She shares the common signs of ADHD in adult women—spoiler: it's not always what you think—and opens up about her experience starting Vyvanse, including what it should feel like when medication is working well. Oh, and because life never slows down, Renee also takes us through the chaotic rollercoaster of her day… because, of course, everything had to happen at once. If you've ever felt overwhelmed, scattered, or like your brain has 37 tabs open, this one's for you. Learn more about your ad choices. Visit megaphone.fm/adchoices

In this episode, we discuss the gut-immune axis and Small Intestinal Fungal Overgrowth (SIFO), with a mechanistic focus on immune activation and epithelial barrier disruption. We detail contributing factors to and symptoms of SIFO. We further discuss the anatomical positioning and immunological functions of mast cells within the lamina propria, highlighting mast cell activation syndrome, while detailing how SIFO and SIBO can serve as upstream triggers for mast cell activation.Topics:1. The Gut-Immune Axis and Mast Cells - The intestinal epithelium forms the innermost selective barrier, coated in a protective mucus layer.- Immediately beneath lies the lamina propria, rich in immune cells including mast cells. 2. Mast Cells in the Lamina Propria - Mast cells are positioned near nerves, capillaries, lymphatics, and epithelial cells within the lamina propria.- Their anatomical location enables rapid immune surveillance and inflammatory response.- Mast cell granules store histamine and more. 3. Mast Cell Mediators - Histamine: Biogenic amine, inflammatory signaling.- Tryptase: A serine protease.- Cytokines and lipid mediators also released upon activation. 4. Mast Cell Activation Syndrome (MCAS) - Systemic: skin, respiratory tract, cardiovascular system, nervous system, and more.- The gastrointestinal tract.- Common triggers for mast cell activation. 5. Transition to Small Intestinal Fungal Overgrowth (SIFO) - Overgrowth of fungal organisms, often candida species, within the small intestine.- Contributing factors including hypochlorhydria, slowed motility, immune suppression, disrupted bacterial populations.- Overlap: SIFO, SIBO 6. Hypochlorhydria and the Stomach's Defense Role - Parietal cells secrete hydrochloric acid (HCl) and intrinsic factor.- HCl denatures proteins and sterilizes ingested pathogens.- Chief cells secrete pepsinogen, which becomes pepsin in acidic conditions to digest proteins.- Consequences of Reduced Gastric Acidity 7. The Role of Intestinal Motility - Enteric nervous system (ENS), myenteric and submucosal plexuses.- Coordinated contractions preventing stasis and microbial overgrowth. 8. Common Symptoms of SIFO and clinical overlap with SIBO 9. Candida and Fungal Pathophysiology in the Gut - Yeast form, regulated by microbial competition and immune defenses.- Hyphal transformation.- Degrade the mucus layer, disrupt epithelial integrity, trigger inflammation. 10. Secretory IgA and Mucosal Defense - Chronic stress, immune dysfunction, dysbiosis can lower sIgA levels and weaken mucosal immunity. 11. SIFO, SIBO, and Mast Cell Activation 12. Conclusion - Root cause approach, multi-factorial Thanks for tuning in!Get Chloe's Book Today! Celiac Disease 101: Signs, Testing, and What to Do Next Could Gluten Be Making You Sick? Here's What You Need to Know About Celiac Disease Are you struggling with digestive issues, fatigue, or brain fog but don't know why? You might have celiac disease, a condition that affects 1 in 100 people worldwide—but shockingly, most people remain undiagnosed. If gluten is harming your health, it's time to take action. In this post, we'll cover: ✅ What celiac disease is and how it affects the body ✅ Common symptoms (including the hidden ones most doctors miss!) ✅ How to get tested for celiac disease ✅ The first steps to take if you're diagnosed Let's dive in! What Is Celiac Disease? Celiac disease is an autoimmune disorder, not just a food allergy. When someone with celiac disease eats gluten (a protein found in wheat, barley, and rye), their immune system attacks the small intestine, causing inflammation and damage to the villi—tiny structures responsible for nutrient absorption. Over time, this leads to nutrient deficiencies, digestive issues, and serious long-term health problems. Unlike gluten sensitivity, which causes discomfort without damaging the intestine, celiac disease is permanent and requires strict, lifelong avoidance of gluten. Common Symptoms of Celiac Disease Celiac disease doesn't look the same for everyone. Some people experience classic digestive symptoms, while others have silent celiac disease, meaning they have no obvious stomach issues but still suffer from long-term damage. Here are some key symptoms to watch for:
