Dr. Kieran Quinn is a General Internist and Palliative Care Clinician-Scientist at Sinai Health in the Department of Medicine and Institute of Health Policy, Management, and Evaluation at the University of Toronto and an Adjunct Scientist at the Institute for Clinical Evaluative Sciences (ICES). His research focuses on using advanced analytic methods and large administrative datasets to improve access and delivery of high-quality end-of-life care for patients with terminal noncancer illness, such as heart failure and dementia, and improving the recognition and care of people with post COVID-19 condition (long COVID). Dr. Quinn was awarded the Governor General's Gold Medal for Academic Excellence and the John Charles Polanyi Prize in Physiology and Medicine. He is co-lead of Canada's national research network on post COVID-19 condition and co-chair for the development of clinical practice guidelines for the treatment of post COVID-19 condition. He served as the Assistant Director of the Ontario COVID-19 Science Advisory Table, the Ontario Public Health Emergencies Science Advisory Committee, and as an Expert Member of the Chief Science Advisor's National Task Force on the Post COVID-19 Condition. His early research success includes over $28 million in competitive grant funding and 7 CIHR grants as the nominated principal applicant. He has mentored 12 trainees who have published and won several awards for their work together. He has 103 peer-reviewed publications, including 52 as first author or senior author in high-impact journals such as JAMA, JAMA Internal Medicine, and BMJ. Dr. James Downar is a Critical Care and Palliative Care physician in Ottawa. He graduated from McGill Medical School and completed residency training in Internal Medicine, Critical Care, and Palliative Care at the University of Toronto. He has a Master's degree in Bioethics from the Joint Centre for Bioethics at the University of Toronto. He is currently the Head of the Division of Palliative Care at the University of Ottawa and a Professor in the Department of Medicine. He is the chair of Palliative and End-of-Life Care at the University of Ottawa, the co-chair of the Pan-Canadian Palliative Care Research Collaborative, the chair of the Ethical Affairs committee of the Canadian Critical Care Society, and the co-chair of the Ontario Palliative Care Network's Provincial Education Steering Committee. He has authored more than 100 peer-reviewed publications, has been principal investigator on more than 20 peer-reviewed grants, and is a former Associated Medical Services Phoenix Fellow. Dr. Downar also led several provincial and national efforts to respond to the COVID-19 pandemic. “When we say ‘treatment for menopause,' it implies that menopause is a disease, when really it's a normal and expected time of life,” says Dr. Anna Barbieri, an integrative medicine physician and specialist in menopause certified by the North American Menopause Society. That attentiveness to word choice is reflective of a new perspective that's driving Dr. Barbieri and her peers to see menopause more holistically than in the past and to forge new approaches to the care they provide. "Menopause care is not checkbox medicine. We have to work with our patients individually," Barbieri shares with special guest host Dr. Deborah Enegess, herself a practicing gynecologist as well as a clinical content writer for Osmosis. Ottawa and the province have recently arrived at a bilateral healthcare agreement. Meanwhile, Trudeau has defended Toronto Liberal MP Han Dong after it was reported that the Chinese government was involved in influencing his nomination in 2019. Also, the federal government has banned the use of Tik Tok on government issued phones to its employees to better protect them from cyber attacks and surveillance. Since the very first days of the pandemic, the National Institute on Ageing based out of Toronto Metropolitan University has been crunching the COVID numbers in long term care homes. In fact, throughout the pandemic they were the only organization to keep comprehensive data on cases, deaths and outbreaks in every province in the country. But on July 28, the NIA announced they would be shutting down their Long-Term Care COVID-19 Tracker Project. The two year endeavour came to a halt, not because the thinktank no longer feels the need to track covid numbers, but because provinces and territories across the nation are no longer providing them with the numbers once used to compile their totals.Dr. Samir Sinha is the director of geriatrics at Sinai Health, and the Director of Health Policy Research for the NIA. Today, I'll talk with him about how the tracker totals were compiled, who used those figures, and the impact comprehensive data can have.

