THE BALANCED MOMTALITY- Pelvic Floor/Core Rehab For The Pregnant and Postpartum Mom
Hey Friend, For those that haven't been following along, I have been struggling with symptoms of Pelvic Venous Disorder or PeVD or better known as, Pelvic Congestion Syndrome, for the last year. It all started with an ovarian cyst like pain, but I later learned that my “menstrual cramps” I was feeling on and off throughout the month, not just during my menses, was also due to these enlarged veins. The heaviness and cramping was starting to get so uncomfortable that standing or sitting for too long was painful, and even exercise started to become painful. Thankfully sex was never painful, but painful intercourse or dyspareunia, especially pain or cramping with and after arousal or orgasm, is a very common symptom of PeVD. I was not a typical presentation for this diagnosis, so it surprised me when I found out from an ultrasound. So I share my story so that others with pelvic pain, constipation, heaviness, sharp abdominal pain, low back pain or pain with sitting, standing, or exercising, can start to become aware of this diagnosis and advocate for yourself. As many gynecologists do not believe this condition is a cause of pain…. And I can tell you… it most definitely IS! After two procedures using coil embolization and sclerotherapy to close off the dysfunctional varicose veins in my pelvis, it was identified that a compression of my Iliac vein was pretty severe and needed to be treated with a stent. I am two weeks postop and recovery has been a journey. Many things I was not expecting, so again, I wanted to share with the World so that maybe you can feel more prepared and aware of the treatment options out there for you! Her patients commonly get referred from OB/GYNs for chronic pelvic pain that is refractory to laparoscopic surgery and undiagnosed. She notes that collaborative relationships with women's health specialists and pelvic pain physical therapists are necessary for adequate patient outreach. Classifying cases by the location and nature of the vessel abnormality (i.e. compressive, obstructive, varicose, reflux, congenital) allows her to think about the best treatment for each patient. Targeting proximal veins can improve back and groin pain, dyspareunia, and heavy periods. Iliac vein stenting can improve compressive symptoms 50% of the time. On the other hand, isolated labial pain is best treated by directly targeting labial varices. Perineal ultrasound is a helpful way to locate some varicosities, but Dr. Spencer prefers MRI and digital subtraction venography to get a comprehensive venous picture and correlate symptoms with imaging. Next, the doctors discuss embolization and foam sclerotherapy. Through her experience, Dr. Spencer has seen sclerotherapy work better in varices with slower outflow and coil embolization work better for varices with more rapid flow. She prefers oversized floppy coils to minimize the risk of migration. With both treatments, there can be significant insurance barriers. It is important to utilize preauthorization specialists and be aware of what the patient's insurance will cover, in order to better frame a conversation about treatment options. After the procedure, maximal pain relief can be achieved anywhere between 3 to 6 months. During this period, it is important to counsel patients over adjunct therapies such as pelvic floor therapy, steroids, and puncture aspiration to remove trapped blood. Overall, Dr. Spencer wants IRs to keep in mind that the chronic pain population has faced many misdiagnoses and insurance barriers, so they might harbor mistrust of the healthcare system. Posterior open-wedge osteotomy and glenoid reconstruction using a J-shaped iliac crest bone graft showed promising clinical results for the treatment of posterior instability with excessive glenoid retroversion and posteroinferior glenoid deficiency. In conclusion, biomechanical analysis of the posterior J-graft demonstrated reliable restoration of initial glenohumeral joint stability, normalization of contact patterns comparable with that of an intact shoulder joint with neutral retroversion, and secure initial graft fixation in the cadaveric model. Currently, his main research focus is investigating FLIA: blood flow limitation of the iliac artery, a condition that affects certain athletes even at the very elite level, sometimes with surgery as the end outcome. Ryan Church, DNP is our guest for this episode of the Mountain Land Pelvic Health Podcast. Ryan has a doctorate degree of nursing practice from the University of Utah and is currently practicing with St. Mark’s Comprehensive Spine Institute. He is a spine care specialist in Salt Lake City. We speak with Ryan on SIJ dysfunction, common presentations, medical and physical therapy diagnosis and treatment approaches.
Rachael Reddy is studying Nutrition and Psychology at The University of Alabama. She runs cross country and track at Alabama and has made All SEC teams for cross country and track, as well as regionals (still with the goal to make a national meet). Rachael competed for USA at the Junior Pan Americas and brought home a gold medal in the 5k. She is passionate about creating healthy recipes, volunteering with kids, writing letters for others, and of course running! Questions we ask Rachael include: How did you get your start in running? In high school you transformed from a soccer player to a runner. What drew you into running rather than soccer? Due to your success, you were able to attend your dream school, the University of Texas. What was the recruitment process like? What did you major in? How did you know Texas was the right school for you? Upon arrive to the University of Texas, you spoke with your coach about your eating disorder. What was this conversation like? How did you gain the confidence to speak with him? Instead of spending your first semester of college at the university, you took a leave of absence and went to The Victory Program. What were some events that led you to seek recovery in The Victory Program? What was your time like here? What was the most valuable thing you gained or learned? What was it like when you were “forced” to transfer to the University of Alabama? Throughout your collegiate career you suffered an incredible amount of injuries...hamstring tear, mono, sacral stress reaction, Iliac crest avulsion fractures, IT band syndrome, and currently an Achilles tear. What was this like emotionally and physically for you? What were some of the aspects of this time that “kept you going”? Did you ever consider giving up your running career? How has the rehabilitation process for your Achilles tear been? A quote you mention in the piece you wrote for the SRC website is, “May the comeback be greater than the setback.” What do you envision for your comeback? What is currently making you thrive?