“You can give someone a survivorship care plan, but just giving them doesn't mean that it's going to happen. Maybe there's no information about family history. Or maybe there's information and there's quite a bit of family history, but there's nothing that says, ‘Oh, they were ever had genetic testing,' or ‘Oh, they were ever referred.' So the intent is so good because it's to really take that time out when they're through with active treatment and, you know, try to help give the patient some guidance as to what to expect down the line,” Suzanne Mahon, DNS, RN, AOCN®, AGN-BC, FAAN, professor emeritus at Saint Louis University in Missouri, told Jaime Weimer, MSN, RN, AGCNS-BS, AOCNS®, manager of oncology nursing practice at ONS, during a conversation about breast cancer survivorship. Highlights From This Episode “I think the biggest thing is to really communicate is that people are living with breast cancer for a long, long periods of time, and a lot of that with really good quality overall.” TS 4:07 “As a general rule, they're going to be seen by the breast surgeon probably every four to six months for a while. After about five years, a lot of times people are ready to say, ‘Okay, annually is okay.' And eventually they may let that drop off. But it also depends on did they have a mastectomy? Did they have breast conserving surgery? And then if they had reconstruction with an implant, how often do they see the plastic surgeon? Because they need to check integrity of the implant. So those schedules are really individualized.” TS 13:24 “When you think about long-term effects, I think you need to kind of think about that survivors can have both acute and long-term chronic effects. And a lot of that depends on the specifics of the treatment they had. I think as oncology nurses, we're used to, ‘We give you this chemotherapy or this agent, and these are the side effects.'” TS 15:36 “The diet issues are huge. And I think we are slow to refer to the dietician, you know, you can get them a couple of consults and because you're saying to them, ‘This is really important. We need you to lose weight or we need you to eat more of this.' Ideally, fruits and vegetables are going to be about half of your plate. And what's the difference between a whole grain and not, less processed foods, making sure that they're getting enough protein. And then once again, really kind of making sure that they're not taking a lot of supplements and extra stuff because we don't really understand all that fully and it could be harmful.” TS 34:53 “Breast cancer is a long, long journey, and I think you should never underestimate the real difference that nurses can make. I think they can ask those tough questions. And I think ask the questions that are important to patients that patients may be reluctant to ask. I think giving patients permission to talk about those less-talked-about symptoms and acknowledge that those symptoms are real and that there are some strategies to mitigate those symptoms.” TS 42:28

Common Symptoms of Low TestosteroneLow testosterone, also known as hypogonadism, can manifest in a variety of ways. Here are some of the most common symptoms:Decreased libido (sex drive)Erectile dysfunction (ED)FatigueReduced muscle mass and strengthIncreased body fatDecreased bone densityMood changes, such as depression or irritability Cognitive difficulties, such as trouble concentrating or remembering thingsHair lossDecreased testicular sizeIt's important to note that these symptoms can also be caused by other medical conditions, so it's essential to consult with your doctor for a proper diagnosis.Total Testosterone vs. Free TestosteroneTotal testosterone measures the total amount of testosterone in your blood, both bound to proteins and unbound.Free testosterone measures the amount of testosterone that is not bound to proteins and is biologically active. While both measurements are important, free testosterone is often considered a more accurate indicator of testosterone deficiency because it reflects the amount of testosterone that is available to your body's tissues.Normal Testosterone RangesNormal testosterone levels can vary slightly depending on the laboratory and the specific assay used. However, in general:Total testosterone: 300-1,100 ng/dL (nanograms per deciliter)Free testosterone: 50-210 pg/mL (picograms per milliliter)It's important to note that these are just general guidelines, and your doctor may consider other factors, such as your age and overall health, when interpreting your testosterone levels.Risks and Benefits of Testosterone Replacement Therapy (TRT)TRT can be an effective