In this episode of the Craving Consciousness Podcast, Melissa Rippe and I go into The Left Hand Side. Melissa seeks to help awaken witches and dispel some of the stigmas associated! From Melissa: I grew up in fear..... I was born a Medium. Growing up was not all sunshine and roses because of this. There were ALWAYS Spirits around me. Being a small child, I did not understand what this was or what it meant. I had no tools for dealing with these gifts and nobody around me who understood them either. I learned to just shut out my abilities. I had things happening to me I didn't understand. Psychic things. They caused fear and anxiety that was paralyzing at times. I tried to ignore them but that fear was real. I didn't want to have other people judge me, so I didn't ask questions. I didn't try to look for answers. I just kept those fears and anxiety hidden inside and tried to act "normal". I felt like I didn't fit in with the world around me. AS AN ADULT, I STARTED TO FIGURE OUT WHAT WAS GOING ON WITH ME I met some people that Spirit put on my path and they opened me up to all that was happening with me, and had always been happening! I learned that I wasn't the only one in my family that had abilities. I started opening up to everyone and searching. Finally I found the answers..... I began heading down the path of opening up my psychic/witch abilities and found the inner strength to really embrace this new information. I found a peace and freedom I had NEVER experienced before! It was a tough journey but I keep going. Once I started, I wanted more. How to Manage Repetitive Behaviors in Alzheimer's Disease “Look for a reason behind the repetition.”— Melissa Batchelor, PhD, RN, FNP, FAAN Are you wondering why a person with dementia develops repetitive behaviors like asking you the same question, telling you the same story, or doing the same thing over and over again? Knowledge is power. In this episode, I’ll walk you through brain development, what’s lost and retained in Alzhiemer’s disease, and help you problem-solve how to manage repetitive behaviors in a person with dementia using the C3P Problem-Solving Framework to explain. Part One of ‘How to Manage Repetitive Behaviors in Alzheimer's Disease’ To put it simply, your brain develops from the back to the front. The back part of your brain is where your feelings and emotions are (the amygdala). The amygdala controls our emotional responses when we encounter anything threatening, and activates the sympathetic nervous system with the fight, fright, or flight response. The front part of your brain is called your frontal lobe, and it doesn't fully develop until you're about twenty-five years old. This part of your brain is basically the “stop sign” - meaning when something comes to mind that you want to say, your frontal lobe might say, "No, don't say that quite yet (or don't say it at all)." What happens with Alzheimer's disease is the brain loses its ability from the front to the back. So you lose your ability to control impulses, use and understand language but your emotions remain intact throughout the disease process. “Focus on the emotion, not the behavior.” — Melissa Batchelor, PhD, RN, FNP, FAAN To give you a visual image of what happens to the brain in Alzheimers. Imagine a grape, that's what a healthy brain looks like. When your brain is healthy, all of the messages can get back and forth without a problem. But what happens with Alzheimer's disease is the brain begins to shrink and shrivel. Alzheimer’s disease basically turns your brain into a raisin. Imagine a raisin. There are deep crevices, and those crevices make it hard for the messages to get back and forth. This is why people lose their memory, decision-making ability, and the hardest thing is that they lose their ability to use and understand language. So our words basically end up sounding like Charlie Brown’s teacher - So words don’t work anymore; but the person does pick up on our emotions and our non-verbal behavior. They will “mirror” us. Part Two of ‘How to Manage Repetitive Behaviors in Alzheimer's Disease’ Let’s talk about the C3P Problem-solving Framework focused on Changing the Person, Changing the People, or Changing the Place. The reason this is important is because there isn’t one thing that will work for any specific behavior. You will have to try different things for behaviors because what works today, may not work tomorrow. You need a “bag of tricks” and a way to think through the three levels that will help you find a solution. Change the Person (with Dementia): Look for a reason behind the repetition. Does the repetition occur around certain people or surroundings, or at a certain time of day? Is the person trying to communicate something? Is the person trying to do something? Trying to understand what the underlying reason is can be helpful. Focus on the emotion, not the behavior. Rather than reacting to what the person is doing, think about how he or she is feeling. Try to figure out the root cause of his or her anxiety can help them manage their anxiety and maybe resolve it. For example, if an older adult said something fifty times already, rather than saying “you’ve already told me that 50 times!” [in a frustrated voice], say something like, “It sounds like you’re anxious.” [in a calm voice]. Turn the action or behavior into an activity. Give them something to do and focus on, something meaningful. If they are picking at their skin or fiddling with their clothes, give them a laundry basket of things to clothes. Ask the person to help you get this “work” done. Change the People (Caregiver Approach) Stay calm, and be patient. One of the gifts that Alzheimer's gives people is that they live in the present moment. On the other hand, it's hard for caregivers to do that because they're living in the