Today's Case A 40-year-old female presents for follow up of hypertension that started 2 years ago. She has been seen frequently in your clinic for titration of her antihypertensive medication and treatment of recently diagnosed hyperlipidemia and type 2 diabetes mellitus. During her visit she reveals that she is frustrated by weight gain that has occurred predominately over the past year. She reports regular menstrual cycles until the past year, when she notes increased time in between cycles and occasional spotting instead of regular periods. She is concerned that she may have early menopause because she is having changes in appetite, difficulty sleeping, and mood swings. She feels like these symptoms are getting worse, and she is frustrated that despite her increase in exercise and change in diet she is more fatigued and has experienced no weight loss. Scott Pescatore, Executive VP of the Rare Diseases Business at Recordati, is focused on rare and ultra-orphan diseases with high unmet needs by acquiring promising compounds from other companies and advancing them through development and approval. An example is the company's drug Isturisa, acquired from Novartis, which is an FDA-approved effective treatment for patients with Cushing syndrome, a rare endocrine condition. Raising awareness of rare diseases among physicians, patients, and the general public is a priority for Recordati to improve diagnosis rates and clinical trial participation, and to encourage more research and funding in the rare disease space. Scott explains, "We have two primary divisions at Recordati. One is our specialty primary care business, and the other is the rare disease business, which I have the honor and privilege to look after. And we have a very simple sort of work ethic or business mantra, if you will, and that's focused on the few. And we really dedicate ourselves to focusing on disease areas and patient groups and therapeutic areas that have a high unmet need and really low or limited options for patients. And really focusing on diseases and areas that are rare and considered ultra-orphan by the definitions in the US, where really there's a very small patient base. And that's where we began back in 2007, when the rare disease business was formed. And that's really what our focus has been since then. And we continue to focus on this segment of the market." "So Isturisa is really a fantastic product. We acquired this product through a deal we did with Novartis Pharmaceuticals back in 2019, and this product has FDA approval for patients who have endogenous hypercortisolemia with Cushing syndrome. So it's quite a nasty disease, but it's a very efficacious product. The product is what's considered a cortisol inhibitor in the blocks in a particular enzyme to help normalize hypercortisolemia in patients with Cushing syndrome. And Cushing syndrome, for those who aren't familiar, is a rare endocrine condition that really has a significant impact on patients' quality of life, on the caregivers, on the families. And the indication I mentioned was supported by quite robust phase 3 trials." #Recordati #RareDisease #FocusedontheFew #CushingSyndrome #IMCD #CastlemansDisease recordati.com Download the transcript here

Dr. Sara Szal, a Harvard and MIT-trained physician, sits down to unpack why women bear the brunt of autoimmune conditions at rates that keep climbing. She introduces the three-legged stool concept that's required for autoimmune disease to take hold, then breaks down why 95% of her patients show cortisol dysregulation long before they're ever diagnosed with anything. What makes this conversation powerful is that Dr. Szal doesn't just study this academically, she's lived it, going through prophylactic mastectomies and converting her own positive anti-nuclear antibody back to normal through targeted interventions. This is a masterclass in understanding the drift from health to disease and exactly what you can do about it before the soldiers come for you. Frankenstein Created Woman (1967), Frankenstein Must Be Destroyed (1969), Frankenstein and the Monster from Hell (1974) Continuing our quest to get through the rest of Hammer's Frankenstein films which starred Peter Cushing, we cover the last three titles that Hammer made. Cushing loved to play this character, and he's done it like no one ever had, or ever has. Along with the screenwriters, Cushing made this character his own, and really made him the real monster in this series. While these films might be well covered by others, sometimes we forget just how good some of these are, as well as what we might be missing the first time or two that we watched it. And as we've mentioned many times before, watching them in sequence in a relatively short time, more things can come to light. Films mention in this episode: Brides of Dracula (1960), The Bride of Frankenstein (1935), Curse of Frankenstein (1957), Curse of the Werewolf (1961), The Evil of Frankenstein (1964), The Exorcist (1973), Frankenstein Created Woman (1967), Frankenstein and the Monster from Hell (1974), Frankenstein Must Be Destroyed (1969), Horror of Dracula (1958), Horror of Frankenstein (1970), Kiss of the Vampire (1966), The Mummy (1959), Night of the Living Dead (1968), Peeping Tom (1960), Psycho (1960), Revenge of Frankenstein (1958), Rosemary's Baby (1968)

Welcome to Monday Motivation!
