A Commonwealth Ombudsman report has found Services Australia knowingly breached child support laws for six years, denying payments to parents with less than 35 per cent care. At least 16,600 people were affected, with some owed up to $10,000. The report highlights unintended legal consequences, financial abuse risks, and recommends law changes, compensation, and stronger oversight to recover funds and protect vulnerable families.
Months after families raised alarms over substandard care at Kimberley Mental Hospital following a damning Ombudsman report, the Northern Cape Health Department confirms investigations are underway into alleged neglect. One patient is currently bedridden after she was discharged, and another allegedly died after being assaulted by a male nurse.

A large insurance bill is looming from the latest weather-related disasters. The landslide at Mount Maunganui combined with flood damage in Northland, Coromandel and Gisborne means that the claims are set to roll in. Insurance and Financial Services Ombudsman Karen Stevens told Ryan Bridge that people affected can expect delays given the high number of claims being made. "I think everybody understands that with a significant weather event, there are going to be more delays than if it's just business as usual." Cybersecurity failures are no longer just IT problems. They are legal, financial, and leadership failures. In this episode of Security Squawk, we break down how a ransomware attack on Ireland's Office of the Ombudsman delayed justice for citizens and what that incident reveals about preparedness, accountability, and real-world consequences of cyber risk. We start with the Ireland cyberattack that forced a key public watchdog agency to halt case processing for months. This was not a minor disruption. Systems were taken offline, legal action was required to prevent potential data leaks, and people relying on the system became collateral damage. The story highlights a hard truth. When cybersecurity fails, mission failure follows. Government or private sector, the outcome is the same. From there, we zoom out to the private sector where the warning signs are flashing red. New survey data shows cybersecurity litigation risk is rising faster than any other legal exposure for U.S. businesses. Corporate legal teams expect cyber and data privacy disputes to intensify, yet fewer of them feel prepared compared to last year. That gap tells us everything we need to know. Companies understand the risk is growing, but they are not investing or aligning fast enough to reduce it. We also examine the dangerous confidence gap in middle market firms. Nearly one in five experienced a cyber incident, yet almost all executives still believe their security posture is strong. Confidence without controls is not resilience. It is exposure. This disconnect raises serious questions about leadership accountability and how security decisions are being made at the executive level. The episode also dives into research showing that many top U.S. companies still fail basic cybersecurity hygiene. Reused passwords, outdated software, poor configuration, and unpatched systems remain common in 2025. These are not advanced threats. These are fundamentals. When organizations cannot execute the basics, the issue is not technical skill. It is culture, discipline, and leadership priority. We discuss the ongoing wave of data breaches affecting insurance, healthcare, and business services organizations, exposing millions of records. These incidents are proof that many companies remain reactive instead of proactive. Third-party risk, weak internal controls, and poor governance continue to amplify the damage. Finally, we tackle a growing blind spot. AI security governance. As businesses rapidly adopt AI tools, many still lack formal rules, oversight, or risk frameworks. Without governance, innovation turns into liability. Attackers move faster than policy, and organizations are left exposed. This episode is a wake-up call for business leaders, MSPs, IT professionals, and security decision-makers. Cybersecurity is no longer about compliance checklists or technology spend. It is about reducing real risk, protecting trust, and leading responsibly. If you want to understand why cyberattacks now lead to lawsuits, why confidence is not the same as security, and why leadership decisions matter more than ever, this episode delivers the insight you need. Nuacht Mhall. Príomhscéalta na seachtaine, léite go mall.