Achilles tendinosis is one of the worst injuries you can get as an injured runner. It is chronic, it's hard to get rid of, and the tendon feels noticeably different. The tendon gets thicker and it becomes constantly painful. Every time you run on it, you may worry that it might rupture or rip apart. There are lots of different ways to treat Achilles tendinosis. Two of those options are PRP or platelet-rich plasma injection or a procedure called "dry needling." Yesterday in a live Runner's Aid Station call, an injured runner asked me to explain the specific differences between these two treatments. I thought it might be helpful to try to explain it to you the way I explained it during that call. What's the difference between dry needling and PRP injection when you have Achilles tendinosis and you're a runner? Well, that's what we're going to talk about today on the Doc On The Run Podcast.

Achilles tendinosis can be a tough problem. If you're a runner who gets tightness in your calves and then develops Achilles tendinosis, it can be very difficult to get it to heal and it can be very frustrating. Today on the Doc on the Run Podcast we're talking about Achilles tendinosis that has failed multiple attempts at treatment.
This question comes up a lot for runners with Achilles tendon injuries. But it can happen with any kind of tendon. The difference between an injury to a tendon which has been labeled "tendonitis" versus "tendinosis" is really a difference in the amount of time that you have the condition and the amount of damage to the tendon itself. There’s been a long running debate about this. I was lecturing at a medical conference in Iceland this the summer. Specifically, it was a lecture on tendon injuries in athletes. One of the key points was that the old research claims inflammation does not play a role in the development of tendinosis is wrong. In this episode I will explain the difference between tendonitis and tendinosis.

In Part 1 of our series on Achilles tendinitis we talked about how runners with mild forms of Achilles tendon injuries can decrease the inflammation, decrease the stress and tension on the Achilles tendon, get better and keep running. But we also talked about the unfortunate reality of some runners having a tendency to ignore these injuries when they are in there earliest stages. Because many runners have a high pain threshold you may not even notice the first stage of Achilles tendinitis. If you don’t notice there is a problem, it’s unlikely you will treat the problem. So you may be one of those runners who just really didn’t realize you were developing a problem with the Achilles tendon. You might have noticed some weird sensations that went on for months. If so, you may have developed one of the more severe forms of Achilles tendon injury. In today's podcast we’re talking about how runners with severe cases of Achilles tendinitis keep running without surgery.

Shoulder pain is a common problem in competitive swimmers, but the structural alterations in elite-level competitive swimmers are not well known. We found a high prevalence of rotator cuff and biceps tendinopathy, which was associated with increased symptoms. Tendinosis was also more common in swimmers with a positive sulcus sign, suggesting a role for shoulder laxity. Click here to read the article.

