Diverticular disease is a condition characterised by the presence of small pouches, called diverticula, in the wall of the colon (large intestine). These pouches can develop when weak spots in the colon's muscular wall give way under pressure. Diverticular disease encompasses two main conditions: diverticulosis and diverticulitis. At the end of Ed's fellowship, Ed spent some time in Basingstoke, UK, with Professor Bill Heald and Professor Brendan Moran further subspecialising within the fields of pelvic cancer. This episode centers around acute, lower gastrointestinal bleeding. Diverticular disease is the most common cause of LGI bleeding. Most cases of LGI bleeding stop spontaneously. Risk factors for poor outcome include: hemodynamic instability on presentation, advanced age, coexisting conditions, the use of dual antiplatelet agents, and fresh blood on digital rectal exam. We also discuss resuscitation with crystalloid initial therapy. Transfusion should be limited to specific hemoglobin goals (~7-9 g/dl). Although it becomes more and more common with age, it's not actually aging that causes it. Rather, it's the accumulation of damage from lifestyle choices throughout the years. Why is this good news?
