Temper tantrums can be so frustrating - for the child, the parent, the teacher, and everyone involved!  On episode 20 of PsychEd4Peds, Dr. Joyce Harrison, child and adolescent psychiatrist at Kennedy Krieger Institute/Johns Hopkins, shares with us an "8 S" framework for evaluating kids with temper tantrums. Q: What is on the differential diagnosis for a 4 year old with temper tantrums?A: Thinking alphabetically, ** A = ADHD, anxiety, and/or autism,** C= communication difficulties, ** D = developmental delay/intellectual disability, ** T=trauma or adverse childhood experiencesQ:  What questions could the pediatrician ask the parent to sort through some of these possible issues?   A:  It can be helpful to use the "Eight S framework"§ 1 - Safety first· Is this child safe?· Are the people around this child safe? · Is this child safe to be treated in the setting that they're in? § 2 - Specific behaviors · What is the child doing that makes the parent think something is wrong· What is the most problematic thing? · What are you most concerned about?§ 3 – Setting· Where are the behaviors happening? (childcare/preschool, home, out w/ other people)§ 4 - Scary/trauma · Has anything scary happened to this child/family? § 5 - Services!!!!! · what kinds of services is the child getting now? She is also on the Faculty at Kennedy Krieger Institute, a hospital serving children with developmental disabilities.  She is a nationally-recognized educator in early childhood mental health and its integration in primary care. She is the Project director for Kennedy Krieger Institute's Early Childhood ECHO programs, and  Co-chair of AACAPs national Infant and Preschool Committee.  She served as the founding  Medical director of Maryland Behavioral Health Integration in Pediatric Primary Care program. What happens inside a child's brain in the first three years of life is critical to their long-term mental health, says Jyrah Knight, early intervention specialist and professional development director at the nonprofit, First3Years.  So, if a child can't even remember their encounters with parents and caregivers, how can it affect them so profoundly? “Understanding Infant Mental Health” is the topic of this episode of The Menninger Clinic's Mind Dive podcast.  Join hosts Dr. Kerry Horrell and Dr. Bob Boland as they explore the fascinating topic of the developing brain in the first three years of life. Knight explains that trust relationships are critical to a child's early brain development and mental health.  “You can't talk about infant mental health without talking about relational health as they essentially go hand in hand,” she says. She notes that infant mental health isn't about illness…it's about helping a child develop well right from the start.  Knight says “Safe, stable, and nurturing relationships are essentially the foundation for all other infant brain development that happens.” “Babies may not have memory, but their brain is keeping score, and it's keeping score every time a need is met,” she says.  For example, when mom and dad come when a baby cries, the brain is keeping score, and those interactions help to build trust.  Babies are aware of when they are cuddled, hugged, talked to and swaddled, says Knight.  She adds that the infant brain is keeping track of those positive interactions, even when memory is not present. So how do you help a parent or caregiver and a child?  Knight says the best approach is a strengths-based perspective and positive reinforcement. Parents and caregivers can easily get overwhelmed with the notion that they are responsible for their baby's mental health. That's why the goal is to highlight things that are going well between the parent and the child dyad. Knight suggests “lighting them up with the positive of what's happening.” She adds that they will then feel empowered to do more. Knight, Dr. Horrell and Dr. Boland all acknowledge the importance of attachment to overall mental health and how the first three years is a critical time to develop these relationships. Sarah currently works as the Vice President of Programs and Practices at The Society for the Protection and Care of Children (SPCC) in Rochester, NY where she oversees multiple IECMH-embedded, multidisciplinary programs. In this episode we talk with Samantha Reisz, EBB Childbirth Class graduate about her experiences taking the class and preparing for a waterbirth in a hospital with her “Golden Ticket” birth team, who were skilled and prepared to manage a placental cord avulsion also known as cord snapping.   