In this KE Report daily editorial, we welcome back Dan Steffens, President of the Energy Prospectus Group, to break down the latest developments across the oil and natural gas markets. With crude prices recently dipping below $57/bbl and natural gas showing relative strength, Dan shares his insights on market fundamentals, storage levels, and the outlook for key producers and income plays. Key Discussion Points: Oil Market Outlook - Despite headlines of oversupply, U.S. crude and product inventories remain below seasonal averages, with Cushing storage levels near record lows. Dan believes strong support around $57 and possible SPR refilling could stabilize prices. Natural Gas Demand Drivers - U.S. LNG exports are expanding rapidly, projected to reach ~20 Bcf/day in Q1 2026. Rising AI data center power needs are emerging as a major new demand source, with multiple gas-fired plants being developed to support this growth. Top Natural Gas Producers - Companies positioned to benefit from rising demand include EQT Corp (EQT), Antero Resources (AR), and Range Resources (RRC). Balanced Oil-Gas Plays – For diversified exposure, Dan highlights Ovintiv (OVV), Devon Energy (DVN), and Coterra Energy (CTRA), each with substantial natural gas and NGL production alongside oil output. High-Yield Midstream & Income Ideas – The safest dividends lie in midstream operators such as Plains All American (PAA) (≈9% yield, tax-advantaged), Enbridge (ENB), and Oneok (OKE) - all benefiting from steady volume-based cash flows. Dan also notes Black Stone Minerals (BSM) and Kimbell Royalty Partners (KRP) as strong royalty-income opportunities. Hedging & Cash Flow Stability - Many of these firms are well-hedged, ensuring consistent dividends even amid commodity price swings. With gas futures near $4, Dan expects renewed investor rotation into gas-weighted producers through year-end. Today, we are exploring how our mindset can influence the decisions we make about challenges to our horses' health.Whether you are dealing with a common issue or a new diagnosis that feels overwhelming, it is essential to stay grounded in practical knowledge and clear reasoning rather than emotion because disease labels can sometimes influence our perspective and choices in ways that do not serve the best interests of the horse. What Disease Labels Really MeanDisease labels are clinical tools that help classify health problems in horses, based on their blood tests, symptoms, and behavior. They can provide a framework for understanding what is happening in the body and guide the appropriate care. Some diagnoses, such as insulin resistance, are straightforward, while others may not be quite as clear. The key is to know whether a diagnosis will guide your next step or limit your perspective. When Diagnosis is UsefulWhen progress has stalled or the symptoms fail to add up, a well-defined diagnosis will clarify what you are dealing with. Knowing the specific problem will allow you to tailor your support holistically, through diet, nutrition, stress management, and other natural approaches. The Power of Belief and FearDisease labels can shape an owner's mindset, often leading to fear-based decisions. When a horse receives a diagnosis, the label can carry emotional weight, narrowing all that the owner may believe is possible. That fear may prevent a deeper exploration of other options, which could influence the horse's energy and its recovery. The Laminitis ExampleBeliefs about laminitis have evolved drastically. Years ago, pads and shoes were considered the only option for comfort. Today, however, many horses recover with patience, proper hoof care, and the right nutrition. Unfortunately, outdated beliefs still cause many owners to give up on their horses prematurely, leading to unnecessary suffering or euthanasia. The Cushing's (PPID) ExamplePPID is another condition that evokes fear. While some horses may require lifelong medication, others can improve naturally through holistic management. High ACTH results do not always mean a permanently broken pituitary. Each case is unique, so it is essential to avoid assuming the worst before exploring all options. How Beliefs Shape OutcomesWhat an owner believes about their horse's condition will directly influence its healing. Negative expectations can impair recovery, while confidence and calm leadership encourage it. Horses mirror the emotions of their owners, so if you believe your horse