I am thrilled to reconnect with Dr. Sanjay Bhojraj today. Dr. Bhojraj is a board-certified interventional cardiologist who became a pioneer in functional medicine. In our conversation, we dive into palpitations, which are a common complaint among perimenopausal and menopausal women. We explore red flag symptoms, the physiological effects of progesterone, estrogen, and testosterone as they relate to heart arrhythmias, EKG changes during the perimenopause-to-menopause transition, and wearable technologies. We unpack the differences between benign and more concerning arrhythmias, risk factors for atrial fibrillation, and the process of taking a thorough history, ordering the correct tests, and using imaging or sleep studies when appropriate. We cover treatment pathways, from lifestyle modifications to medications, channelopathies, and the genetic propensities for conditions such as Long QT, Brugada Syndrome, WPW (Wolff-Parkinson-White syndrome), and sudden cardiac death. We also highlight the importance of genetic testing for individuals with a family history of those conditions. Today's conversation with Dr. Sanjay Bhojraj is full of practical wisdom and clinical pearls, so you will most likely want to listen to it more than once. IN THIS EPISODE, YOU WILL LEARN: Why thyroid function should always be taken into account when assessing heart rhythm issues   How stress and life circumstances can trigger palpitations  The benefits of magnesium supplementation for supporting heart health What ventricular arrhythmias (from the bottom chambers) and atrial arrhythmias (from the top chambers) are commonly related to The value of monitoring for identifying the nature and severity of arrhythmias How sleep apnea can increase the risk of arrhythmia   The importance of exercise, stress management, and healthy lifestyle habits for supporting heart rhythm Why certain arrhythmias may require procedural interventions Why various types of athletic activity matter when evaluating arrhythmias How genetic factors can impact specialized heart assessments Connect with Cynthia Thurlow   Follow on X, Instagram & LinkedIn Check out Cynthia's website Submit your questions to support@cynthiathurlow.com Join other like-minded women in a supportive, nurturing community (The Midlife Pause/Cynthia Thurlow)  Cynthia's Menopause Gut Book is on presale now! With a deep commitment to improving health outcomes, Evelyn uses her personal and professional experiences to raise awareness about heart health, particularly for young adults. As a strong voice for change, she aims to inspire others to take proactive steps in managing their heart health and to advocate for better care and resources for those affected by heart disease.     Definition: A heart arrhythmia, also known as a cardiac arrhythmia, is an abnormal rhythm of the heart. It occurs when the electrical impulses that control the heart's contractions do not function properly.    Causes:    Heart arrhythmias can be caused by a wide range of factors, including:    Heart disease (e.g., coronary artery disease, heart failure) Electrolyte imbalances (e.g., low potassium or magnesium) Certain medications (e.g., stimulants, caffeine) Thyroid disorders Damage to the Vagus Nerve Stress Genetics    Types:    There are many different types of heart arrhythmias, which can be classified based on the rate and rhythm of the heart:    Tachycardia: A rapid heart rate (over 100 beats per minute)    Bradycardia: A slow heart rate (under 60 beats per minute)    Atrial fibrillation: A quivering or irregular rhythm of the upper chambers of the heart (atria)    Ventricular fibrillation: A life-threatening rhythm where the lower chambers of the heart (ventricles) contract irregularly and chaotically    Premature beats: Extra heartbeats that occur early in the rhythm    Symptoms:    Some people with heart arrhythmias may not experience any symptoms. Others may have symptoms such as:    Palpitations (feeling like the heart is racing or fluttering), Chest pain or discomfort, Dizziness or lightheadedness, Fainting, and Shortness of breath.    Diagnosis and Treatment:    To diagnose a heart arrhythmia, your doctor will likely perform a physical exam, ask about your medical history, and take an electrocardiogram (ECG). Treatment options depend on the type and severity of the arrhythmia and may include:    Medications (e.g., beta-blockers, antiarrhythmics) Lifestyle changes (e.g., exercise, stress management) Surgery (e.g., ablation procedure) Implantable devices (e.g., pacemakers, defibrillators)    Outlook: The outlook for people with heart arrhythmias varies depending on the underlying cause and severity of the condition.