Samantha Reisz, she/her, is a Scholarly Assistant Professor of Human Development at Washington State University in Vancouver, Washington, just outside of the Portland, Oregon area. She completed her bachelor's in Psychology and master's in Infant Mental Health from Mills College in Oakland, California when she first began studying childbirth. Samantha then earned her PhD in Human Development and Family Sciences from the University of Texas at Austin, where she studied infant parent relationships and the transition to parenthood. Samantha is a passionate scholar and educator. After years of studying these topics academically, she finally was able to live her own research with the birth of her first child. Samantha lives in Vancouver, Washington with her baby, partner, and two dogs.    Samantha shares how she prepared to give birth for the first time with the use of a doula who recommended the EBB Childbirth Course. Samantha and her partner planned for a waterbirth in a hospital with an OBGYN attending the birth. After experiencing the beautiful waterbirth she had desired, complications arose in the 3rd stage with a placental cord avulsion. An Interview with Dr. Barbara Stroud Barbara Stroud, PhD, is a licensed psychologist with over three decades worth of culturally informed clinical practice in early childhood development and mental health. She is a founding organizer and the inaugural president (2017-2019) of the California Association for Infant Mental Health, a ZERO TO THREE Fellow, and holds prestigious endorsements as an Infant and Family Mental Health Specialist/Reflective Practice Facilitator Mentor. In 2018 Dr. Stroud was honored with the Bruce D. Perry Spirit of the Child Award. Embedded in all of her trainings and consultations are the activities of reflective practice, demonstrating cultural attunement, and holding a social justice lens in the work. Dr. Stroud's book “How to Measure a Relationship” [published 2012] is improving infant mental health practices around the globe and is now available in Spanish. Her second book, an Amazon best seller, “Intentional Living: finding the inner peace to create successful relationships” walks the reader through a deeper understanding of how their brain influences relationships. Additionally, Dr. Stroud is a contributing author to the text “Infant and early childhood mental health: Core concepts and clinical practice” edited by Kristie Brandt, Bruce Perry, Steve Seligman, & Ed Tronick. Dr. Stroud received her Ph.D. in Applied Developmental Psychology from Nova Southeastern University, and she has worked largely with children in urban communities with severe emotional disturbance.  Dr. Stroud's professional career path has allowed her to work across service delivery silos supporting professionals in mental health, early intervention (part c), child welfare, early care and education, family court staff, primary care, and other arenas. Misty Richards, MD, MS is the Director of Infant Mental Health of the UCLA Maternal Mental Health Program. She is the Co-Founder and Medical Director of Perinatal Psychiatry for the Maternal Outpatient Mental health Services (MOMS) Clinic in the Department of OB-GYN, where she treats patients with perinatal mood disorders, anxiety, and psychosis while educating the next generation about reproductive psychiatry. She is a child, adult, and reproductive psychiatrist at UCLA who holds a joint appointment in the Division of Child and Adolescent Psychiatry and the Department of Obstetrics and Gynecology. She is double board-certified in child, adolescent and adult psychiatry and specializes in infant mental health, with recent completion of the NAPA Parent-Infant Mental Health Fellowship. This week we bring you a long awaited interview with our Gentle Sleep Coach colleague Macall Gordon. Macall specializes in working with "live wires:" Children with very alert temperaments. She shares with us her immense knowledge gained from working with these children as well as research insights. Macall has a master's degree in Applied Psychology from Antioch University, Seattle with a research-based specialization in infant mental health, the development of emotional regulation, and parenting psychology. She also has a B.S. in Human Biology from Stanford University. She is currently a Senior Lecturer in the graduate Counseling Psychology program at Antioch University in Seattle. She has conducted and presented research on temperament, sleep, and parenting advice at infant and child development conferences around the world including the World Association for Infant Mental Health, the International Society for Infant Studies, the Society for Research in Child Development, and the American Psychological Association. She has been a featured speaker at international sleep conferences and has led webinar-based advanced training for sleep coaches, mental health providers, and others. Macall has just finished the manuscript for a book on sleep techniques for livewires. She lives outside of Seattle with her livewire husband and two very dramatic dogs.
