And the last Antonov An-22 “Antei” heavy military transport, breaking apart and falling out of the sky is just such a headline grabbing event. The An-22 heavy military transport is a Soviet-era aircraft, incredibly rare – in fact this was the last of its type that was considered airworthy. It broke apart mid-flight and crashed in Russia's Ivanovo region. Welcome back to the tasty morsels of critical care podcast. Today we’re going to have a quick overview of the oesophageal balloon. If you’re directed to a patient in your long case who has an oesophageal balloon in, then you’re probably having a bad day. It would seem very unfair to have too many questions on this but an awareness of their existence and some cliff notes on their basic use might come in handy especially if you’re doing well and you’re in the medal type territory of the exam. Exams aside they’re a useful gateway drug into some important respiratory mechanics that are relevant to all of us. At their most basic these are fancy NG tubes with an inflatable balloon that should end up in the lower third of the oesophagus. Inflating the balloon with a small amount of air allows you to transduce the pressure at the area the balloon lies. While that sounds straightforward there are large sections of review papers dedicated to troubleshooting placement and means of assuring the number you generate is actually accurate. I refer you to the below references for further reading. The pressure measured is called the oesophageal pressure, often abbreviated to Pes because it seems the Americans won the spelling war on that one. Oesophageal pressure is a reasonable surrogate (with assumptions of course) for pressure within the pleural space. Once we have an estimate of pleural pressure we can subtract that from the plateau pressure displayed on the vent and we end up with a fancy number called the transpulmonary pressure. The transpulmonary pressure or Ptp is the distending pressure applied to the lung either from the muscles of spontaneous ventilation or from positive pressure ventilation from the ventilator. Whoopdy do says the examiner – you now have another number you don’t really know what to do with. What should we use this data for, the examiner is asking? Well a short list of useful aspects you can look at with the oesophageal balloon include compensating for the effect of the chest wall on respiratory mechanics appropriate titration of PEEP assessing the contribution of respiratory muscle use to potential lung injury assessing triggering and synchrony issues At this stage you’d be hoping the examiner is satiated and you can move on to something else but in the unlikely and terrifying event that they ask for more detail you might want to mention some of the following. Our typical approach to safe ventilation in the passively ventilated patient is to look at driving pressures and tidal volumes. But this takes no account for the contribution of the chest wall. In the very obese patient there is a lot of flesh pressing down on the chest wall, this leads to an increasingly positive pleural pressure. It would make sense that we would need more pressure to distend the lungs in this scenario. The balloon in this scenario will allow you to set your PEEP appropriately. The Ptp at end expiration needs to sit somewhere in the 0-10cmH20 range to avoid derecruitment and in end inspiration it needs to be less than 25cmH20. This may need a lot more PEEP or less driving pressure than you’re used to giving and the balloon can help you feel safe about doing that. In the patient weaning from the ventilator in a spontaneous mode the oesohpageal balloon can be used to make an estimate of the contribution of the patients muscular effort to the transpulmonary pressure. Your patient may be on 10/5 on a