Originally uploaded September 23rd, re-edited October 28th. Jeffrey Mosher welcomes Sarah Milanowski, LMSW, CDP, Enrollment and Marketing Manager at LifeCircles PACE, Muskegon, MI. Questions Sarah and Jeffrey discuss: ● Can you explain how smart home technologies, like motion sensors and automatic lighting, are specifically adapted for senior safety? ● What are some of the challenges in introducing new technologies to older adults, and how does LifeCircles address these challenges? ● We often hear about smartwatches and fitness trackers being used for health monitoring. How are these consumer devices being repurposed or adapted for fall prevention in seniors? ● Voice-activated assistants like Amazon Alexa or Google Home are popular in many households. How can these be utilized to enhance safety and prevent falls for older adults? ● Can you share any success stories or data on how these technologies have reduced fall incidents among your participants? ● How do you balance the use of technology with more traditional fall prevention methods like physical therapy or home modifications? ● What role do family members and caregivers play in implementing and managing these technologies? ● Are there any emerging technologies in fall prevention that you're particularly excited about? ● How does LifeCircles ensure that these technologies respect the privacy and autonomy of seniors? ● Can you discuss the cost implications of implementing these technologies? Are they accessible to most seniors? ● How do you see the future of fall prevention technology evolving, especially with the rapid advancements in AI and IoT? Talking Points ● The scale of the issue: Falls among older adults result in about 3 million emergency department visits annually, according to the CDC. ● Holistic approach to fall prevention: Stress that while technology is important, it's part of a broader strategy that includes proper use of mobility aids, maintaining physical strength, and creating safe home environments. ● Customized solutions, as there's no one-size-fits-all approach to fall prevention. ● Technology as a component of care, not a replacement. ● How technologies aim to keep seniors safe while allowing them to maintain their independence and dignity. ● The importance of making technology user-friendly for seniors who may not be tech-savvy. ● How data is used to understand fall patterns and improve prevention strategies. About LifeCircles LifeCircles PACE has provided an in-home alternative to traditional nursing home living since 2009. We offer comprehensive medical and support services to seniors, allowing them to live independently in their homes while receiving the care they need. Our innovative approach combines in-home services, day centers, and community outreach to provide holistic care that enhances quality of life and promotes independence while also supporting family caregivers. Do you have a specific end goal for your private practice, but aren't really sure how to get there? This episode is perfect for you!I sat down with Heather Anderson, a speech-language pathologist with a private practice in Monroe, Michigan. Heather started out as a medical SLP, later became a pediatric SLP to start her own private practice, and now sees all ages through her practice, Your Voice Speech Therapy. I helped Heather get started through the Start Your Private Practice Program. As her business took off, she leveled up to the Grow Your Private Practice Program, which has helped her create a thriving practice doing what she loves. In this episode, Heather shares what it was like starting her practice on the side of a full-time job, why she pivoted to pediatric services, and how that flexibility eventually allowed her to get back to her zone of genius and experience huge growth. Heather graduated from The University of Toledo in 2016 with a Master of Arts. Her first exposure to speech pathology was as a junior in high school in 2008. While looking through possible majors at The University of Toledo, the title of speech-language pathology instantly caught her attention. Her mother recommended shadowing a speech-language pathologist at the hospital where she worked, and after taking her advice, decided after just one hour that it was the career for her. After graduation, Heather began working in a transitional care setting (a segway from acute care into a skilled nursing facility). While working in transitional care, she also worked in acute care at Toledo Hospital, where she learned valuable information for evaluating and treating strokes (in the neuro ICU), dysphagia, cognitive disorders, tracheostomies, motor vehicle accidents, and traumatic brain injuries. She also became certified to perform swallow studies (MBSImP certified).Heather then went into an outpatient pediatric setting while conducting swallow studies for acute care patients and outpatients. From there, she worked in skilled nursing settings in which she worked heavily with voicing disorders, dysphagia, dementia care, memory impairments, and more. Heather became a Certified Dementia Practitioner while working in skilled nursing and has expertise working in specialized Alzhiemer's/dementia care facilities. She has since continued working in acute care and skilled nursing settings but has always dreamed of owning a private practice. In Today's Episode, We Discuss:Heather's background and how she got into SLPWhat motivated Heather to start her own practice on the side of her jobWhat helped her take the leap to enter a school contractA look into the networking opportunities that came with pursuing her own practice The reality of pivoting your vision to serve the communityHow the Start and Grow programs helped Heather get back to her zone of geniusThe power of becoming the one that people refer toI hope you find Heather's story inspiring! Rob Liebreich, CEO and President of Goodwin Living, discusses why the 60-year-old faith-based not-for-profit organization is a provider of choice, employer of choice, and champion of social good. Learn how Goodwin Livings complimentary Stronger Memory program has positively impacted tens of thousands of lives by improving brain health in three simple steps. About Rob Robert “Rob” Liebreich started his service as President and CEO of Goodwin Living in July 2019. He brings energy, compassion and a creative drive, as well as proven leadership in senior living for over 20 years.  Prior to joining Goodwin Living, Rob was the executive director of the Asbury Methodist Village (AMV), a 1400 person not-for-profit continuing care retirement community in Maryland. He has also held senior positions in marketing and operations for senior living organizations in the Washington-Baltimore metropolitan area and the Pacific Northwest. Rob studied Business Administration and International Business and Marketing at Georgetown University and graduated 1997. He earned a Master of Business Administration with a concentration in Senior Care and Housing from Johns Hopkins University in 2005. Rob is a Certified Dementia Practitioner, and his other passions include his family, travel, mentoring and brain health. Key Takeaways Goodwin Living offers complimentary educational and career path opportunities for team members—80 to 100 team members every year work on future educational goals, with Goodwin Living taking on hundreds of thousands of school debt. Goodwin Living's diverse team of 1,200 members represents over 65 different countries of origin, with a quarter of the team not holding U.S. citizenship. The Goodwin Living Foundation has supported more than 160 team members to become citizens, pairing them with resident mentors to study for the exam. A study from Japan shows that reading aloud, writing by hand, and doing simple math problems quickly on a consistent basis levels cognitive decline and, in many cases, improves brain function. Early interventions hold off the need for care. Stronger Memory clears the brain pathways by engaging the brain's prefrontal cortex and hippocampus, which improves the “working memory” and improves the processing of information, based on research from George Mason University.

