The Neurological Hand Deformity Classification: Construct validity, test-retest, and inter-rater reliabilitySimon Garbellini, Melinda Randall, Michael Steele, Catherine Elliott, Christine ImmsAbstractBackground: The Neurological Hand Deformity Classification (NHDC) is an impairment-based tool that classifies hand deformity into one of two ordinal scales: flexion or extension deformities. Classification is made from live observation or from recorded video footage. Differentiation between the levels is determined by wrist position and wrist and finger movement.Purpose: To examine aspects of validity and reliability of the NHDC.Study design: A measurement study design.Methods: Data from a convenience sample of 127 children with cerebral palsy, 66 males: 61 females, ranging in age from 8 months to 15 years, across all Manual Ability Classification System levels I to V, were analyzed. Construct validity was assessed by testing predetermined hypotheses of relationships between the NHDC and measures of body function and activity measures with observed performance using the Chi Squared Test of Independence and Spearman Correlation Coefficient. Test-retest and inter-rater reliability were assessed by calculating agreement between repeated measures and paired raters using weighted kappa and Cohen's kappa with 95% confidence intervals.Results: Predicted hypotheses for the NHDC were met in nine of 10 Spearman's rho correlations with body structure measures and in 2 of 7 correlations with activity measures. Test-retest for flexion deformities: κw = 0.84; 95% CI 0.70-0.98; and extension deformities: κ = 1.0; 95% CI 1.0-1.0 was good to excellent; inter-rater reliability for flexion deformities: κw = 0.76; 95% CI 0.67-0.85; and extension deformities κ = 0.75; 95% CI 0.43-1.0 was moderate to excellent.Conclusion: Expected relationships between the NHDC and other measures, stability between repeated measures and acceptable between-rater agreement supports confidence classifying hand deformity in children with cerebral palsy with the NHDC.

Chest wall deformities are structural abnormalities in the chest. While present since birth, chest wall deformities might not become noticeable until children hit their adolescent growth spurt. The most common chest wall deformity, pectus excavatum, is a sunken breastbone that can be repaired with surgery. Another deformity, pectus carinatum, causes the breastbone to protrude out. It is typically treated with bracing. A third, difficult-to-diagnose condition, is slipped rib syndrome. This occurs when cartilage grows abnormally and the ribs rub together, causing nerve pain. On Episode 13 of Back Talk Doc we spoke with Dr. Matt McGirt about Adult Spinal Deformity (Ep. 13 - Adult Spinal Deformity with Dr. Matt McGirt.) It proved to be a popular and valuable episode for our community, so today we’re following up that episode with a discussion of scoliosis and spinal deformity in pediatric patients. Joining us is Carolina Neurosurgery and Spine Associates’ new partner, https://www.cnsa.com/providers/michael-bohl-md (Dr. Michael Bohl). Dr. Bohl attended medical school at the University of Michigan and completed his residency at Barrow Neurological Institute. A U.S. Marine Corps Veteran, Dr. Bohl was involved in humanitarian aid missions in East Africa. While he was not considering medical school at the time, the impact of that experience led him to return to East Africa after leaving the Marine Corp to work as a nurses aid. Dr. Bohl and Dr. Lakhia first differentiate between the terms ‘scoliosis’ and ‘spinal deformity’, which are frequently used interchangeably. (10:17) The latter is the umbrella term which describes a condition wherein the shape of the spine is leading to a problem. Scoliosis, on the other hand, is a type of deformity where the spine shows an S or C-shaped curve. They examine the causes of pediatric spinal deformities and urge parents to be mindful of their child’s posture as they grow. (13:20) Indicators of pediatric scoliosis include a visible curve in the spine, a curve that worsens over time, and whether or not the spinal deformity is causing symptoms like pain or neurological symptoms. The goal of treatment is to keep the curve from getting worse, usually through minimally invasive methods like bracing and physical therapy. (16:32) And, as with many medical conditions, early intervention is the key to more successful outcomes. (21:21) Scoliotic curves get worse during periods of active growth, so a brace can help stabilize and control the growth of the curve. (21:55) Early intervention is important here because once the curve reaches a certain severity the problem is likely to continue getting worse into adulthood. If bracing is ineffective and the curvature is becoming more sever, spinal surgery may be an option. However, in a pediatric patient, smaller, less invasive surgeries are performed to manage the curve and not impede the child’s growth. (25:10)  Candidates for surgery include