Hello everyone, I was first 'sucked in' to this forum when I was directed to a post I might find interesting - it was about a research organisation with EA endorsement that was straying into my area of work, mental health. I'm a UK doctor specialising in psychiatry, with some research experience. To be honest, I was baffled and a little frustrated by how far this organisation strayed from what I would expect from mental health research - hence the (perhaps overly) technical diatribe I launched into on a website I hadn't visited before, about an organisation I hadn't heard of prior. However, that's not usually my style, and once I took a step back from my knee-jerk reaction, I wanted to understand how people with the same goals could arrive at completely different conclusions. It's led me to do a lot of reading, and I wanted to see if I could try on a makeshift 'EA' hat, with most of my philosophy knowledge gained from The Good Place, no economics experience, and see where it went. What I wanted to understand: Where has the interest in 'wellbeing' arisen from, and what does it mean? What are 'subjective wellbeing' (SWB) measures, and are they useful? Are we at a point of putting monetary value on SWB (e.g. like QALYs) for the sake of cost-effectiveness analysis (CEA)? When people are in this space talking about mental health, are we talking the same language? Why are RCTs the 'best' evidence for subjective wellbeing? What would I come up with from my perspective of working within mental health for a way of comparing different interventions based on their intended effects on wellbeing? a. Spillover effects b. Catastrophic multipliers How does my guess stack up against existing research into wellbeing? How could my framework be helpful in practice? What would I be suggesting as research areas for maximal gains in wellbeing from my biased perspective? I'm aware this might be well-trodden ground in EA, which would make me embarrassingly late to the party, and consequently a complete bore. To lay my cards firmly on the table, I did approach these questions from the perspective that mental health is desperately underfunded, I spend a lot of time with patients who are severely affected by mental illness and therefore I'm biased towards seeing 'wellbeing' as an opportunity to rebalance this scale and acknowledge the impact mental illnesses have on people. I also feel the term 'mental health' is used in a way which is often confusing and occasionally unhelpful or stigmatising. This is not meant as an attempt to further an argument against any person or organisation; it will also not be high in tech-speak as this was the first lesson I learnt very quickly on my journey - while jargon is a useful shorthand for talking with people in the same field, as an outsider it is exhausting. This post does not reflect the attitudes or opinions of anyone but me - this is my personal quest for common ground and understanding, not a representation of 'UK psychiatry' - I'm speaking in an entirely personal capacity and, accordingly, I'm assuming I've gotten a lot of it completely wrong. To make this less self-indulgent, I've arranged this post to follow that question-and-answer format. For the sake of transparency, this was how this work came to be: I started with a long piece of writing about my concerns with assumptions made about mental health interventions in low- or middle-income country (LMIC) settings. I then did a quick Google on the WELLBY and wrote a lot about the idea of asking people to rate their 'satisfaction with life' on a scale from 0-10 which was essentially just entirely critical. The memo says that migrants can be allowed into the country on parole — a process typically reserved for "urgent humanitarian reasons or significant public benefit" — if Customs and Border Protection (CBP) faces overcrowding. The memo calls the practice "parole with conditions" as migrants are required to make an appointment with Immigration and Customs Enforcement (ICE) or request a Notice to Appear by mail. Under a parole release, migrants are rapidly released into the country, do not get an alien registration number and do not receive a court date. The use of parole is being authorized if a sector capacity goes above 125%, if agents apprehend 7,000 a day over 72 hours or if average time in custody goes above 60 hours. Agents have been encountering over 10,000 migrants a day since Monday, and there are no signs of that slowing down with the looming end of Title 42, which is expected to bring an even bigger wave with it. The memo noted the enormous numbers agents are encountering. "For the past 7 days, USBP has averaged over 8,750 encounters per day. This is over double the average daily encounters of 4,284 in May of 2019, the highest month of the 2019 surge. Even with significant personnel along the SWB, a significant detention capacity, and interagency resources supporting the effort, this situation requires urgent action," the memo states. The public health order has been used since March 2020 to expel over two million migrants due to the COVID-19 pandemic. The Biden administration has told migrants that it will not be easier to enter the U.S. once the order drops as there will be stiffer penalties until Title 8. Parole is not a Title 8 authority. The use of parole is legally questionable, with critics arguing it is not supposed to be used to wave in thousands of migrants. The state of Florida successfully sued the Biden