Jean Hartnett, MHSA, NHA, CDP, YTT, C.N.A., talks about her move from CEO of the largest nursing home in Nebraska to founding Radical Sabbatical, supporting individuals and senior living communities on their paths to healing. About Jean Jean Hartnett is a visionary health care leader with more than 30 years of executive experience serving the older adult population. She holds a master's in health services administration, is a Certified Dementia Practitioner, a Certified Yoga Instructor, and a Certified Nursing Assistant. Ms. Hartnett is an expert in trauma-informed care and adverse childhood experiences (ACEs) and founded Radical Sabbatical to help others turn past adversity into self-advocacy. With a deep understanding of the profound effects trauma can have on development and well-being, Jean brings a compassionate, evidence-based approach to support individuals and communities on their healing journey. Key Takeaways Trauma informed is an approach that can be applied to resident's and staff in senior living. Practitioners must view the individual through the lens of why they act a certain way, not simply their behaviors. All behavior has meaning. Adverse childhood experiences impact an individual's entire life, including their older adult years. Moving into senior living can lead to trauma resurfacing because the coping mechanisms that have provided emotional support throughout an older adult's life may be cut off. Medicare-eligible older adults spend fifty calendar days in clinical environments that are not hospitals. That includes physician, physical therapy and occupational therapy, and other specialists.

In this revisited episode, we are joined by OPG's Director of Aging Services, Sam van Kalkeren. Sam discusses how to help a loved one faced with a dementia diagnosis. He gives excellent information on how to communicate with a loved one to ensure their voice is heard as they navigate this difficult path. Sam talks about the benefits of having a care partner and what type of support they can offer the family. It is vital to have a care partner if the ailing family member lives out of state. We touch on what happens when someone with a dementia diagnosis gets a psychiatric admission and give excellent tips on keeping the loved one safe. This episode contains great information to help your aging parent handle cognitive decline.    IN THIS EPISODE [2:20] How does a child support a parent facing cognitive decline?  [5:20] What are the benefits of having a care partner?  [8:52] What is the best way to communicate with someone with  dementia?  [10:40] What does Sam recommend to children when they first hear the diagnosis?  [12:04] How do family members help someone struggling with dementia avoid being taken advantage of financially?  [14:24] What happens when someone goes into the hospital with a psychiatric admission? [20:25] How do you help a family member who doesn't want to accept their diagnosis?    KEY TAKEAWAYS   Have hard conversations about how the ailing parent wants to live the rest of their life. Do they want a DNI (do not intubate) or DNR (do not resuscitate)? Know all medications and have everything laid out to help the parent. Form a relationship with a care partner if you are out of state or need additional support. Those with dementia need a lot of support. Some people can continue to live at home, work, and speak for themselves, but the proper support needs to be in place. If your loved one goes into the psychiatric emergency department, they may get scared and become aggressive. If they are sedated due to that behavior, it can be difficult to go back to inpatient care. An impatient home may not want to take the patient back if they display aggressive behavior. It's important to have a care partner to help avoid the loved one being put in a psychiatric department.     RESOURCES: Beyond the Balance Sheet Website  https://www.linkedin.com/in/sam-van-kalkeren-msn-rn/   BIO: Sam van Kalkeren, MSN, RN, CDP, is OPG's Director of Aging Services. Communicating with someone who has dementia can be especially challenging because relating to them becomes increasingly hard as their memory declines. Drawing upon our years of experience, we have seen what is most successful for families. This, along with our Certified Dementia Practitioner certifications, sets the stage for this episode about communication approaches. Three take-ways from this episode: What is truth? Truth is a matter of perspective, especially for people with dementia. People who have dementia process information very differently than we can relate to. We cannot control their thoughts or behaviors, but we can control ours in order to best support them. Validation and redirection are crucial approaches to successful communication with those who have dementia. What is a Certified Dementia Practitioner? The mission of the NCCDP® (https://www.nccdp.org/) is to promote, encourage and enhance the knowledge, skills and practice of all persons who provide care and / or services to Dementia clients by means of requiring excellent standards of education, Dementia specific training and incentives for professional development of those who are dedicated to the ever growing field of Dementia Care. Today we are joined by OPG's Director of Aging Services, Sam van Kalkeren. KEY TAKEAWAYS   Initiate the difficult conversation about how the ailing parent wants to live the rest of their life. RESOURCES:   Beyond the Balance Sheet Website   BIO:   Sam van Kalkeren, MSN, RN, CDP, is OPG's Director of Aging Services. He has been working as a psychiatric RN since 2012 and is a Certified Dementia Practitioner. Sam has worked with all age populations suffering from mental illness, from pediatrics to geriatrics. He has experience working with clients suffering from substance use and co-occurring disorders, complex mental health diagnoses, neurocognitive disorders, and personality disorders. Most of his nursing career has been in the inpatient psychiatric setting. Sam joined OPG from Tufts Medical Center in Boston. Before Tufts, he was the Patient Care Director for the geriatric and adult inpatient psychiatric units at Carney Hospital in Dorchester, Massachusetts.
