Participants in the usual diet group will be encouraged but not required to follow the Dietary Guidelines for Americans diet. The study will also measure the impact of the study diets on walking function, hand function, vision, serum neurofilament (blood biomarker), and brain imaging (presence of enhancing lesions and changes in brain volume).  The study will be held at the University of Iowa Hospitals & Clinics over two years. It will consist of three visits to Iowa City, Iowa. The first visit occurs at month 0. The second visit is at month 3, and third and final visit is at month 24. The first visit may last 4-6 hours. The second visit will last about 2-3 hour. The third visit may last approximately 4-6 hours. Participants will be randomly assigned to follow one of the three study diets. Participants will continue to take their current medications, including any disease-modifying treatments for multiple sclerosis. Participants may take recommended supplements, eat more non-starchy vegetables, complete daily food logs (3 questions) on a free smartphone application, complete online surveys every three months, watch videos to learn about the study diet to which they are assigned, meet with the study dietitian to learn about the study diet to which they are assigned, and attend optional online support meetings. If a participant is assigned to the ketogenic diet, they will be asked to take a blood ketone measurement using a drop of blood daily for the first month, then twice per week. They will have a device that pricks the finger to conduct the blood ketone measurement. Participants will have a fasting blood draw at each of the study visits and may bring a snack after the blood draw. They will complete assessments of walking, hand, and vision function at each visit. They will have a NON-CONTRAST MRI of the brain at the first and last visits. Key requirements to participate in the study include the following: 1) be willing to share medical records with the study team, 2) do not have clinically significant heart disease, liver disease, kidney disease or type 1 diabetes, 3) do not have a serious psychiatric disease that would make adopting a study diet more difficult (for example schizophrenia or disabling depression), 4) are not taking insulin, coumadin, or weight loss medications, 5) have a smart phone or tablet to download a free app, and 6) have access to high-speed internet and a computer, tablet or smart phone to complete surveys and participate in video conferences via the Zoom platform. He was 88 pounds and dealt with severe anorexia.  Plus, Ryan was suffering from hardcore mental health problems. Eventually, Ryan got his weight up to 130. However, that wasn't the end of his health problems.  Despite gaining weight, Ryan's energy never returned, and he never addressed his emotional trauma from childhood.   Later, Ryan was diagnosed with small fiber sensory neuropathy (SFSN).  [11:40] Mercury in Tuna: Measuring Heavy Metals in Human Hair  One of Ryan Brown's heroes is Dr. Terry Wahls. Learn more about Dr. Wahls here: https://terrywahls.com When Ryan did a deep dive on functional medicine, he looked at his mercury levels. At one point, Ryan had been eating tuna daily for four years.  When Ryan tested his hair follicle levels for mercury, his levels came back extremely high.  Blood tests will look at acute mercury, but they won't give you chronic levels of mercury.  The bigger the fish, the more toxins they are going to have.  [18:20] Mold Toxicity: Getting Your Energy Levels Back With a Heavy Metal Detox Another deep dive Ryan did on his health was regarding mold toxicity.  In LA, Ryan found black mold in his home; he lived in it for over a year. After moving out of that environment, Ryan's energy levels finally increased. Detoxing from heavy metals and staying in ketosis have been total game-changers for Ryan's health.  [21:15] Free Tools to Improve and Support Your Mental Health  Ryan will start his day by journaling. He writes about good things that are going on in his life.  Once you believe in something, great things are going to happen.  Plus, Ryan will spend as much time outside as possible.  He loves to ground himself and practice breathing. Breathwork is one of the most potent mindset tools that we have.  [27:40] Why Ryan Is Such a Big Fan of Getting Into Ketosis  Ketosis is one of the most potent therapeutic tools anyone can use as a human being. Ketosis will help reset your body and give you humanity back. Infants are in ketosis, and women during pregnancy are in ketosis too.  So many people in the United States are metabolically inflexible. Every single one of these people can benefit from ketosis.  [30:10] Ryan Debunks Common Misconceptions About Keto  Too many health coaches and influencers are making ketosis complicated. Ketosis is not complex; it's simple.  One of the biggest mistakes people make is eating things that are labeled “keto.”  Nothing in the world labeled “keto” beats eating real and whole foods.  You need to focus on real food instead of packaged and processed goods. Another thing people are getting wrong is worrying about eating too much protein. It's actually pretty challenging to get into gluconeogenesis.  [35:40] Animal-Based Proteins Are Amazing for the Human Body  Pretty much everything you hear mainstream, almost the opposite is true.  Animals have the full array of amino acids that the human body needs. While on the other hand, plants do not have the full array of amino acids we need. Ryan is a massive fan of eating fatty red meat.  You need cholesterol to synthesize vitamin D. Also, don't be afraid to eat eggs; eggs are great for your hormones.  [39:30] Cholesterol Is Critical for the Body and Your Overall Health More people die from heart disease with normal to low cholesterol than with high cholesterol.  Cholesterol is essential; your membrane is made up of partly cholesterol. Tune in for the second part of our discussion with world-renowned Dr. Terry Wahls, creator of The Wahls Protocol. In today's discussion, we'll learn more about Dr. Wahls' role as a researcher. She shares about her upcoming randomized study, Efficacy of Diet on Quality of Life in Multiple Sclerosis.  