*Inniu an tríú lá déag de mhí na Nollag. Is mise Siubhán Nic Amhlaoibh.Tá teaghlach Patrick Rooney, an chéad leanbh a fuair bás sna Trioblóidí, le "cúiteamh suntasach" a fháil tar éis caingean dlí i gcoinne Sheirbhís Póilíní Thuaisceart Éireann as a dhúnmharú mídhleathach sa bhliain 1969. Lámhachadh Patrick, buachaill 9 mbliana d'aois, ina cheann ag Árasáin Dhuibhise i mBéal Feirste le linn oibríochta ag Constáblacht Ríoga Uladh (RUC), a scaoil suas le 200 babhta ó ghunnaí meaisín suite i bhfeithiclí. Bhí Patrick ina árasán lena theaghlach, ag lorg foscadh i seomra leapa, nuair a bhuail piléar é. Chuir an RUC bac ar imscrúduithe ina dhiaidh sin, agus fuair tuarascáil Ombudsman na bpóilíní in 2021 teipeanna suntasacha oibríochtúla agus imscrúdaitheacha. Mar thoradh ar an chaingean dlí, a ghlac máthair Patrick, Alice Rooney, thángthas ar shocrú lena n-áirítear leithscéal foirmiúil ón PSNI, ina n-aithneofar fulaingt an teaghlaigh. Tugann an socrú, tar éis 56 bliain, roinnt ceartais don teaghlach.Seoladh foclóir nua Gaeilge an tseachtain seo ag an iarsmalann EPIC i mBaile Átha Cliath. Is é an foclóir seo, a sheol an tUachtarán Catherine Connolly ag imeacht Dé Máirt, an chéad fhoclóir cuimsitheach aonteangach Gaeilge-Gaeilge. Dúirt an tUachtarán Connolly gur "acmhainn ríthábhachtach in aon teanga bheo é foclóir aonteangach nua-aimseartha, ina leagann pobal na teanga amach saibhreas agus uathúlacht na teanga ina cuid focal féin, seachas trí mhéan teanga eile". Cuireadh tús le hobair ar An Foclóir Nua Gaeilge i mí Mheán Fómhair 2022, agus bhí príomheagarthóir foclóir Fhoras na Gaeilge Pádraig Ó Mianáin agus clárbhainisteoir foclóra Cormac Breathnach i gceannas ar an tionscadal. Tá sciar tosaigh de 20,000 iontráil, ina bhfuil 40,000 ciall focail, beo ar Focloir.ie. Tá príomhchéim an tionscadail foclóra le bheith críochnaithe faoi Lúnasa 2027, agus faoin am sin beidh 30,000 iontráil agus 80,000 ciall ann.Tá balla ollmhór faoi uisce aimsithe ag seandálaithe mara na Fraince amach ó chósta na Briotáine, atá thart ar 7,000 bliain d'aois. Síltear gur ó shochaí na Clochaoise a d'fhéadfadh sé a bheith agus gurbh é a imeacht faoi na farraigí mar bhunús le finscéal faoi chathair áitiúil a chuaigh faoi uisce. Creideann na seandálaithe gur gaiste éisc nó claí a bhí sa bhalla 120 méadar (394 troigh) – an foirgneamh faoi uisce is mó a fuarthas riamh sa Fhrainc – chun cosaint a thabhairt i gcoinne leibhéil na farraige ag ardú. Nuair a tógadh é ar Île de Sein ag rinn thiar na Briotáine, bhí an balla ar an chladach, ach tá sé faoi naoi méadar uisce sa lá atá inniu ann. Tá an balla 20 méadar ar leithead agus dhá mhéadar ar airde ar an mheán. Fuair tumadóirí clocha móra eibhir – nó monailití – ag gobadh amach os cionn an bhalla i dhá líne chomhthreomhara. Más ceart an hipitéis faoin ghaiste éisc, is dócha go mbíodh líonra déanta as bataí agus craobhacha faoi chúram na monailítí ag gobadh amach chun iasc a ghabháil de réir mar a tharraing an taoide siar. A new survey from the Children's Ombudsman has found that 63% of young people come across extremist views online, when only 6% of them come across those views in real life. The content seen by younger people of secondary school age include negative commentary on immigration and views that society should reverse to ‘traditional' gender roles. However, children still say that parents or guardians are the biggest influence in their livesWe discuss this all further with Colman Noctor, Child and Adolescent Psycotherapist.
