August 23, 2024 Mark, Scott, and Ray talk about two questions that came into the PRS Communities:I have Optum requesting records on all of our clot evac cases where something in the bladder was fulgurated during the same session and after record review we fail the audit.Now compliance is very quick to say refund it, stop billing it, its bundled, etc.But we know a modifier is allowed so when is it allowed????Everything I find online like AAPC says to bill both 52214 and 52001-59 but there is nothing recent and nothing concrete about what is considered distinct. I mean using a clot evacuator is different than using a loop electrode. Below are the Optum rationales.So the urologist does a cysto on hematuria work up, and they wind up using an evacuator to remove huge amounts of clot burden. Then use a loop electrode to fulgurate some areas that look suspicious.Why is that service not considered distinct? Compliance says its incidental but they can't tell me when it wouldn't be incidental and I don't want to let optum take back money without an argument.Hi, I would like to ask a coding question please. Thank you so much!For Robot-assisted left partial nephrectomy and Resection of a left renal hilar lymph node, Do we code any additional CPT to capture renal hilar lymph node, in addition to partial nephrectomy 50543?Pathology report had a separate report for Renal hilar lymph node dissection, which was negative for carcinoma. Question i have is if you are just reviewing results that were ordered during a visit, this is essentially not a billable event. Isnt it considered double dipping because the review of the results and test order is included when ordered. Aside from the results, going over a treatment plan could be a billable event. Truly depends on what you are using this visit for. I have added an AMA sheet that will help with the different scenarios.Please let me know your thoughts Ordering a test is included in the category of test result(s) and the review of the test result is part of the encounter and not a subsequent encounter.”And, from the CPT Assistant, (AMA publication) November, 2020, page 5: “It is assumed that the physician or other QHP would review the results of the test ordered; therefore, the physician or other QHP would not receive dual credit toward MDM for service-level selection for both ordering and reviewing the test.” Hi, a question please. Having completed General Surgery residency at McGill University and Colorectal Surgery residency at Cleveland Clinic Florida, her clinical interests are advanced minimally invasive approaches to benign and malignant colorectal and anorectal diseases. She treats colon and rectal cancer, inflammatory bowel disease, diverticular disease, fecal incontinence and all anorectal disorders. Dr Boutros is a federally funded researcher who currently leads an internationally renowned research program focused on assessing innovations to improve recovery and functional outcomes after colorectal surgery using multicentre prospective trials, patient-centred interventions and patient-reported outcome measures. Within her research program, Dr Boutros has mentored over forty trainees and graduate students. Dr Boutros also has leadership roles in multiple international societies including Society of American Gastrointestinal and Endoscopic Surgeons, American Society of Colon and Rectal Surgeons, Canadian Society of Colon and Rectal Surgeons and the Canadian Association of General.

March 8, 2024Mark, Scott, and Ray discuss questions that came into the PRS Network.Hello all, I have a CPT coding question,Indications = Urinary Obstruction // Post-Op DX = Obstruction of neck of Urinary Bladder Resectoscope per urethra into the bladder under direct vision. There was moderate trabeculations with no evidence of mass or stones. The bladder neck was noted to be elevated as seen on prior clinic cystoscopy. This was causing obstruction of the bladder outlet without significant prostatic hypertrophy.The bipolar resectoscope was used with the plasma button attachment to perform a TUIP at 5 o'clock, vaporizing the bladder neck fibers until the bladder neck was able to open properly and drain appropriately. Electrocautery was used for any small bleeding vessels and hemostasis was adequate at the end of the procedure.The recommended Code is: 52450, but the Coder is recommending Code: 52500.We're also taking Codes: 52276 & 52214 into consideration.... Please advise on what you recommend with the Rationale. Any assistance is greatly appreciated.I completed a  tapered ureteral reimplant  50783 on the left and a standard Non tapered ureteral reimplant on the right 50780 - does adding LT and RT modifiers allow both codes to be billed as follows:50783 LT50780.51.RT  does adding R /L mods  address the bundling issue?Coders noting that 50783 includes 50780 -- however this is  2 different sides and can't bill bilateral tapered 50783.50, as only tapered one side, and bilateral reimplant 50780.50 is undercoded for the additional time and complexity of ureteral tapering Has anyone billed for bladder injection of Lipiodol before? I have a provider who resected a large bladder tumor, then injected Lipidol in and around the resection site to aid with the patient's radiation planning for invasive bladder cancer. I was thinking this may be included in the 52240, but the office is suggesting unlisted code 53899 benchmarked to 52287 or 52283. This figure has not changed at all over the past 20 years; however, systemic therapy options have evolved dramatically during this time and have significantly increased life expectancy for these patients. We're back with the second episode of our image guided surgery video series! In the second part of this episode we're discussing lung nodule resection in hybrid OR with some of our Interventional Radiology and Pediatric Surgery colleagues at Cincinnati Children's Hospital. Our guests are Dr. Anusua "Roshni" Dasgupta, Dr. John Racadio, Dr. Neil Johnson and Nicole Hilvert. And we're joined by Dr. Denise Liu, radiology resident at University of Toledo. Hosts: Rod Gerardo, MD, Em Tombash, MD With image guided surgery, surgeon uses real-time images of the inside of the body during an operation. These images are generally produced by a combination of X-rays, ultrasound, CT's etc. Hybrid OR (operating room) at Cincinnati Children's is a collaboration between Radiology's Dr. John Racadio and Surgeon-in-Chief Dr. Daniel von Allmen. The hybrid OR offers imaging capabilities in a sterile operating room. It creates an environment for surgeons and interventional radiologists to combine their individual skills and expertise in order to benefit patient care. The room is equipped with X-ray, fluoroscopy (live X-ray), ultrasound and C-arm Cone Beam computed tomography (CBCT). Surgeons work together in the hybrid OR with interventional radiologists, who specialize in using these imaging technologies. Imaging can be performed just before the surgery to help plan the procedure, during the surgery to identify important anatomy or disease, and after the surgery to evaluate results in the hybrid OR. Any surgery requiring radiologic imaging including open and minimally invasive surgeries, may take place in the hybrid OR. The hybrid OR at Cincinnati Children's opened in 2018 and serves for general surgery, urology, orthopedic surgery, neurosurgery, pulmonology, and many more.
