Today, we are joined by Dr. McMahon and Jacqueline TrumbI'll as we discuss Borderline Personality Disorder, its development, and treatment. Jacqueline is the co-host of A Little Help For Our Friends, a member of Bachelor Nation, and as a current Ph.D student at Duke University: a soon-to-be clinical psychologist. Mental health has been a prevalent part of Jacqueline's life and as she has a BA in Philosophy from the University of Virginia, she has always been interested in studying humans, their emotions, and purpose. Throughout her career thus far, she has primarily focused in research with her work at the Citadel first, and then for two years at the Mood and Personality Disorders lab at Mount Sinai in NY. Having been trained in several evidence-based psychotherapies, such as the Unified Protocol, Exposure and Response Prevention, and Behavioral Activation, Jacqueline treats adults with a range of mental health struggles, particularly depression, anxiety, misophonia, and OCD. Dr. Kibby McMahon is a clinical psychologist, fitness studio owner, yogi and the co-host of A Little Help For Our Friends. Kibby completed her undergraduate degree at Columbia University, her PhD at the Duke University clinical psychology program, and her predoctoral internship at Weill Cornell/NY Presbyterian Hospital in 2021. She is currently a postdoctoral associate at Duke University Medical Center. Having been trained in several forms of evidence-based psychotherapies, such as Dialectical Behavior Therapy, Acceptance and Commitment therapy, Interpersonal Psychotherapy and Mindfulness Based Cognitive Therapy, Kibby treats adults with a range of mental health struggles, particularly depression, anxiety, personality disorders, trauma, and relationship problems in general. In addition to her work as a psychologist, she runs a fitness studio, Intent FitHouse, in downtown Durham with the mission of creating an empowering, inclusive community that promotes holistic health and fitness. It can create definitive schemas, which are the way we perceive ourselves in the world. It may be that we develop a schema that we don't deserve love or that we are not important in a relationship. As we grow older and get into relationships, we attract people who make us feel like love made us feel when we were smaller. We stay in the pattern until we change it and we play out stories that don't feel resolved. The love you received as a child becomes your narrative for what love looks like in your life going forward. Our identities are flexible and can change over time. We have a choice in the identity we want to take on. Some people are afraid of what change will look like for them. We can change without destabilizing ourselves. Becoming Emotionally Bonded & Regulating Emotions [19:15] Marni asks the co-hosts of A Little Help for Our Friends what contributes to our emotional bonding. At certain times in our lives, we may emotionally bond with people who fill certain needs even though they may not be suitable for long-term commitments. It's important to ask ourselves what the relationship is serving. If it fills an unmet need from childhood it may not last. Happiness and living a meaningful life are living in accordance with our values. But, to know our values we have to dig deep into what we want for ourselves. Emotions make us do things that may not be aligned with our values. Negative emotions aren't bad, they just direct us to do certain things. Inaction is a choice. In this Ask The Egg Whisperer episode, Penny sent in a question. Here's what she asked, “I'm 38 and a half. In the past six months, I've done three egg retrieval to preserve my fertility. In cycle one, I had 15 eggs frozen. Cycle two, four embryos with donors from one normal five AB one, lone mosaic, five DB. Cycle three, seven embryos. Five day five, one day six, one day seven waiting for PGT results. I'm hoping to start a family as a single mom by choice. I want to try to conceive as soon as next month after having an HSG. I ultimately want to have two kids, maybe both with the same donor or possibly with a partner with my frozen eggs, depending on life unfolds. And my fertility doctor recommended trying at least one IUI now that I've eggs and embryos banked, because I could possibly avoid the cost of a transfer and avoid shots. This sounds appealing, but shots sound terrifying. Would you recommend trying IUI with donor sperm first? What are the pros and cons? If yes. In this Ask The Egg Whisperer episode, Gulbahar sent in a question. Here's what she asked, “My doctor recommended that I take a one month off between egg retrievals. There are only so many times to ask 'why' and not be answered before feeling you can't ask again. What is the science between a month wait? I have low ovarian reserves, so overstimulation is not likely. Why do some women have back to back IVF cycle while others don't?” And the answer is you don't have to take a break. You really don't. I always say when your period starts, you think of me and you come on in by cycle day three, you just keep showing up. I say this because you just never know when you're going to have an awesome cycle. So how would you know that the cycle right after one egg retrieval, is going to have a lot of sparkling eggs unless you actually go and look? So I would advocate for yourself. Ask your doctor if you can come in at the time of your period, just to check, understanding that you might have an ovarian cyst that might make it so that you just need to take a break. I don't necessarily recommend a one month break. I do have patients that just feel like they want to keep coming in cycle after cycle without a break. But if let's say you're 41 years old, you have a follicle count of three the next month after your egg retrieval, you could have a follow up count of four. So I would suggest, showing up, find out what's happening in that cycle and then make a decision as to whether you should take a break or wait for your next cycle. Megan wrote in to ask The Egg Whisperer about freezing her eggs. She's 37, and tragically lost her fiancé last year. Here's what she had to say, "Hi, Dr. Aimee. I got my eggs frozen at 36, after my fiancé passed away. I got 13 eggs. My AMH was 5.7. FSH was 5, and it's now a year later. And I'm still worried about my chances. I really want two kids someday. Should I freeze more eggs when I can? FYI, my mother and grandmother both went through menopause at 42. Thank you for your time and advice." Because of her life philosophy to say “Yes!” to as many opportunities as possible, she found herself on Season 22 of ABC’s The Bachelor, yet said “No!” to the prospect of giving up said Ph.D. and moving to Arizona for an admittedly dashing race car driver. She has spent several years in psychology research and currently focuses on mood and personally disorders, with a particular interest in narcissism. Dr. Monica O’Neal is a Clinical Psychologist and Relationship Expert with a private practice in the Back Bay. Popularly known as "Dr. Monica," she specializes in the treatment of relationship challenges and interpersonal conflicts. When Dr. Monica isn’t at her practice, she is a lecturer at Harvard Medical School and consults for various local and national media outlets.
