Alex Paradies educates us on how root cause analysis can be used to improve industrial operations.

We don’t find it surprising that you find 5-Whys root cause analysis lacking. The lack of effectiveness of 5-Whys RCA has been shown in simple investigations and major investigation. Even smart, well-trained users (Like Taiichi Ohno, himself) frequently jump to conclusions, follow a single root cause trail, and fail to adequately analyze the causes of human error. That’s why we find it so surprising that people still recommend 5-Whys root cause analysis because it is “easy.” Easy but wrong is the wrong answer when you are counting on your root cause analysis to save lives, stop injuries, improve equipment reliability, or stop quality issues. And there are other techniques that are easy but effective. Let’s look at one of the most famous 5-Why examples used by Taiichi Ohno), who created the 5-Why root cause analysis technique. He is quoted using the following 5-Why example to teach 5-Whys to others. Here is an example: 1. “WHY did the robot stop?” The circuit has overloaded, causing a fuse to blow. 2. “WHY is the circuit overloaded?” There was insufficient lubrication on the bearings, so they locked up. 3. “WHY was there insufficient lubrication on the bearings?” The oil pump on the robot is not circulating sufficient oil. 4. “WHY is the pump not circulating sufficient oil?” The pump intake is clogged with metal shavings. 5. “WHY is the intake clogged with metal shavings?” Because there is no filter on the pump. That’s it. The fifth WHY.  Taiichi Ohno says the “Root Cause” is:  “NO FILTER ON THE PUMP” Would you call that a root cause? I wouldn’t. What would the corrective action be? Probably to install a filter on the pump. What is wrong with this root cause analysis? To find out, we would have to dig deeper into the failed robot’s root causes. Provides a clear picture of what happened and clarification of the incident so we can identify all the areas of potential improvement      b. Can be used as planning and data collection tool      c. SnapChart® is very simple to do      d. Early versions will be very simple, assumptions and missing info, fill that info in with (3 P &R), interview techniques. Have conditions detailing the events. A s/c with 0 conditions is an indication of incomplete. Answers to our dictionary questions like to live in the conditions. “What do we know about each event?”      g. Remove blame focus in our SnapChart®      h. Why?      i. How?      j. Use on action and 1 verb per event avoid “and”      k. Use active voice opposed to passive voice to provide clarity      l. Ex “Pump 2B stop button is pushed by Operator A vs Operator A pushes 2B stop button     m. Be specific and precise      n. Quantify – instead of “hot” temperature was 100 F      o. Be neutral and objective      p. While TapRooT® occasionally utilizes discipline as a corrective action that must mean it is sometimes effective, right? But if you think about it, most people don’t do things wrong just for the heck of it unless somewhere they think it’s probably okay. They saw their co-worker take a shortcut just the other day and their manager told him “job well done” for getting the task done so quickly. Time is money, right! Time is money when things go right and when they go wrong. So, this time you took the same shortcut as you’ve seen others do, but this time something went wrong and now they have had to shut things down (at a minimum). Downtime is money too. You knew it was a shortcut, you knew you skipped a step in the procedure, but so does everyone else. What should the company do with you now that something bad happened? If your company is great at enforcing its policies, you know if you take a certain shortcut to get done quicker than something negative will happen to you. There will be a consequence for not following the procedure and your company always enforces it. If that’s your company, then you are likely to think twice before taking that shortcut. However, if you are at a company where people feel it’s okay to take shortcuts to get things done quicker (hoping all will be well and nothing bad will occur by taking that shortcut), it gets easier and easier within that company’s culture to take some shortcuts to get the job done. Because other people do the same shortcut and they sometimes get a pat on the back! But this time something bad occurred. Are you at fault for taking a known shortcut and then need to be disciplined? Not necessarily. This topic is near and dear to Marcus Miller’s heart. Listen in as he discusses how enforcing boundaries and policies along the way consistently can help deter people from taking the wrong actions to begin with. Then if a shortcut is taken by an employee when there is a known constant, negative response to that action, perhaps then, discipline might fit. Even then when we dig deep enough, we can probably still find a deeper cause. Taking the time to investigate properly to get to the real root causes of why an incident happened is worth it! Benna and Ken discuss an example of how Equifactor® troubleshooting leads to human error root cause analysis.

