With about 700 pregnancy-related deaths every year, the United States has the worst maternal mortality rates among developed countries.One response to this crisis has been the creation of state and local maternal mortality review committees (MMRCs), which are multidisciplinary committees that examine the context in which maternal deaths occur.The October 2021 issue of Health Affairs is focused on the topic of perinatal mental health. MMRC reports provide a treasure trove of information that can help us understand the role perinatal mental health plays in maternal mortality and what it might take to achieve better results.Susanna Trost, an Oak Ridge Institute for Science and Education Fellow at the Centers for Disease Control and Prevention, joins Health Affairs Editor-in-Chief Alan Weil on A Health Podyssey to discuss addressing perinatal mental health as a factor in maternal mortality.Trost and co-authors published a paper in the October 2021 issue of Health Affairs analyzing data from 14 state and local MMRCs regarding pregnancy-related maternal death. Studying these determinations from 2008 to 2017, they identified the characteristics of, and factors contributing to, pregnancy-related deaths caused by mental health conditions, including substance use disorder.They found that mental health conditions are the underlying cause of nearly one-in-nine pregnancy-related deaths, all of which were considered preventable by MMRCs. Ridgeview's Continuing Education Committee members and presenter(s) have disclosed they have no significant financial relationship with a pharmaceutical company and have disclosed that no conflict of interest exists with the presentation/educational event. SHOW NOTES: Chapter 1: CPR was introduced in the 1960s. At the time, research at John Hopkins was being conducted on pigs in ventricular fibrillation and ventricular tachycardia who required defibrillation.  It was noted that when pushing defibrillation paddles against the chest wall, the arterial pressure of the subjects increased. It was concluded that with compression, pulsatile flow was generated. This was essentially the first iteration of closed chest CPR. Over the next 50 years or so, clinicians have been looking for better ways to improve survival outcomes. Key factors include early identification, early CPR, along with obtaining resources and assistance to improve ventilation, perfusion, and defibrillation. Poor predictors of survival for cardiac arrest include CPR for greater than 30 minutes and individuals with coronary artery blockage. With the integration of ECMO, it was found that by bypassing the heart and lungs and essentially taking on their functions, outcomes improved. Dr. Yannopoulos reiterates that CPR for greater than 30 minutes results in poor outcomes, thereby creating a goal to both normalize pressures during this time while also fixing the offending cause, such as a coronary artery occlusion.  Around 2015, in the Minnesota metro area, visionary EMS directors implemented an alternative option: If a patient fails three shocks, with ongoing CPR, the patient was transferred to a tertiary care facility with ECMO cannulation capabilities. Once on ECMO, these patients were transferred to the University of MN where they underwent PCI to evaluate for reversible causes such as occluded coronary vessels, PE, etc. Outcomes of this therapy showed 30-40% patient survival, which is a game changing result, thus the need for a randomized control trial, the ARREST Trial. Chapter 2: The ARREST Trial studied a group of patients with out-of-hospital cardiac arrest (OHCA) in ventricular fibrillation, refractory to defibrillation and initial ACLS treatment. This was a randomized control trial where one group was randomized to an ECMO intervention arm, versus a standard ACLS therapy arm. It was a phase 2, single center, open-label, adaptive, safety and efficacy randomized clinical trial.  Specific subject criteria included adults aged 18-75 with OHCA, refractory ventricular fibrillation, with no return of spontaneous circulation (ROSC) after three shocks, with an automated cardiopulmonary resuscitation device or LUCAS device and estimated transfer time shorter than 30 minutes. The primary outcome of the trial was survival to hospital discharge. Secondary outcomes included safety, survival and functional assessment at hospital discharge, at 3 months and 6 months after discharge. Results of the ACLS arm showed a 7% survivability, and zero at both three and six months, while the ECMO arm fared better with a 43% survivability. After enrolling 30 patients, the study was terminated at the first pre-planned interim analysis by the National Heart, Lung and Blood Institute after a unanimous recommendation from the Data