Central Ohio's healthcare giants are galloping to keep up with the region's growth, building new facilities that'll need to be staffed by legions of trained healthcare professionals that don't yet exist. As reported in Columbus Business First, Central Ohio's four major hospital systems are right now trying fill more than 2,200 open positions in nursing alone. Rural Ohioans too are suffering from the ill effects of too-few healthcare professionals, with "care deserts" leaving entire Ohio counties without critical specialists. What's being done to close the huge gap in the ranks of Ohio's healthcare professionals? In April of 2022 the American Association of Colleges of Osteopathic medicine or AACOM graciously invited the D.O. or Do Not podcast to attend their national meeting, “Educating Leaders” conference in Denver, Colorado. Our two producers, Lerone Cark, MS-II at the New York Institute of Technology College of Osteopathic Medicine and Brooke Pillegi, MS-II at the Alabama College of Osteopathic Medicine, as well as Ben Berg, our pre-medical student liaison, attended the conference to continue or  mission of promoting awareness of osteopathic medicine by interviewing leaders in the field.We would like to thank Dr. Robert Cain, president of AACOM, Joseph Shapiro, director of media relations and Helene Cameron, Vice President of medical education services for having us at the conference and supporting the podcast. With these announcements has come a rise in vaccine hesitant Canadians who oppose the idea of mandates, many of whom are concerned their rights and freedoms are being infringed upon. It has also become a key wedge issue in the federal election. Do vaccine mandates violate Canadians' charter rights? GUEST: Andrew Furgiuele, Lecturer with the Faculty of Law at the University of Toronto - Doctors must be judicious about handing out medical exemptions to COVID-19 vaccines, the head of Ontario's medical regulator said, urging physicians to do their part to slow the pandemic's fourth wave. The message from the CEO of the College of Physicians and Surgeons of Ontario comes after the province announced a vaccine certificate program that will require residents to be inoculated against COVID-19 to access some non-essential services, unless there's a medical reason they can't be vaccinated. Listen in to today’s interview with the author of The Heartbeat of Success: A Med Student's Guide to Med School Admissions, Dr. Alexa Mieses. The journey of a family practitioner, author, advocate [Show Notes] Our guest today is Dr. Alexa Mieses, who has an MD and MPH and in her own words, "strives to have influence beyond the confines of the examination room." She is a resident physician in the Department of Community and Family Medicine at Duke University Medical Center. In addition to providing clinical care to marginalized patient populations, Dr. Mieses' interests include population health, increasing diversity in the physician workforce, mentoring premedical students, writing, and research.” Dr. Mieses graduated from CUNY magna cum laude in 2011. She then studied for her MD/MPH at Icahn School of Medicine at Mount Sinai from 2012 to 2016 and graduated with distinction in Research and Excellence in Family Medicine. She has been a Family Medicine Resident at Duke University since 2016 and has earned the Harvey Estes, MD Memorial Award for Leadership and Advocacy. Can you tell us about your background outside of medicine? Where you grew up? What do you like to do for fun? [2:45] I am a native New Yorker. I grew up in NYC in Queens and had a very interesting childhood. I am bi-racial - my dad is from the Dominican Republic and my mom is American of Italian/Irish descent, so I grew up in a very ethnically diverse family and neighborhood, which shaped who I am and what I have done in my career. I have been a writer since childhood and have incorporated that into my career and leisure time. I really enjoy spending time with friends and family, and I am a mom of three pets – two cats and a dog. How did you know you wanted to be a doctor? [3:57] I really feel like medicine is my calling, and it’s something I’ve thought about since I was a kid. I was first interested in science when I was 3 or 4 and told my parents that I wanted to be a marine biologist. I was a big nerd and loved watching National Geographic specials, and I would read encyclopedias. I was always fascinated by life science. My mom actually got me interested in medicine – not because she is a doctor, and in fact neither one of my parents went to college. My mom had diabetes, but I remember helping her check her blood sugar and going with her to her doctor and seeing the positive relationship she had with her doctor and wanting to have that same positive impact on someone’s life. What was the hardest part of the med school application process for you? [5:26] I remember that it was a marathon and not a sprint. In hindsight I feel like I lived, breathed, and slept my dream of becoming a physician, which meant I was very intentional about what I did. For example, I was accepted to both a science high school and a performing arts high school. I probably would have been fine going to either, but at the time it felt do or die, and so I chose the science high school. As another example, in my first and second years of college I was working fulltime in addition to going to school fulltime so I had to make serious decisions about the type of job I took that would allow me enough time to study. So it was all about being really thoughtful and intentional about things. Every decision you make has the ability to make your path easier or more challenging. I would never want to go through the agony of being an applicant again, though the good news is it gets easier each step along the way. Being a premed student is by far the hardest. You took a gap year between undergrad and med school. What did you do and are you happy you did it? [9:00] I knew early on that clinical medicine alone would not allow me to have the kind of impact I wanted to have on my community.

99 Topics for the CCFP

Here are some resources I have used and would recommend for your study for the CCFP Exam, roughly in order of how effective I feel they are: Swanson's Family Medicine Review — My favourite study resource bar none! An American textbook (although I've been told Swanson himself was Canadian) focused on the AAFP examinations, but nonetheless broad enough in scope to touch on most if not all of what we need to know for the CCFP exam. Why do I love it? It's case-based: it holds my attention as I work through a presenting complaint, differential, management, and then I come out on the other side knowing more about a particular condition. Highly recommended, and a good resource that can hold your attention when you're burnt out from learning "bullet point medicine". The Practice Based Small Group (PBSG) Learning Program (a.k.a. "The McMaster PBLs") — The McMaster PBLs are a good resource for the CCFP, and useful for Mainpro credit once you're out into solo practice. If your Department doesn't provide you access, then beg them to get it. The University of Saskatchewan academic half-days usually include at least one of these. Each PBL works through a topic in great detail, with 2-3 cases, questions to prompt learning, as well as thoroughly-referenced and researched explanations. Dr. Mike Kirlew’s CCFP podcasts (iTunes) — Dr. Mike Kirlew practices in Sioux Lookout, Ontario, and his lectures are mostly focused on OSCE content rather than the 99 Topics, which I think complements this podcast nicely. I find his lecture style a tad scattered, and unfortunately the lectures don't have an accompanying description at this time (to match up to particular topics), but the content is fantastic, and he has a knack for simplifying complex content. Highly recommended. The Guide to the Canadian Family Medicine Examination — Published in 2013, it was created as an initiative of the University of Saskatchewan Family Medicine Residents at the time, along with faculty support. I think this book is great, although it's getting a bit out of date. It is brief but thorough — if you read through it from cover to cover, which won't take you very long, you'll have a great idea of what you know and what you don't, and where you need to focus your studies. Family Medicine Notes by Dr. Danielle O'Toole — These are compiled study notes from Dr. Danielle O'Toole, a former Family Medicine Resident, now practicing in Academic Family Medicine at McMaster. They are updated yearly, and comprehensive in scope. These study notes are quite condensed, but fairly comprehensively cover the topics. She has sample pages available on her website — have a look and see what you think. Bates' Guide to Physical Examination and History-Taking — Last but not least, if you need to brush up on your physical examination skills, which are a core component of both the MCCQE2 and CCFP examinations (but are generally considered prior knowledge from medical school), this is the book for you.