But the palpitations were real as ever— and now— On a Saturday night in the Jamaica, Queens medical center emergency room, There they were again. Only this time I knew exactly why. ‘Too Bizzare' by Skrillex begins to play, via Complications 003- The Trauma Method. Irony. It was ironic, but still startling, Started with some nostalgic traumas, Every other time I saw an ER doctor (Why I don't go) Fuck, I just realized I have to airdrop myself 880 times. DEADMAU5 exits. … … After a few moments of comic tension, the Deadmau5 head rolls back in through the exit which he has taken. Ugh. Fuck this. No matter what you do, you're a superstar DJ. What. No! Yes. The answer is yes. NO. Fuck. What the FUCK. No matter what you do. You want to go, Go, you want to die, Die, you want to try, try You want to cry, cry Do what you want; As so will I, Demand is demand— Supply is supply. EDX So then, I followed this long hallway under the stage deck. Uh huh. And it led to a door— Uhhuh, where'd the door lead? To a portal. Woah. Pasqualle! You made it! I—yes. Congratulations! *blows party horn* *Daisies/ confetti* You're like 25! I'll be 25 forever. Nice! Yeah. I guess that's why it's called ‘music'—a musician without muses is just useless. ‘Well, whose next?' I wondered. All of my muses were not just so wonderful to me, but adored by many—and perhaps this is what allured me most—beffldled ans confounded me; once my mind was set on somehh th int, there was nothing else its eye could see—and for how long one God could only know, how deep the love would go and that the blood would run deep, and the scars to show for it, only upon my heart and never by soul—for a love was a love, and even once came and gone, to the end of my life I knew I would still ponder upon them, at one time or another, my muses—star studded lovers, rather than crossed, shiny and golden like all diamond and trophies so treasured and thought of as precious. ‘Yes, you are—precious.' Another tongue in cheek thought, for the other that I was, and also was not, as summer drew onward as short as it would come and go—a reminder to leave the apartment more often, and to mind my manners, to find the upper echelon wherever it was and come quietly into its doors, to open my world and wordform of thought, into a place where my heart always was; then, and only then, would I be home. Amongst the men and women of the uppercut and classy, luxurious big fishing ponds and flocks of doves upon olive branches—the peaceful world long parted from where mine was, by only the fault of my own. What had been done just certainly was, and yet, what was to come was an open poem, not of mine, but Godform in thought. ‘I wonder what's at the top of Rockefeller Plaza.' —perhaps, a gander at the bottom of an even larger entertainment complex. Then, again, only God would know what was beyond all that I wanted; a job—and not just any job at all— the one that I had always wanted. Mmm. Birthday cake. Suddenly the taste of a white confetti crème filled my mouth with a delicious remnince of what it might be like to taste a confectionary sugar again—but i couldn't imagine ever making it just on talent and charisma alone—no. Indeed, it seemed something had damaged my charm, and perhaps it was just the swarms and droves of phone controlled masses that saw me as nothing more than dust, I had started to surrender my desire to perform, and the quality of my music—along with my ability to make it, suffered with the awful thing that had been crowding my soul at all—whatever it was, evil and dark in nature, sure saw to it that it wanted to hurt me in all the ways that it could—and in all the ways it could not, I stayed away from most others, favoring my delisuins of love. ‘Nobody seems to understand that the pain they cause will only harm themselves.', I thought Younger souls, however, they were—and they would be kept in the pain that I was in one way or another until eventual death, far behind me on the infinite road to the source. Far enough behind, that it seeker to destroy my progress, and for all that it could, it also couldn't. The infliction of pain would simply not act as a measure for control any longer. Of into my own world, where I was at least free from the thoughts and judgement of others. She's the most beautiful girl, And I'm the most beautiful boy; So naturally, we belong together, don't we? Ablation or Meds for AFIB

