Dee's mother has been struggling with a non-healing wound on her toe since 2019, which has led to a series of complications. From oxygen therapy to the diagnosis of osteomyelitis (bone infection), they have explored various treatments, but wounds persist. Now, Dee is determined to find the best specialist and facility to give her mother the greatest chance of success and minimize complications, which may include clearing blockages in her Mom's arteries below-the-knee and below-the-ankle into the pedal-plantar artery loop in the foot. In this episode, the hosts bring together a panel of experts in the field of PAD treatment, including Dr. Christopher Lee Stout, a Vascular Surgeon from Ozark Vascular in Arkansas, and Dr. Dan Simon, an Interventional Radiologist from Edison Radiology Group in New Jersey. In this conversation we will examine the principles of coagulopathy and novel approaches to coagulopathy within pre-hospital care. We will examine the utility of fibrinogen concentrate, the distinct features of fibrinogen concentrate with cryoprecipitate, its longevity of use compared to other blood products, how you constitute fibrinogen concentrate for use in the pre-hospital environment amongst other topics. To do this I have Ross Davenport with me, Ross is a Consultant Trauma & Vascular Surgeon at the Royal London hospital in the UK. He is also a Senior Lecturer in Trauma Sciences and has previously worked in prehospital care for both London and Essex & Herts Air Ambulance. His research has focused on trauma-induced coagulopathy, acute response to injury and the treatment of major trauma haemorrhage. His work in traumatic coagulopathy and massive transfusion, inflammation and organ dysfunction has had international renown. He begins by recounting the first time he was approached by a patient suffering from Stage 3 lower extremity lymphedema, along with his initial thought process/treatment plan for the patient. He then goes on to speak on several similarities/challenges he has encountered through treating a number of patients with lymphedema, including managing/treating psychological body dysmorphia in most patients after successful surgical treatment of lymphedema. Kuldeep underscores the gravity and risks associated with moving forward with the Charles procedure (which is also known as lymphedema debulking operation), how such a surgical option should only be considered for patients with Stage 3 lymphedema, and how to encourage and promote consistent pre-operative care. Kuldeep goes onto discuss the operation itself in detail - explaining how the surgery is best done as a team (Vascular (dissecting soft tissue and placing Wound VAC for 1 week to promote formation of granulation tissue), Plastics (skin grafting/reconstruction after 1 week), and Pathology), working within the fascial plane (differentiating fascia, muscle, and fat), dealing with calcified fat effectively, difficulty of identifying fascia in setting of lymphedema, and working near the ankle (avoiding iatrogenic injury of anterior and posterior tibial arteries and achilles tendon). Show Notes Sharif is a vascular surgeon currently living in Manhattan. After graduating from Harvard in 1992, he went to NYU for medical school. He completed his residency and fellowship at Mount Sinai and was on faculty there for 12-13 years before going to Cornell in 2016. Sharif is also a father of two sons, and, throughout the years, he has managed to maintain some of his Harvard friendships and connections to his family in Egypt. He is currently part of the Society for Vascular Surgeries Communication Council to help raise awareness of the field. The Role and Responsibilites of a Vascular Surgeon Vascular surgeons are responsible for diseases involving blood vessels outside the heart and brain. Sharif explains how they treat these diseases with open surgical techniques, including bypasses, stents, and balloons. Vascular surgeons also work with orthopedic surgeons, cancer surgeons, neurologists, or any time there is a challenge when dealing with blood vessels. Sharif talks about the diseases he commonly treats and the techniques he uses to perform surgery such as magnification in addition to muscle memory through repetition, and vascular repair techniques. Sharif discusses his work as the Program Director for the Vascular Surgery Fellowship of New York Presbyterian Hospital, which he has been running since 2016. To further trainees' education, Sharif has been involved in simulation training and runs a boot camp that has been operating since 2017. The boot camp is an in-person course with faculty from across the country and focuses on the technical side of vascular surgery and is designed to teach techniques without putting patients at risk. The program uses endovascular simulator and cadaver workshops to provide trainees with hands-on experience. This also gives them a chance to get to know the wider vascular community. The Vascular Podcast, Audible Bleedings