pressure support mode and you may well be lulled into a false sense of security that because the pressure numbers on the vent are modest then the pressures being exerted across the lung are also modest. What we are not measuring in this scenario is the distending pressure being applied to the lungs by the respiratory muscles, the Pmus. The balloon in this scenario can give an estimate of this as it reflects the negative pleural pressure generated by the patients inspiratory efforts allowing us to come up with a Ptp number that takes Pmus into consideration. Sometimes this might encourage you to increase the support from the vent, sometimes this might encourage you to increase the sedation depending on the context. So given all the wonderful things the balloon can do for us why are we not doing it on everyone? A list of reasons not to use oesophagaeal balloons might include cost – these fancy NG tubes are pricier than you would think compatible software on the ventilators. These frequently don’t come as standard appropriate placement. These are tricky to get right and knowing that the number generated is valid is not entirely straightforward. Lots of assumptions are made the Pes number reflects pleural pressure only at a single location and does not take account of heterogeneity. the evidence base is unclear if this adds anything over doing something like simply following the high PEEP table from ARDSnet. Interestingly several research groups (thinking the folk from Toronto or Luigi Camporata in london)  have used balloons to identify surrogate ways of measuring recruitment or estimating Pmus that we can easily measure on a standard ventilator set up. This may well be a way of bringing the important concepts of transpulmonary pressure to the bedside. Reading: The Toronto Mechanical Vent Course was an excellent intro for resp mechanics for me. Rok 2025 bol rokom ďalšej vládnej koalície. Bol to rok transakčnej dane, zvýšenej dépéháčky a slabého ekonomického rastu s dlžobami takmer 80 miliárd eur. A rok, keď chudobe a sociálnemu vylúčeniu čelilo na Slovensku takmer milión ľudí. Bol to aj rok protestov a rok kedy vládne strany strácali svojich voličov a opozičné zas v prieskumoch silneli.Bol to aj rok, v ktorom sa Robert Fico stretával s Putinom, rečnil o nutnosti zmeny politického systému na Slovensku a Európskej únii odkazoval, že ak má skapať, tak nech skape. Popritom stihla vláda spolu s KDH a Hnutie Slovensko dostať do našej ústave dve pohlavia a národnú identitu. Nie všetko sa vláde podarilo. Na konci roka jej ústavný súd zrušil novelu o mimovládkach a k tomu pozastavil účinnosť zákona, ktorým bleskovo rušila Úrad na ochranu oznamovateľov.Čo nám rok 2025 dal a čo nám zobral? Európska Únia poskytne Ukrajine pôžičku vo výške 90 miliárd eur na najbližšie dva roky. Dohodla sa na tom dvadsaťsedmička po pätnásťhodinovom rokovaní bruselského samitu. Slovensko, Maďarsko a Česká republika sa však nebudú podieľať na jej garanciách.Miliardy Ukrajine si pritom Únia požičia na trhoch v rámci svojho dlhového programu. Na avizovanom použití zmrazených ruských aktív vo výške 210 miliárd na tzv. reparačnú pôžičku ešte musí pracovať Európska komisia. Európa požičala peniaze Ukrajine – teda okrem Maďarska, Česka a Slovenska. Tam, kde podľa Fica „nie je nič“, je Ústavný súd, ktorý mu pije krv. Proti novele trestného zákona sa postavili prokurátori.