treatment for low testosterone, but it's essential to weigh the risks and benefits before starting therapy.Risks:Prostate enlargementIncreased risk of blood clotsSleep apneaAcneBreast enlargementTesticular shrinkageIncreased risk of heart problemsBenefits:Improved libido and sexual functionIncreased muscle mass and strengthDecreased body fatImproved bone densityImproved mood and cognitive functionIncreased energy levels Here are some proven natural ways to potentially increase testosterone levels without replacement therapy:1. Exercise and Lift WeightsExercise is a powerful way to boost testosterone levels. Resistance training, like weightlifting, is particularly effective. High-intensity interval training (HIIT) can also be beneficial. 2. Eat a Balanced Diet3. Manage StressLong-term stress can elevate cortisol levels, which can negatively impact testosterone. 4. Get Enough SleepAim for 7-9 hours of sleep per night.5. Get Some Sun or Take Vitamin D SupplementsVitamin D is linked to testosterone production. 6. Maintain a Healthy Weight7. Understanding Scoliosis: Symptoms, Impacts, and Management In today's insightful episode of our podcast, Dr. Tony Nalda discusses the complexities of scoliosis, focusing on its symptoms, progression, and the implications of various treatment approaches. This episode aims to demystify scoliosis for patients and their families, offering a comprehensive look at how this condition affects individuals differently depending on numerous factors. What is Scoliosis? Definition and Diagnosis: Scoliosis is characterized by an unnatural sideways curvature of the spine, accompanied by rotation. To be classified as scoliosis, the curvature must be 10 degrees or greater, as measured by the Cobb angle analysis. Types of Scoliosis Idiopathic Scoliosis: Making up about 80% of cases, idiopathic scoliosis has no single known cause and is considered multifactorial. Neuromuscular Scoliosis: Associated with conditions affecting muscles and nerves. Congenital Scoliosis: Caused by vertebral anomalies present at birth. Degenerative Scoliosis: Results from age-related spinal degeneration. Traumatic Scoliosis: Caused by traumatic injuries leading to immediate curvature. Symptoms Across Life Stages Adolescent Scoliosis: Most diagnoses occur between ages 10 and 18. Surprisingly, during these years, even significant curvatures often do not cause pain. Adult Scoliosis: In contrast, adults are more likely to experience pain as scoliosis progresses due to spinal compression from gravity. Common Symptoms and Signs Pain: Not directly correlated with the curvature size but rather with age and the rate of progression in adulthood. Postural Changes: The most noticeable signs of scoliosis include uneven shoulders, misaligned hips, and rib protrusion, which can affect how clothing fits and overall body alignment. Functional Impairments: In severe cases, scoliosis can lead to issues with lung function and digestion, although these effects vary greatly. Treatment Considerations Surgical Approaches: While surgery, specifically spinal fusion, can halt the progression and reduce curvature, it comes with the cost of spinal immobility and potential long-term complications. Conservative Management: Dr. Nalda advocates for non-surgical options that focus on reducing curvature while maintaining spine flexibility. These include customized exercises, bracing, and chiropractic care, aimed at preventing further curve progression and managing symptoms without the risks associated with surgery. Proactive Treatment and Early Intervention The Value of Early Detection: Identifying scoliosis early in its progression offers the best chance for effective management, especially in children and adolescents. Early intervention can prevent the curve from worsening and reduce the likelihood of needing invasive procedures later in life. Patient-Centered Care: Each scoliosis case is unique; thus, treatment plans should be tailored to individual needs, taking into account the type of scoliosis, its severity, and the patient's overall health and lifestyle. Conclusion This episode underscores the importance of understanding scoliosis beyond the curvature of the spine, highlighting how symptoms can vary and the critical role of tailored treatments. For anyone dealing with scoliosis or responsible for someone who is, proactive management and a thorough evaluation of treatment options are key to improving quality of life and managing this complex spinal condition effectively. At Sanctuary you will find the best of the conventional medical field combined with the best of Functional Medicine.Beyond the science, Dr. Potter has personally experienced healthcare crises from