administration in March to block a similar policy, "Parole + ATD." On Thursday, Fox Digital reported that Florida sued this week over the latest plan to release migrants into the U.S. interior, saying it is "materially identical" to the policy that the federal court enjoined. "Florida seeks a temporary restraining order to preserve the status quo until the parties can brief motions for a preliminary injunction or to postpone the effective date of the new policy," the lawsuit states. "The Biden Administration’s behavior, if left unchecked, makes a mockery of our system of justice and our Constitution." The judge has given the Biden administration a 4 p.m. deadline Thursday to respond to the request for a temporary restraining order, adding that it appears the administration is "preparing to flout" the court’s order. The bill was introduced to the House of Representatives on May 2 in anticipation of the flood of illegal immigrants expected in the wake of Title 42, which allowed expulsions due to the pandemic emergency. The bill calls for the "immediate resumption of border wall construction" no later than seven days after the bill is enacted, using previously set aside funds and materials for its construction. No later than 180 days after the bill is enacted, a report shall be submitted to congressional committees that outlines a strategic 5-year technology investment plan for border security, including potential upgrades in security technology, communications equipment, screening equipment, incorporating in input from the private sector. In an effort to retain Border Patrol personnel, the bill will authorize up to $100,000,000 in appropriated funds for the Commissioner of the US Customs and Border Protection to give to front-line agents who hold a position equal to or below level GS-12, has been with Border Patrol for five years or longer, and commits two additional years of service with Border Patrol upon acceptance of the bonus. The bill also requires that the Commissioner hire and train enough Border Patrol agents to maintain an active duty presence of no less than 22,000 full-time agents. The bill also includes a provision prohibiting the requirement to take a Covid-19 vaccine or adverse actions taken against a Department of Homeland Security employee. The Secretary of Homeland Security would be required to send a report no later than 60 days after the bill is enacted on whether Mexican drug cartels meet the criteria for being designated as a foreign terrorist organization. The bill amends the Immigration and Nationality Act, stating that those found not to be eligible for parole shall be deported from the country. The bill also increases penalties for illegal entry or presence in the country from $50-$250 up to $500-$1000. Illegal immigrants who overstay a visa are subject to imprisonment of no more than six months and/or a fine for the first offense, and a fine and/or no more than two years of imprisonment for subsequent offenses. Wesleyan University announced last week that, following a pressure campaign from the university's Young Democratic Socialists of America chapter, the school would now pay for any student to get an abortion or emergency contraceptives. The school has also committed to footing the bill for any transportation costs incurred by students seeking abortions. “Consistent with the health center’s mission to provide education and support in the decision-making process, students will be able to access emergency contraceptives by consulting with a nurse at Davison Health Center," the university told NBC Connecticut in a statement. The decision comes after the Young Democratic Socialists of America chapter at Wesleyan collected over 700 signatures on a petition that urged the school to "ensure all students are able to overcome financial burdens to and lack of transportation for abortion procedures." "This victory comes in the wake of landmark efforts by Barnard College students to secure abortion pills on campus as well as a broader movement for reproductive justice nationwide," the student organization said in a press release. "Wesleyan now plays a pioneering role in reproductive healthcare among private liberal arts colleges by committing to cover the entire cost of abortion after insurance for all students." After the Supreme Court issued a ruling that overturned Roe v. Wade last year, the availability of abortion for college students has gained increased attention, as numerous states with widely recognized state universities have banned abortion altogether. Students have increasingly reported that access to abortion has become a factor in their college decision-making process. "We cannot provide any additional information until he has been arraigned in Manhattan Criminal Court, which we expect to take place tomorrow." On May 1, Penny, a 24-year-old Marine veteran and college student, put 30-year-old Jordan Neely in a fatal chokehold after what police described as an altercation on a northbound F train. Neely was allegedly threatening passengers on the subway and having a mental episode. He was yelling and pacing back and forth, when Penny interjected, tackled him to the ground and put him in a chokehold, witnesses and police said. The incident sparked protests and violent clashes last week after the Marine veteran was taken into custody and released. However, based on new information, that might be over soon, and potentially in Araiza’s favor. During a nearly year