狀的前五天內拿到抗病毒藥。這是決戰關鍵。 但這牽涉到一個比較麻煩的問題,台灣目前還是要經過PCR才能算確診。但以現在病患這麼多,先做快篩,快篩陽再去做PCR,陽性確診後進入居家照護,這個流程本身可能就3~4天了。如何能在五天內讓病患拿到藥物? 我想在盛行率高的地方或族群,有症狀加快篩陽者可以直接視為確診,應該是接下來趨勢。沒有力氣每個人都去做PCR了,也沒有此必要。有重症高風險者要在五天內確診給藥,這是可以有效降低後續住院和死亡的重點。如果因為要做PCR確認增加確診的時間,接下來問題會非常大。這問題一定要有方法解決。這不是把PCR操到一天做22萬就能解決的事情,真的做這麼多時可能還更惡化。當你PCR做到22萬時,請問結果出來的時間會不會塞車?這都要考慮。 本篇會提到的抗病毒藥物包括: 莫納皮拉韋(Monupiravir) Nirmatrelvir + ritonavir (Paxlovid) 瑞德西韋(remdesivir) 之前的podcast 2021.9.28 新冠口服藥物即將問世?是否能補上防疫的最後一塊拼圖? 2021.10.1 Merck的新冠口服藥molnupiravir可以減少50%的住院或死亡 會盡快送EUA 2021.12.23 美國FDA給予輝瑞和默克新冠口服抗病毒藥物緊急使用授權 2022.1.24 默沙東口服藥莫納皮拉韋抵台 輝瑞也將到位 兩萬五千份療程夠嗎? The case fatality rate of 2019-nCoV (aka Coronavirus, COVID-19) is still uncertain, with estimates floating around ranging from 0.16%-5.7%, higher among the elderly and people with preexisting conditions, and lower among everyone else. However, the death rate doesn't capture all of the harms from infection. There is also time lost during the infection and recovery, there is the possibility of accelerated aging, and there is the possibility of long-term nonfatal disability, such as chronic fatigue. Since 2019-nCoV has only existed for about two months, there is no data on the long-term outcomes of its survivors. However, the rate of lasting disability among survivors is important for deciding what responses are appropriate. I'm particularly interested in estimating the risk of chronic fatigue from nCoV infection. If that risk is high, this would greatly increase the importance of avoiding it personally and of suppressing it in communities of people doing important work, and would also greatly increase the expected economic impact. As a starting point, I chose a similar but more severe virus, SARS, which was successfully contained in 2003. Out of 208 Canadian survivors of SARS, 22 (10%) appear in this study of subjects "who remained unable to return to their former occupation" with "clinical similarities to patients with fibromyalgia syndrome". This implies a high lower bound on the rate of disability among SARS survivors. However, this is only one virus, and may not be representative of severe respiratory illnesses. Good answers to this question would be: Papers estimating the rates of postviral fatigue from other viruses, especially respiratory viruses, viruses with severity comparable to 2019-nCoV, and among non-elderly patients Models of postviral fatigue and how they relate to 2019-nCoV Data on whether and how much lung damage from non-viral sources causes chronic fatigue Early data on 2019-nCoV which bears on this question Any research help on this question is greatly appreciated, even if it provides only a bit of information about a small corner of the problem, or reports that a strategy for answering the question failed to pan out. This keeps coming back to me as Covid-19 spreads across our nation and we look at what other countries have had to do to control the spread. I am tempted to talk about science and epidomolgy but I'm not a public health scientist or a doctor - I'm a journalist by training - and that makes me a jack of all trades. I know enough to be scared. I'm concerned about what I'm seeing - a lack of organization or awareness. I feel anxious because I know I can only control so much. Here is what I am doing now: *Looking at only a select few, non-inflamatory, news sources. See Learn to Discern in Season 1 for more on that. *Come back to the present moment and be thankful for 3 things in that very moment. *Being grateful is the remedy for a scarcity mindset - nobody is making good decisions when they panic.*Trying to remember that other people's actions and behavior is a symptom of their own issues - and I honestly can't do anything to help them other than show leadership and courage in my own actions and behavior.This whole thing has come at a weird time for me. I love being socially and had actually taken a break over the winter months - and now this happens. I actually wrote a family statement to decline social events: That is so kind of you to ask! In order for a quarantine to be effective we must all practice it. This will allow our public health officials and medical doctors/nurses to take care of the upcoming surge of serious cases. Looking at what's happened in other countries (and the lack of organization and acknowledgement on the forefront here) many of us are carrying Corvid-19 and passing it on unknowingly. We've made a decision to keep to ourselves to allow the spread to slow down some.” Trying to fully understand what's happening is like taking a picture of a fast moving river and saying “this is the river.” The river is changing every second. Have grace and compassion for the people who work in public health as they try to work with private businesses, government (which has failed on a national level) and schools. Have grace and compassion with those who aren't taking this seriously. Have grace and compassion for yourself because most of us having never dealt with anything like this. I feel really called to say and remind everyone that all of us are strong and resilient people. We will be inconvenienced for months possibly. We may not have the certainity we want and crave. We will have to be mindful with our resources and grateful for everything we have. We can all show true leadership to our communities by preparing and taking steps now. If nothing else remember that you are strong and you can persevere. I don't mean in just recovering from this virus - I mean in all the things you may experience in the coming months. Be comfortable being uncomfortable. You are here because your ancestors faced and overcame adversity. COVID-19 has taken the world by storm and it's all anyone seems to be talking about. Today, I want to give you 5 tips to staying "sane" during this stressful time and how to survive the mom guilt and anxiety that comes along with it all. COVID-19 is responsible for thousands of deaths in China where the virus originated, and around the world, with the elderly and those with underlying health issues mostly at risk. As the pandemic continues to spread, with growing numbers in the U.S., more and more employers are telling their domestic workers to stay home. On Friday, President Trump declared a national emergency over the coronavirus outbreak. Now, it's important to note that a heck of a lot of people that have been diagnosed with coronavirus have made a full recovery.
