In this week's episode, we feature a specialty requested by a lot of this podcast's listeners. The 17th of November each year marks World Prematurity Day. It's a day to raise awareness of the causes and complications of preterm birth and the concerns of preterm babies and their families worldwide. Approximately 15 million babies are born preterm each year, accounting for about one in 10 of all babies born worldwide. It's a day that's really close to my own heart, as all three of my beautiful children were born premature. My firstborn son was born at 33 and a half weeks, my daughter at 35weeks, and my youngest son born at the the very stroke of 28 weeks gestation. All of them spent some time in the neonatal ICU, or ‘NICU' before coming home: my firstborn spent exactly 50 days in hospital, my daughter was in NICU for just one night before they gave her the all clear, and our youngest spent 142 days in NICU before coming home! I'm disappointed that I wasn't able to get an episode about prematurity out to coincide with World Prematurity Day, but nevertheless I'm really glad to share this episode with you, which is all about the paediatric sub-specialty of neonatology. Dr Robin Saggers is a consultant paediatrician and neonatologist at Charlotte Maxeke Johannesburg Academic Hospital, who is also currently completing a fellowship in paediatric critical care. Dr Saggers always dreamed of becoming a paediatrician and he shares with us his journey from junior doctor to consultant, and what it is like actually working as a neonatologist. In the US, a baby boy born at 21 weeks and a day, weighing just 420g is the world’s most premature baby to survive, after celebrating his first birthday in July this year. Closer to home, in Singapore, a baby girl weighing just 212g when she was born is believed to be the world’s smallest at birth, and was discharged in August after spending 13 months in the hospital. Every November Prematurity Awareness Month is observed, with World Prematurity Day being recognized globally on the 17th.  One common reason for a preterm delivery is when a patient's water breaks before being full term (or 37+ weeks pregnant).  During pregnancy, the baby resides inside a fluid-filled sac, called an amnion which contains amniotic fluid. When someone says their water has broken, they really mean the membranes of the amniotic sac have ruptured. Rupture of membranes (ROM) after 37 weeks usually happens after the onset of labor (or the start of contractions). However, when this rupture occurs before 37 weeks, this is called preterm premature rupture of membranes (PPROM), and this can cause a world of trouble.  Check out this episode as Dr. Plenty lets you know how term ROM and PPROM are both handled.  Also, learn about how you can reduce your risk of PPROM and preterm delivery with subsequent pregnancies. November is Preemie Awareness month and November 17th is World Prematurity Day. 1 in 10 babies in the US are born too soon, which is before 37-40 weeks. Premature babies fight for their very survival and are at risk for significant short-term and long-term health challenges.The health equity gap in our health care system puts women and babies of color at the greatest risk, with racial and ethnic differences in preterm birth rates. In 2020, the rate of preterm birth among non-Hispanic black women was about 50% higher than the rate of preterm birth among non-Hispanic white women  or Hispanic women with African-American mothers. World Prematurity Day was created on November 17, 2011, to raise awareness for the millions of children every year who are born prematurely. While technologies and medical procedures in America have increased over the years, preterm babies still carry a huge vulnerability to develop cerebral palsy, delays in development, hearing problems, and sight problems.   Additionally, while neonatal death in the United States is low in comparison to most of the world, premature births account for a quarter of all neonatal deaths.   That’s part of the many reasons why World Prematurity Day was created – to shine a light on the risk and hardships created by premature births, cost-effective and proven solutions, and spreading compassion for families who have experienced premature births. Kate Morrow and Frankie Robertson join Marjy Marj for a conversation about the health of mothers and babies About KateKate Morrow has worked in healthcare non-profit management, marketing and fundraising for over 15 years. After the birth of her twins at 28 weeks gestation, who spent 76 days in the Neonatal Intensive Care Unit, Kate knew more needed to be done to combat many of the challenges she witnessed for mothers of critically ill children. She has combined both her personal experience and professional expertise to be a true advocate for premature births, birth trauma, and high-risk mothers.Today, Kate supports the local and statewide March of Dimes Chapters and is also involved at the national level. In 2019, her family served as the March of Dimes Ambassador family and she was one of hundreds of women chosen to be an inaugural Gretch Carlson Fellow for the national March of Dimes. Her fellowship project was focused on maternal mental health, specifically, Post Traumatic Stress Disorder, faced by mothers after critical or lengthy hospital stay for their child. Her most notable moment was having May 1 established as South Carolina Healthy Mother's Day; with a proclamation signed by Governor Henry McMaster in 2019. Kate feels strongly that mothers need to be mentally healthy to take care of the physical needs of their premature and critically ill children. Additionally, Kate is involved in the PRISMA-Midlands Foundation as an advocate and also supports the hospital system through a new role on the NICU Family Advisory Council. She works tirelessly to improve outcomes for families who have gone through medical stays similar to her family's. Today, Kate is involved in a variety of pediatric healthcare causes; most recently leading special projects and events at Children's Cancer Partners of the Carolinas. About FrankieFrankie Robertson has worked in public administration for more than two decades. Her maternal and child health work spans over a decade and began as the state director of the Louisiana Chapter of March of Dimes. During her tenure as state director, her talented team partnered with key stakeholders on successful statewide initiatives such as the 39 Weeks Initiative, Group Prenatal Care, Baby and Me Tobacco Free, and Go the Full 40.Frankie also served as Regional Director of Advocacy and Government Affairs at March of Dimes. She supported a talented team, contract