Jaishree currently heads the Laboratory for Antimalarial Resistance Monitoring and Malaria Operational Research (ARMMOR) at the NICD. The core focus of ARMMOR is to improve malaria diagnostic and treatment options and to assist South Africa and Africa eliminate malaria. To this end, ARMMOR hosts the South African National Surveillance Programme for Antimalarial Drug and Diagnostic Resistance Monitoring. As the chair of the case management sub-committee of the South African Malaria Elimination Committee, Jaishree has developed and delivered training courses aimed at capacitating the malaria programme staff on case management protocols and practices. She is a member of several regional networks, such as the Elimination 8 and Pathogen Diversity Network Africa, that are aiming to strengthen malaria genomic and bioinformatics capacity across Africa. She co-developed a leadership and management course for malaria elimination targeting malaria programme staff currently being delivered by GIBBS and the University of Pretoria and is also a mentor on the recently launched Pan-African Malaria Control Association (PAMCA) LiftHerUp initiative, which is focussing on increasing African women leaders in science, with a major focus on vector-borne diseases. Abstract: The Notch receptor is a titratable, context-specific counter of intercellular interactions that translates productive interactions with ligands on neighbouring cells into corresponding changes in gene expression via the nuclear localization of the Notch intracellular Domain (NICD). Using an Optogenetic Notch1 construct in combination with a live imaging transcriptional reporter and super-resolution imaging, we show that the N1ICD activates gene expression through spontaneous self-assembly into transcriptional condensates whose phase separation is driven by C-terminal Intrinsically Disordered Regions (IDR) of the N1ICD. N1ICD condensates recruit and encapsulate a broad set of core transcriptional proteins, thereby facilitating gene expression and promoting super enhancer-looping. We produced a model of Notch1 activity, whereby discrete changes in nuclear NICD abundance is translated into precise changes in target gene expression through the assembly of phase separated N1ICD molecular crucibles that catalyze gene expression in a concentration-dependent manner by enriching essential transcriptional machineries at target genomic loci. This as the outbreak has now been declared in five provinces namely, Limpopo, Mpumalanga, Gauteng, the Free State, and the North West. The NICD said SA is ready should a monkeypox outbreak occur. This comes as the world faces the largest outbreak of the virus in non-endemic countries. According to the NICD, the South African healthcare system has put measures in place to deal with any cases or outbreaks and that a more classical containment approach would be adhered to rather than mass vaccine rollouts as seen with Covid-19.

Local newspapers can hold back the rising tide of political division in America by turning away from the partisan battles in Washington and focusing their opinion page on local issues. When a local newspaper in California dropped national politics from its opinion page, the resulting space filled with local writers and issues. What comes from this experiment are lessons for all of us as we think about the role of local media, especially newspapers. As they argue, an opinion page that ignores national politics and, instead, focuses on news issues and concerns could help people think about public issues and politics differently. According to researchers from the National Institute for Communicable Diseases, the fourth wave of Covid-19 infections in South Africa has resulted in a surge in hospital admissions in children under the age of five. Dr Waasila Jassat, a public health specialist at the NICD spoke to Radio Islam International; says that the admissions should not cause concern, noting that there could be several explanations for the rise in children's admissions, one of which could be an increase in Covid-19 testing.
Active coronavirus cases in South Africa have climbed to 133 063. The NICD says an increase of 22 391 new infections was recorded nationally, in the last 24 hour cycle.