Sharif's most proud accomplishment is the podcast he was asked to produce with the trainees. This podcast, Audible Bleedings, which was bought out to be the official podcast for the Society of Vascular Surgery, was trainee-driven and has become increasingly popular in the past couple of years. This is a great way for trainees to develop their content and professional development. On the podcast, Sharif interviews prominent vascular surgeons, giving listeners insight into the personal life of a vascular surgeon, how they approach the difficulties in their work, and what the process of being a vascular surgeon looks like. During the pandemic, his podcast provided the vascular community with knowledge and insight. He interviewed two of his colleagues from Seattle, Ben Starnes and Nitin Singh, who are both vascular surgeons and military surgeons, to discuss the impact of the COVID-19 pandemic in Seattle. He also had another guest, Mel Sheehan, a vascular surgeon at LSU, who had been there during Katrina and stayed throughout the pandemic. The conversation focused on how to respond to unexpected situations and the process of being a vascular surgeon. Recent Developments in Vascular Surgery Sharif points out that there are many specialists in the field, and talks about his focus, his practice, and recent developments in vascular surgery, including endovascular simulation and remote access points when the focus is on endovascular repair of abdominal aortic aneurysms. This procedure has revolutionized the treatment of these aneurysms, allowing patients to come in, have the procedure done percutaneously, under local anesthetic, and go home the next day. This technology has allowed for a much less invasive and much more convenient way of treating aneurysms, making it a major development in the field. He also talks about the limitations of current technology when it comes to repairing aneurysms. He explains that it can be difficult to maintain blood flow to branch vessels, such as the arteries to the kidneys and intestines, while also excluding the aneurysm. To solve this problem, devices have been developed that have branches with holes to maintain blood flow while still excluding the aneurysm. It is also possible to print out a 3D model of the vessel ahead of time to help in designing the device. CAT can be used to create a 3D model of a patient's blood vessels. Industry Collaboration in Health Innovations Vascular surgeons continuously need to develop technology to help patients. To do this, they have to partner with industries that have the resources to develop new technology, and that it is important to seek out industry partners to provide necessary support and experience. In order to create solutions for patients, it is essential that vascular surgeons and industry collaborate. He mentions that it is the surgeon's goal to do what is best for the patient and that they must remain aware that the job of industry is to sell the device. When reflecting on his career, Sharif initially found general surgery attractive because of the dynamic nature of the work and the ability to offer patients medical and surgical therapy. However, he grew to like vascular surgery due to its meticulous nature and the fact that it allows for minimally invasive procedures. He found his mentor, Dr. Michael Marion, Chief of the Vascular Division, and Dr. Larry Hollier to be inspiring and both were a major factor in his decision to pursue the specialty. Professors and Courses of Influence Ellozy also recalled his college days at Harvard where he found courses such as Literature Arts where he read Chaucer, and Ethics, and International Relations. However, he found the most impactful course was Cultural Illness and Healing taught by Arthur Kleinman. He specifically remembers a book they read called Forgive and Remember, which was written to help physicians deal with medical errors. In honor of International Women's Day, we are re-airing our interview with Dr. Averil Mansfield, Pioneering Vascular Surgeon and the UK's first female professor of surgery. She recently published her autobiography, Life in Her Hands. From a young age, Averil Mansfield wanted to become a doctor. But it was the early 1950s, and women were not expected to pursue a career in medicine. Against all expectations, Dr. Mansfield forged ahead and became a Vascular surgeon in the UK. In addition to her illustrious career, she has worked to support and encourage other women to enter the field of surgery. Dr. Mansfield was voted one of "100 Women Who Have Changed the World" and founded Women in Surgery, a Royal College of Surgeons initiative to encourage more women to enter the field. She was born into a working-class family and was educated at her local primary and grammar schools in Blackpool and then at Liverpool University graduating M.B.Ch.B in 1960. She plays the piano and music has been a prominent feature throughout her life. She learned to play the cello in retirement. After