Jozef Golonka, jedna z najväčších legiend slovenského hokeja, sa v podcaste Góly z bufetu s Tomášom Prokopom a Marekom Marušiakom rozhovoril o neuveriteľných príbehoch zo svojej kariéry. Od ponúk z NHL cez stretnutia so svetovými lídrami až po takmer smrteľný úraz – rozhovor je plný momentov, o ktorých málokto vie.Jozef Golonka na úvod odhalil svoj vzťah k Nitre, kde strávil detstvo počas vojny a kde neskôr pomáhal budovať hokej. Spomína, ako s krompáčom v ruke stál pri stavbe prvej ľadovej plochy a ako neskôr vybojoval v Prahe 365 miliónov korún na postavenie krytej haly vo Zvolene. „Za to som dostal župan od Zvolena. To je pre mňa väčšia pocta ako niektoré iné veci,” povedal dojato.Najšokujúcejšie sú jeho spomienky na rok 1960, keď dostal ponuku z Toronta Maple Leafs. „Bol som prvý Európan, ktorý sa mal dostať do NHL, keď bolo len 6 klubov,” vysvetlil s tým, že v hokejovej Sieni slávy v Toronte má dodnes dres s číslom 19. Režim ho však nepustil a táto situácia sa opakovala každý rok. Jozef Golonka vtipne spomínal na kanadských „skautov”, ktorí mu dávali podpisovať prázdne papiere len preto, aby mali zaplatený výlet do Československa.Legendárny útočník otvorene hovoril aj o najtemnejšom momente kariéry – v roku 1962 sa po zákroku švédskeho hráča ocitol na pokraji smrti. „Mal som 34 kg, všetci ma odpísali,” spomína na 3,5 mesiaca v nemocnici, kde ho liečili na tuberkulózu, hoci išlo o zápal pohrudnice. Zachránilo ho až radikálne rozhodnutie lekárov. O rok neskôr už hral na olympiáde v Innsbrucku a Rusom dal 2 góly.Zaujímavé sú aj príbehy z medzinárodnej scény. Jozef Golonka rozprával, ako potriasol rukou anglickej kráľovnej, švédskeho kráľa aj prezidenta Richarda Nixona, ktorého musel opraviť, že nie je Slovinec, ale Slovák. „Tieto zážitky mi nikto nezoberie. Ľudia ponúkajú peniaze za tieto memorabílie, ale ja ich nepredám,” skonštatoval s hrdosťou.Kriticky sa vyjadril k aktuálnej situácii v slovenskom športe: „Protekcionizmus v športe neexistuje – buď vyhráš, alebo prehráš. A u nás sme sa naučili niečo iné.” Mrzí ho, že slovenské médiá nedostatočne pripomínajú velikánov hokeja a návštevnosť na zápasoch klesá.Jozef Golonka odhalil aj príbeh o neuznanom góle v Štokholme, ktorý mu neuznal legendárny rozhodca Josef Kompalla. „Ak by mi ten gól uznal, mal by som zlatú medailu,” povedal o stretnutí s Josefom Kompallom po rokoch v Garmisch-Partenkirchene.Dojemné sú spomienky na spoluhráčov – Jardu Holíka, Gusta Havla či Stana Mikitu, ktorý ho priamo na ľade prehovával, aby utiekol do Kanady. „Hovoril mi: ‚Budeme hrať spolu, budeme najlepší.' Ale ja som mal len pás na pančuchy a niečo k tomu,” spomína so smiechom na ich tajné stretnutie za zatvorenými dverami.„Žiletka”, ako znie prezývka Jozefa Golonka, chystá aj štvrtú knihu, v ktorej by chcel napísať veci, o ktorých doposiaľ mlčal. „Teraz už môžem povedať to, čo som predtým nemohol,” avizuje ďalšie šokujúce odhalenia.

Neustála polarizácia, šírenie lží a demagógie a živenie strachu - dokonca i voči niečomu takému, ako je veda či progres. No a do toho postupný, no čoraz viditeľnejší úpadok štátu a všetkého toho, čo tento štát môže a aj má pre občanov robiť.Havarujúce vlaky, padajúce mosty, nedostavané diaľnice, charitatívne zbierky na život zachraňujúce lieky, zhoršujúce sa výsledky školákov, čoraz masovejší útek mladých mozgov do zahraničia, zdravotnícke služby za stále viac narastajúce poplatky, rozklad ochrany prírody i kultúrnej sféry.No a do toho všetkého čoraz vyššie dane, odvody a tri mnohomiliardové vládne konsolidácie bez prakticky akéhokoľvek zmysluplného výsledku ako i prepad našej krajiny prakticky vo všetkých merateľných parametroch na samých chvost Európskej únie.I tak vyzerá obraz Slovenska na sklonku roka 2025. Štátu postavenom na parlamentnej demokracií. Je však toto ešte demokracia alebo sa to už skôr blíži k tomu, čomu sa hovorí ochlokracia či dokonca kakistokracia, teda vláda toho najhoršieho? Vládne tu ešte rozum alebo nás už opanovali vášne, klanová mentalita a bezohľadné sebectvo na úkor spoločenstva, v ktorom žijeme?Aký príbeh si teda dnes Slovensko vlastne píše? Vieme si to vôbec zadefinovať a chceme aby tento malý kúsok zeme, ktorý voláme našim domovom, mal aj nejakú