Vulvar cancer screening refers to checking for signs of vulvar cancer in individuals who may not have symptoms. Vulvar cancer is rare, and there isn't a standard screening test specifically for it like there is for cervical cancer with Pap smears. However, early detection often relies on regular pelvic exams and self-examinations to look for any abnormalities or changes in the vulvar area. Dr. Melissa Mauskar joins me to discuss vulvar screening in this episode.Dr. Melissa Mauskar is a Dermatologist and Associate Professor in the Departments of Dermatology and Obstetrics and Gynecology at UT Southwestern in Dallas, Texas. She is the Director of Genital Dermatology and Women's Health and is an International expert in Lichen Sclerosus and Vulvar dermatoses. Melissa is a fellow of the International Society of the Study of Vulvovaginal Diseases (ISSVD) and the Secretary General of the North American Chapter of the ISSVD. In addition to caring for patients with lichen sclerosus, she is active in clinical research and passionate about patient quality of life and health literacy initiatives. In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Dr. Maaike Oonk to discuss the ESGO Vulvar Cancer Guidelines for 2023. Dr. Oonk is a gynaecological oncologist in the University Medical Center Groningen in the Netherlands. Her research focuses on new treatment options for vulvar cancer patients to improve quality of life. She is PI of the GROINSS-V studies and was chair of the 2023 ESGO vulvar cancer guidelines working group. Highlights: Inguinofemoral radiotherapy is a safe alternative for a lymphadenectomy in patients with micrometastasis in the sentinel node. In case of unilateral sentinel node involvement, it is probably safe to omit treatment of the contralateral (sentinel node negative) groin. Studies are ongoing on application of the sentinel node procedure in locally recurrent disease, tumors > 4cm or multifocal tumors: outside these studies, the sentinel node procedure should only be applied in patients with unifocal primary squamous cell vulvar cancer < 4cm. In advanced disease, both primary chemoradiation and radical surgery are treatment options. The Vulcanize study is investigating the role of neoadjuvant chemotherapy in these patients. Given the rareness of this disease, treatment of vulvar cancer should be centralized in specialized centers.

According to the American Cancer Society, vulvar cancer accounts for 6% of all cancers of the female reproductive organs. Over 1600 individuals will die from vulvar cancer per year. Cancers of the uterus, cervix, and ovaries are far more well-known, identified, and diagnosed. Yet, cancer of the vulva is a diagnosis that is often hard to obtain. Often, the symptoms can be mistakenly attributed to other skin conditions or yeast and bacterial growth. As a result, many individuals may experience a delay in diagnosis and treatment. This can often result in a delay of treatment for literally years. As with any cancer early detection and treatment leads to improved outcomes. Dr. Nasioudis is from the Division of Gynecologic Oncology at the Abramson Cancer Center of the University of Pennsylvania. His current research focuses on translational therapeutics and population-based research with an emphasis on rare gynecologic tumors. Highlights: • Prognosis of patients with vulvar melanoma and inguinal lymph node metastasis is poor. • In our study using real-world data approximately 1 in 4 patients received adjuvant immunotherapy with no clear overall survival benefit. • Further research to identify biomarkers to select patients who may benefit from immunotherapy is needed.