graduation, she trained in surgery in Liverpool and in addition spent two years in the USA. At the age of 34 she became a consultant general surgeon first in Liverpool and later moved to London. In London she practiced as a vascular surgeon and later [1993] became Professor of Vascular Surgery at Imperial College and Director of the Academic Surgical Unit at St Mary's Hospital. She was the first woman in the UK to become a professor of surgery and to head up an academic surgical unit. Her research interests centered on thrombosis and on arterial diseases along with their surgical treatment. Teaching and passing on skills have always been her passion and she hopes to inspire the next generation to achieve to the maximum of their capabilities. She has been President of The Association of Surgeons of Great Britain and Ireland [the first woman], President of The Vascular Surgical Society [the first woman], and the section of Surgery of the RSM. She was Vice President of the Royal College of Surgeons of England and chaired a number of its boards and the Court of Examiners and in 2009 became President of the BMA. She was recently a member of the working party looking at Diversity in Surgery under the chairmanship of Baroness Helena Kennedy. In retirement, she chaired the Council of the Stroke Association, the Board of Science of the BMA, and two medical charities. Charitable commitments and ambassadorial roles eg for Liverpool University are a continuing and important feature of her life. In 2019 she was awarded the Aneurin Bevan Lifetime achievement award at the celebration to mark the 70th anniversary of the NHS. In 2020 she was a castaway on Desert Island Discs.
Averil Mansfield was the UK's first woman vascular surgeon and first female professor of surgery. She qualified as a surgeon in the early 1970s, at a time when only two per cent of her colleagues were female – and was often met with disbelief bordering on amusement when telling people what she did. She talks to Woman's Hour about her medical achievements, which she downplays to, ‘It's just glorified plumbing,' as detailed in her memoir ‘Life in Her Hands.' A a new app to block child abuse images has received £1.8m pounds of EU funding, with the aim to help combat what has been described as a "growing demand" for child abuse images. According to the NSPCC, child abuse image offences have reached record levels with more than 30,000 reported in the last year. It also revealed that the police have recorded the first child abuse crimes in the metaverse, with eight instances recorded last year. We hear from Rani Govinder, Senior Child Safety Online policy officer from the NSPCC and John Staines, former police officer from E-Safety Training who goes into schools to educate children and teens about online safety. Dada Masilo is a South African choreographer, who is known for her re-working of classic stories to reflect black female identity. Her latest show is called Sacrifice, inspired by Stravinksy's iconic ballet Rite of Spring is on a national tour of the UK, and will be performed at the Sadler's Wells in London this weekend. Dr. Rasmussen is a retired Air Force Medical Corps officer and is currently a Professor of Surgery and Vice Chair of Education in the Department of Surgery at the Mayo Clinic College of Medicine and Science in Rochester, Minnesota. Col Rasmussen had the opportunity to deploy multiple times across the globe and shares how Military Medicine prepares and provides care from the tip of the spear through the evacuation chain to medical centers in the United States. Dr. Rasmussen explains how the work of he and his colleagues at the US Army Institute of Surgical Research and with the DoD Combat Casualty Care Research Program used data-driven research to improve how coalition forces care for wounded service members. He also is an incredibly talented Vascular Surgeon, and he shares some insights and lessons he's learned from memorable cases over his distinguished career. Dr. Rasmussen received his medical degree from the Mayo Clinic College of Medicine and trained in General Surgery at Wilford Hall Air Force Medical Center. He returned to the Mayo Clinic for fellowship training in vascular surgery. In this episode, we explore the role of vascular surgeons in oncologic surgery. We will be discussing the nuances of vascular involvement in oncologic resection, common practices, and future directions of this collaborative surgical culture. Dr. Adam Beck (@AWBeckMD) is a vascular surgeon and Professor of Surgery at the University of Alabama at Birmingham where he serves as the Director of the Division of Vascular Surgery and Endovascular Therapy, holds the Holt A. McDowell, Jr, MD Endowed Chair in Vascular Surgery and serves as the Director of Quality and Associate Chief Medical Quality Officer for the UAB Cardiovascular Institute. Dr. Beck completed his general surgery residency training and a surgical oncology research fellowship at the University of