curves greater than fifty degrees and curves that are rapidly progressing and not adequately responding to a brace. Dr. Bohl stresses that while surgical outcomes are overwhelmingly positive, non-surgical treatment options remain a priority for treating pediatric spinal deformities. So my name is Brittany and here’s my story!!I’m 30 and I live in Michigan and I do live with my parents,I used to be embarrassed to say that because I felt in society’s eyes I should have my life together but I’m learning to care less what people would think. ⁣⁣I’m the only one in my family that’s born with deformities on my hands and feet so I was no stranger to the hospital, I don’t want to say that I’m disabled cause I’m not cause I can do what everybody else can do but I do it a different way. ⁣⁣I’m the second oldest in my familyI have an older sister(3 years apart younger sister (2 years apart)and brother (5 years apart)School growing up was hard for me because I was always picked on cause of my hands and feet. So the people pleaser in me thought if I did this or that they would like me more.After high school I never went to college.I have no kids of my own but I’m aunt to my 7 year old niece and my 5 year old nephew.I use to live in Minnesota for 7 years and l lived in Colorado for a year then moved back to my home state.In the past I’ve worked in the fast food industry and decided to change it up and now, I’ve been at Target for 2 years now as a guest advocate (cashier) and as much as I have stressful days at work ( but who doesn’t) they took care of me when I broke my femur last year.And that brings me to the reason why I started working out cause when I broke my femur I had the worst mindset and became depressed then when I started feeling better and stronger my brother suggested to get a gym membership.Then a friend reached out to me and was like hey I see what you are doing, have you ever thought about being a Beachbody coach.. so I dove into that and here I am now almost a year in January being a coach.So Back in July I was at a Beach Body retreat and we were doing yoga and one moment I was fine and then the next I woke up on someone’s lap. I remember I wasn't feeling good and felt like throwing up but the ladies that were with me(aside of being Beach Body coaches they were nurses, mothers, you name it) they told me that I had a seizure so they called 911 and I was sent off in an ambulance to the nearest hospital. I was there for about a good couple of days cause they did tons a test on me because this never happened to me before and wanted to know the root cause of it.Turns out I have absent seizures so they put me on seizure medicationFast forward to my check up; according to the doctor and my past surgeries I’ve been having these absent seizures ever since I was little.... so basically I’m a walking medical mystery!!With everything I’ve been through I’ve always dealt with it but then one day something set me off and I had a breakdown, a breakdown so severe my parents thought they had to call an ambulance.. cause for a good couple minutes I was having a hard time breathing.Using Facebook and Instagram was my outlet to let it out cause I felt I couldn’t talk to no one because they wouldn’t understand. I even get considered going to therapy because at my lowest time in my life I felt like a beaten dog.So ever since that mental breakdown ofMine I decided enough was enough I’ve been focusing on myself more; Working out and listening to podcasts and it’s a game changer for sure... no therapy yet but it’s still on the back burner.Sorry if it’s all scrambled together and also my punctuation haha I tried to include more details in it.But yes this is my life basically in a nutshell!Support the show (http://cash.app/$thefitboss)

Sunday, July 5, 2020 1 Kings 18:1 And it came to pass after many days that the word of the Lord came to Elijah, in the third year, saying, “Go, present yourself to Ahab, and I will send rain on the earth.” 2 So Elijah went to present himself to Ahab; and there was a severe famine in Samaria. 41 Then Elijah said to Ahab, “Go up, eat and drink; for there is the sound of abundance of rain.” 42 So Ahab went up to eat and drink. And Elijah went up to the top of Carmel; then he bowed down on the ground, and put his face between his knees, 43 and said to his servant, “Go up now, look toward the sea.” So he went up and looked, and said, “There is nothing.” And seven times he said, “Go again.” 44 Then it came to pass the seventh time, that he said, “There is a cloud, as small as a man’s hand, rising out of the sea!” So he said, “Go up, say to Ahab, ‘Prepare your chariot, and go down before the rain stops you.’” 45 Now it happened in the meantime that the sky became black with clouds and wind, and there was a heavy rain. So Ahab rode away and went to Jezreel. 