can heal, you create the space for that healing to happen.Emotional LeadershipYour emotional state guides your horse. If you stay grounded, hopeful, and consistent, your horse will feel safer and more able to recover. That mindset will also protect you from burnout by replacing your anxiety and grief with trust.Managing Financial FearAvoid letting your financial fears drive your decisions before fully understanding what is truly needed. Financial pressure often amplifies the stress of a diagnosis, so owners may assume they will be liable for massive costs before exploring all options. Yet many horses recover with far less expense and intervention than expected. Old BeliefsMany beliefs about equine health stem from decades-old assumptions or limited studies. Always question the source of your information. Was it from a solid, well-designed study, or just anecdotal experience? Our modern understanding continues to evolve, and open-mindedness can make all the difference between despair and recovery.Health and Recovery Gray AreasEquine health often lies in the gray areas, and recovery could...

Reel Britannia - a very British podcast about very British movies...with just a hint of professionalism. Episode 181 - The Gorgon (1964) " Don't use long words, Inspector; they don't suit you." Get ready to have a monstrously good time with one of Hammer Film Productions' most creative and atmospheric chillers, 1964's The Gorgon! This isn't just any old creature feature; it's a vibrant, gothic mystery that brings together the titans of terror, Peter Cushing and Christopher Lee, for a truly legendary showdown. Forget what you know about Medusa, because this film introduces a new snake-haired sister to the mix, and she's ready for her close-up! The fun kicks off in the spooky little German village of Vandorf around the turn of the 20th century. This town has a serious problem: its residents have a nasty habit of turning to stone. When a young artist is found petrified, his family refuses to accept the flimsy official story. His determined father and brother arrive to uncover the truth, but the locals, terrified of a legendary curse, are tighter-lipped than a statue. This creates a wonderfully suspenseful puzzle, with our heroes piecing together clues while the village doctor (the ever-brilliant Peter Cushing) seems to know more than he's letting on. The mystery deepens with the arrival of the dashing Professor Karl Meister, played with commanding authority by the one and only Christopher Lee. He storms into town, ready to challenge superstition with science and solve the stony situation once and for all. The scenes between Cushing and Lee are electric, a fantastic duel between two masters of the genre who bring gravitas and excitement to every line. They elevate the film from a simple monster movie into a compelling human drama set against a wonderfully spooky backdrop. The film does a masterful job building the tension, giving you glimpses of slithering snakes and haunting reflections before the big reveal. When Megaera finally appears in her full, terrifying glory, it's a fantastic piece of classic horror magic. Her ghostly presence, combined with the eerie wail that precedes her, makes for some of the most memorable scenes in Hammer's history. The special effects, which show victims slowly hardening into stone, are delightfully retro and add to the film's unique charm. The Gorgon culminates in a thrilling and dramatic finale set in the ruins of Castle Borski. It's a fantastic climax filled with heroic sacrifice, shocking revelations, and a final, unforgettable confrontation with the creature. If you're looking for a horror film that is less about jump scares and more about rich atmosphere, brilliant performances, and a truly unique monster, then The Gorgon is an absolute must-see. It's a stylish, exciting, and wonderfully entertaining ride from start to finish! Mare reproductive challenges can range from endometritis to ovarian irregularities such as hemorrhagic anovulatory follicles, ovarian hematomas, and persistent corpora lutea, all of which can disrupt normal cycles and fertility. Older mares often face anatomical complications that increase contamination and impair conception. Hormonal tumors such as granulosa-theca cell tumors might spur erratic behaviors and cycle abnormalities, while pituitary pars intermedia dysfunction (PPID, formerly Cushing's disease) can further dampen