Emergency physicians Resa E. Lewiss and Courtney M. Smalley discuss their article, "Why point-of-care ultrasound belongs in emergency department triage." Amid the crisis of overcrowded waiting rooms and long wait times, they argue for the immediate integration of a powerful, underutilized tool: point-of-care ultrasound (POCUS). Resa and Courtney use compelling clinical examples, like identifying a collapsed lung or a ruptured ectopic pregnancy in under two minutes, to show how POCUS can dramatically improve risk stratification and save lives before a patient even leaves the triage area. They counter potential objections by explaining that emergency physicians are already highly trained in POCUS, the technology fits existing workflows, and it is a billable procedure that can generate revenue. The conversation serves as a direct call to action for hospital leaders to redesign triage spaces and protocols, making POCUS a standard of care to ensure the sickest patients are identified and treated without delay. In Part Two, Resa gets tactical. She explains why Micro Skills begins with time, trust, and self-care—because none of the communication tricks matter if you're running on empty. She unpacks workplace blind spots that slow careers down, shares why she wrote this book for more than just doctors, and explains why clarity, not complexity, wins in high-stakes situations. Also: BCC etiquette, post-traumatic growth, and the exact moment she realized the workplace comes with an unwritten playbook—and how she set out to write it.This is about what really sustains you: self-respect, situational fluency, and knowing what's worth your time.Key Highlights of Our Interview:The Workplace Doesn't Come with a Playbook“I didn't get the playbook—so I wrote one.”Why Micro Skills is the field guide she wishes she had years ago.Why the Book Starts with Time“Time can only be spent. You can't save it. You can't refund it.”What trauma bays taught her about urgency, and how it applies to every job.Letters of Recommendation and Hidden Rules“No one told me how the game worked. That one insight saved me years of inefficiency.”The invisible workplace norms that trip people up—and how to name them.Rest Is Strategy, Not a Luxury“If you're not well, nothing else works. We call that micro skill #1.”Why self-care opens the book—and isn't just about bubble baths.Intentional Email Etiquette“BCC isn't sneaky—it's smart. You're protecting privacy and reducing clutter.”How respect shows up in the smallest digital behaviors.Practicing Under Pressure“People think it's all adrenaline. In Part One, Dr. Resa Lewiss reflects on the experiences that led her to emergency medicine, from early memories of gender inequity at the dinner table to the interdisciplinary studies that shaped her worldview. She shares why she chose a career in high-pressure medicine, how a love of procedures led her to ultrasound, and why teaching globally changed how she practices and leads.Key Highlights of Our Interview:Medicine Was a Calling, Not a Family Trade“I didn't grow up around doctors. But from early on, medicine was in my bones.”Resa explains how her internal pull toward healthcare was stronger than any external influence. The First Fight for Fairness“My dad said, ‘Girls, clear the table.' I said, ‘What about the boys?'”She recalls early moments that sparked her refusal to accept unequal expectations.Finding Her Fit in Emergency Medicine“Once I rotated in emergency, I thought—this is it.

Guest Michelle Barker Panelist Richard Littauer Show Notes In this episode of Sustain, host Richard Littauer welcomes Michelle Barker, Director of the Research Software Alliance (ReSA), for an in-depth conversation about the critical yet often overlooked role of research software in open science. Michelle shares her journey from sociologist to open science advocate, unpacking how ReSA fosters global collaboration to support software developed for research. Together, they explore what it means to build “social infrastructure” in the open source ecosystem, the challenges of aligning international stakeholders, and how ReSA is shaping the future of research through strategy, connection, and community-driven solutions. Press the download button now to hear more! [00:01:58] Michelle explains how she got involved in open science and open source software. [00:04:35] Why Research Software? Michelle shares that ReSA was founded to coordinate globally on research software, which lacked unified international representation unlike open data. [00:07:21] We hear about ReSA's engagement strategy and the three main strategies: knowledge sharing, stakeholder collaboration, and governance evolution. [00:09:37] ReSA includes RSEs as one of many stakeholders and works broadly across funders, policymakers, infrastructure providers, and more. [00:10:26] Research software is defined as software developed within a research context to solve a research problem and most is open source but not all. [00:13:12] Richard asks about tracking engagement, and Michelle shares it's hard to quantify outcomes, but standard metrics include newsletter subs, citations, and forum attendance. [00:15:08] Michelle explains the role of social infrastructure. [00:17:37] What's hard about being a social infrastructure? Michelle talks about the challenge of how to motivate groups of people to work together. [00:19:52] Michelle shares her personal approach to networking: research, targeting key individuals, emotional engagement, and strategic planning for conferences. [00:24:35] A new strategy plan is rolling out and Michelle shares what's different. [00:27:32] ReSA is working to establish research software as its own recognized field. [00:29:57] Michelle recognizes shared challenges of both open source and research sectors. The keys to success are spotlight wins and demonstrate value through supported infrastructure and recognized contributions. [00:30:40] Find out where you can find out more about ReSA. In Part 2, Adaira shares how she and Resa shaped Micro Skills into a fast-impact, high-utility guide for early career professionals—and why it intentionally skips fluff in favor of action. She opens up about saying yes too often, burning out from “non-potable work,” and how she finally embraced what she calls JOMO—the joy of missing out. We also hear how they trimmed the book's original title (“Chisel”) and why ambition without discernment leads to a flat career, not a rising one.Key Highlights of Our Interview:How the Title Came to Be“We almost called the book Chisel—but MicroSkills came from a medical lecture I never forgot.”The Real Goal“We wanted people to finish the book Friday and be better at work Monday. No degrees, no fluff.”The Myth of Saying Yes“I believed the mantra: say yes to everything. I ended up burned out, doing too much that didn't scale.”From FOMO to JOMO“The joy of missing out means you choose what matters. You don't chase everything.”Horizontal vs. Vertical Growth“I wasn't climbing—I was just adding. In Part 1, Dr. Resa Lewiss shares how growing up in Rhode Island, challenging gendered assumptions at home, and studying the liberal arts all shaped her path to medicine. In Part 2, Resa explains why she and Adaira started their book MicroSkills with the most overlooked chapter: self-care. From emotional and civic health to better rest and boundaries, she unpacks how showing up well starts before you speak. She also shares practical tools for navigating hard moments—like having a failure buddy—and reveals why thoughtful email etiquette isn't just about manners, but about professional respect. This episode is about what sustains you—before, during, and after the work. Micro skills begin with you.Key Highlights of Our Interview:Why Micro Skills Start with Self“We opened the book with self-care—because everything else depends on it.” Physical, civic, emotional, and financial health are non-negotiables.Support Isn't Optional“Have a failure buddy. Or a personal board of directors. Someone you can call when things go wrong.”What Real Rest Looks Like“Rest isn't just sleep. It's knowing what recharges you. And choosing to protect it.”Respect Through Email“BCC isn't a trick—it's about being thoughtful.” Communication is a reflection of how much you value someone's time and dignity.Small Language, Big Impact“Even experienced professionals said, ‘I didn't think I'd learn anything from a book on communication—but I did.'” Thoughtful communication isn't about polish.