At the University of Toronto, he is a Professor in the Dalla Lana School of Public Health and the Department of Family and Community Medicine, affiliate member of the Institute for the History and Philosophy of Science and Technology, Member of the Centre for Environment and Adjunct Senior Scientist at the Institute for Clinical Evaluative Sciences. He is a Staff Physician at Bridgepoint Active Healthcare, Sinai Health. In 2015, he was named one of the Top 20 Pioneers in Family Medicine Research in Canada by the College of Family Physicians of Canada and was a Tier 2 Canada Research Chair from 2005-2015. He is an elected Fellow of the Hastings Center and the Canadian Academy of Health Sciences. During COVID-19 he has served as the co-Chair of the WHO Ethics and COVID-19 Working group and is a member of the WHO ACTA Ethics and Governance Working Group.Rushay Naik is an MSc student in Health Policy & Global Health at the University of Toronto. His research centres on health service delivery in conflict-affected states, and has conducted fieldwork for research initiatives in Greece, Mongolia, and Switzerland. Rushay has held research fellowships at UofT's School of Cities, at the Center for the Study of the Presidency & the Congress in Washington, D.C., and at the University of Oxford's COVID-19 Government Response Tracker. The growing field of art therapy argues that paintings and sculpture can improve your wellness. In collaboration with Art Toronto, host Shaun Francis examines the evidence with such experts as the contemporary artist Marcel Dzama, RxArt founder Diane Francis, Sinai Health psychiatrist-in-chief Dr. Lesley Wiesenfeld and Montreal Museum of Fine Arts art therapist Stephen Legari. Plus a 2017 study showed how just one art therapy session improved participants' mood, and reduced pain and anxiety. A 2014 article suggested art therapy can affect those with Alzheimer's and other dementias.    The World Health Organization's 2019 scoping review on the role of the arts in improving health and wellbeing   INSIGHTS   “When we're patients, we're still people, and we're people who are going through stress and strain.” According to Dr. Lesley Wiesenfeld, our healthcare experience can be improved by the presence of art that is beautiful, engaging or thought-provoking. [04:31]   The art on the walls at Medcan has been carefully picked out by Stacy Francis and art consultant Marshall Webb. The pieces were chosen for their potential to provoke attention and create a reaction in the people who see it. “I wanted [clients] to just have a contemplative moment that was not about the next test that they were about to receive,” says Francis. [07:03]   Art therapy is the combination of therapy with the presence or creation of art. Stephen Legari is an art therapist at the Montreal Museum of Fine Arts. “Art therapy allows people to externalize their experience, enjoy a therapeutic relationship with art and feel supported as they're doing that,” says Legari. [10:36]   When we do things that don't come naturally to us (like art!), we use and connect more neurons in our brains. And that becomes more important as we age, according to Dr. Lesley Wiesenfeld. “This is why people get advice about, if you're left handed, use your right hand, [and] if you love crosswords, you should actually try a number puzzle instead,” she says. [13:54]   When we realize we aren't good at a hobby or activity, we tend to move on to something else. But Dr. Lesley Wiesenfeld says that “there's good support for the value of developing habits that are about both mastery, and tolerating our absolute mediocrity at something.” [26:24]   Interest in art therapy is growing fast. RxART is a nonprofit organization that commissions artists to transform hospitals and healthcare settings, and its founder Diane Brown has seen the interest first-hand, both from hospitals and from artists. Every RxART project is fully funded by the charity and free to the hospitals. Artists receive small honorariums.  “We don't have problems getting artists to collaborate. Rarely does an artist turn us down,” says Brown. [29:06]

One way to spot a flash point is to notice when people are using different words to describe the same concept. Throughput is one example of this. On one side of the table, you have those who grasp that if a provider organization is concerned about patient outcomes, with few exceptions, building relationships with said patients is essential. It’s not entirely clear to anyone anywhere how you manage to build relationships and trust without spending a certain amount of time with patients. These “we need time with patients” people will bring up the Quadruple Aim issues that arise from rigid 7-minute appointments or even 50-minute appointments really. On the other side of the table, you have those who have built practice fiscal models on the backbone of however-many-minute appointments. They use different terminology for this whole concept, however. They call it throughput. How many patients can a physician manage to squeeze into a day? Some of these folks will tell you that throughput success is “more is more.” In other words, throughput is one