Send us Fan MailThe fire service is full of people who will sprint toward danger without hesitation, but we rarely talk about what the job quietly takes from us over 20 or 30 years. Host Kevin Ferrara dives into the culture of wellness in fire and rescue, and why the old “tough it out” mindset is not just outdated, it is costing careers and lives. We break down the physical realities that outsiders never see: the constant redline of adrenaline when the tones drop, the heavy gear and awkward movements that destroy backs and shoulders, and the cardiovascular risk that remains one of the leading causes of firefighter line of duty deaths. Sleep deprivation gets a hard look too, not as a bragging point, but as a health and safety problem that wrecks immune function, slows reactions, and piles onto other concerns like PFAS exposure and cancer risk. Then we move into the part many stations still avoid, the mental load. Using the “backpack of rocks” metaphor, we talk about cumulative trauma, burnout, depression, and the heartbreaking suicide reality among first responders. A real firefighter wellness program cannot treat behavioral health like an afterthought. It needs peer support, trusted access to culturally competent clinicians, and routine check ins that are as normal as checking your SCBA.Finally, we get practical: what a structured, comprehensive health program looks like on the ground, built on occupational medical screening, functional fitness and injury prevention, behavioral health, and nutrition that fits station life. We also make the cost argument for chiefs, city managers, and policymakers: the real question is not “how much does it cost,” but “how much is doing nothing already costing” in workers' comp, overtime, and lost expertise. If this hits home, subscribe, share it with your crew or a decision maker, and leave a review with one change you want to see in your department.DISCLAIMERThe views, information, or opinions expressed by guests during this podcast are solely those of the individuals involved and do not necessarily represent those of AFSO21's Weekend Wrap-up Podcast and its hosts. Send us Fan MailTurnout gear is supposed to be the last line of defense, so what happens when the fabric itself contains PFAS forever chemicals that were never clearly disclosed? This week, we pulled a crucial interview from our archives with Dr. Graham Peasley, the researcher who tested dozens of firefighter turnout gear sets and found fluorine consistent with PFAS treatments across manufacturers, enough that his lab changed how they handled the samples. We talk about what that discovery means for firefighters who wear the gear, touch it daily, and store it around the station and sometimes at home. We also slow down and make the numbers make sense. Parts per trillion versus parts per billion is not a trivia question when PFOA and PFOS have documented links to cancers and immune impacts at very low levels. We address the online pushback and the consultant op-ed culture head-on: how to evaluate sources, what “trace” really implies, and why uncertainty about skin absorption is not the same as safety. Along the way we cover real exposure pathways, from hand transfer to station dust, and how AFFF firefighting foam fits into the broader PFAS story. Then we get practical. We share straightforward steps departments can take now: keep gear at work, wash and decon consistently, rethink storage locations, and treat labeling and ingredient disclosure like essential safety information. We also dig into the hard end-of-life problem: landfilling vs high-temperature incineration, and why repurposing old gear into bags or blankets can create new risks. If you care about firefighter health, PFAS exposure, and fluorine-free turnout gear that still meets NFPA performance needs, this conversation gives you a clear place to start. Subscribe, share this with your crew, and leave a review so more firefighters can find it. What policy change would you implement first at your station?DISCLAIMERThe views, information, or opinions expressed by guests during this podcast are solely those of the individuals involved and do not necessarily represent those of AFSO21's Weekend Wrap-up Podcast and its hosts. Send us Fan MailCalling EMS “ambulance drivers” sounds small, but it exposes a huge gap in how the public and even parts of healthcare understand prehospital medicine. We sit down with Pennsylvania Paramedic Justin Whitney during National EMS Week to get specific about what the job really looks like on the street and why the future of EMS is trending toward community medicine, not nonstop high-acuity 911 calls. If you're considering EMT training, AEMT, or paramedic school, this conversation helps you reality-check the work before you commit. We unpack why “routine” calls still deserve real assessment, how pressure changes as you climb the certification ladder, and what mentorship should look like when it's done well. Justin explains community paramedicine and why many calls are really about gaps in the healthcare system: confusing discharge instructions, medication dosing issues, chronic conditions, isolation, and limited access to care. We also talk EMS Officer training, field training officer (FTO) responsibilities, and when an incident command mindset matters most. From there we zoom out to the big operational challenges: siloed agencies, too little collaborative training, and the friction between EMS and hospitals when each side doesn't understand the other's scope and workflow. We close with direct, grounded burnout solutions including fair wages, sustainable schedules, and dependable equipment so clinicians can deliver safe care without running on fumes.Subscribe for more leadership and operations conversations. Share this with someone in your station, and leave a review if you want more EMS-focused guests. What's one change that would improve outcomes in your area right now?DISCLAIMERThe views, information, or opinions expressed by guests during this podcast are solely those of the individuals involved and do not necessarily represent those of AFSO21's Weekend Wrap-up Podcast and its hosts. Send us Fan MailFire calls may have built the modern fire service, but EMS calls now define it and that demands a different kind of leader. We walk through why the EMS Officer 1 is not a “nice to have” title, but a practical response to how fire based EMS really operates today, where medical responses can make up 80% of the workload and outcomes depend on more than individual clinical skill. We break down what an EMS Officer 1 owns day to day: clinical oversight, crew support, and system performance. That includes standards and professional expectations tied to NFPA 450 and NFPA 1021, plus the real work behind the paperwork like coaching leadership, emotionally intelligent debriefs after high impact calls, and building a culture where critical thinking thrives. We also dig into clinical quality improvement done the right way, using data to spot trends and fix training, equipment, or workflow problems before they become patient harm. The conversation goes beyond medicine into the areas that can make or break public trust: HIPAA compliance, consent law, end of life ethics, and the organizational risk that follows a bad decision on a chaotic night shift. We also talk about the EMS Officer 1 as the first responder to the responders, watching for burnout and PTSD, advocating for peer support, and protecting the workforce. Add in financial stewardship, supply and medication logistics, smarter unit placement, hospital partnerships, scene safety, and the coming growth of community paramedicine and mobile integrated health care, and the case becomes clear. If your department still promotes based mostly on seniority, this will challenge you to rethink what competence means in clinical leadership. Subscribe for more, share this with an EMS leader in your circle, and leave a review with your take: what's the hardest part of building strong EMS leadership where you work?DISCLAIMERThe views, information, or opinions expressed by guests during this podcast are solely those of the individuals involved and do not necessarily represent those of AFSO21's Weekend Wrap-up Podcast and its hosts. Send us Fan MailNFPA 1750 is here, and it's an earthquake for the fire service. Are you prepared for the radical transparency it demands, or are you still relying on "tribal knowledge" that's about to put your department on the wrong side of a legal liability claim? In this explosive episode of The Weekend Wrap-up, host Kevin Ferrara pulls no punches, breaking down why NFPA 1750, the unified standard replacing 1201, 1710, 1720, and 1730, has officially ended the era of "we've always done it this way."The Gut-Punch: Learn why this standard is a "cold, hard mirror" that doesn't just show your wins, but exposes systemic neglect, administrative failures, and the cultural weaknesses many departments have hidden behind the "hero" narrative for decades. The Four Critical Tasks: Discover the deadly math of the Effective Response Force and how running a fewer than four-person crew in a high-risk zone is statistically incapable of meeting operational requirements, turning a safety issue into an economic drain on your city.Meet the Data Diplomat: Municipal officials are asking, "Prove it." Find out how the new species of fire leader is using Gap Analysis and cost-avoidance models to translate risk into the language of the budget hawk, ensuring survival for the next twenty years. This isn't a suggestion; it's the new standard of care. Send us Fan MailThe podcast episode details the generational crisis of occupational cancer among firefighters and the cultural shift required to address it.The key points include:Cultural Shift: The traditional image of the "salty" veteran, identified by soot-stained gear, has been exposed as a marker of exposure and a "death sentence". The culture is shifting toward a "Professional" standard where cleanliness, decontamination, and wellness are prioritized as essential survival skills. The Hazard: Modern fires involve "solidified oil" (plastics, synthetic materials) that, when burned, create a toxic chemical soup, unlike the natural materials of the past. The Evidence: Landmark NIOSH studies (2013 and 2015) confirmed that firefighters face a 9% higher risk of cancer diagnosis and a 14% higher risk of death compared to the general population. Cancer accounts for approximately 66% to 80% of career firefighter line-of-duty deaths (LODDs) for IAFF members. The PFAS Paradox: Firefighters are exposed to carcinogenic "forever chemicals" (PFAS) through Aqueous Film-Forming Foam (AFFF) and, disturbingly, the moisture barriers in their own turnout gear. The research shows that for every 5-degree increase in skin temperature, the skin's absorption rate increases by 400%, allowing toxins to be "cooked" into the body. The Roadmap: New protocols focus on prevention and early detection, including on-scene "Gross Decon" (scrubbing down with soap and water), the "Clean Cab" initiative (storing contaminated gear externally), and a push for annual, fire-specific medical physicals. Early detection at Stage 1 offers a high survival rate, underscoring that wellness is a survival skill. Host Kevin Ferrara explores three strategic imperatives for modern survival and excellence: The Shift from Response to Prediction, via predictive analytics and AI-powered syndromic surveillance, challenging leaders to become "Data Translators";The Digital Privacy Crisis for First Responders, defining digital exposure and doxing as a critical safety and wellness concern requiring proactive "Digital Privacy Audits"; and The Death of the "Hero" Recruitment Model, advocating for a pivot toward sustainability, work-life balance, and combating "Mission Creep" through strategies like Community Paramedicine, arguing that retention is the new recruitment. This episode concludes by urging leaders to abandon broken, reactive frameworks and embrace this transformative opportunity to build smarter, safer, and more human-centered organizations. Outwit. Outplay. Outlast. The Survivor creed is being put to the test by the Parramatta Eels - particularly the whole outlast part. A heart-breaking loss to the Tigers has seen them lose Bailey Simonsson, Apa Twidle, Jonah Pezet and Sean Russell for varying lengths of time ranging from the season to a game or two. Just another four more bodies to pile into the burgeoning injury ward for the Eels. Who does Jason Ryles turn to? Does he even have 22 healthy players to name this week? Sixties and Forty20 put on their finest PPE as they attempt to avoid injuries covering the week that was and the week that will be for the Blue & Gold. The boys break down a very broken down Team List Tuesday with Araz Nanva set to make his NRL debut...but could there be more rookies in the mix this week? The Weekend Wrap at least has some joy to be found in the lower grades as all four pathways squads qualified for the finals. Stuff happened in the NRL, of that I am certain but it all feels a bit distant given the disastrous sequence of events plaguing the Eels. Still, the boys review Round 5 as the inconsistent results continue to fly in from all angles while the injury bug reaches some of Parramatta's competitors.

I caught up with Ya'akov Katz on Thursday afternoon, and we got into some very sticky issues that are being talked about in Israel. We are one month into this war with Iran and it is not exactly going as we'd thought it might. Then again, political leadership has been anything but clear in communicating with the Israeli people about war goals, how things are going and where they are going. There's a lot of talk about numbers. Numbers of missile launchers hit, missiles intercepted, targets hit. But the weird part is - the numbers don't add up. And we had a very similar experience with the war on Hamas. Israelis are famously resilient, yes, but they are also worn out after almost three years of constant war. And there is no letup in sight. The population overwhelmingly supports war with Iran if it will degrade the country militarily and, ideally, lead to regime change. But people are beginning to wonder if we will be “back at it” in another six months or a year. Following the June attacks on Iran's nuclear facilities, we were led to believe that we had diminished the nuclear and general military threat substantially. Now? There are many questions. This past week there has been a lot of buzz about the U.S. negotiating with Iran…..President Trump says the talks are going very well. Iran denies that there are any talks. The missiles keep coming at us from Iran. The sirens keep wailing. The intercept rate is high but there are always cluster bombs that break up and score direct hits. And we have a terrible shortage of troops in the IDF - but we don't get into that today. I do write about it in the Weekend Wrap - which we drop tomorrow. In this podcast, we just try to sort out what's real and what's, well, not.Show your support for STLV at buymeacoffee.com/stateoftelavivYaakov Katz is an Israeli-American author and journalist.