both sides of care–as a physician and as a family member to patients. Working within our broken healthcare system and taking his family members through that same system inspired him to look for new ways to deliver the quality of healthcare he envisioned. Stepping out of the insurance-based model of medicine allows him to provide whole person care in longer visits. Providing wholesale pricing on lab tests and supplements removes the profit motive for these services, allowing patients to trust his recommendations. Practicing Functional Medicine allows him to treat root causes rather than superficial symptoms. He endeavors to be the patient's trusted advocate with the goal of restoring a healthier more abundant life.Connect with Dr. Eric Potterhttps://sanctuaryfunctionalmedicine.com/about-us/meet-our-team/

This conversation between Dr. John Stenberg and Dr. Cameron Bearder explores the topic of post-traumatic cranial cervical disorders from a postural control perspective. The discussion covers the mechanisms of insult with mild traumatic brain injury and whiplash injuries, as well as their impact on postural control. The conversation also delves into the biopsychosocial aspects of managing patients with prolonged recovery and the importance of assessing psychosocial implications. The chapters cover various systems involved in postural control, including the ocular sensorimotor system, vestibular system, vestibulospinal system, cervical spine, and stomatognathic system. Click here for the full text PDF: Post-Traumatic Craniocervical Disorders From a Postural Control Perspective: A Narrative Review Takeaways Post-traumatic cranial cervical disorders can result from mild traumatic brain injury and whiplash injuries, leading to symptoms such as dizziness, vertigo, headache, neck pain, visual complaints, anxiety, and neurocognitive dysfunction. Psychosocial factors, including pre-existing anxiety, depression, and post-traumatic stress disorder, can affect the expression of symptoms and contribute to prolonged recovery in patients with post-concussion syndrome and whiplash-associated disorders. Impaired postural control is commonly observed after cranios cervical trauma, involving the ocular sensorimotor system, vestibular system, vestibulospinal system, cervical spine, and stomatognathic system. Manual therapy, patient education, range of motion reassurance, strengthening exercises, and multimodal care are useful interventions for managing neck pain and post-traumatic cranial cervical disorders. TMJ disorders can be a potential cause of post-traumatic cranial cervical disorders, and their diagnosis relies on a history of TMJ complaints associated with movement, local discomfort, and physical examination. When evaluating patients, it is important to track progress over time and consider the different systems that may be affected. Treatment of temporomandibular joint disorder (TMD) and related headaches may have poor outcomes when associated with whiplash. Postural control is influenced by ocular sensorimotor, vestibular, cervical proprioceptive, and somatic-nathic systems, which may be damaged by cervical spine trauma. Future research should consider all these systems from a postural control perspective in the context of cranioservical trauma. Other factors such as adverse childhood experiences, attachment disruptions, biological factors, and cultural/societal factors can also contribute to dissociation. Dissociation serves as a way for individuals to cope with overwhelming emotions by mentally disconnecting or escaping from distressing situations. It often co-occurs with other mental health conditions such as PTSD, depression, anxiety, and substance abuse. Recognizing the common symptoms of dissociation, such as memory loss, distorted perception, and a sense of unreality, is crucial. Understanding these symptoms helps individuals evaluate whether they still rely on dissociation when it is no longer necessary. Sometimes, people may experience a distorted sense of reality or a feeling of unreality in their surroundings. However, it is important not to add fear to these experiences but rather approach them with curiosity. Keeping a journal and discussing these perceptions with a therapist or coach can provide insights and understanding. Feeling detached from oneself, emotions, or having a blurred sense of identity can also be signs of dissociation. These symptoms are often present when individuals are coming out of an anxious or panic state. Inability to cope with emotional or professional stress is another indicator of dissociation, as individuals feel disconnected and unable to handle such challenges. Learn more about your ad choices.