Researchers at the Council for Scientific and Industrial Research (CSIR) briefed the media on Friday on some of the organisation's research, development and innovation (RDI) efforts aimed at addressing the HIV/AIDS and Tuberculosis (TB) epidemics in South Africa. CSIR researcher Dr Advaita Singh shed light on an innovative plant-based, highly potent anti-HIV antibodies research initiative. “The CSIR's plant-based anti-HIV antibodies research is aimed at developing a cost-effective pre prophylaxis vaccine which will passively immunise against HIV/AIDS, pre and post exposure. This initiative was inspired by the desire to empower young girls and women to protect themselves against the virus as studies have indicated that women are theoretically, at higher risk of HIV acquisition,” said Singh. Working with the National Institute of Communicable Diseases (NICD), the CSIR is developing broadly neutralising antibodies against HIV-1, the most widespread human immunodeficiency virus, for the prevention and treatment of HIV/AIDS. CSIR scientists and their research partners were able to demonstrate the ability to engineer tobacco plants to produce unique glycosylation and rare sulfation post-translational modifications, a result that is not usually seen in plants. Glycosylation is a biochemical process during which a glycan – a large carbohydrate molecule – attaches to a protein, a lipid, or another organic molecule. The glycoengineered Nicotiana benthamiana (a relative of the tobacco plant) expressed two broadly neutralising antibodies named CAP256-VRC26 08 and CAP256-VRC26 09. These two antibodies were originally isolated by collaboration Aids research centre CAPRISA and the NICD from a South African HIV positive patient during a trial conducted by CAPRISA. Another technology presented by the CSIR at the media briefing aims to provide rural areas with a diagnostic assay that will test for HIV and TB simultaneously using one device in the comfort of the patient's home. CSIR lab technician Kanyane Malatji provided insight into a multiplex HIV and TB point-of-care diagnostic test being developed for use in remote and rural areas. “The effective management of patients infected with HIV and TB is restricted by separate diagnoses. The situation is exacerbated in remote areas where patients must either travel long distances to reach a healthcare facility or wait a long time before obtaining their TB diagnostic results. Therefore, through our multiplex HIV and TB point-of-care diagnostic assay, we hope to mitigate these challenges with the objective of improving treatment outcomes for patients in rural areas,” said Malatji. The locally developed diagnostic test uses a solid surface that couples the HIV and TB antigens and makes use of a locally produced fluorescence detector. “The benefits of this technology are that it offers a cost-effective solution with a low turnaround time of approximately 2 hours. Additionally, no laboratory infrastructure is required, the device is easy to use and can be performed at point-of-care without highly trained personnel,” added Malatji. The CSIR's multiplex HIV and TB point-of-care diagnostic assay is funded by government and the plant-based anti-HIV antibodies research work is funded by the Department of Science and Innovation and the South African Medical Research Council.
Health Minister Dr Joe Phaahla said citizens can be confident that in a matter of weeks, perhaps days, the Department of Health's (DoH's) scientists will have gotten to the bottom of the Covid-19 Omicron variant. Phaahla reiterated President Cyril Rampahosa's message on Sunday evening, when he said the best weapon to fight the virus and the pandemic was vaccination. The President urged citizens not to panic as the world dealt with the new variant. The variant was first discovered in Botswana and South Africa, and scientists have also identified cases in Hong Kong, Australia, Belgium, Italy, the UK, Germany, Austria, Denmark and Israel. “We reiterate that there is no basis for some of the leaders of countries which have imposed these restrictions on travelling for us in South Africa and other countries in Southern Africa. There is no basis for South Africans to panic, We have been here before, this is no new territory. We are now more than twenty months' experience in terms of Covid-19, the various variants and various waves,” Phaahla said. While scientists work around the clock to understand the new variant, the DoH is preparing health facilities for the fourth wave. Phaahla said the detection of the new variant and the reaction of some countries had made this matter of even more public interest to all South Africans. Epidemiologist Professor Abdool Karim said, based on what the scientists know, they expect that the protection of the current vaccines against severe Covid disease, including hospitalisation, was likely to remain strong. Phaahla said government was engaging countries, especially those which had initiated travel bans against South Africa, to convince them that it was unnecessary. Karim explained that closing the borders had almost no benefit as it would not stop the Omicron variant from spreading – the virus is already present in many countries. NEW COVID-19 CASES The National Institute for Communicable Diseases' (NICD's) Dr Michelle Groome said current infections were driven by younger age groups, with Gauteng accounting for 80% of all new cases. NICD's Dr Waasila Jassat explained that there had been an increase in hospital admissions in Gauteng in the past 14 days, adding that other provinces were not yet showing an increase in admissions or deaths. Head of Hospital Services at the Gauteng Health Department Dr Freddy Kgongwana said Gauteng has 4 407 beds for the fourth wave. He explained that the department was "boldly secure" in terms of oxygen availability. “We are seeing mild cases. It is still early days for us as general practitioners. Vaccinated patients are doing better than unvaccinated ones,” said Gauteng general practitioner Dr Unben Pillay.