of those things that you can never have too much of. Let me back up for a sec and mention the mission of this show. It is to connect health care leaders together by helping everyone understand each other well enough to communicate effectively, which is rate critical numero uno for any collaboration. You can’t collaborate if parties don’t really grasp what anyone else is actually saying when they communicate their WIIFMs (their “what’s in it for me?”) or their organizational imperatives. If we consider that the health care industry can only transform when multiple stakeholders collaborate, these little “language discrepancies” actually can have macro implications. In this respect, this throughput example—not in all cases but at a minimum—it’s an exemplar illustration and certainly something to contemplate. Consider people arguing against 7-minute appointments without mentioning the word throughput. They’re probably not going to even reach the headspace of those who just spent the past two decades in meetings to increase throughput. It’s like two ships passing in the night. You could be sitting there right now pooh-poohing what I’m saying, but I’ve sat in enough meetings where people talk around each other using different terminology, think they’ve agreed on some collaboration or compromise or solution, except nothing happens because everyone got to walk out without addressing the elephant in the room. It sounds something like this: DOCTOR OR NURSE: We need you to enable patients to have quality time with their doctors and the rest of the care team. SOMEBODY ELSE: We need to get rid of inefficiencies, which means driving maximum throughput. ANOTHER PERSON: OK, let’s compromise. Doctors should have quality time while maximizing throughput. Don’t laugh. I’ve heard “action items” like this often enough, and so have you if you think about it. That’s why I originally started this podcast—because I can also guarantee you if this is the action item, no action will actually take place. The only way this conversation is going to net any change is if people around that table head-on confront that quality time with patients means less throughput. And how much less are we going to agree on and/or how are we going to creatively change the practice model so throughput is an archaic term (ie, asynchronous stuff, etc)? I say all this to say that this throughput business also leaks into the technology space in ways that we should probably think about. Increasing throughput, after all, is one of the key ways to increase FFS (fee-for-service) revenue. FFS is all about the need for speed. The faster you can smack a billing code on a patient visit, the more patient visits you can pack into a day, the more billing revenue you can rack up. To some extent, throughput is code word for an addiction to FFS. You can always tell a tech vendor who is used to selling in an FFS environment because the second slide of their pitch deck is always one of two things: either how much faster the tool will get patients in and out of a doctor’s line of sight or what the billing code is for the tool (but that’s a whole different topic). I just described the second slide in an FFS-centric technology vendor deck. The first slide in those “use our AI thingamajig to revolutionize your throughput” decks is always some mission statement about improving patient care. And this is where not everybody using the same language creates immense wiggle room for profit over patients under cover of mismatched terminology. To add one point of context, when I say throughput here or increasing throughput, nobody is talking about making the front desk more efficient, minimizing faxing things around, or streamlining prior auths or duplications in the workflow (ie, fixing things that are in desperate need of a fix). What we’re talking about in this health care podcast are tools like the one I saw the other day. This biz dev person of this company was up and about early promoting some AI diagnostic tool. With this tool, so their slide deck promised, a physician could see 50 patients a day. Even for this particular vendor, I guess a full-throated “Hey, let’s burn out all your doctors and make patients wonder if they imagined their doctor visit would happen so fast”—a blunt message like that—presumed a little too much avariciousness on the part of the practice. So, they tempered their message by stating the inarguable fact that there is a physician shortage in rural America and that this tool will help resolve that. OK … that’s a worthy thing to fix. But, seriously, is the goal to get rural patients an automagical visit with a doctor that, in hindsight, they wonder if they hallucinated it was so fleeting? Or is it to actually help patients get better health? Also inarguably, health care that leads to better health requires less than pedal-to-the-metal throughput. If you think differently and want to change my mind, feel free but show me the study. I say all this to say that I called up Arshad Rahim, MD, MBA, FACP, a little bit ago to see what he thought of my aforementioned burning premises (aka rants) about throughput; and he kindly agreed to come on the show again.