reproductive performance by causing abnormal cycles, anovulation, and recurring endometritis. Vigilant diagnostics—including ultrasound, hormone panels, and uterine evaluation—are essential in identifying and managing these issues efficiently. During this Ask TheHorse Live Q&A two experts answer your questions about common mare reproductive problems. This episode is sponsored by Equithrive. About the Experts: Carleigh Fedorka, PhD, assistant professor of equine reproductive physiology at Colorado State University, in Fort Collins, is a global expert in reproductive immunology. She has focused her research on understanding the relationship between the immune system and the reproductive tract, evaluating the efficacy of treatments on various reproductive disorders, and discovering biomarkers for reproductive health. Fedorka earned her BS degree from St. Lawrence University, in Canton, New York, and her PhD in Veterinary Sciences from the University of Kentucky, in Lexington, after working in the industry as a manager of a commercial Thoroughbred breeding farm. She maintains a presence in the equine industry by retraining countless off the track Thoroughbreds, including her personal event horses Judge Johnny (JJ) and Strike Two (Jeter). In her spare time, she enjoys fly fishing, spending time with her husband and their dog, and cheering on the Buffalo Bills.Jenna Ward, DVM, Dipl. ACT, is a clinician and lecturer in large animal reproduction at the University of Pennsylvania's New Bolton Center, in Kennett Square. She earned her DVM at Texas A&M University, in College Station. Ward's research includes stallion reproductive health and genetic-related embryonic loss in horses.

In this episode, I explain cortisol and science-based protocols for properly setting your cortisol rhythm, which can significantly increase your daytime energy, focus, mood, and stress resilience, while also improving your sleep quality. Most people mistakenly think cortisol is bad, and many assume their levels are too high, when in fact many health and performance challenges simply stem from a disrupted cortisol rhythm. Getting your cortisol rhythm right can be transformative for your health and performance. I outline behavioral, nutritional, and supplement-based strategies to raise or lower your cortisol levels at the appropriate times of day and night. I also provide specific protocols for overcoming burnout. If you're dealing with stress, low energy, hormone or sleep challenges—or simply want to optimize these for the sake of your physical and mental health and performance—this episode offers science-backed protocols to help. Heavy crude supplies to the US Gulf Coast have tightened. Waterborne imports have dropped sharply, driven largely by declines from Mexico and Venezuela. Canadian wildfires have further limited access to heavy barrels, and refiners like Valero are starting to feel the squeeze. Why are Mexican and Venezuelan crude imports collapsing, and can they recover? Why aren't USGC refiners just turning to Canada via pipeline to replace lost barrels? What Canadian pipeline expansion plans are in the works to bring more crude to the USGC or overseas markets? Jeff Mower, director of Americas oil news, speaks with senior oil news editor Ashok Dutta and Canadian oil market reporter Alex Saucer to break down what's driving the tightness and where new supply might come from. Tucked deep within the brain and no larger than a pea, the pituitary gland is often called the body's “master gland.” It oversees the endocrine system—a network of glands and organs that produce hormones regulating everything from growth and metabolism to reproduction and mood. At Pacific Neuroscience Institute's Pituitary Disorders Center, endocrinologist Dr. Noa Tal specializes in identifying and treating hormone and endocrine imbalances. Patients may present with a wide range of symptoms—unexplained weight gain, irregular menstrual cycles, hot flashes, or mood changes—that often seem unrelated. In cases like Cushing's disease, a pituitary tumor triggers excess cortisol production, resulting in a puzzling array of symptoms. Sometimes the cause is even more elusive, such as side effects from medications. That's where careful clinical investigation comes in. Dr. Tal and her interdisciplinary team work closely to uncover the root of hormonal issues and guide patients toward not just better outcomes, but better quality of life. Listen in!
