In this episode, we talk to Denise Tedder, former teacher, ED Nurse, and now Quality Programs Manager specializing in Medicaid for CHESS Health Solutions about North Carolina's transformation to Managed Medicaid Care, Medicaid Expansion, and what healthcare can do to prepare for the influx of a new population of patients into the program.Denise Tedder, thank you for joining us today on the move to value podcast!Thank you, Thomas it's a pleasure to be here.I have a couple of questions for you about Managed Medicaid. So can you tell me a little bit about the move to Managed Medicaid in North Carolina?Sure. Well it started back in like 2015, they passed legislation transitioning Medicaid from a fee for service to managed care and what that means under managed care the state contracts with insurance companies, which are paid a predetermined set rate for each enrolled person to provide the services. North Carolina Medicaid Managed Care actually started back in July 1st 2021. So now our patients have options to choose a health plan and get care through the health plans' network of doctors. Fast forward to now, effective December 1st, North Carolina has passed Medicaid expansion which will provide an estimated 600,000 more North Carolinians with access to health care coverage. That sounds like a really good opportunity for a lot of folks to get the healthcare coverage they need. Yes, it's going to be amazing for our communities!So what opportunities are there, you know because CHESS is in the value-based care space, right? So what opportunities are there for managed Medicaid to accelerate that?So, one of one of the key features of North Carolina's Medicaid, Managed Medicaid program is the requirements of all the pre-paid health plans to align their population health and prevention strategies with the state's goals. The ultimate goal is obviously to make everyone healthier under Medicaid but there's opportunities to reward providers for keeping their patients healthy. This expansion means more people will have access to healthcare which improves their health and that's what it's all about. What does that do with the improvement perhaps of health equity in our communities?So, it just means that everyone has the opportunity to be as healthy as possible. It gives patients increased access to preventative care for things like well visits, immunizations, screenings, and it also causes for better management of chronic conditions. So having this equitable access to healthcare means we're focusing on keeping our communities healthy which will positively impact health outcomes.So, how does this address the social determinants of health?So, Medicaid programs are increasingly focused on social determinants of health needs, including food insecurity, access to housing, reliable transportation. I recently read an article from the CDC which said 40% of a person's health outcomes are driven by their social determinants of health. So, I mean no one can focus on a health problem that they have when they're worried about housing or food for their family. With a focus on these needs we can put them in the best position to be successful.And what are your thoughts on how prepared providers are for this December 1st expansion deadline we have?Well, one of the big challenges for providers will be a sudden increase of all the patients seeking care. So, we have 600,000 across the state and most of our providers are operating at or near capacity. So, it could cause short term delays in scheduling appointments, but having this, you know, this previously underserved population access to care, I think providers are ready to make a difference.That's great! Christian Costazza, is a RN BSN who was born in Italy in 1984. He grew up with a pretty challenging childhood, losing his dad when he was 10. He was bullied in middle school, went straight to work after high school, and decided that he wanted to change his life by pursuing his BSN in Nursing. He graduated in 2012 with a BSN. He opened the first nurse-lead business in my hometown and decided that he wanted more, so he moved to Ireland with basic English skills. After a few rejections, he chose to study and landed a job in NICU. After a while, he moved to England, where he became a cath lab nurse and went through all the roles, from RN to Director of two departments. He then started looking into the personal development space after having a period of burnout in the middle of the COVID pandemic. He decided to move to the US last year. Before becoming a nutrition coach, Ellie received her RN/BSN from Samford's Ida B. Moffet School of Nursing. Ellie describes her ideal client as anyone without a preexisting medical condition who wants to improve their health. She takes each client on a highly personalized journey to a lifestyle change that is effective and sustainable. Who Is Geena Pitts? During her competitive CrossFit years, Geena was also pursuing a Master's degree in Exercise Science. She held a graduate assistantship position and worked with the Children's Physical Activity and Research Group. She also aided with multiple research projects about exercise and supplementation (one is in the process of being published). After graduation, Geena taught Exercise Physiology at the University of South Alabama and continued as a research assistant. Nutrition has always been one of Geena's favorite topics of research & discussion. She has dedicated hours of study to nutrition and habit-based changes. NICU nurses do what we just can't do for our babies—and that in and of itself is magical and special and makes me over-the-moon grateful whenever I think about it. And while I'm grateful for all nurses that care for my son, there are certainly nurses that are better at their jobs than others. Better at loving, better at caring, better at making the family's experience as meaningful and positive as possible. Sam Keirsey is one of those such exceptional nurses. In this episode, she shares some of her lows and highs, both of which are dripping with love for the babies and families she's served. She's held babies as they passed away when parents couldn't make it in time, and she's attended birthday parties of thriving NICU grads. Hospice Nurse Julie delves into the details of dying, so that we can all learn more ahead of time and be better prepared when someone we love is at the end of their life. “I know, as a hospice nurse who's used to seeing a body dying, that this is very normal: their face is going to look different; their eyes are often open, not blinking; they are not making eye contact; mouths are usually open, as all our muscles are relaxing; breathing changes; they are usually not eating or drinking; and they are probably unconscious for the most part." All of this is normal – we just don't know it, so we can find it unsettling at best and terrifying at worst. Julie and I have a mutual goal: to normalize talking about death.
Episode #37: Prescribing Medical Cannabis with Rebecca Abraham, RN, BSN Cannabis has been used as a medicinal substance for more than 5,000 years. And, for good reason. Cannabinoids, the active compounds found inside cannabis, offer potential solutions and symptom relief for a wide range of ailments including chronic pain, neuropathy, muscle spasms, and anxiety. Until the 19th century, cannabis was part of the United States Pharmacopoeia and widely used by doctors. But, in 1942 the U.S prohibited the substance and all forms of use. Today, cannabis is still a Schedule 1 Substance, which is defined as a drug having a high potential for abuse and no accepted medicinal use. And yet, acceptance and attitudes toward cannabis are rapidly evolving. In the U.S., 37 states have approved medical cannabis and more than 3.2 million people report using the substance for wide range of conditions. As acceptance of cannabis grows and demand increases, more health care providers are seeking guidance on prescribing methods. Our guest today is Rebecca Abraham, a critical care nurse, medical cannabis expert, and founder of Acute on Chronic – a medical cannabis clinic working to help patients navigate the safe and effective use of cannabis for hard-to-treat medical conditions. In this episode, Rebecca discusses how nurses are leading the charge in prescribing medical cannabis. She describes how nurses assess patients for cannabis use. Dr. Victoria Maizes and Dr. Andrew Weil ask important questions about dosing recommendations and ingestion methods for safe and effective treatment, and the challenges cannabis faces within our healthcare system. Rebecca describes common conditions and symptoms that respond well to cannabis and also shares common drug interactions to look out for.