vyššiu ideu a zmysel?Sedliak je od slova sedieť, teda je to o tom, vysedieť problém, nevšímať si ho, prikrčiť sa a azda ho i nejako prežiť. Myšlienka stavať celé domčeky sa objavila po rozprávke o Jankovi a Marienke, ktorej perníková chalúpka inšpirovala nemeckých pekárov vytvárať jedlé domy.Keď sa tradícia rozšírila po Európe a neskôr do Ameriky, každá krajina pridala vlastný štýl. Na Slovensku sa medovníky zdobili jemnou bielou polevou pripomínajúcou čipku. Hoci medovníkové domčeky nepatrili medzi pôvodné slovenské zvyky, postupne sa stali súčasťou adventu, často zdobené folklórnymi vzormi a jednoduchým, milým dizajnom typickým pre miestne remeslo.Dnes sú medovníkové domčeky spojením rozprávkovej fantázie, sladkej vône a rodinnej zábavy. 2025-12-16 | UPDATES #080 | A post on X caught my eye, the gist of which was “Only a Russian official can invade and bomb you while simultaneously calling themselves a peacemaker and the victim a warmonger.” The author is referring to Kiril Dmitriev, one of the key so-called ‘negotiators' from the Russian side, and it highlights the absurdity of trying to build a peace process with the same regime that initiated the aggression and committed untold war crimes in Ukraine. Any such process pre-supposes that the aggressor has seen the error of their ways, in some sense recanted for their mistakes, even if not admitting culpability for the crimes themselves or any contrition. But the reality is so far from this. Russia seems to want a temporary “ceasefire,” because it is exhausted, and needs time to rearm and internalise the lessons of failure – the failure of its military strategy, that is, so it can come back again with greater lethality and success. Hol a határ a köszönet és a kényszer között, és miért néznek rád furcsán, ha nem nyomsz rá automatikusan a 25–30%-ra.   Hosszabban beszélünk a New York-i munkahelyi kultúráról, az „everyone is amazing" világáról, ahol mindenki kedves, mosolyog, majd egyik napról a másikra eltűnik. Összehasonlítjuk ezt a magyar mentalitással, őszinteséggel, direkt kommunikációval.   Külön blokkot kapnak a lakások, amiket látni kell, hogy elhidd. Konyhában zuhany, hálóban hűtő, kreatívan értelmezett szobák, és horror alaprajzok. Olyan történetek, amiken a magyar hallgatók egyszerre nevetnek és szörnyülködnek.   Beszélünk a biztonságérzetről New Yorkban, a metróról, az utcai jelenetekről, az ICE raid-ekről és arról, hogy mi az, amit kívülről túlreagálnak, és mi az, amit nem vesznek elég komolyan. Nem hírekből, hanem saját tapasztalatból.   Fontos téma a side hustle kultúra is. New Yorkban szinte mindenkinek van második, harmadik élete. Zenész, Uber-sofőr, DJ, csomagkihordó, alkalmi munkák. Szóba kerül, hogy ez szabadság vagy kényszer, és kinek való ez az életforma.   Nagyon emberi rész a barátságok New York módra. Gyors kapcsolódások, még gyorsabb eltűnések. Miért működik ez így egy ekkora városban, és mi hiányzik ebből egy magyar szemének.   Az epizódban kitérünk a zenei életre, zenekarokra, stúdiózásra, jammelésre, arra, hogy miért nem lehet levetkőzni azt, ami igazán belülről hajt. Arról is beszélünk, hogy mikor jön rá az ember, akár 50–60 évesen, hogy mit nem kellett volna abbahagynia.   Szóba kerül a foci és a világbajnokság, a New York-i páholyárak abszurditása, a sport üzleti oldala, és az is, hogyan próbálják az amerikaiak „amerikaisítani" a focit. Innen kanyarodunk vissza a magyar kötődéshez, a magyar tévécsatornákhoz, a honvágyhoz, és ahhoz, mi marad meg az emberben itthonról.   Az epizód egyik fontos gondolata a hiteles információ hiánya. Miért lenne fontos, hogy a külföldre költöző magyarok valós, használható tudást kapjanak bankról, munkáról, lakhatásról, nem legendákat és Facebook-kommenteket.   Ez a beszélgetés nem ad kész válaszokat, de jó kérdéseket igen.