The vulva, quite often erroneously referred to as “vagina” or many other names, is the external part of the female genitalia. This complex organ is comprised of the mons pubis, labia majora, labia minora, clitoris, vestibule of the vagina, bulb of the vestibule and the Bartholin glands. The vulva is made up of mucous membranes, several glands, and highly innervated and sensitive skin and hair follicles. This makes it one of the most anatomically and physiologically intricate body parts.Vulvar health is dependent on many aspects of a woman's health, including:The presence of urine or fecal incontinenceMenopause and its changesMenstrual abnormalitiesSystemic or local dermatologic conditionsAll of these can have a negative impact on the vulva. Her patients commonly get referred from OB/GYNs for chronic pelvic pain that is refractory to laparoscopic surgery and undiagnosed. She notes that collaborative relationships with women's health specialists and pelvic pain physical therapists are necessary for adequate patient outreach. Classifying cases by the location and nature of the vessel abnormality (i.e. compressive, obstructive, varicose, reflux, congenital) allows her to think about the best treatment for each patient. Targeting proximal veins can improve back and groin pain, dyspareunia, and heavy periods. Iliac vein stenting can improve compressive symptoms 50% of the time. On the other hand, isolated labial pain is best treated by directly targeting labial varices. Perineal ultrasound is a helpful way to locate some varicosities, but Dr. Spencer prefers MRI and digital subtraction venography to get a comprehensive venous picture and correlate symptoms with imaging. Next, the doctors discuss embolization and foam sclerotherapy. Through her experience, Dr. Spencer has seen sclerotherapy work better in varices with slower outflow and coil embolization work better for varices with more rapid flow. She prefers oversized floppy coils to minimize the risk of migration. With both treatments, there can be significant insurance barriers. It is important to utilize preauthorization specialists and be aware of what the patient's insurance will cover, in order to better frame a conversation about treatment options. After the procedure, maximal pain relief can be achieved anywhere between 3 to 6 months. During this period, it is important to counsel patients over adjunct therapies such as pelvic floor therapy, steroids, and puncture aspiration to remove trapped blood. Overall, Dr. Spencer wants IRs to keep in mind that the chronic pain population has faced many misdiagnoses and insurance barriers, so they might harbor mistrust of the healthcare system. Even though a HUGE number of women of all ages have vulvodynia, it just isn't a topic that you talk about in the break room at work. In fact, it is estimated that 40% of women who have it have NEVER discussed this chronic problem with their health practitioner. Until recently it has been a syndrome with a lot of proposed causes but no definite proof that any of these are the real issue. However, more research is being done with the vaginal microbiome and how changes in the microbiota might act upon pain nerves, the immune system, and more. Vulvar lichen sclerosus is a chronic condition that causes thin, white patches of skin, usually in the genital area. This condition occurs primarily in girls who haven't started menstruating and in postmenopausal women. Lichen sclerosus is thought to be related to genetics, immune disorders, trauma, or infection. Lichen sclerosus can cause discomfort, itching, and easy bruising or tearing of the skin. In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Dr. Mario Preti to discuss the ESGO, ISSVD, ECSVD, and EFC consensus statements on pre-invasive vulvar lesions. Dr. Preti received his Specialization in Gynecology and Obstetrics and Specialization in Medical Oncology at the University of Torino. He is Associate Professor at the Department of Surgical Sciences, University of Torino, Italy, and the Immediate Past President of the ISSVD (International Society for the Study of Vulvo-vaginal disease). Dr. Preti is the author of 4 books on vulvo-vaginal diseases, and as of June 2022, he has published 108 papers indexed in Scopus/Pubmed, with 2,014 citations and an H-index of 23. Highlights: Epidemiological data show no decrease in vulvar invasive cancer incidence. This means that accurate diagnosis and treatment of pre-invasive vulvar disease must be improved: these are among the ESGO-ISSVD-ECSVD aims. The two carcinogenic pathways of vulvar squamous neoplasia recognize as precursors two vulvar intraepithelial neoplasia (VIN): HPV-associated squamous intraepithelial lesions (High Grade VIN) and HPV-independent VIN (mainly differentiated VIN). These precursors have different clinical approach, treatment and oncological risk. Their histologic features can be subtle, and the histological diagnosis may be further complicated by coexisting conditions. For differentiated VIN, an excisional procedure must always be adopted. Medical treatment (imiquimod or cidofovir) and ablative treatment can be considered for high grade VIN to preserve anatomy and function. These conservative approaches must be preceded by representative biopsies to exclude invasive disease. Follow up should be modulated according to the risk of recurrence (type of lesion, patient age and immunological conditions, other associated lower genital tract lesions). Pre-invasive vulvar lesions deserve specific attention because they affect not only functionality and body image, but also psychosexual factors.
In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Dr. Gerda Trutnovsky to discuss the use of imiquimod versus surgery for the treatment of vHSIL. Dr. Trutnovsky studied at the University of Graz/Austria and the University of Sydney/Australia. She works as an associate professor in the Department of Obstetrics and Gynecology at the Medical University of Graz/Austria, with a special interest in vulvar disease. Highlights: This was the first prospective trial of imiquimod versus surgery for the treatment of vulvar high grade squamous intraepithelial lesions (vHSIL). Complete clinical response at 6 months was observed in 80% of patients using imiquimod per-protocol, compared to 79% in women who had one surgical intervention. Imiquimod was used in a slowly escalating dosage scheme up to 3 times a week and reduced in case of side effects – local toxicity was mostly mild to moderate. Imiquimod is a safe, effective and well accepted alternative to surgery and can be considered a first-line treatment option.