46 Then the hand of the Lord came upon Elijah; and he girded up his loins and ran ahead of Ahab to the entrance of Jezreel. Acts 1:4-8 4 And being assembled together with them, He commanded them not to depart from Jerusalem, but to wait for the Promise of the Father, “which,” He said, “you have heard from Me; 5 for John truly baptized with water, but you shall be baptized with the Holy Spirit not many days from now.” 6 Therefore, when they had come together, they asked Him, saying, “Lord, will You at this time restore the kingdom to Israel?” 7 And He said to them, “It is not for you to know times or seasons which the Father has put in His own authority. 8 But you shall receive power when the Holy Spirit has come upon you; and you shall be witnesses to Me in Jerusalem, and in all Judea and Samaria, and to the end of the earth.” Acts 2:1-2 When the Day of Pentecost had fully come, they were all with one accord in one place. 2 And suddenly there came a sound from heaven, as of a rushing mighty wind, and it filled the whole house where they were sitting. We believe in the prophetic. We believe that God still speaks to His people, by (but not limited to) His logos or written word, but by the Rhema or spoken word of God as well. When I look back over the past couple of years at the things The Lord has prophetically declared over this body there are some things that they all have in common. A prophetic word is a divine invitation by God to release your faith WITH corresponding action toward the fulfillment of that promise. They don’t just happen without your cooperation. January 11, 2019 “I am stirring a hunger, a passion within the spirits of the people of Ignite. A passion that will ignite a relentless pursuit of My presence! Relentless, relentless, relentless! I have not relented in my pursuit of you and from this time forward you will not relent in your pursuit of Me! I reward those who relentlessly pursue Me. I reward them with unimaginable manifestations of my Glory! As you release your worship and pursue me relentlessly I will release waves of my Glory! Waves of Glory that will be spoken of for miles around. My manifest Glory is what you will become known for! People will come from three states away seeking my Glory! People who are hungry and will not be satisfied with anything less than Glory! You purse me and and I will do the rest!” May 31, 2020, Pentecost Sunday The Lord gave me prophetic message concerning the Rain (revival) and this word. “I have not called you to celebrate Pentecost annually, but to live in the power Pentecost daily!”. “I am moving you into a kairos marked by signs, wonders, and miracles and sustained by my presence!” June 28, 2020 “God is raising up a people who have been placed in a position of sonship. Their posture before Him will be one of expectation. Their posture before the world (meaning unbelievers) will be one of love, compassion and empathy. Their posture in the spirit will be one of absolute victory taking territory with no thought for selfish promotion but only Kingdom promotion. They will be compassionate with people, but violent with the spirits driving people!” Relentless pursuit of His presence leads to manifestation of His Kingdom which releases His Power that demonstrates His love, carried out through His people which bring HIM Glory! Many times God will give us a prosthetic word, an invitation, and then allow agitation in our life to move us to respond! These two passages of scripture have something in common that I believe is for us today. Posture in response to the prophetic. Posture and how it relates to the church. Noun; The position in which someone holds their body when standing or sitting. Mike mentioned 3 things that bad posture can lead too. One was Deformities of the spine. Everything in the body is affected by the spine. A misaligned spline pulls on the surrounding muscles, over working some and underworking others. These imbalances causes injures. One of the quickest ways to cause the spine to become misaligned is sitting too much. Not enough movement. The body of Christ as a whole has become deformed, dysfunctional because there has been too much sitting and not enough doing! Some muscles (believers) have become overworked and burned out while others are have become underworked and weak! Muscle when it’s not being used atrophy. It shrinks and looses strength! USE IT OR LOOSE IT! Deformities lead to injuries. The portion of the body that is overworked becomes bitter and it causes division. They become hurt and eventually walk away from the church and in some cases The Lord! Injuries lead to Dysfunction which leads to LOSS OF FUNCTION! The Church as a whole in dysfunctional and has moved away from its original function… What is the original function: To prove the will of God in every situation by demonstrating the Kingdom through the power of God destroying the works of the devil! Digestion issues. We have become obese on knowledge and spiritually malnourished. Noun; A particular way of dealing with or considering something; an approach or attitude. You could easily say it is a perspective. Our posture must be one of The Kingdom of God! Verb; Behave in a way that is intended to impress or mislead. We have postured ourselves to impress the world which has robbed us of power to change the world thereby misleading them! We should focus on impressing The Lord! What determines our posture? How we see a word from God will determine the posture we take. Both Elijah and the 120 received a Prophetic word form The Lord. “the word of the Lord came to Elijah, in the third year, saying, “…I will send rain on the earth.” “… He commanded them not to depart from Jerusalem, but to wait for the Promise of the Father…” “And He said to them, “It is not for you to know times or seasons which the Father has put in His own authority. 