long investigation by San Diego police and detailed review by prosecutors, they have come to the conclusion that the alleged gang rape that the female plaintiff accused Araiza of being part of stands in question since evidence shows that Arazia, then a college student at the time when the plaintiff was 17-years-old, was not even present during the alleged crime. Yahoo Sports has obtained a 200-plus page transcript, covering a meeting in which a San Diego deputy district attorney broke the back of the case. According Yahoo Sports’ summary, “Araiza couldn’t have led the girl into the alleged gang rape because he had ‘left’ the home at about 12:30 a.m., an hour prior to when evidence suggested the alleged gang rape would have occurred.” Deputy district attorney Trisha Amador stated that “He wasn’t even at the party anymore. All I know is that at that point, suspect Araiza is gone from the party.” Prosecutors additionally told the plaintiff that the recordings that were made of the situation “made it impossible” to identify Araiza or any other potential suspects since their faces and voices aren’t clear enough to make a prosecution. Further, there are questions as to whether a rape occurred at all as the plaintiff claimed, and not a consensual sexual situation. “In looking at the videos on the sex tape, I absolutely cannot prove any forceable sexual assault based upon what happened,” said Deputy DA Amador. Because of this lawsuit, then-rookie Araiza was cut from the Bills team, crippling his rookie season so he could “take care of this situation,” according to Buffalo general manager Brandon Beane. But Araiza isn’t out of the woods yet, despite the prosecutors themselves admitting he wasn’t present at the event – civil suits have a much lower standard of proof than criminal cases do. Summary This shallow cause area report explores the impact of lead exposure in childhood on subjective wellbeing (SWB) in adulthood. It was completed in two weeks. We performed non-systematic searches to estimate the impact of lead exposure on SWB and to find potential cost-effective interventions. We found studies investigating two correlational longitudinal cohorts (following children to adulthood) from New Zealand and Australia analysing the relationship between childhood blood lead levels (BLLs) and adult affective mental health (MHa). Based on this data, our best guess estimate is that an additional microgram of lead per deciliter of blood for each year for ten years of childhood, leads to a total lifelong (62 years) loss of 1.5 WELLBYs, and a larger overall 3.8 WELLBYs loss when we include some guesses about household spillovers. Hence, we estimate that a modest amount of lead exposure severely impacts on wellbeing across the lifespan. From several back of the envelope calculations, we tentatively conclude that lead-reducing interventions would be 1 to 107 times more cost-effective than cash transfers. Advocacy against lead in paint, food, cookware, and cosmetics seems particularly promising. The scarcity of causal and context relevant data means that we are very uncertain about the effect and cost-effectiveness of these interventions. But, given the potentially high cost-effectiveness, we think this is a promising area for additional research. We especially encourage further research into the causal relationship between lead exposure and SWB and the most common sources of lead exposure to reduce uncertainty about the cost-effectiveness of lead interventions. It's unclear if the top organisations working to reduce lead exposure, like Pure Earth or the Lead Exposure Elimination Project (LEEP), have sizable funding gaps. Therefore, we're unsure how much more work should be done to evaluate funding opportunities related to reducing lead exposure for philanthropists aiming to maximise their impact. Notes This report focuses on the impact of lead exposure in terms of WELLBYs. One WELLBY is a 1 life satisfaction point change for one year (or any equivalent combination of change in life satisfaction and time). In some cases, we convert results in standard deviations of SWB to WELLBYs using a 2 point standard deviation on 0-10 life satisfaction scales (i.e., 1 SD change is the equivalent of 2 point changes on a 0-10 life satisfaction scale). We consider the limitations of converting from affective mental health measures to WELLBYs in Appendix A4. This naive conversion is based on estimates from large scale data sets like the World Happiness Reports. See our post on the WELLBY method for more details. Our calculations and data extraction can be found in this Google Spreadsheet and this GitHub repository. The shallowness of this investigation means (1) we include more guesses and uncertainty in our models, (2) we couldn't always conduct the most detailed or complex analyses, (3) we might have missed some data, and (4) we take some findings at face value. Clare Donaldson was co-director of HLI before becoming the current co-director of the Lead Exposure Elimination Project. We do not think this influenced our choices or analysis. Their debut album, Trailer Park Trash, has a near-legendary reputation among many music fans. The guys welcome SWB guitar player Drew Fischer and bass player Emma Dalton into Thunderlove Studio to chat about the band. Object Level Interventions / Reviews by EJT, CarlShulman Linkpost for this paper, which uses standard cost-benefit analysis (CBA) with detrimental assumptions (eg. giving no value