In this AJNR Article Summary, Dr. Francis Deng reviews CTA-based Carotid Plaque-RADS and its role beyond stenosis grading. He explains the key imaging features that define plaque vulnerability, then highlights two AJNR studies showing that the system is learnable, reproducible, and clinically meaningful for predicting future stroke risk, especially in patients with only mild or moderate carotid stenosis. Nick Homer has done employer branding in-house and in agencies across his career, most recently at Chapter 2. In this conversation, we get into where agencies genuinely add value, where the current model is falling short, and what a more relevant version of the relationship between agencies and in-house teams might look like. The honest answer isn't that agencies are irrelevant. It's how we can close the gap between what clients need and what agencies deliver. The opportunity is real — but so is the need to change. Here are some of the points we cover:- Why the in-house vs agency cost comparison is forcing harder questions- Where creativity is still the clearest case for agency involvement- Why knowledge transfer — not just execution — could redefine what agencies offer- How independent consultants are filling gaps that neither agencies nor in-house teams cover well- What the RADs this year suggests about where employer branding creativity is heading- Why disruption from smaller entities is probably good for the whole industryThis episode was recorded during a session for a broader episode about the future of employer branding - you can watch that episode here: https://open.spotify.com/episode/165mXweEyhXN0scVxR85yI?si=ry3YOz80QEmOHMcZimELpQ

PSA spikes, abnormal MRI results, and high PI-RADS scores often trigger immediate fear and for many men, that fear leads straight to biopsy. In this episode, Dr. Stephen Petteruti breaks down what PSA actually measures, how MRI technology fits into modern prostate cancer management, and why a high PI-RADS score does not automatically equal aggressive disease. Dr. Stephen discusses active surveillance, non-biopsy monitoring strategies, cardiovascular risk, hormone balance, and why overtreatment may compromise quality of life more than the cancer itself. For men who value proactive healthcare, evidence-based medicine, testosterone preservation, and long-term vitality, this conversation offers clarity in a space dominated by urgency and assumption. It reframes prostate cancer care around informed consent, individualized risk assessment, and protecting both lifespan and healthspan. Before agreeing to your next scan or biopsy, press pause. We explore why maintaining a healthy respect for radiation remains crucial even as we champion nuclear power's expansion.Read the extended shownotes on Substack.

Population based data tells us that 15 to 20% of women will develop an ovarian cyst during their lifetime. Although most ovarian cyst have a low potential for malignancy, their presence causes anxiety for both the patient and the provider. Our typical mental-exercise is balancing the risks of expected management vs choosing surgery prematurely. Which ovarian masses are likely to resolve if given enough time? Do septated ovarian cysts resolve the same as simple cysts? In this episode, we will highlight and summarize a new publication coming out in Obstet Gynecol (Green Journal) in December 2023. We will also review a uniform, standardized approach to reporting and interpreting ovarian masses, originally published in 2020 by the American College of Radiology. This scale is called the O-RADS classification and provides extremely helpful tools for ovarian cyst prognostication and follow up.