future thinking about all the things that they need to do. Be aware of the impact of your own emotions in the moment. Caregivers may get very upset that this person can't do today what they could do yesterday. Patience is essential because while that person's feelings remain intact, their ability to pick up on your feelings also remains intact. If you express feelings of being frustrated or mad, that energy is in the room and they are also going to pick up on that. Demonstrate what you want them to do. Another part of the brain that remains intact is the ability to “mirror” us; meaning whatever emotional state we come into a room with, the person will mirror it back to us. This is also why mimicking a behavior you want the person to do works - they understand our non-verbal communication better than when we use words. Reassure the person with a calm voice and gentle touch. Don’t use so many words. Don't argue or try to use logic; Alzheimer's affects memory, and the person may not remember he/she asked the question already. Provide an answer. Give the person the answer that he or she is looking for, even if you have to repeat it several times. If the person with dementia is still able to read and comprehend, it may help to write it down and post it in a prominent location. Accept the behavior, and work with it. If it isn't harmful, don't worry about it. Find ways to work with it. Go with what the person can or wants to do in the moment. Resistance is usually met with resistance. As caregivers, we often have our own agenda. Things we want and feel like we need to get done. Pushing our agenda on someone else is likely to be met with resistance. If we push harder, they resist harder. So if this happens in an interaction, stop pushing and it’s very likely the person with Alzheimer’s disease will stop resisting. Wait until another time and come back and try again. Flexibility is key for this one. Share your experience with others. Find your own support group, a group of people that you can talk to and share your experiences with. Learning from other people is going to be helpful. What worked today may not work tomorrow and this can be frustrating. Be sure you have the emotional support that you need. “Turn the action or behavior into an activity.” — Melissa Batchelor, PhD, RN, FNP, FAAN Change the Place (Environment) Use memory aids. If the person is calling you every day asking the same questions over and over again, try to use visual cues to offer reminders. Using notes, clocks, calendars or photographs, if these items are still meaningful. None of us can remember everything, so that might be another strategy that could help with a repetitive question. Manage Noise. Too much noise or activity can also raise a person’s level of anxiety. Sensory overload can increase their anxiety and end up with a behavior that they didn't really mean to get. Some behaviors do have the same solution though. One patient that I cared for a long time ago had daily repetitive verbalizations. He would get started up in the afternoon and you could hear him all over the building. When he did this, all of the staff knew it was time to take him out for a cigarette. He calmed right down after he smoked, until the next time he wanted one. And while I don’t think smoking is a great idea, in this case, it was the lesser evil than giving him a medication that would have likely caused greater harm. Tiffany served in the Army for ten and a half years. While she was in the Army, she was a logistician and she deployed to Iraq twice. She left the military with her masters degree using Tuition Assistance while on active duty and this past December graduated with her second masters degree using her GI Bill. Tiffany decided to join the military because she felt lost and didn’t know what she wanted to do. Although she had been a good student in high school, she didn’t want to have her parents pay for college without her knowing what she wanted to do. So, she decided to join the Army. After she had committed to the Army the one school she had applied to came back with a scholarship upon her acceptance. She was hoping she could get out of her military commitment and go to college instead. But her parents told her she had already committed to the Army and needed to fulfill her commitment. So off to boot camp she went. Not long after being in the Army she deployed to Kuwait for the initial invasion to Iraq. While waiting for the invasion to take place they were in Iraq and a Non-Commissioned Officer told her that they were waiting to go to war. She couldn’t believe it. It seemed like someone would have told her before it. It wasn’t until she saw the scud missiles in Iraq that everything came together and she realized she might not come home. As they pushed into Iraq, she expected they would enter war and there would be fire fights and bullets. But crossing the border was uneventful and instead their days were full of driving. They were always hot, always dirty and almost always on the move. They lived in their vehicles and kept pushing forward. About a year and a half after she deployed to Iraq she went back to Iraq. This time there were camps built up and since she went back to almost the same location, she could see how much change had happened since she had left. In 2003 you had your vehicle and your uniform, but it wasn’t like that the second trip. There were showers, you had a room, there were internet cafes, tents to call home, and more. Another big change between 2003 and 2005 was how the war was being fought. They hadn’t seen a lot of combat in their first round while they had been looking for landmines in 2003. In 2005, Improvised Explosive Devices, suicide bombers and convoy ambushes were a real threat. The war had evolved. The threat was amplified. She watched the