to future generations, only assessing benefits to Americans, and only assessing value from preventing existential threats) to show that even under those conditions governments should be spending much more on averting threats from nuclear war, engineered pandemics, and AI.Their analysis primarily relies on previously published estimates of risks, concluding US citizens alive today have a ~1% risk of dying from these causes in the next decade. They estimate $400B in interventions could reduce the risk by minimum 0.1 percentage points, and that using the lowest figure for the US Department of Transportation's value of a statistical life, this would result in ~$646B in value of American lives saved. They suggest longtermists in the political sphere should change their messaging to revolve around this standard CBA-driven catastrophe policy, which is more democratically acceptable than policies relying on the cost to future generations. They suggest it would also reduce risk almost as much as a strong longtermist policy (particularly if the CBA incorporates an argument for citizens ‘altruistic willingness to pay' ie. some level of addition for the benefit to future generations). by GiveWell The Happier Lives Institute (HLI) has argued that if Givewell used subjective well-being (SWB) measures in their moral weights, they'd find StrongMinds more cost-effective than marginal funding to their top charities. Givewell assessed this claim and estimated StrongMinds is ~25% (5%-80% pessimistic to optimistic CI) as effective as these marginal funding opportunities when using SWB - this equates to 2.3x the effectiveness of GiveDirectly. Key differences in analysis from HLI, by size of impact, include: GiveWell assumes lower spillover effects to household members of those receiving treatment. Givewell translates decreases in depression into increases in life satisfaction at a lower rate than HLI. Givewell expects lower effect in a scaled program, and lower durations of effects (not passing a year) due to the program being only 4-8 weeks. Givewell applies downward adjustments for social desirability bias and publication bias in studies of psychotherapy. These result in an ~83% discount in the effectiveness vs. HLI's analysis. For all points except the fourth, two upcoming RCTs from StrongMinds will provide better data than currently exists. HLI has posted a thorough response in the comments, noting which claims they agree / disagree with and why (5% agree, 45% sympathetic to some discount but unsure of magnitude, 35% unsympathetic but limited evidence, and 15% disagree on the basis of current evidence). Summary Subjective wellbeing (SWB) data, such as answers to life satisfaction questions, are important for decision-making by philanthropists and governments. Such data are currently used with two important assumptions: Reports are comparable between persons (e.g., my 6/10 means the same as your 6/10) Reports are linear in the underlying feelings (e.g., going from 4/10 to 5/10 represents the same size change as going from 8/10 to 9/10). Fortunately, these two assumptions are sufficient for analyses that only involve the quality of people's lives. However, if we want to perform analyses that involve trade-offs between improving quality and quantity of life, we also need knowledge of the neutral point, the point on a wellbeing scale that is equivalent to non-existence. Unfortunately, evidence on all three questions is critically scarce. We propose to collect additional surveys to fill this gap. Our aim with this report is two-fold. First, we give an outline of the questions we plan to field and the underlying reasoning that led to them. Second, we present results from an initial pilot study (n = 128): Unfortunately, this small sample size does not allow us to provide clear estimates of the comparability of wellbeing reports. However, across several question modalities, we do find tentative evidence in favour of approximate linearity. With respect to neutrality, we assess at what point on a 0-10 scale respondents say that they are 'neither satisfied nor dissatisfied' (mean response is 5.3/10). We also probe at what point on a life satisfaction scale respondents report to be indifferent between being alive and being dead (mean response is 1.3/10). Implications and limitations of these findings concerning neutrality are discussed in Section 6.2. In general, the findings from our pilot study should only be seen as being indicative of the general feasibility of this project. They do not provide definitive answers. In the hopes of fielding an improved version of our survey with a much larger sample and a pre-registered analysis plan, we welcome feedback and suggestions on our current survey design. Here are some key questions that we hope to receive feedback on: Are there missing questions that could be included in this survey (or an additional survey) that would inform important topics in SWB research? Are there any questions or proposed analyses you find redundant? Do you see any critical flaws in the analyses we propose? Are there additional analyses we should be considering? Would these data and analyses actually reassure you about the comparability, linearity, and neutrality of subjective wellbeing data? If not, what sorts of data and analyses would reassure you? What are some good places for us to look for funding for this research? Of course, any other feedback that goes beyond these questions is welcome, too.