This week we will discuss possible treatment methods for AFIB.   Atrial fibrillation (AFib) is a common type of irregular heart rhythm (arrhythmia) that occurs when the upper chambers of the heart (atria) beat chaotically and out of sync with the lower chambers (ventricles). This can lead to a variety of symptoms, including: Palpitations (a feeling of a racing or irregular heartbeat) Fatigue Shortness of breath Dizziness or lightheadedness Chest discomfort AFib can increase the risk of blood clots, stroke, heart failure, and other heart-related complications. Treatment Options for AFib Management of AFib focuses on controlling the heart rate and rhythm, preventing blood clots, and addressing underlying conditions contributing to the arrhythmia. The choice of treatment depends on the individual's symptoms, overall health, and risk factors. 1. Medications Medications are often the first line of treatment for AFib. These include: Rate-Control Medications Aim to slow the heart rate to a normal range. Common drugs: Beta-blockers (e.g., metoprolol), calcium channel blockers (e.g., diltiazem, verapamil), and digoxin. Rhythm-Control Medications Help restore and maintain a normal heart rhythm. Common drugs: Antiarrhythmics like amiodarone, flecainide, or sotalol. Anticoagulants (Blood Thinners) Reduce the risk of stroke by preventing blood clots. Examples: Warfarin, direct oral anticoagulants (DOACs) like apixaban (Eliquis) or rivaroxaban (Xarelto). 2. Ablation Therapy Ablation is a minimally invasive procedure aimed at correcting the electrical signals causing AFib. It is typically recommended for individuals who: Do not respond to or cannot tolerate medications. Have recurrent or persistent AFib that significantly impacts quality of life. Types of ablation: Catheter Ablation Uses thin tubes (catheters) inserted into blood vessels to deliver energy (radiofrequency or cryotherapy) to destroy small areas of heart tissue causing abnormal electrical signals. Surgical Ablation (Maze Procedure) Often performed during open-heart surgery for other conditions, creating scar tissue to block abnormal signals. Both options have high success rates, but catheter ablation is more commonly performed due to its minimally invasive nature. Choosing the Right Treatment Deciding between medications or ablation depends on factors such as: The severity and frequency of symptoms. The presence of other medical conditions. Patient preference and lifestyle. Consultation with a cardiologist or electrophysiologist is crucial to tailor treatment to the individual's needs.

Zack of Del Paxton joins me to discuss Del Paxton's history, their record "Auto Locator," and his love of Duplicate Keys Icaro, Note to Self, and Things I Like from Failed States. Listen to Del Paxton: https://delpaxton.bandcamp.com/album/auto-locator Follow Del Paxton: https://www.instagram.com/delpaxtonbuffalo/ DEL PAXTON BIO Buffalo's Del Paxton recorded their second full length Auto Locator straight to tape over three cloudy days in a solar powered barn, somewhere in New York's Finger Lakes. Named for the rewind mechanism on the reel to reel tape machine on which the band tracked the album, Auto Locator coils up the chemical trails of ribbon binding old places and head spaces to our consciousness. Static and hiss leach between analog track layers, blurring the normal and paranormal to remind us that no memory is totally separate from a fiction – just like no drum track is totally separate from bass. Over Auto Locator's ten songs, Del Paxton rip shit like a band entering their second decade of existence. On-the-nose opener “Freight Train Metaphor” throws listeners face first into the band's first album since 2017, with a syncopated pulse and arpeggiating guitar foreshadowing a record replete with punkish, serpentine composition. “Up With a Twist,” the album's lead track, deals in Del Paxton's crashing punk vibrance, through which the band explores and laments the uninspired architecture of American sprawl. Second single “Chart Reader”, whose origins trace back to a psychedelic dream inspired by Wayfaring Strangers: Cosmic American Music, sees the trio dabbling in emo Americana, even featuring a nod to John Denver's “Take Me Home, Country Roads.” Such unashamed references abound throughout Auto Locator, which takes further inspiration from bands like Mock Orange, Third Eye Blind, and Jimmy Eat World, all of whose influence is clear on “Palpitations” and third single “Spiritual Gymnastics.” Del Paxton's mix of nostalgic references aims to serve as their primary tool for exploring the contemporary, resulting in a post mortem of the American Empire. Whether intentional or otherwise, it's an appropriate result for the Upstate NY trio, whose self-awareness feels like a main component of Auto Locator, a feature as crucial to the band as the eponymous tool they used to record their music. text by Joe McGrath and William Osiecki