8 But you shall receive power when the Holy Spirit has come upon you…” Faith in His word must supersede what we see in the natural! Elijah had plenty of opportunities to give up on the word from the Lord. “So he went up and looked, and said, “There is nothing.” And seven times he said, “Go again.” They were in the upper room for 10 days. Not knowing what the fulfillment of the word would look like. The correct posture brings the desired result! The posture of expectation brings the fulfillment of the Word! There has to be a tenacity in us to grab ahold of His promise and not let go no matter what! “…And Elijah went up to the top of Carmel; then he bowed down on the ground…” Up into His presence and bowed down in worship! Worship will ALWAYS be the correct posture! (not matter what situation you find yourself in worship is always the correct posture!) Why? Because WORSHIP IS THE POSTUE OF OF FAITH! What were they doing all that time in the upper room? I would imagine they were worshipping and praying with expectation of the fulfillment of a promise! The enemy is after your worship! He is trying to stop your worship and redirect it because he ultimately desires to be worshipped. (California mandate) Anytime we focus on it out our trust in someone or something other than The Lord, we are redirecting our worship! The correct Posture brings increase! 44 Then it came to pass the seventh time, that he said, “There is a cloud, as small as a man’s hand, rising out of the sea!” So he said, “Go up, say to Ahab, ‘Prepare your chariot, and go down before the rain stops you.’” At the first indication of rain, Elijah responded as if it were flooding! The response to the drop brings the flood! The response to the little brings that overflow! 45 Now it happened in the meantime that the sky became black with clouds and wind, and there was a heavy rain. He responded before the sky became black… The correct posture releases power! 46 Then the hand of the Lord came upon Elijah; and he girded up his loins and ran ahead of Ahab to the entrance of Jezreel. He was overcome by supernatural power and he outran a chariot! 8 But you shall receive power when the Holy Spirit has come upon you… We have worked hard to preserve pentecostal theology of tongues and missed the power! Today we chat to the first professional female distance runner, Anne Audain. Anne is from New Zealand and was born with deformities in the bones of her feet. She went on to break down the barriers for women runners and in sport in general. A truly inspiring story of overcoming physical and societal roadblocks to achieve what is possible in your life. She was banned (temporarily) in 1981 for accepting prize money at a track and field event. Anne qualified for the Olympic games 6 times in her career (from 1972 to 1992) and has been inducted into the Running USA Hall of Fame, the New Zealand Sports Hall of Fame, and the RRCA Distance Hall of Fame. Today’s Guest Anne Audain was born in New Zealand, adopted as an infant and suffered through her younger years with bone deformities in both her feet. After successful reconstructive bone surgery at age 13, she joined a local athletic club and a remarkable running star was born. Through her career Anne set records and pioneered professional running for women. She has since been inducted into the New Zealand Sports Hall of Fame, the USA Running Hall of Fame, and honored with a Member of the British Empire Medal from Queen Elizabeth II of England for her contributions to her sport worldwide.  What you will learn about: Anne moved to the US thanks to rumors that the US was going to open the longer running distances to women where previously 800/1500 meters were the longest distances women were allowed to run. The difference in running between New Zealand and the US is that NZ is based on the club system vs the school based systems in the US. How she believes the US system burns young runners out. Anne was given up for adoption as an infant as her mother was a teenaged mom. Her adoptive parents noticed that she did not walk properly and she was identified as having a bone deformity similar to large bunions, but the doctors felt she should not be operated on until she was a young teen and her feet were not actively growing as rapidly. The doctors created a leather