Continuing our conversation with warrior, Jenny Jones from S1E27. Jenny lives with two rare, chronic conditions called Familial Adenomatous Polyposis (FAP) and Short Bowel Syndrome and more recently was diagnosed with Post-Concussion Syndrome, Fibromyalgia, and Esophageal Dysmotility. Due to FAP, she underwent a total colectomy at age nine. Complications resulted in six additional surgeries and an eighth surgery to remove her gall bladder. Her genetics and additional factors led Jenny to decide not to have biological children. As fate would have it, she ended up becoming a very involved auntie to her twin nieces and stepmom to her bonus son. Abstract: Inorganic polyphosphate (polyP), a linear polymer of orthophosphate residues, is found in all living organisms. While the biological and molecular functions of polyP have been studied in prokaryotes and unicellular eukaryotes, in metazoans it has often been referred to as a molecular fossil. Although certain functions of polyP, e.g. in blood clotting, have been uncovered in humans, a genetically tractable model system has not been developed to systematically study polyP function in metazoans. To explore Drosophila as a model to study polyP, we first developed methods to quantify polyP in flies and then analysed polyP levels in various tissues during different developmental stages. Interestingly, we found that the levels of polyP increase before pupal formation and significantly drop during metamorphosis. To check the in-situ localization of polyP, we used polyP binding domain of bacterial protein, namely PPBD to detect endogenous polyP in fly tissues. We report the presence of polyP in cytoplasm and enrichment in the nucleolus in most tissue types. Interestingly, during oogenesis, we found polyP is enriched in early stage egg chambers but get depleted in later stages suggesting that polyP may play a crucial role during ovariole development. Finally, we found polyP to be enriched in fly hemocytes and showed that polyP promotes hemolymph clotting suggesting that polyP function between flies and mammals is conserved. These observations strongly suggest that combining powerful Drosophila genetic and biochemical tools can accelerate the discovery of unknown biological functions of polyP in metazoans. Abstract: Dictyostelium discoideum is a soil-dwelling unicellular eukaryote that accumulates extracellular polyphosphate (polyP). At high cell densities, when the cells are about to overgrow their food supply and starve, the corresponding high extracellular concentrations of polyP allow the cells to preemptively anticipate starvation, inhibit proliferation, and prime themselves to begin development. In this report, we show that starved D. discoideum cells accumulate cell surface and extracellular polyP. Starvation reduces macropinocytosis, exocytosis, and phagocytosis, and we find that these effects require the G protein-coupled polyP receptor (GrlD) and two enzymes, Polyphosphate kinase 1 (Ppk1), which is required for synthesizing intracellular polyP, cell surface polyP, and some of the extracellular polyP, and Inositol hexakisphosphate kinase (I6kA), which is required for cell surface polyP and polyP binding to cells, and some of the extracellular polyP. PolyP reduces membrane fluidity, and we find that starvation reduces membrane fluidity, and this effect requires GrlD and Ppk1 but not I6kA. Together, these data suggest that in starved cells, extracellular polyP decreases membrane fluidity, possibly as a protective measure. In the starved cells, sensing polyP appears to decrease energy expenditure from ingestion, and decrease exocytosis, to both decrease energy expenditures and retain nutrients. The Iterative SKOUT polyp detection tool provides algorithmic assistance to physicians during a colonoscopy to improve the ability to detect and remove polyps. With colon cancer getting diagnosed at a younger age, it is increasingly important to detect polyps as early as possible. Shrujal explains, "I think there's a lot of science in the space for detection of precancerous tumors throughout the body. In colon cancer currently, it's still very anatomic, which is we look for polyps, we look for abnormalities, dysplasia, we look for things that we can see on the end of the camera that's a colonoscopy. We flag them, and we try to remove them. Now, many of those lesions may never have gone on to become cancer. Part of this is our learning, over time, that there are certain polyps or certain lesions that are more likely to become cancerous, and can we target our interventions and only resect those." "What we found is that in studies, and these are out there, when physicians do colonoscopies in the morning versus when they're done in the afternoon, there's more polyp detection in the morning than in the afternoon. And we can postulate why that might be, but it suggests that pattern recognition requires a level of sort concentration that is still a human-dependent process." "By having AI in the background running alongside the physician's interpretation, if the physician found every one of those polyps on their own, that's great. In this episode, I interview Jenny Jones, a fighter of Familial Adenomatous Polyposis and Short Bowel Syndrome. Jenny was diagnosed with the rare, hereditary colon cancer syndrome Familial Adenomatous Polyposis at age 8 and had her first surgery to remove her colon at age 9. After experiencing life-threatening complications, Jenny required 4 more surgeries that year and developed another rare disease - Short Bowel Syndrome as a result. She lived with an ileostomy for 6 years before having it reversed into a Straight Pull Thru and again experienced life-threatening complications. Jenny has been able to surpass all the odds though and went on to earn a Master's Degree and maintains full time employment in addition to her rare disease awareness and advocacy efforts through her blog, Life's a Polyp. Arkady Broder, MD, will introduce the audience to the benefits of colon cancer screening, and ultimately, prevention. Dr. Broder is the chief of the division of Gastroenterology and Hepatology at Saint Peter's University Hospital. He holds a clinical faculty position at Rutgers Robert Wood Johnson Medical School and is the program director of the Medical School's Gastroenterology Fellowship Program at Saint Peter's University Hospital. Dr. Broder is a recipient of the Excellence in GI Practice Operations Recognition Program from the American Society of Gastrointestinal Endoscopy.