Here is what the DOJ has to say about corporate compliance programs and sentencing for corporate crimes: The Principles of Federal Prosecution of Business Organizations require prosecutors to assess “the adequacy and effectiveness of the corporation’s compliance program at the time of the offense, as well as at the time of a charging decision.” JM 9-28.300. Due to the backward-looking nature of the first inquiry, one of the most difficult questions prosecutors must answer in evaluating a compliance program following misconduct is whether the program was working effectively at the time of the offense, especially where the misconduct was not immediately detected. U.S. Department of Justice Guidelines The DOJ guidelines for federal prosecutors says: To determine whether a company’s compliance program is working effectively at the time of a charging decision or resolution, prosecutors should consider whether the program evolved over time to address existing and changing compliance risks. Prosecutors should also consider whether the company undertook an adequate and honest root cause analysis to understand both what contributed to the misconduct and the degree of remediation needed to prevent similar events in the future. The DOJ guidelines say that the company should document the company’s response to a compliance issue including discipline taken or remediation (corrective actions). The guidance specifically says: Have the company’s investigations been used to identify root causes, system vulnerabilities, and accountability lapses, including among supervisory managers and senior executives? What has been the process for responding to investigative findings? How high up in the company do investigative findings go? DOJ says: Finally, a hallmark of a compliance program that is working effectively in practice is the extent to which a company is able to conduct a thoughtful root cause analysis of misconduct and timely and appropriately remediate to address the root causes. These two guidelines imply that your root cause analysis system should identify Management System root causes and have SMARTER corrective actions. Importance of Compliance Your corporate counsel can explain the importance of your compliance program IF you get in regulatory trouble. But from the brief explanation above, you need to know about advanced root cause analysis and you need to have the best root cause analysis program implemented to reduce your potential liability under the sentencing guidelines. But there is an even MORE IMPORTANT reason to have the best root cause analysis as part of your compliance program. Why Virtual RCA Training?This may seem obvious since many training providers took their live courses and started presenting them virtually when the COVID pandemic struck. But we knew the value and quality of our live face-to-face training and we saw many poor quality virtual courses that just didn’t deliver. Therefore, we waited. We hoped that COVID would pass and we could return to effective, live, face-to-face training.However, by June COVID was still around. Some companies were starting to hold live, face-to-face training or sending people to our public courses, but many were begging us to provide a virtual course experience that was just as effective as our 2-Day TapRooT® Root Cause Analysis Training.We could see that COVID was sticking around and that many TapRooT® Users might wait until mid-2021 to have live training, so we decided to try to produce training to match the quality of our 2-Day TapRooT® Course.Therefore, we started assessing various virtual training platforms and testing ways to provide virtual training that would be interactive and effective.Virtual RCA Training DevelopmentWe knew that just providing our 2-Day TapRooT® Root Cause Analysis Course virtually was not an option. Why?Eight hours of virtual training in a day was just too long.Our instructors were practiced at making live, face-to-face training interactive, but this was more difficult in the virtual classroom.The classroom environment is different than the virtual environment. We knew that picking the right training delivery software environment would be important.We didn’t know how many people we could effectively teach in a virtual course.Therefore, we started testing different software virtual delivery systems and experimented with changes to content and timing.We found a delivery system that provided a great virtual training experience that included virtual breakout rooms for team exercises.We found that students could remain alert and interactive in three-hour segments that were structured with the right amount of interactive exercises, instructional content, exercises, and breaks.We started with 10 person classes with two instructors to allow good interaction between students and instructors and provide an instructor to handle technical challenges while the other instructor taught.We found ways to add quizzes, exercises, and software practice to the course to keep people involved.Virtual RCA Training Testing & RefinementBy early July, we had a draft course that was ready for testing. Our first test used our staff and our contract instructors. We tested:Our course material conversions to a virtual environment.The timing of segments, breaks, and lunch.Various quizzes and new exercise ideas.The number of people to have in a virtual course and the capacity of our wifi with multiple students and breakout rooms.How to coach the use of TapRooT® Software in a virtual environment.The studio needed to conduct an effective course.Ken teaching in the virtual classroom studio.It was now August. After testing on our staff, we partnered with three TapRooT® Users (three different companies) to test our refined virtual course with a follow up post-course coaching and evaluation regimen to ensure the training was effective.The testing continued through November. We found that:The six-hour per day, three-day course was effective and received excellent student reviews.Post-course coaching of the student’s first application of the TapRooT® System was very effective to reinforce the classroom learning. Not only did this question and answer/coaching session after their first investigation provide the students with a chance to ask questions about their use of TapRooT® RCA, but it also provided the instructors with feedback on the students’ use of the techniques and ideas of ways to make the training more effective.Some of our initial post-course training (we initially held two post-course mini-review sessions plus an incident review) wasn’t needed and could be dropped to save student’s and instructor’s time.Some quizzes and exercises could be modified to simplify the course administration but keep the interactive elements of the training,We could increase the number of students in the class to 16 and reduce the number of instructors to one once the instructors were fully familiar with the virtual training environment and the new course materials.During the later part of the testing, We started testing contract instructors’ wifi connections to ensure the needed capacity for conducting multiple, simultaneous courses and certifying more of our instructors to teach the new virtual RCA course.When the testing was complete, we held two review and refinement meetings to improve the course once again.That brought us to the end of the year.Virtual TapRooT® Root Cause Analysis CourseWith all this course testing and refinement, what did the virtual course content turn out to be?