Roger Mairlot, a retired car mechanic in his 70s, takes center stage in this episode of Seeing Them Live. Renowned for his passionate dedication to live music, Roger's story has attracted the attention of media giants like Vice News, The Guardian, and CBC Radio Canada. With an astonishing personal record of attending over 725 consecutive gigs and accomplishing the feat of attending six gigs in one night at different venues across London, Roger has become an icon in the city's live music scene. His distinctive ornate military jacket, adorned with badges, has earned him the affectionate name "The Gig Slut" from Debbie Smith of the band Blindness.Reflecting on his early music experiences, Roger shares memories of a Joan Baez concert in the 1960s, marking the beginning of his journey through London's rich music scene. The cultural and social aspects of live performances are explored, emphasizing the intimate connection between the audience and the artists. Roger's passion for live music goes beyond attendance; he meticulously documents gig timings and schedules in "The Book of Bands," showcasing his dedication to discovering new bands and genres. The conversation delves into Roger's connection to the Mod Culture, influenced by bands like The Who and The Kinks, and explores his experiences in London's music scene during the 1970s. Notable bands from that era, including Canned Heat and Queen, are mentioned, while Roger reflects on the changing music landscape in the '70s.Roger's transition to retirement in the early 2000s allowed for greater flexibility in attending gigs, leading to a revival of his interest in live music. Charles and Roger discuss the impact of Mod Culture, Roger's favorite band, The Kinks, and his experiences seeing iconic artists like Jimi Hendrix. Roger's signature look, the guard's jacket, is born out of his paper rounds, which he took on to finance his gigging passion during his early waking hours. The episode delves into Roger's mad gigging phase, discussing his gigging routine, how he justified spending money on gigs through his paper round earnings, and the development of detailed schedules for nightly gig adventures. The conversation also touches on Roger's extensive guard's jacket collection and the experiences he had wearing them.In 2014, Roger pushed the limits by attending six gigs in one night, facing challenges in managing multiple venues and early start times. The episode explores Roger's criteria for choosing gigs, post-pandemic changes in the gig scene, and the decline in fan attendance. Despite the challenges, Roger emphasizes the importance of enjoying a gig and shares his diverse taste in music, spanning genres from jazz to chamber rock.BANDS: Adam Ant, Blindness, Blue Crime, Canned Heat, Color TV, Cream, Fleetwood Mac, Jimmy Hendrix, Joan Baez, Ogunquit, The Breeders, The Kinks, The Libertines, The Palpitations, The WhoVENUES: 100 Club, Camden (general reference to the area), Pure Groove, Rough Trade, Royal Festival Hall, Sculpture of St. James, The Clarendon Hotel, The Dublin Castle, The George Robey, The Hammersmith Odeon, The Hope and Anchor, The Kings Head, The Kursal, The Lock Tavern, The Marquee Club, The Monarch, The Old Blue Last, The Purple Turtle, The Roundhouse.More information including photos and links available at https://www.SeeingThemLive.com. Palpitations are not a pleasant sensation, woman experiencing them can actually feel their heart beating. For some, it is just the awareness of their heart at work, for others it feels like their heart is pounding, racing, or skipping a beat. Unfortunately, it's a common experience for many women in the menopausal transition. Research indicates that 42% of perimenopausal and 54% of post-menopausal women experience them. If with your palpitations you are also experiencing shortness of breath, chest pain, chest tightening, or dizziness it's best to head to emergency. If not, then book an appointment with your regular doctor. They can check your medications, as some medications can cause palpitations, and they can investigate for other conditions such as hyperthyroidism that can also produce palpitations. And rule out other cardiovascular issues such as arrhythmias. Once you are cleared of anything more serious it's time to look at your diet and lifestyle. These factors increase the risk of palpitations: Blood sugar irregularities Dehydration and low electrolyte status Low Iron Anaemia Blood pressure issues Stress, anxiety, or panic.