Diet is often heralded as the key ingredient to managing MS symptoms. But, with over a dozen "MS diets" out there that are often contradictory to each other, how do we really know which one is best? Or is there even one diet that works for everyone? Hello everyone and welcome to a very special 2-part episode of the MS Gym podcast. At the moment, Jodi and I are currently on our usual summer podcast hiatus, but when author Dr. Terry Wahls, the creator of The Wahls Protocol, reached out to us to help share information about a new study she's working on, we were honored and MORE than happy to oblige.  We both thought it would be a perfect opportunity to host an episode together and, of course, we couldn't wait to share the episodes with our listeners. So, you might be thinking, who is Dr. Terry Wahls? What's the Wahls Protocol? Dr. Wahls is currently a clinical professor of medicine at the University of Iowa Carver College of Medicine in Iowa City, Iowa, U.S.A., where she teaches internal medicine residents in their primary care clinics. She also does clinical research and has published over 60 peer-reviewed scientific abstracts, and papers. What gives Dr. Wahls a unique perspective in her field, is the fact that in. 2003 she was diagnosed with RRMS  and quickly transitioned to SPMS a few short years later.  At that time, she put her researched-oriented brain to work and discovered that most MS treatments centered around drugs that were years away from approval. Given that, she diverted her attention to the role of vitamins and supplements, which were readily available, in helping brain disorders, as part of a treatment plan. She added to her research repertoire functional medicine, a biology–based approach that focuses on identifying and addressing the root cause of disease and from there started compiling a list of supplemental nutrients that would assist in brain health. Her ah ha moment came when she asked herself, what if she created a diet that would include those important brain nutrients, not from supplements, but from the foods she ate...and the rest is Wahls Protocol history!  In this episode one of two, we'll get a closer look at Dr. Wahls own physical decline and how she ate her way back to health and onto creating the now infamous Wahls Protocol. Multiple sclerosis (MS), is an auto-immune condition affecting more than 1 million Americans. And like other autoimmune conditions, it it's becoming more prevalent. The role of genetics in MS is not significant. Lifestyle choices however may play important roles in regulating the immune system, and therefore should certainly be looked at as it relates to this and other autoimmune conditions.   My guest today, Dr. Terry Wahls, is herself a multiple sclerosis patient. On the podcast she describes how aggressive her multiple sclerosis became, how she herself did not seem to realize any dramatic improvement from pharmaceutical intervention, and how she ultimately discovered the important role of nutrition in terms of regulating her immune system and as such, the disease course of her multiple sclerosis situation.   Dr. Wahls then took these observations and created programs for other  MS patients, applying rigorous scientific principles to study the effects of nutritional interventions.   Her latest study compares her nutritional protocol with a long-standing nutritional approach to multiple sclerosis called the Swank diet that actually began being utilized in the late 1940s. I will say that the results she has obtained, which we discussed today, are extremely impressive.    There is certainly good reason to consider the use of pharmaceutical intervention in multiple sclerosis. Nonetheless, that's one intervention that, in my opinion, should certainly be coupled with other proven therapies, like nutrition. Dr. Wahls makes it very clear that there are multiple mechanisms by which this dietary change may prove helpful in the MS patient;  by targeting things like inflammation, the functionality of the gut bacteria, and even the function of mitochondria.   I am certain you will enjoy today's interview. Show Notes On cellular health: The theory of mitochondrial metabolism at the cellular level gained tractions in her practice. Poor cellular health, insulin resistance are common factors in many chronic disorders, metabolic disorders. It is generally common to many conditions affecting organ systems.   The Wahls Protocol's diet piece is here in a flash: 9 cups vegetables 3 cups greens 3 cups cabbage family 3 cups onion family 3 cups mushroom vegetables 3 cups deep pigmented vegetables/fruits like beets, berries, roots, and tubers Remove sugar, gluten-containing grains, dairies, eggs.   Best proteins are from fishes from the wilds. Liver meat and heart are fabulous superfoods.   Time-restricted eating or fasting - 3 to 7 days of fasting infuses the body with new stem cells to keep you young and healthy.   It takes efforts to change habits and extinguish harmful habits. If it can be linked to why it's important to us, it will drive us to be invested more to do the work.    Steps to Behavioral Change: Inspiration.Talk about the why's to go and change. Get people fired up in learning to do the work. Talk with the patient and his/her family to plan even small things that can be done realistically to achieve tangible successes.   Study your biosensors. Hear what the body is saying about your health. Teach patients that pain is handy to sense what might be happening inside your body. Learn to  pinpoint what changes were made that triggered these biosensors to show symptoms. Learn from it or ignore it, then you can anticipate more cellular damage.

Today's guest is the amazing Dr. Terry Wahls. I became a big fan of Dr. Wahls ever since I first heard her speak on the very first TED talk I ever listened to about 10 years ago on her journey into multiple sclerosis, which went totally viral and now has over 3 million views. Dr. Terry Wahls is an Institute for Functional Medicine Certified Practitioner and a clinical professor of medicine at the University of Iowa where she now conducts groundbreaking clinical trials. In 2018 she was awarded the Institute for Functional Medicine's Linus Pauling Award for her contributions in research, clinical care and patient advocacy.
