The AHA/ACC clinical practice guidelines for perioperative cardiovascular management for noncardiac surgery have recently been reviewed. While oral surgeons aren't likely to perform high-risk surgery, it is important to know what these guidelines suggest to avoid perioperative complications. To break down these guidelines, we are joined by Dr. David Salomon. In this conversation, you'll hear all about why we decided to discuss pre-op anesthesia considerations today, how the 2014 AHA/ACC guidelines differ from the 2024 ones, some key points in the new guidelines, and how they have changed how Dr. Salomon practices medicine. We delve into what a high-risk surgery is, how to identify a high-risk surgery, and what the seven risk modifiers mentioned in the study are before discussing the three possible next steps a surgeon will follow once they've done a risk analysis. Dr. Salomon even stresses the importance of asking the patient about functional capacity and shares how these guidelines have helped him be aware of what's important to consider pre-op. About a quarter of all patients with newly diagnosed triple-negative breast cancer will not benefit from neoadjuvant checkpoint inhibitor immunotherapy with an agent such as pembrolizumab—even though it improves outcomes among the remaining majority. At the 14th European Breast Cancer Conference, held in Milan, Italy, Laura van ’t Veer, PhD, Program Leader of the Breast Oncology Program at the UCSF Helen Diller Family Comprehensive Cancer Center, reported findings from the I-SPY2 TRIAL showing that analysis of “response predictive subtypes” identified a subset of patients with triple-negative early-stage breast cancers with a very low likelihood of response to neoadjuvant immunotherapy and can be spared potential toxicities. After her talk in Milan, van ’t Veer called in to the OncTimesTalk studio to talk about the I-SPY findings with Peter Goodwin.

Visit was 2 days after surgery. Documentation by nurse states; Pt in for foley cath removal. 10 ml sterile water removed from balloon, balloon deflated, foley cath removed. Pt tolerated well. Should we be billing a 99211 nurse visit, a 51700 for bladder irrigation or considered part of the surgical package and would not be billable? My question is what if I use a modifier that Coding Today indicates a modifier is required, and the insurance company still denies the second procedure with the modifer as included in the primary procedure, such as CPT's 52332 and 52005-59. This week Natalie sat down with three pre-op patients and BariNation community members Ashley, Helina, and Lisa. They chatted about all things pre-op, including what they wish they knew before taking the leap into the bariatric journey. From not realizing how tedious the process could be, to learning that mindset work is a vital component to success. Ashley is a wife, mom of two (2yrs and 5yrs), and school psychologist in NY. Her VSG is scheduled for 4/5/23. She enjoys watching documentaries (including too much true crime), watching reality tv, and learning about/trying new things. Ashley's hoping VSG helps fight the fatigue that holds her back from doing things she wants to do. She's looking forward to spending this summer working on and caring for herself and being active with her kids and husband. Helina struggled with her weight for as long as she can remember and tried just about every diet out there. Over a year ago she started researching WLS, got referred to OHSU in February 2022, went through personal hardships, was waitlisted for the program at OHSU, but was finally able to start her pre-op journey on 07/12/22. She's struggled with a food addiction for a long time and has found food as a comfort. But with going back to therapy and mindset work she knows she can do the hard things. She also knows and believes she's worthy and deserving of choosing herself and her health. Lisa has been working through a lot of mindset work to prepare for her journey. Quad Inhibition and Muscle Atrophy can really slow down the recovery process. However, Jackie Kleihege and Jena-Claire Auten know it is essential for healing. Finding the balance between helpful and hurtful is kind of an art form. Looking at the timeline is essential so we can see the progress of where we have come. What is the research showing us? We do not have all the data yet from the quad tendon ACL We have 10 years of data on it What are some reasons we have inhibition? In the acute phase, it looks about the same. If we can interrupt the inhibition early then we have better outcomes It is really hard to study the brain when we are studying the knee How can we find this loophole or workaround to muscle atrophy? Decrease swelling Decrease local inflammatory response ICE??? Is there a time frame where are going too fast in turning off the inflammation? We still want the natural healing process to continue through that phase. And then for that to quietly fade out. NMES JK: I have probably underutilized it most of my career.       I needed to give something more and then added NMES and made a difference. What are your ideal volume and exercises to prevent muscle atrophy? PRE-OP therapy is critical There is not a magic wand protocol Pre-Op nutrition is super important as well. When we try to activate the quad and then the patient is absent for a week what is the problem? Latency is key - I want those first 7 days like they are falling downhill, then I dial it back. Often day 1 postop patients do not look bad, but then days 2-3 are worse. JA: Genetics plays a role in the inflammation process. Consistency is key, similar to Instagram and Facebook, they keep putting the ads in front of you and then you end up buying it… Continue to use the same methods so the patients know what is coming and can buy in Looking at the 12-16 week patient OPEN KINETIC CHAIN - the type of graft makes a difference as well I need to be able to read the signs and customize the options for them. The goal is quad communication Determining when to progress: Biofeedback can help give visual and auditory feedback. BFR: It is really as good as they say. The uses keep growing.