Der Einsatz von klinischen Scores zur Bewertung des Therapieergebnisses ist heute bei Erkrankungen der Schulter, z.B. der Tendinosis calcarea, etabliert. Nach Empfehlungen der European Society for Surgery of the Shoulder and the Elbow (ESSE, SECEC) soll insbesondere mit dem klinischen Constant-Murley-Score (KCMScore) der subjektive und objektive Schulterzustand erfasst und dieser somit gegenüber anderen Studien vergleichbar werden. Da die Erhebung des KCM-Scores aufwendig ist und nicht selten vom Patienten abgelehnt wird, wurde in der vorliegenden Arbeit der KCM-Score als Fragebogen umgearbeitet und dieser neu erstellte Score den Ergebnissen des KCM-Scores gegenübergestellt. Der KCM-Score enthält subjektive (Schmerz, Aktivitäten des täglichen Lebens, Arbeitshöhe) und objektive Parameter (Beweglichkeit, Kraft) im Verhältnis 35:65. Maximal 100 Punkte werden bei schmerzfreier, frei beweglicher und funktionstüchtiger Schulter erzielt. Von insgesamt 117 Patienten (128 Schultern), bei denen wegen einer Tendinosis calcarea der Schulter eine extrakorporale Stoßwellenbehandlung erfolgte, wurde zuerst der FBCM-Score selbst ermittelt, dann wurde von einem Untersucher der KCM-Score erhoben. Die 81 Frauen (63,3%) und 47 Männern (36,7%) waren zum Zeitpunkt der Behandlung durchschnittliche 55,8 Jahre alt, der Nachbeobachtungszeitraum betrug 48,7 Monate. Hinsichtlich der Punktmittelwerte der subjektiven Parameter Schmerz (KCM 12,7; FBCM 11,8), Aktivitäten des täglichen Lebens (KCM 8,8; FBCM 8,3) und Arbeitshöhe (KCM 9,9; FBCM 9,5) sowie den objektiven Parametern Flexion/Abduktion (KCM 19,4; FBCM 19,3), Außenrotation (KCM 9,4; FBCM 9,3) und Kraft (KCM 15,2; FBCM 16,2) fanden sich keine statistisch signifikanten Unterschiede zwischen dem KCMScore und dem FBCM-Score. Nur die Messung der Innenrotation ergab im KCMScore einen signifikant niedrigeren Wert als im FBCM-Score (8,1 vs. 8,9). MW15,2 Punkte; (Beweglichkeit, Kraft). Das Gesamtergebnis von KCM-Score und FBCMScore war nicht signifikant unterschiedlich. 53 Somit erscheint der FBCM-Score geeignet den subjektiven und objektiven Zustand bei Patienten nach extrakorporaler Stoßwellentherapie wegen Tendinosis calcarea zu erfassen. Insgesamt war der KCM-Score und der FBCM-Score vergleichbar. Unterschiede zwischen beiden Scores ergaben sich bei isolierter Analyse des Ausmaßes der Innenrotation. Diskutiert wird die Technik der Bestimmung der Innenrotation und der Kraftmessung. Eine Übertragung der vorliegenden Ergebnisse auf andere Schulterpathologien ist nicht möglich.

Überprüft wurden die Hypothesen: 1. Die stoßwellenvermittelte Desintegration und das klinische Ergebnis nach extrakorporales Stoßwellentherapie sind vom maximalen Durchmesser sowie vom Umfang und der Fläche der Verkalkungen der Rotatorenmanschette abhängig. 2. Verkalkungen der Rotatorenmanschette weisen Unterschiede in ihrer quantitativen mineralogischen Zusammensetzung auf. In der ersten Untersuchung wurden mittels computergestützter Bildanalyse definierte Größenparameter vor und nach Stoßwellentherapie bestimmt. Keiner der bestimmten Größenparameter beeinflusste das klinische oder röntgenologische Therapieergebnis. In der zweiten Untersuchung wurden operativ entfernte Verkalkungen bei Tendinosis calcarea der Schulter atomemissionsspektrometrisch hinsichtlich ihrer mineralogischen Zusammensetzung analysiert. Die Ergebnisse zeigten keine Relation bezüglich Geschlecht und Alter der Patienten. Eine hohe individuelle Variabilität vergleichbarer Verkalkungen können als Hinweis auf die mögliche Abhängigkeit der Desintegration von Verkalkungen von ihrer mineralogischen Zusammensetzung mittels ESWT angesehen werd

Effekte extrakorporaler Stoßwellen analog der etablierten Erfahrungen mit der Stoßwellenapplikation bei der Desintegration von Verkalkungen der Tendinosis calcarea der Schulter sollten an Verkalkungen anderer Lokalisation, wie der Achillessehne, überprüft werden. Als Tiermodell diente die mineralisierte Gastrocnemiussehne des Truthahnes, die ein weit akzeptiertes Modell für Sehnenverkalkung darstellt. Nach in-vivo und in-vitro Versuchen konnte keine Desintegration der Verkalkungen bei den Gastrocnemiussehnen festgestellt werden, auch nicht bei Energieflußdichten vom doppelten der klinisch angewandten. Bei diesen hohen Energieflußdichten konnten allerdings Beeinträchtigungen in der Stabilität der Sehnen festgestellt werden, so daß von hohen, insbesondere aber ultrahohen Energieflußdichten, wie in der Literatur beschrieben, abgesehen werden sollte.