Safety Monitoring Board, because the posterior probability of ECMO superiority exceeded the prespecified monitoring boundary. Cumulative 6-month survival was significantly better in the early ECMO group compared to the standard ACLS group. No unanticipated serious adverse events were observed. Conclusion: early ECMO-facilitated resuscitation for patients with OHCA and refractory ventricular fibrillation, significantly improved survival to hospital discharge compared with standard ACLS treatment. Chapter 3:  The Minnesota Mobile Resuscitation Consortium or MMRC, which was the first program to serve an entire metropolitan area in order to rapidly deliver extracorporeal membrane oxygenation (ECMO)-facilitated resuscitation to patients with refractory ventricular fibrillation/ventricular tachycardia (VF/VT) out-of-hospital cardiac arrest (OHCA). This was an observational cohort study that analyzed consecutive patients prospectively enrolled in the MMRC's ECMO-facilitated resuscitation program. Entry criteria were identical to the ARREST Trial, adults 18-75 with an out-of-hospital cardiac arrest in VF or CT with no return of spontaneous circulation post 3 shocks, use of a LUCAS or automated cardiopulmonary resuscitation device, and an estimated transfer time of less 30 minutes. The primary endpoint was functionally favorable survival to hospital discharge with Cerebral Performance Category (CPC) 1 or 2. CPC 1 results in good cerebral performance: conscious, alert, able to work, that might have some mild neuro or psych deficits. CPC 2 results in moderate cerebral disability; conscious, sufficient cerebral function for independent activities of daily living (ADLS). Secondary endpoints included 3-month functionally favorable survival. Between the period of December 1, 2019 and April 1, 2020, 63 consecutive patients were transported, and of these 58 were treated by the mobile ECMO service. Post EMCO treatment 25 of the 58 or 43% were both discharged from the hospital and alive at 3 months with CPC 1 or 2. This first, community-wide ECMO-facilitated resuscitation program in the US demonstrated 100% successful cannulation, 43% functionally favorable survival rates at hospital discharge and 3 months. Chapter 4:  As the research continues, reaching 25-30% positive outcomes would be adequate to start a viable program. Dr. Yannopoulos speculates that with overall system improvement and refined protocols, that number can be as high as 70-80%, which is an incredible number. The future of ECPR will require a combined effort of the healthcare systems, policy makers, administrators, and communities. With a successful program in the urban and suburban areas, future research can hopefully expand access to more rural populations. Dr. Yannopoulos emphasizes that this is not a hospitalized based program, it relies on the EMS system, and those providers, especially medics, are key to the success of the program. At the time of this interview, the mobile ECMO program was on hold due to COVID, but currently the mobile ECMO program has a pending restart date of March 1st. Thanks for listening.

A new monoclonal antibody called Isatuximab is advancing through clinical trials and is getting closer to FDA approval. This antibody targets CD38. Learn more from Dr. Ravi Vij of Washington University as he discusses the findings from recent clinical trials, how this drug will be used in the clinc and how it is different from other monoclonal antibodies previously approved. Dr. Vij was the senior author on the first Isatuximab paper published in Blood and has been involved in several clinical trials to date.  Dr. Ravi Vij is Associate Professor of Medicine at the Washington School of Medicine in the Bone Marrow Transplant section. He received his medical degree in India and did follow-up training and residency at Rush-Presbyterian St. Luke’s Medical Center in Chicago and fellowships in Oncology and Bone Marrow Transplantation with Stem Cell Biology at Washington University. He’s a member of ASCO, ASH, the St. Louis Society of Clinical Oncology, and the American Association for the Advancement of Science. He serves on numerous national committees including the Myeloma, Transplant, Leukemia Committees of the Alliance for Clinical Trials in Oncology and the Steering Committee of the MMRC. His honors include the Multiple Myeloma Research Foundation Innovator Award in 2013 and the MMRC Center of Excellence Award. He’s the author of over 100 peer-reviewed publications, as well as a book called Contemporary Management of Multiple Myeloma, and several book chapters. He served as a reviewer for several journals, including Blood, Journal of Clinical Oncology, Bone Marrow Transplantation, Experimental Hematology, and Haematologica.