關於Omicron變種病毒的消息,資訊肯定會變化很快,部落格文章我會隨時更新。 https://linshibi.com/?p=39815 1.Omicron變異株為何讓科學家特別擔心? 此變異株的棘狀蛋白上有32個突變,集先前各種變異株的突變特徵於一身,包括與病毒進入細胞,免疫逃脫或是感染力增加等等有關的突變。且初步在南非已經證實可以很快的取代原本的Delta變成主流病毒株,證實了他的能耐。 2.Omicron傳播力比Delta更強嗎?為何你說這個魔王可能是玩真的? 之前的幾個假魔王(比方Delta plus,Mu,C1.2.)都沒有辦法對Delta造成太大的威脅。但Omicron新變異株最早在南非及波札那發現,近2週相關病例數在南非東北的豪登省快速攀升,取代原本流行的Delta變異株,預估可能成為下一波南非主流病毒株,WHO判斷已經符合了高關注變異株(Variants of Concern, VOC)其中一個定義:新冠的流行病學上產生了不利的改變,因此決定將B.1.1.529界定為高關注變異株,命名為Omicron。這是第五株VOC,加入了Alpha、Beta、Gamma和Delta變異株的行列。之前的假魔王,沒有一個被界定為VOC。而且Omicron是短短14天內就被界定為VOC。 3.Omicron會不會更容易重症? 南非初步報告說以輕症為多,但你要知道這個病毒從11月12日才在南非第一次偵測,且一開始以年輕人為多,也有些人是二次感染。因此現在觀測到輕症居多是正常的。可能需數週時間累積更多臨床資訊,分析在老人家或有重症風險因子,還有沒有打疫苗者是否會有更高的重症風險。之前不管是Alpha或是Delta,我們都是大概兩個月以後才有比較確認的研究。 我真心希望雖然Omicron變得比較有傳染力,但疾病的嚴重度若是會變輕的話,這反而是這個病毒往輕症化,流感化的方向走呀。 4.新冠疫苗對Omicron還有用嗎?單株抗體,口服藥的效力呢? 現有新冠疫苗幾乎都是針對病毒的棘狀蛋白產生抗體,而Omicron變異株的棘狀蛋白上就有32個突變,很令人擔心疫苗能夠產生的中和抗體是否會更低。各疫苗公司已經開啟實驗室研究,初步的結果應該兩週左右會出來。南非NICD是預估,也許會有一些免疫逃逸,但疫苗應該對於重症和死亡還是可以提供高的防護力的。 就算很不幸現有疫苗完全失效了,mRNA的技術平台也可以很快地做出新疫苗。輝瑞/BNT和莫德納疫苗六週的時間就可以改變mRNA序列做出新疫苗,輝瑞聲稱可以在100天內開始出貨。 Omicron變異株在單株抗體所作用的受體結合區域(Receptor-binding domain; RBD)上有很多個突變。因此單株抗體的確有可能會效用降低,這需要後續研究。至於口服藥,因為其作用機轉和疫苗完全不同,不是作用在棘狀蛋白上,因此很可能是不太受影響的。但這當然也需要後續研究證實。 5.各國如何因應?國際交流又會就此停止嗎? 目前仍以入境限制為主。英國開第一槍禁航。美國11月29日起限制南非等8國旅客入境,並呼籲民眾盡快施打疫苗追加劑。歐盟建議對非洲南部7國人士實施旅行限制。日本強化對非洲南部9國入境管制,相關人士須在指定場所隔離10天。 台灣指揮中心將非洲南部6國:南非、波札那、納米比亞、賴索托、史瓦帝尼、辛巴威列重點高風險國家,11月29日0時起自上述國家入境或轉機的旅客,須入住集中檢疫所並配合專案採檢。陳時中指揮官表示,現階段國際驗出新變種病毒確診個案人數少,且台灣沒有直飛航班,直接威脅相對小,還是要以完整阻絕為先,會持續關注國際狀況。他也呼籲國人盡速接種疫苗,目前75歲以上長者仍有27%未打疫苗。 南非衛生部批評各國的管制措施太過嚴苛、不科學且違反WHO建議。這很似曾相識,像去年12月英國宣布alpha之後全世界幾乎都對英國禁航。但在這株病毒的擴散範圍,還有詳細特性被釐清之前,各國採取比較謹慎的措施,我認為是合理的。至於會持續多久,就要看之後對此病毒的了解,還有此病毒的擴散狀況了。 6.現有PCR可以偵測Omicron變種株嗎? 目前的PCR是可以檢測出此變異株的,沒有漏掉的問題。南非的幾個實驗室反應主要會檢驗的三段基因中,其中的S基因會檢測不到Omicron(稱之為 S gene dropout或 S gene target failure),這可以在完整定序之前較快速的辨識出這大概是Omicron。 7.台灣的防疫需要做什麼改變? 先不用太擔心,維持目前阻絕疫情於境外的策略。指揮中心會根據最新資訊隨時滾動式調整的。而民眾,做的事情都一樣。出外戴口罩,勤洗手,盡可能保持社交距離主,注重室內通風。能打疫苗的,連第一針都還沒打的,第二針還沒打的,快點去打。我們明年準備要打加強針了。 林氏璧醫師的電子名片 https://lit.link/linshibi 小額贊助支持本節目: https://pay.firstory.me/user/linshibi Powered by Firstory Hosting

South Africa said on Friday a British ban on flights from six southern African countries over a new Covid-19 variant seemed rushed, as EU authorities prepared similar moves and the World Health Organization (WHO) convened an emergency meeting. Scientists have so far only detected the B.1.1.529 variant in relatively small numbers, mainly in South Africa but also in Botswana and Hong Kong, but say they are concerned by its high number of mutations which could possibly make it vaccine-resistant and more transmissible. Britain said the variant was the most significant one found yet