Texas-Southwestern Medical Center. He trained in vascular surgery at the Dartmouth-Hitchcock Medical Center and then completed a fellowship in advanced endovascular techniques, including branched and fenestrated endografts for aortic aneurysmal disease, at the University Medical Center of Groningen in The Netherlands. Dr. Xavier Berard (@XavierBerardMD) is a French vascular surgeon and serves as a Consultant in the Department of Vascular Surgery and Professor of Vascular and Endovascular Surgery in Bordeaux University Hospital in Bordeaux, France. He is board certified from the French College of Vascular and Endovascular Surgeons and from the European Society for Vascular and Endovascular Surgery. He has also completed a PhD in Vascular Biomaterials and has built a library of educational video content. He works closely with Institut Bergonié Bordeaux Cancer Center for sarcomas. In the summer, your feet are exposed to the elements more often which is especially concerning for those with diabetes, neuropathy, and Peripheral Artery Disease (P.A.D.), which is poor circulation due to plaque build-up in the arteries. Podiatrist Dr. David Alper joins hosts Kym McNicholas and Dr. John Phillips to talk about how to prevent common foot problems and complications. An important part of today's discussion was about how lack of circulation can put patients at-risk of more severe complications during the summer especially. Wounds don't heal as quickly and easily without proper blood flow. So, if you have diabetes or P.A.D., it's important to have a vascular specialist and podiatrist team up to help both on the inside of your arteries and outside of your foot. Dr. Anthony Pozun, an Interventional Cardiologist with Modern Vascular in the Phoenix area of Arizona, was listening to the show and called in to drive home that point. He added that tools and techniques for restoring blood flow have advanced to the point where some physicians know how to carefully navigate clear into the small vessels in the foot to help clear a path if necessary for even more flow to heal wounds faster, especially in diabetics who tend to have more vascular issues in their lower extremities. Patients wrote in many questions during today's broadcast as well about choosing appropriate shoes and proper toe nail care. Dr. Alper emphasized that flip flops are not the most supportive choice for the feet especially because they force you to grip the shoe with your big toe and second toe, which can lead to bunions and hammer toe problems. That can also pose a problem for those with neuropathy who might not feel a friction wound brewing between the two toes. Dr. Alper suggested that summer footwear should have at least a band around the heel so the foot remains secure and supported without effort. And if you do a lot of walking or running, that sneakers should be replaced every six months to ensure the best support for exercise programs. As for toe nail care, he discussed the importance of learning from a podiatrist how to cut your toe nails properly and to shy away from nail salons if you have diabetes, neuropathy, and/or P.A.D. One of the most important points Dr. Alper raises about avoiding summer foot problems is the use of sunscreen. He said that some of the most frequent visits to his clinic during the summer were by those with second degree burns or worse, not just on the top of their feet, but also the bottom. The latter of course is due to forgoing shoes on hot pavement. Also in this episode, you don't want to miss our Save My Piggies segment where a patient, Sheryl and her Vascular Surgeon in Oklahoma City, Dr. Bernardino Rocha, share an inspiring story of how a second opinion made the difference between life and limb. She found Dr. Rocha through nonprofit advocacy organization The Way To My Heart after she spent months in a wheelchair with no hope of ever walking again. Her primary care physician had misdiagnosed her symptoms of leg pain and cramps as gout and when medicine didn't help it, he offered her muscle relaxers and told her to drink plenty of fluid to relieve the cramps in her foot and calf. It wasn't until she went to a podiatrist for an unrelated issue that he discovered signs of a circulatory problem. Further testing uncovered she had Peripheral Artery Disease, restricted blood flow in the leg arteries due to plaque build-up. That is what was causing all the leg cramps. She thought the proper diagnosis would lead her to an easy solution, but her vascular surgeon told her that the only thing he could do was an extensive bypass that he didn't think would last very long and amputation was in her future. The Way To My Heart sent her to Dr. Rocha for a second opinion and he was able to easily clear the artery blockages using minimally invasive tools and techniques. After the angiogram procedure she felt immediate pain relief and was able to get out of the wheelchair, with a regular walking regimen each day whether on pavement or in the pool.
