This podcast was motivated by over a decade of patients telling and encouraging us to record our conversations and perspectives on various health subjects. For years we kept talking about needing to do it and finally after kicking the can down the road long enough we launched our podcast in January of 2023. Over the years, we would have conversations over the phone on our commute home. We would talk about the various things we would see in our office that day, discuss all the potential complexities of a case, the patient history that was missed from other providers. This would eventually lead to us ranting and going on and ultimately passionately discussing and advocating for a better path going forward for those patients. We would often end those calls saying that we should have recorded that! These conversations and our daily strive to help educate and empower our patients and community made this launch into a podcast not only make sense but ultimately inevitable. We pride ourselves on fighting for patients to get better care, better answers and empowering them to take control of their health and life. This podcast is an extension of this wider push for empowering more people through education and asking for nothing in return (well maybe a share and pass long to those who would benefit from listening). In the world of functional and integrative health there are varying shades or providers. The hope is our podcast will help listeners be better interviewers of providers out there and weed out the ones that like the sound of being an "integrative/Functional provider" but are still practicing an outdated, protocol driven and non specific model of healthcare. The world of chronic disease is growing by the minute and more and more are being impacted by chronic conditions and need providers who are willing to listen, dig deep, be humble and be willing to put the patient first above all else to figure out why the individual is where they are. We found our way into the functional health world first with functional neurology and becoming experts in brain and neurological health. Quickly into this journey you realize the brain can not thrive without proper metabolic and cell health, Immune and cardiac function and optimal biochemistry...ultimately leading us to layer both functional and integrative medicine and functional neurology into our clinical approach. In this episode we jump in and address why someone can have optimal levels of hormones on labs but still not see or feel the results of "so called" optimal hormones. Unfortunately, the majority of people get prescribed hormones and yet the provider does not address: 1-Why their hormones were low to begin with and 2-What can prevent hormones from finding their landing/receptor site on the cell... essentially making them less effective, even at an optimal dose. The net result... people having hormones that are normal or often even high normal but still have all the same damn symptoms that led them to go on the hormones to begin with. Initially, they may see a month or two of mild to moderate benefit and then this eventually goes away because all the ways our Landing sites for hormones get beat up and less efficient, goes overlooked and forgotten. In this episode on Gut Health, we dive deep on the the importance of a great functioning gut, what's is normal and what happens when it starts to break down and become problematic. Most commonly known as leaky gut, intestinal permeability is the more accurate and the term that will help track down better research on the subject matter.Many try diets, detoxes but do they truly address the enough of the stresses that lead to leaky gut? The truth is many do not and so it is a repetitive cycle of attempts to solve the issue. The key aspect to the anatomy of the small intestine that determines if the gut is leaky or not is referred to as Tight Junctions and Tight Junction Proteins. In this week's episode we tackle headlines from the month in research and have a candid analysis and discussion around each article.This is a common in office discussion that our team naturally has and we figured we would do an episode reflecting our common discussion and insight of the various headlines.There is so much research being published each day, week, and month. What influences which ones reach the media outlets and the public eye? Is it their significance? Is it their financial ties? Each day there are hundreds to thousands of articles published across the globe on various health subjects. Weekly, there are several thousand published and monthly this number easily hits tens of thousands.What is even more astonishing is that last year over 10,000 articles were retracted, setting a new annual record. This number of retractions reflects issues with an increase in fraudulent papers and compromised peer-reviewed processes. An article is retracted because there are findings that question the merits of the article and its findings and/or its legitimacy. Now how often do we hear about the hype of a new article but fail to hear if that article has later been retracted? This happened in the case of COVID 19 and early treatment using a pre-existing medication. Major media outlets pushed the retracted article and offered no apology when the article was later retracted. This is despite numerous articles demonstrating benefit and yet one article that was soaked up to push away from the majority of these other articles. This is why it is critical to always question and allow discussion. The moment the ability to question and have open discussion is taken away around a subject is the moment one should have suspicion on the article in reference. The world of Dysautonomia and all the various symptoms and conditions that fall