boot that allowed her to begin to develop the proper walking motion right away. It seemed natural to her when the casts came off to be like the other kids and run with the sports club. She didn’t want to do the field sports because she didn’t want to jump or drop things on her feet so she started running and then kept going finding she was actually good at it. How the bullying she endured as a child and being adopted and viewed as different made her very independent, self-motivated and comfortable with her own company and training alone. She felt she wanted to run as hard as she could herself, and if she got beaten, so be it. So she didn’t run tactically, she just ran as hard as she felt she could each time and had done her very best. If that meant she was competitive, then she was competitive with herself and the clock.  How Anne found her birth parents in her 30s and found they had married a year after giving her up as an infant and she had 6 siblings. Her birth family had watched her as a runner and didn’t realize she was their daughter/sister. Her adoptive parents were supportive and her dad had raised her as an observer of sport so she was prepared to participate when her body was ready. How she ran the fastest debut marathon before the first Olympic marathon that allowed women to run but New Zealand still required her to prove herself before allowing her on the team. As a result she had to run only a few months before the Olympics to qualify and then failed to finish at the Olympics as she ended up in the medical tent with dehydration. How she had watched her male counterparts get paid under the table for doing European races but that women got nothing. She heard about a Portland race that Nike was offering prize money to the first ten male and female finishers and went ahead and said she would accept prize money if she qualified. At the time track sport was still an amateur only event, and she was temporarily banned by the NZ Sport Federation when she won and accepted the prize money. She also was in the US on a tourist visa so was not allowed to accept prize money and was eventually deported. She put her trust in the race directors and those who wanted the sport to change and allow prize money. How she won over 75 road races and believes in consistency of training and going back to the same races where she was familiar with the course. Deformities, both congenital and acquired, have a profound and often negative impact in the Emerging World. Deformities may be internal as well as external. Though internal deformities may not be seen they are often more severe and debilitating. The negative effects of external deformities are seen world-wide and in every culture and civilization. In the West, children born with deformities are often operated on early and may not be seen in public. Though family members may often ask “why us,” any negative thoughts are kept to themselves. In the emerging world, children with deformities, especially external ones, are often kept inside the house where they are hidden from view, and they are only taken outside with a shawl or blanket covering the deformity or at nighttime. Treatment for deformities is often not readily available in the developing world. When they are seen by other members of the community, some think the family has been cursed or that the deformed one is indwelt with an evil spirit. When these children are brought to a mission hospital, reconstructive surgery may not completely eliminate the deformity or its consequences; however the significant improvement allows the children to be accepted into the local society. The treatment of these children gives the missionary doctor and hospital staff a great opportunity to show the love and compassion of Jesus Christ and many parents and older children have come to know Christ as their Savior.

For several years biologists were noticing fairly widespread malformations among amphibians - frogs missing legs or having extra legs. There was much speculation about the cause of this disfigurement, including heightened suspicion of pesticides and other chemicals. Researchers have recently uncovered what may be at least part of the explanation for legless frogs – dragonflies.

Chest wall deformities are structural abnormalities in the chest. While present since birth, chest wall deformities might not become noticeable until children hit their adolescent growth spurt. The most common chest wall deformity, pectus excavatum, is a sunken breastbone that can be repaired with surgery. Another deformity, pectus carinatum, causes the breastbone to protrude out. It is typically treated with bracing. A third, difficult-to-diagnose condition, is slipped rib syndrome. This occurs when cartilage grows abnormally and the ribs rub together, causing nerve pain. Medical and surgical options can be used to treat slipped rib syndrome.On the Mayo Clinic Q&A podcast, a special edition of "Ask the Mayo Mom" focuses on chest wall deformities in children.