evolution of the war. As the military tried to respond to the threat. Going from Humvees with no armor to trying to armor the Humvees, but sometimes causing more damage and having sand bags as protection. The war progressed in such a way and eventually the military was able to create a vehicle that added more protection. And if you have ever seen a side by side comparison of a Humvee and a Mine Resistant Ambush Protection (MRAP) vehicle you can understand why the change was so important and necessary to fight the war. She also dealt with a helicopter crash where she didn’t allow herself to grieve and really struggled for years after the incident happened. She felt guilty for living and she felt guilty for struggling with the deaths of the people she knew who passed. We talked about the importance of getting mental health help along the way and for military members to process all the different experiences they go through. I pulled a card for the collective today. I was guided to talk to you about fear. Everything you want is on the other side of fear, for many of you. I talk to you about how I deal with fear and how I have been able to over come things the are fearful to me. Hint: It has nothing to do with being "fearless". Introduction 2:30 Have a new caregivers retreat scheduled in Santa Fe in 2020 3:33 Received the support of Alzheimer’s Organisation to access caregivers, but wanted to offer something very different from the many resources of the Alzheimers’s Organisation. The retreat is similar to going to a yoga class – greeted at the door, checking in how they feel. Many caregivers come year after year. Most have no experience with yoga, nutrition, or self-care, massage, etc. Many don’t have access to these things either time or financially. 10:00 A free retreat for the caregivers funded by donations and volunteer work. No volunteers are compensated. Might turn this into a non-profit in the future. 80% of the volunteers have some link to dementia/Alzheimers. 11:58 A one day retreat: Volunteers come in and set up and prep. As caregivers arrive, a volunteer acts as a caregiver for them. Shows them around, helps them with the schedule, and check back in during the day. Start with gentle or chair yoga Welcome Brunch, Array of offerings: massage, reiki, acupuncture or yoga therapy or Ayurveda sessions. 3 sets of sessions with 3 speakers. May talk about the Alzheimer’s Association, self-care. Asks the speakers to make it a conversation and to have only one thing that the listeners come away with. May explore nature Last set of speakers, Q&A, or breathwork Meal. It is a wellness retreat; experiencing things they never would have the opportunity to do so otherwise. 18:00 Management task: Melissa delegates to 30+ volunteers on the day. 19:15 Melissa’s Dad was a caregiver for over 10 years for his wife who had early onset Alzheimers. His experience was that you are “witnessing your loved one die each day” There is grief each day and the grief lingers even after their death. After his wife died, he has been diligent with his own self-care; still longing for a sense of healing. Melissa recommends “It’s OK that you’re not OK by Megan Devine to help with grief. 22:24 With the new retreat in Santa Fe, Melissa has looked for other solutions to implement the retreat. She also used FB to do a fundraiser for a tent for the Calgary retreat. Start with the gifts you have and work from there. So many ways to create what is needed. 26:00 Getting the message out: uses groups that already are in contact with caregivers – adult day care centers, Alzheimers associations, Alzheimers Society in Calgary, non-profits in Santa Fe, other groups. 29:40 Anything is possible, everything I need is provided for, being able to give back is a reward that is indescribable. Dr Melissa J Ree, Clinical Psychologist, BSc (Hons) MPsych (Clinical) PhD Melissa’s main specialty is in the psychological management of adults with Insomnia and sleep difficulties. Melissa also works with many patients who are struggling with anxiety problems, low mood, and low self-esteem. “Generally, my approach to psychological treatment is 'here and now' focussed, although past issues will be discussed when appropriate. I use evidence-based treatments such as Cognitive Behaviour Therapy (CBT) and Acceptance and Commitment Therapy (ACT).” Melissa completed a master’s degree in Clinical Psychology and a Ph.D. with distinction at University of Western Australia in 2001. After her Ph.D. Melissa worked at the University of Oxford as a Postdoctoral Research Fellow in the Department of Psychiatry. During this time, she worked on a treatment trial of Cognitive Therapy for Chronic Insomnia. Since returning to Perth Melissa has continued to be active in treatment, training, and research into psychological aspects of insomnia, anxiety, and depression and has published a number of papers on the subject and has written two treatment protocols for Insomnia. You can see her publications on Research Gate. Melissa works in both clinical and research roles at private psychiatric hospitals in Perth and has been in private practice since 2004. What does the law say about listing operators on the mass auto policy? Hint: Honesty is the Best Policy Here....and the truth is that "creative auto rating" just isn't worth it. And why updating the policy with changes as they happen is also important. The Laws discussed in this episode.... MGL c.175 s.186 - Misrepresentation or warranty by insured. MGL c.266 s.111B - Motor vehicle insurance policies; penalties for fraudulent claims. MGL c.90 s.34O - Property damage liability insurance or bonds. Irene is quick to point out that answering the application honestly and completely is key and reminds us that it's always cheaper to pay the true premium than to be denied coverage.
