La fistule obstétricale est une lésion provoquée par un accouchement prolongé et aggravée par l'excision ou lorsque la maman est très jeune. Un fléau qui concerne 500 000 femmes dans le monde, dont 250 000 se trouvent en Afrique subsaharienne. Direction la région de Kolda, en Casamance, dans le sud du Sénégal, où une ONG fait de la prévention et accompagne jusqu'à la guérison les femmes qui en souffrent. De notre envoyé spécial à Kolda,À peine arrivées dans le village de Sare Kanta, les équipes de l'ONG Tostan distribuent des brochures sur la fistule obstétricale aux habitantes. Parmi elles, Hothia Mballo, qui connaît déjà le sujet. Pendant près de cinq ans, après un accouchement difficile, cette femme de 41 ans a souffert des conséquences physiques et sociales de la fistule : « Je vivais isolée des membres de ma famille, j'avais honte de ne pas pouvoir retenir mes urines et de sentir mauvais. Mais mon mari ne m'a jamais haïe à cause de la maladie et contrairement à d'autres femmes, il m'a toujours soutenue. Aujourd'hui, je peux dire que le tabou est brisé, parce que tout le monde m'a vue guérir. »Une évolution dans le comportement au sein des communautés qu'a observées Penda Damso, une agente de mobilisation sociale qui intervient dans toute la région : « Au départ, il y a de la honte. Voilà, les femmes, des fois, quand la sensibilisation se termine, vous allez dans d'autres villages, c'est après qu'on vous interpelle pour vous dire que dans le village que vous avez quitté, il y a une femme porteuse de fistule. »À écouter aussiFistules obstétricales : les prévenir grâce aux consultations prénatalesFinté Boiro est le coordinateur de Tostan dans la zone sud du Sénégal. Lui aussi souligne l'importance de la prévention et l'accompagnement dans les soins : « Les capacités des femmes ont été renforcées, c'est pourquoi on a eu aussi à prendre en charge ces femmes victimes de la fistule obstétricale à travers le traitement, la réparation de ces femmes-là. »Une réparation qui prend du temps et sur laquelle le Dr Emmanuel Kazubwenge est spécialisé depuis plus de dix ans. Le chef du service de chirurgie de l'hôpital régional de Kolda évoque sa prise en charge : « Elle est complexée, elle est multidisciplinée, elle est sociale, elle est médicale, elle est économique. On nous les amène, on les consulte. S'il faut opérer, on les opère. C'est une chirurgie délicate et une chirurgie, peut-être pas urgente, mais une chirurgie parfois avec des échecs, ce n'est pas étonnant d'avoir une femme multi opérée. »Le médecin se réjouit tout de même de voir de moins en moins de femmes venir consulter pour une fistule obstétricale. Grâce, selon lui, à la multiplication des structures sanitaires de proximité qui offrent aux femmes un suivi tout au long de la grossesse et après l'accouchement.

In her role, she is responsible for the organization's overall strategy and management of the global Open Road Alliance team. Since its inception in 2012, Open Road has made over $50 million in grants and low-interest loans to organizations across the impact sector which have encountered unexpected roadblocks. Through this strategy, Open Road has kept over $411 million of impact on track across all sectors and geographies, for both registered nonprofits and for-profit social enterprises. The organization also conducts research and advocates for the adoption of long-term, system-wide risk management practices in the philanthropic sector. Maya served as Open Road's inaugural Executive Director, and previously worked as an Associate Director with the consulting firm williamsworks where her clients included the Eastern Congo Initiative, Nike Foundation, PATH, Tostan, and TOMS Shoes. Prior to williamsworks, she served in multiple roles across the nonprofit and government sectors focusing on program development, fundraising strategy, and corporate partnerships. Maya writes and speaks frequently about Open Road's work and its growing research expertise on ‘Risk Management in Philanthropy.' She is a regular speaker at forums including Grantmakers for Effective Organizations, Skoll World Forum, and PEAK Grantmaking. Her work has appeared in Stanford Social Innovation Review, Chronicle of Philanthropy, The Foundation Review and many other publications. Seit 20 Jahren reist eine der bekanntesten Schauspielerinnen Deutschlands, Katja Riemann, als Botschafterin für humanitäre Hilfe durch die Welt. 2020 hat sie ein Buch über ihr Erlebtes in den Projektreisen mit dem Titel „Jeder hat.
