Austin Allred is a dairyman located in the Columbia basin of Washington State and operates Royal Family Farms. His parents started farming 50 years ago, and now Austin, as well as his brothers, own their own operations. Karıncaları cezbettiği için Hintli doktorlar buna madhumeha (bal idrarı) adını verirler. “Kayserili Arete” olarak da bilinen Kapadokyalı Aretaeus, MS 1. Yüzyılda; “…içeceklerin hiçbir önemli kısmı vücut tarafından emilmezken, etin büyük bir kısmı idrarla sıvılaştırılır.” diye gözlemlerini aktarır ve “diyabet” (Yunanca, "sifon") kelimesini literatüre kazandırır. “Mellitus” (Latince, "bal gibi tatlı") teriminin eklenmesi için aradan yüzlerce yıl geçmesi gerekir. Hastalığı, idrarın tatsız olduğu diğer diyabet hastalığından (insipidus) ayırt etmek için 1798'de İngiliz Genel Cerrahı John Rollo “Diyabetes Mellitus” terimini oluşturur. Ortada test imkanı olmayınca, iş doktorlara düşer. Mesela Londralı bir doktor olan Willis, yüzlerce yıldır uygulanan bir yöntemin iyice duyulmasına yol açar; hastalarının “idrarının tadına bakma”sıyla meşhur olur. İdrar şekerli ise hasta Diyabetli tanısı alır, dosya kapanır. Neyse ki ilerleyen yıllarda teknoloji aşama aşama gelişir, doktorlar glukometre, diller test strip'i olmaktan kurtulur. Sürekli Kan Şekeri Takibi Nasıl Mümkün Oldu? İdrarda glukoz miktarını belirleme girişimleri, insülinin keşfinden daha öncesine, 1800'lerin ortalarına kadar uzanır. 1883'de George Oliver yatak başı idrarda glukoz ölçümünü tanımlar. Stanley Benedict, bu yöntemi 1908'de modifiye eder ve bakır reaktifi ile idrar glukozu ölçümü yapılır. Bu yöntem 50 yılı aşkın süre bazı modifikasyonlara uğrayarak kullanılmaya devam eder. Yöntemin en büyük zorluğu ısıtma işlemi gerektirmesidir. Ancak 1945'te modifiye bakır reaktif tabletleri içeren Clinitest (Ames, Elkhart, ABD)'in piyasaya çıkması ile işler biraz kolaylaşır. Bu testte glukoz oksitlenir ve “glukozüri miktarının, ısıtılan çözeltinin rengiyle orantılı olduğu” prensibiyle sonuç elde edilir. Aynı yıllarda benzer yöntemle idrarda keton da bakılmaya başlanır. İdrarda Glokuz ölçüm sistemi Clinitest (1950'ler) 1950'lerin ortalarında, glukoz oksidaz içeren “kuru strip”ler geliştirilir. 1957'de Miles-Ames laboratuvarı, idrar glukozu için daldır ve ölç (dipstick) denilen Clinistix adındaki stripleri geliştirir. Ancak bu yöntemin birçok kısıtlılığı vardır. Bir kere örneğin alım şeklinden ve örnek miktarından fazlasıyla etkilenir. Sonra glukozun saptanabilmesi için “bir hayli” yüksek olması gerekir. 1965 yılında Ames firması ilk kan şekeri test stripini geliştirir: Dextrostix adı verilen bu strip, üzerine bir damla kan damlatılması 60 saniye sonra yıkanması ile çalışır; ortaya çıkan renk şişe üzerindeki bir tabloyla karşılaştırılarak sonuç elde edilir. Haliyle bu strip, ev kullanımı için değil, doktor muayenehaneleri için üretilmiştir ve “Göz var izan var” prensibine dayandığı için sonuçları birazcık “göreceli”dir. İlk glukometre 1970'lerde Dextrostix ile kullanılır, ancak sonuçlar hala istenildiği düzeyde tutarlı değildir. 1970'lerin ortalarında, hastaların evde kan şekeri takiplerini yapabilmeleri fikri iyiden iyiye düşünülmeye başlar. Hem teknoloji elverişlidir, hem de hastalığın matematiği enikonu ortaya çıkar. 1977'de Glikozile Hemoglobin (HbA1c) ölçümü ile ilgili çalışmalar başlar. Uzun süreli kan şekeri kontrolünün iyi olmasının, diyabete bağlı komplikasyonların önlenmesinde çok önemli olduğu barizdir; ancak sıkı glisemik kontrol, beraberinde ölümcül bir risk de taşımaktadır: Hipoglisemi. Tucked away in a warren of testing cubicles and curious contraptions, we find Franco Rigolon hard at work. His job involves making just one thing, and it's not something he can make very quickly. It is late summer 2015 and nobody knows that Tom Dumoulin is on the verge of a breakthrough. Not the 24 year-old