At the University of Michigan, Dr. Craig Cole is working on a collaboration with the MMRC to perform genomic sequencing on 500 hundred relapsed and refractory myeloma patients over a two year period. The goal is to share each patient's genomic profile to identify patient-specific treatment options. The study will be open at 11 myeloma academic centers to begin, with more to open in the future. The tissue samples collected at these centers are shipped to the University of Michigan Comprehensive Cancer Center, which then evaluates the alterations of 1700 genes. The study aims to find the specific mutations that are "actionable," or that have a drug that can be used. Hintergrund: Von den weltweit mehr als zwei Milliarden mit Würmern (griech. Helminthen) infizierten Menschen sind schätzungsweise etwa 22 Millionen zusätzlich mit HIV koinfiziert und verbleiben durch die krankheitsbedingte Produktivitäts- und Leistungsminderung in einer Abwärtsspirale. Trotz großangelegter globaler Versuche, Helminthosen mit präventiven Massenbehandlungen einzudämmen, gibt es weiterhin großen Forschungs- und Handlungsbedarf. Eine sensitivere und zuverlässigere Diagnostikmethode als die klassische Stuhlmikroskopie ist dringend erforderlich. Problemstellung und Zielsetzung: Etablierung eines molekularen Wurmdiagnostikverfahrens (Multiplex-Real- Time-Polymerase Chain Reaction) am Mbeya Medical Research Center (MMRC)-Labor im einkommensschwachen Tansania zur Detektion verschiedener Helminthen im Stuhl und direkter Vergleich mit der Merthiolat- Jod-Formalin (MIF)-Mikroskopie sowie dem derzeitigen Diagnostikgoldstandard der Kato-Katz (KK)-Mikroskopie. Klärung der Frage, ob das neue Verfahren wirklich sensitiver und für weitere Untersuchungen einsetzbar ist. Prüfung der Wirksamkeit von zur Massenbehandlung eingesetzten Medikamenten und Untersuchung von Besonderheiten bei HIV-Helminthen-koinfizierten Probanden mit Hilfe des neuen Verfahrens. Material und Methoden: Zwischen 2008 und 2011 Probensammlung, Erhebung der HI-Viruslast und CD4-Zellzahl, Stuhlmikroskopie nach der MIF- und KK-Methode, sowie Etablierung, Validierung, Durchführung und Evaluierung der Real-Time-PCR (RT-PCR) für 179 Probanden in Mbeya, Tansania. 118 Studienteilnehmer wurden nach einer Einmalgabe von 400 mg Albendazol gegen intestinale Nematoden und 40 mg/kg Praziquantel gegen Schistosomeninfektionen mit der Mikroskopie sowie der RT-PCR nachuntersucht. Zusammenfassung 108 Ergebnisse: Bei fast allen Probanden lagen laut WHO-Klassifikation leichte Infektionen vor. Eine externe sowie interne Qualitätskontrolle bestätigte die Validität unserer RT-PCR. Der Methodenvergleich ergab eine hohe Diskordanz zwischen den einzelnen Verfahren. Die RT-PCR wies 2,2 mal mehr Infektionen als MIF und 1,4 mal mehr Infektionen als KK nach. Bei etwa 17% der untersuchten Probanden lag ein Polyparasitismus vor. Mit der RT-PCR wurden mehr Mischinfektionen nachgewiesen. Ein niedriger Cycle Threshold (CT) korrelierte mit einer hohen Eizahl und umgekehrt. Die Infektionsstärke bei HIV-Wurmkoinfizierten Probanden unterschied sich nicht von der HIV-negativer Probanden. HIV-Positive schieden trotz ihrer Immunschwäche nicht weniger Schistosomeneier am Tag aus als Nichtinfizierte. Ein Polyparasitismus lag bei HIV-Positiven (37% Mischinfektionen) nicht häufiger als bei HIV-Negativen (43% Mischinfektionen) vor. Schlussfolgerung und Ausblick: Die RT-PCR erwies sich als wesentlich sensitiver als die Mikroskopie bei der Detektion von Hakenwürmern und Schistosomeninfektionen; nicht aber bei Askarideninfektionen. Als alleinig eingesetztes Diagnostikverfahren am MMRC hat sie daher keinen Bestand. Sie wird für zukünftige Studien als sensitives Zweitverfahren, das Hakenwurm-, Schistosomen- und Mischinfektionen besser nachweisen kann, neben der klassischen Kato-Katz-Mikroskopie Anwendung finden. Durch diese Arbeit konnte der Grundstein für die Diagnostik zukünftiger Studien am MMRC gelegt werden, die im Rahmen des IDEA-Projektes, einer großen afrikanisch-europäischen Forschungsinitiative, die unter anderem wurminduzierte Immunantworten in Bezug auf Koinfektionen mit HIV, Malaria und Tuberkulose und den Einfluss von Würmern auf Impfstoff-induzierte Immunantworten untersucht, durchgeführt werden. Es sollten weitere Studien zur Sensitivität der verwendeten RT-PCR-Protokolle an anderen Standorten durchgeführt werden. In Mbeya wurde das Verfahren nach meiner Abreise erfolgreich weitergeführt, sodass inzwischen fast 1000 Stühle mit der RT-PCR untersucht worden sind.

Did you miss us? We missed you sooooo much. Now, with the Holidays out of the way, we are super excited to get back into the recap groove. This is our first (and only) recap recap. Yes, we are recapping "Guy Walks into an Advertising Agency" from season three, and this was our very first recap over 2 years ago. And in true MMRC fasion we explore the dangers of office lawn furniture, the peril of having stupid fingers and the shame of being related to Paris Hilton.
