As BMIs and weights increase across the US population, there have been increased calls for universal screening for existing DM at entrance to prenatal care, if under 20 weeks. Others, including the ACOG, prefer to screen early those with additional risk factors (like prior GDM HX, prior macrosomia, BMI >30, PCOS, first degree relative with diabetes, or age >40). In July 2024, the ACOG released its publication, “Screening for Gestational and Pregestational Diabetes in Pregnancy and Postpartum”. In this guidance, it states, “At this time, there are insufficient data to support the best screening modality for pregestational diabetes in pregnancy, but consideration can be made to use the same diagnostic criteria as for the nonpregnant population (A1c value 6.5 or higher, or fasting plasma glucose value 126 mg/dL or higher, or 2-hour plasma glucose value 200 mg/dL or higher during a 75-g OGTT, or random plasma glucose value 200 mg/dL or higher in patients with classic hyperglycemia symptoms)”. However, a new proposed protocol has been published in AJOG for early screening for DM in pregnancy. This also describes the differences in diagnosis and care for Standard GDM diagnosed at 24-28 weeks, vs a diagnosis of pregestational DM diagnosis made prior to 20-weeks vs “early” GDM also diagnosed under 20 weeks of gestation. Hey friend — buckle up. This episode is peak messy and oddly heartwarming: imagine a Victorian house party with deceptive chocolate cocktails (they sneak up on you), a disco ball spin-off, and one of our hosts getting so tanked she ends up barfing in the shower. Classic. We also dig into the restaurant trenches — you know the ones: forced prix-fixe menus, last-minute menu swaps, entitled Valentine's diners who stiff servers and act like their big romantic show excuses everything. If you ever wondered why hospitality folks roll their eyes at Feb 14, this episode explains it in gruesome detail. On the flip side, there's a genuinely sweet chaos: a sneaky ring in a jacket pocket (disguised among empanadas, naturally), a bout of nausea from cigar smoke, and then—after showers and teeth-brushing—the perfect, quiet proposal at home. It's hilarious and tender all at once: puke, pajamas, and a very sincere “will you?” We rant about how Valentine's can be performative and cruel, celebrate Galentine's and the small rituals that actually matter, and trade childhood Valentine memories (cupcakes, cheesy hearts, and all). It's raw, funny, and totally relatable. Want to swap your best or worst V-Day stories? Slide into our DMs — misery, triumph, and barf tales welcome. Love ya. Cheers.

In this episode of the Federal Help Center Podcast, Zach Golden walks through how contractors can use OpnGovIQ to break down an RFP quickly and avoid falling behind as opportunities begin dropping faster and deadlines shrink. He explains how uploading key documents like the PWS and price schedule can instantly generate a compliance matrix, helping businesses identify exactly what the government is asking for before wasting weeks on the wrong bid. Zach also highlights the importance of spotting early "beige flags," like difficult overnight staffing shifts or specialized medical cleaning requirements inside VA hospital environments. Instead of bidding blindly, this approach allows contractors to make smarter bid/no-bid decisions, understand evaluation factors beyond lowest price, and prepare stronger proposals with confidence and speed. Key Takeaways OpnGovIQ helps contractors evaluate RFP requirements fast by building a submission matrix from uploaded documents. Deadlines are shortening and opportunities are accelerating, so speed in bid/no-bid decisions is now a competitive advantage. Specialized requirements (medical spaces, surgical suites, shift coverage) must be flagged early before committing to a proposal.