under this broad heading, can leave people impacted by it, feeling miserable, defeated and like their world is limited due to their inability to properly function within it.Dysautonomia is an umbrella term used to described disorders related to the autonomic nervous system (ANS). Anxiety impacts a large portion of our population and it can look and take different forms in various segments of the population. Mast Cell Activation Syndrome, MCAS for short, is likely something you may not have heard of but can guarantee you know someone that is impacted by the complexity of this diagnosis and all its potential symptoms. Unfortunately, there is a strong chance that your healthcare provider may also not be aware of this diagnosis and how many of your symptoms may be connected and due to MCAS.Mast Cell Activation Syndrome is estimated to impact 17 to 20% of the population and is a relatively new diagnosis. Most impacted will not know they have MCAS and will likely have multiple other names for all the symptoms that can be attributed back to MCAS. Migraine headaches, irritable bowel syndrome, allergies, low blood pressure, elevated heart rate, itchy red and reactive skin, itchy eyes, anxiety, bone density issues, brain fog, diffuse numbness and tingling, extreme fatigue, dizziness, fainting and lightheadedness, chronic nasal congestion, pelvic pain and bladder urgency to name a few are all potential symptoms that may be due to MCAS.What is MCAS? Mast cells are essential immune cells for the immune system and help provide our body with some of the quickest immune reactions. When appropriate this allows for quick and proper responses to initiate protection and ultimately recovery. In MCAS, mast cells begin to overreact and ultimately respond inappropriately to ordinary stimuli. When this happens, they dump their chemical and immune mediators into circulation, leading to potential local and systemic responses.The best recognized chemical we know released from mast cells is histamine and then likely followed by tryptase and heparin. Love it or hate it, the world of weight loss is a growing one as well as the discussion around it. We are seeing the need for weight loss solutions and strategies increase over the years as well as seeing stubborn weight loss become more common. What is the reason behind this pattern of weight gain? Detoxification, also known as biotransformation, too often can be over simplified and made "gimmicky" by many wanting to simplify an incredibly deep and complicated process. This process of taking chemicals and eliminating them from our cells, tissues and circulation and moving them out of our body is incredibly important and when it becomes problematic can lead to numerous health issues that are not always immediately correlated with impaired detox. Some providers and health coaches like to sell detox as a simplified process requiring a protocol of supplements and products and unfortunately this "protocol" approach is not always the right fit for many with ongoing health issues.To make matters worse, it is very common for the traditional medical world to underestimate the world of detox and how essential this process is to the human body. When you hear the list of side effects certain medications have, often these are due to their impact to our detox and processing pathways. When we look at the world of genetics and why some are more vulnerable to certain medications, again this often is due to how these genetic snips impact our liver pathways involved with processing certain chemicals. These points illustrate the vital importance of optimal and intact detoxification pathways and enzymatic processes.In this episode we review the numerous considerations that should go into supporting this process, from routine labs to cell health and mitochondrial health. In Episode 10, We dive into the world of Neurobehavioral disorders, from Autism, ADHD, OCD, Tourettes. We touch on the difference between acquired brain injuries to developmental disorders.Currently, up to 1/5 kids under seven have been or will be diagnosed with a neurobehavioral or developmental disorder. This number has been growing and increasing at an alarming rate with no slow down in sight.During the scope of this podcast we discuss what are some essential tools that can be added to the neurological examination, from oxygen and fuel metrics and monitoring fatigue to primitive reflexes. We all should be born with primitive reflexes but as the brain develops and integrates properly we see the higher brain centers develop and begin to suppress these initial survival promoting reflexes. By 18 months of age these reflexes should be non-existent until a delay in development or an acquired brain injury has occurred. We discuss how these reflexes can be good baselines and gauges for where to focus your sensory therapy and which regions of the brain to drive neuroplasticity.As we wrap up discussing how to maximize the neurological exam we morph into additional testing that can provide valuable insight into the metabolism of those suffering with neurobehavioral and developmental disorders.Since the brain is constantly active and never slows down function, the demand for oxygen and glucose are incredibly high for the overall size and weight of this prized organ. Mastering and bolstering your traditional labs to be confident and oxygen delivery is great is the first line of testing then using again routine labs to evaluate for any inflammatory or infectious etiology is also critical. PANDA's is one presentation that can be due to an infectious cause and it is essential to capture this early and get the right team of providers in place to address these aspects to brain health.Lastly, we discuss the importance of the "mitochondria" a key part to cell health and how well we breakdown cells that have been misguided and how well we create the energy to make all the developmental connections to hit our milestones. In Episode 9 I sit down and have the pleasure of interviewing NHL Alumni and recently retired American Airlines Captain, Al Secord.Over the last couple of years we have been fortunate to get to know Al off the ice and he is by far one of our favorite people to sit down with and chat. From hockey to health, Al is honest and passionate about everything he touches in life and this makes for great conversation as well as life perspective.Drafted in the first round of the NHL draft, 16th overall, to the Boston Bruins Al began a very successful NHL career in 1978. Al made the Bruins roster as a 20 year old and in our podcast we discuss what his first taste of an NHL camp was like and what it took to make the roster of the an NHL team.Al had his best season while playing for the Chicago Blackhawks where he scored 54 goals and 32 assists in the 1982-83 season. In total Al played 766 games in the NHL and notes he was told he was in approximately 170 fights. That is approximately one fight every 4 to 5 games for 12 years. This was at a time where the NHL was like other contact sports and did not fully know the long term effects of concussions and traumatic brain injuries.After Al retired from the NHL he started his second successful career as a pilot. Al eventually was hired by American Airlines where he served as captain and just recently retired last month after a long and successful career in the air.Regardless if it was the NHL or the American Airlines, what we love best about Al is how he pours his heart into everything he touches and it is contagious when you sit down and talk with him.In our podcast we discuss his journey as an NHL player, American Airline Pilot, parent and health advocate and what he has done to keep his health in tip-top shape. 65 years young and Al looks like he is in his 20-30's on the ice, keeping up with the younger generations still on the ice.Al shares his experience with the Amen clinic and his brain scan and we discuss how his follow up brain scans at the Amen clinic actually improved over the years as he implemented various brain based strategies to support and improve his brain function. Pain is a vital sensory sense that we need to be able to survive but when it is felt at the inappropriate time it can turn one's life upside down.In an ideal scenario Pain should function as an alert signal to protect us from further injury but when it becomes chronic and persistent and is no long associated with a threat or injury it quickly can become debilitating.In this episode we explore how pain messages are relayed from the periphery to the spinal cord and up to the brain for perception. We discuss how the brain has ways to adapt to pain and modify pain from perception as well as modifying at the level of the spinal cord.We also discuss how we have many other pleasant senses that are there to be felt before pain and ultimate protect us from long term pain.Pain can be divided up neurologically in 2 main ways, Peripheral pain and central pain. Peripheral pain refers to the receptors in the skin that sense pain, the wires and pathways that bring them to the spinal cord and the biochemistry that can increase how easy these nerves can relay brain to the central nervous system. Central pain refers to the brain and the spinal cord and how pain can become intensified due to failure in these central systems.Quick example when someone gives you an anti-inflammatory they are trying to modify pain in the periphery versus when one is prescribed an SSRI medication it is looking to target central mechanisms modifying pain.This episode starts to dig into why pain pathways can become chronically turned on and what things need to be considered.We also discuss what labs can help identify some sticking points in why the pain has become chronic.This will likely morph into a second and third podcast where we go deeper into more and more of the biochemistry, lifestyle influences as well as our team approaches and our therapies we often use to break the cycle of chronic pain for our patients.We hope you enjoy!

We see athletes from all levels, from High school to professional and there is one common theme…that concussion and baseline testing of the brain and its performance can be much better.This episode dives into the current trends in concussion baseline testing as well as what other options are out there to help refine and improve the assessment and ultimately improve brain performance and safety.Both Drs Rayner and Nutt have played sports at a high level, from lacrosse to hockey, and know first hand that getting a concussion can be devastating. Even more devastating is when you, your child or the athlete returns too soon before a TRUE return to normal brain function has occurred.Your baseline concussion is only as good as what you initially and consistently measure and when it comes to athletes most baseline brain assessments do not measure much beyond cognitive function. Using measures like the VNG to assess eye movements has been demonstrated to be a predictable measure of brain performance and pull in many regions of brain function. Eye movements accurately capture the frontal lobe, parietal lobe, vestibular and cerebellar function as well as regions of the brain stem. Not only does this allow for better baseline testing this can also identify areas of weakness in the athlete that can be targeted and improved with functional neurological techniques that drive neuroplasticity in these areas. This ultimately leads to a better athlete that can interact with their environment more precisely.
