In this episode, Dr. Jonathan Chung and Dr. John Stenberg discuss the biopsychosocial model of pain and its impact on cervical chiropractic care. They emphasize the importance of individualized care and the limitations of the biopsychosocial model. They share personal experiences and highlight the role of stratifying patients and understanding biomarkers. The conversation also explores the nocebo effect and its impact on patient psychology. The episode concludes with a discussion on teaching patients to control pain and understand flare-ups. This conversation explores the importance of managing patient expectations, avoiding nocebos, and using patient-centered communication in upper cervical chiropractic care. It emphasizes the need to be mindful of language and to avoid using scare tactics or exaggerated claims. The conversation also highlights the significance of the therapeutic relationship and understanding the patient's life and circumstances. By focusing on empowering patients and promoting their resilience, chiropractors can provide more effective and meaningful care. Takeaways: The biopsychosocial model of pain expands our understanding of pain medicine beyond just tissue injury and emphasizes the complex interactions of biological, psychological, and social factors. Individualized care is crucial in cervical chiropractic to address both the bio and psychosocial aspects of a patient's condition. Stratifying patients based on their presentation and understanding relevant biomarkers can help provide more targeted and effective treatment. The nocebo effect, where negative beliefs and expectations can lead to negative outcomes, should be considered when communicating with patients. Teaching patients to control pain and understand flare-ups can empower them to manage their condition and improve their outcomes. This conversation between Dr. John Stenberg and Dr. Cameron Bearder explores the topic of post-traumatic cranial cervical disorders from a postural control perspective. The discussion covers the mechanisms of insult with mild traumatic brain injury and whiplash injuries, as well as their impact on postural control. The conversation also delves into the biopsychosocial aspects of managing patients with prolonged recovery and the importance of assessing psychosocial implications. The chapters cover various systems involved in postural control, including the ocular sensorimotor system, vestibular system, vestibulospinal system, cervical spine, and stomatognathic system. Click here for the full text PDF: Post-Traumatic Craniocervical Disorders From a Postural Control Perspective: A Narrative Review Takeaways Post-traumatic cranial cervical disorders can result from mild traumatic brain injury and whiplash injuries, leading to symptoms such as dizziness, vertigo, headache, neck pain, visual complaints, anxiety, and neurocognitive dysfunction. Psychosocial factors, including pre-existing anxiety, depression, and post-traumatic stress disorder, can affect the expression of symptoms and contribute to prolonged recovery in patients with post-concussion syndrome and whiplash-associated disorders. Impaired postural control is commonly observed after cranios cervical trauma, involving the ocular sensorimotor system, vestibular system, vestibulospinal system, cervical spine, and stomatognathic system. Manual therapy, patient education, range of motion reassurance, strengthening exercises, and multimodal care are useful interventions for managing neck pain and post-traumatic cranial cervical disorders. TMJ disorders can be a potential cause of post-traumatic cranial cervical disorders, and their diagnosis relies on a history of TMJ complaints associated with movement, local discomfort, and physical examination. When evaluating patients, it is important to track progress over time and consider the different systems that may be affected. Treatment of temporomandibular joint disorder (TMD) and related headaches may have poor outcomes when associated with whiplash. Postural control is influenced by ocular sensorimotor, vestibular, cervical proprioceptive, and somatic-nathic systems, which may be damaged by cervical spine trauma. Future research should consider all these systems from a postural control perspective in the context of cranioservical trauma. Stay tuned for these episodes each month as we continue throughout the Diplomate in Chiropractic Craniocervical Junction Procedures program over the next 3 years. This is the FIRST and ONLY behind the scenes look at how the doctors grow through this process. Takeaways The Diplomate Program is a higher level of study and specialization in chiropractic, demonstrating advanced knowledge and proficiency in a specific topic. Cognitive dissonance is the discomfort experienced when confronted with conflicting ideas, and it is important to approach it with an open mind and allow for further exploration and understanding. The practical application of chiropractic knowledge should focus on improving patients' health and function, rather than solely on structural alignment. The upper cervical community can benefit from further development in the areas of science, art, and philosophy to enhance clinical outcomes and understanding. The philosophy of chiropractic is stable but may require advancements based on new scientific discoveries. Advancements in science have significantly grown in the field of chiropractic over the years. Challenges exist in improving techniques due to the influence of personal experiences and the resistance to change. The fallacy of mastery assumes that one's current approach is correct, even if it may be incorrect. Openness to change and willingness to adapt techniques based on compelling scientific evidence is essential for progress. Learning from others and engaging in conversations with peers can lead to personal growth and new perspectives. Documenting thoughts and experiences allows for reflection and continuous improvement in chiropractic practice.