Dutchman himself, nor his team-mates at Giant-Alpecin. It doesn't figure in his employers' plans. The top tens of Grand Tour contenders put out by magazines and websites ahead of such races do not feature him. The best three-week riders in the world might know his name but have no idea he is set to join their ranks. Dumoulin has shown promise in week-long races, winning stages at the Tour de Suisse and the Eneco Tour, placing twice overall in successive years at both. He finished second to Tony Martin in the penultimate stage TT at last year's Tour de France. By more than a minute but still, Tony Martin. A podium at Paris-Nice, maybe Romandie, should be next, though Dumoulin has already said his target for 2016 is an Olympic medal. A gold one. He is thought of by his team as an exciting prospect for the future but they see him, for now at least, as “a world class time trialist who's very good on the hills”. He can make it over the high mountains (more or less) but he is not expected to challenge in them. That is about to change.He's one of the best Grand Tour men of a generation now; in 2015, Tom Dumoulin was a 1,000-1 outsider before the Vuelta a España that kickstarted his career. There was a lot of crying in the time after.”A professional cyclist's heart is trained to be as big and strong as possible, allowing it to pump massive amounts of blood and oxygen around the body. Most cyclists have a ticker 50 per cent bigger than that of a regular person. Miguel Indurain possessed a superhuman resting heart rate of around 30 beats per minute; a normal, well-trained adult heart has one of 72.But, in pushing themselves to the extreme, they are in danger of developing arrhythmia.Two months prior to the 2016 Tour of Oman, doctors had found a life-threatening arrhythmia in Johan Vansummeren's heart. He had been through dozens of checks and was equipped with a heart monitor during the Middle Eastern race.Vansummeren's legs felt worse than ever, and momentarily cramped while going up a steep climb.His cardiologists back in Belgium had seen the exact moment his legs cramped, and it correlated with a spike in his heartbeat. They called him.Aware of the initial tests, the AG2R doctors banned Vansummeren from finishing the race. Hours later, Vansummeren was on his way back to Lommel, where he was born and still lives, transformed from a healthy elite athlete to a man with a life-threatening heart arrhythmia. Oliver Onions Oliver Onions was born in Bradford, Yorkshire, England in 1873 and died in Aberystwyth in Wales in 1961, His father was a bank cashier. When he was a young man he was an amateur boxer. He didn’t like the surname Onions and changed it legally to George Oliver but still published his stories under the name Oliver Onions. Onions is in fact a Welsh name, being from the Welsh Ab Einion. Onions trained as an artist and worked as an illustrator. His most famous ghost story is The Beckoning Fair One. He is reckoned as one of the best writers of ghost stories. He has a very conversational lyrical style and the characters in his story come out via the tone of their voice. The Cigarette Case is a neat little story with a lovely twist in the end. We probably guess what’s going to happen, but it’s very well done. I particularly like Loder’s asides to other members of the circle, such as Marsham the romantic and Smith the sceptic. I was reminded by this story of An Adventure which is the reportedly true tale of two English ladies having a time slip experience in France at the Paris of Versailles in the early 20th Century. The Cigarette Case was published in 1911 in the collection Widdershins and I believe An Ad venture was published in 1910, so it is not impossible that the tale of two English ladies in France who mysteriously disappear was inspired by An Adventure.