lobbyists, and volunteers in LA, MS, AL, TN, FL, GA, NC, SC, and Puerto Rico. Key advocacy initiatives included maternal mortality and severe maternal morbidity, health equity, workplace protections, paid leave, maternal mental health, protecting immunization laws, healthcare coverage, newborn screening, neonatal abstinence syndrome, substance use disorders, and smoking cessation.Frankie has an unwavering and unapologetic commitment to social justice and equity. She is passionate about maternal mental health, addressing toxic stress, and experienced the premature birth of her daughter, Zoe Amandla, at 28 weeks gestation. She applies her experiences as an activist mom of a Black son and a preemie, coupled with training as a birth doula and education in structural racism, to advance policies to address the root causes of health inequities for Black birthing people. Il problema della Prematurità è un problema sempre più attuale (1 neonato su 10 è prematuro), ed è trasversale: non ha confini, non ha colore, razza, nazione, status sociale, religione, ruolo, può colpire tutti noi. "The World Prematurity Day", evento internazionale celebrato il 17 Novembre, vuole andare a sensibilizzare sempre più le persone su questo tema, sulla battaglia che ad oggi combattono tanti piccoli guerrieri. "SIAMO TUTTI NATI PREMATURI" sarà infatti lo slogan per la Giornata Mondiale della Prematurità 2020. "In quest'anno così difficile per tutti, abbiamo pensato di portare fuori dai confini dell'isolamento della termoculla e della nostra Terapia Intensiva Neonatale, il soggetto delle nostre cure: il neonato prematuro." Con queste parole il Dott. Cont Gabriele, medico neonatologo presso la Terapia Intensiva Neonatale dell'IRCCS Burlo Garofolo di Trieste, diretta dal Direttore Dott. Francesco Maria Risso, chiede la partecipazione di tutti all'iniziativa pensata in occasione dell'evento del 17 Novembre. In occasione di questo appuntamento verranno stampate oltre 200 magliette viola (colore scelto per indicare la Prematurità), che avranno sul petto i loghi dell'Associazione Scricciolo (www.scriccioloassociazione.org), di VivereOnlus (a livello nazionale) e ECFNI (a livello europeo), nonché quello del Burlo Garofolo, ma cosa ben più importante avrà sulla schiena scritto il nome di un giocatore coraggioso e tenace, con un numero rappresentante le cifra del suo peso neonatale " SERGIO 420 gr ", scricciolo di peso estremamente basso, ma così basso ma da far tanto rumore! A tutti coloro che vorranno partecipare all'iniziativa, richiedendo la maglietta che verrà spedita senza alcun costo, verrà chiesto di indossarla e fotografarsi di schiena, con sotto la "vostra divisa" nel posto di lavoro o in ufficio, per poi postarla sulla personale pagina social o sito aziendale e dar voce agli scriccioli di tutta italia e oltre. Today we are chatting with a couple of incredibly powerful women about a potentially less desired topic.  Our intention is to bring awareness to mamas and families dealing with premature birth. In honor of World Prematurity Day this coming Saturday, November 17th, we are talking about our tiny warriors and their warrior mamas. We are speaking about the miracles that exist to provide hope and to empower the mamas in the trenches of dealing with prematurity, the NICU (Neonatal Intensive Care Unit) and all of the fears and anxieties that come along with it. We have some incredibly inspiring and well-versed guests who have plenty of experience with this: Charleyne Oulton and Riana Luck.       Charleyne is a confident and happy mom of three children who were all born prematurely. Charleyne has a passion for writing and is a well recognized published author with Golden Brick Road Publishing House and regular contributor for YGTMAMA. She is a popular health and wellness coach with It Works, works in the Royal Canadian Navy as a Reservist and is a Pisces who lives on beautiful Vancouver Island, BC. She is also an upcoming contributor in You've Got This Mama Too and You've Got This, Healthy Mama. Charleyne shares a brief snippet of her story. She has three children that were all born prematurely, within thirty-two months of each other. She had no idea that she was at risk for premature delivery. She was so uneducated around any other possibility of a birth plan other than a full term healthy pregnancy. She claims that it was horrible and terrifying, an event that she still suffers from PTSD (Post-traumatic Stress Disorder) because of. Her first son's birthday brings up mixed emotions, joy is combined with having to relive really scary times. Her second pregnancy had her more prepared with the risks of premature delivery only because of her previous experience. The lead up to her son Jeffrie's birth was terrifying but born at twenty-eight weeks, he was a fighter and her strongest preemie. Less than a year later her daughter Juley Anne was born at twenty-six weeks, at less than 2 lbs. Her pregnancy was the most joyful pregnancy. She knew that she was going to have another preemie, her waters ruptured at twenty-two weeks and she was encouraged to terminate the pregnancy because the baby would not be viable at that time. She chose life and decided to try to try to keep her inside as long as possible. Combined she spent 110 days in the NICU with what she calls her little ‘miracles'. Riana is the mom of five; seven-year-old triplet boys, a four-year-old daughter, and a one-and-a-half-year-old daughter. Four out the five were preemie babies that spent a total of 119 days in the NICU. Riana is also an owner of two businesses, one being a successful cheerleading gym in Red Deer called Premier Academy, and co-owner of three others. She is also an upcoming contributor in You've Got This, Boss Mama. Riana shares that one of the biggest struggles from her NICU time was when she had other children at home depending on her and missing her. With her triplets, although it was hard, it was expected and they knew what they were in for. There were no other children at home who also needed attention, so they could devote all their time to be in the NICU. When her waters burst early with her last pregnancy she walked into the hospital in tears. She knew the long road ahead, and a lot more tears were shed as a coping mechanism. She also praises our outstanding health care system: “It's amazing what they can do for these little babies.” We are here to bring awareness to this difficult topic. Riana starts off by sharing her two different NICU experiences. Her triplet sons were born at thirty-one weeks, five days, which was expected and actually very good for triplets. Even though almost thirty-two weeks is a great milestone for triplets,