after banning flights from South Africa, Botswana, Lesotho, Eswatini, Zimbabwe and Namibia from midday on Friday. European Commission chief Ursula von der Leyen said the EU also aimed to halt air travel from the region, describing the variant in a tweet as "of concern". A WHO working group on virus evolution is due to meet on Friday to discuss whether to officially give it that label, a designation only given to four variants so far. WHO epidemiologist Maria Van Kerkhove said in a video posted on Twitter that it could take a few weeks to understand the impact of the variant's mutations. The rand slumped over 2% against the dollar early on Friday, as the variant unnerved investors. South African hospitality stocks also plummeted. South Africa will speak to British authorities to try to get them to reconsider their ban, the foreign ministry in Pretoria said. "Our immediate concern is the damage that this decision will cause to both the tourism industries and businesses of both countries," Foreign Minister Naledi Pandor said in a statement. As Asian countries also moved to tighten curbs, two Welsh rugby clubs in South Africa for a tournament scrambled to leave as soon as possible, and British and Irish golfers withdrew from the Johannesburg Open. Although the NICD did not link the resurgence to the B.1.1.529 variant, leading local scientists suspect it is the cause. The Africa Centres for Disease Control and Prevention strongly discouraged travel bans on countries that had reported the variant. "Imposing bans on travellers from countries where a new variant is reported has not yielded a meaningful outcome," it said.

The National Institute for Communicable Diseases says South African scientists have detected a new coronavirus variant with multiple mutations but are yet to establish whether it is more contagious or able to overcome the immunity provided by vaccines or prior infection.The variant, known as C.1.2, was flagged last week by the KwaZulu-Natal Research and Innovation and Sequencing Platform in a preprint study that has yet to be peer-reviewed. Driving back from a visit to the VOA Greenville transmitter site, we passed through Fredericksburg That reminded me that Mike Bird has started referring to the Fredericksburg Index in his solar forecasts. It turns out there is a connection. On the last Monday in the month Radio Glasnost, a new programme on a West German FM station is airing messages from East German (religious and environmentalist) groups. The show airs via Radio 100. But conservative groups object to the programmes and attempts have been made to silence the broadcasts. There has also been talk of jamming of recent broadcasts.
