Mobile Medicine in Rural Areas

EMS Today

Play Episode Listen Later Jul 15, 2022 54:26

Technologist Jonathon Feit has often said that the greatest innovations serving Mobile Medicine (Fire & EMS) come from rural spaces. The most obviously reason, perhaps, is that agencies have fewer resources to begin with, and it takes longer for their neighbors to arrive on-scene, therefore they have no choice but to learn how to do more for themselves. This has always been the hallmark of those who live and work where the wild spirit of self-reliance abides. But there is something else, too: big skies, clean air, and the ability to better afford life and spend less time in traffic tend to attract those who need to be able to afford aspects of life that would be impossible where the norm is sitting in traffic going back-and-forth from long hours in an office: for example, the time to care for a child or parent who is seriously ill or disabled and need more regular attention. For this reason, while they have fewer available resources that most cities, countrysides tend to attract a higher concentration of people who themselves — or whose parents or children — are more likely to need emergency healthcare. That sets up a Catch-22 and an important risk of health inequity that rural Mobile Medical agencies navigate around the clock. Reuben Farnsworth, who spearheaded Community Paramedicine, ET3, and other specialty care programs at Colorado's Delta County Ambulance District — an agency that has won statewide honors, including as the first ambulance service to interoperate with the Quality Health Network health information exchange and local hospitals — will explain how a data — and economics-centric management style has powered his service's success, from clinical medicine, to education and policy development, to advancing legislative priorities with partners.

Leveraging ePCR Data to Identify the Risk of Post-Traumatic Stress Disorder This special episode of "Sacred Cows & Data Cubes" was recorded at the HCA (Hospital Corporation of America) EMS Kansas City Symposium in November 2021. It features an emotional anecdote of post-traumatic stress and recovery by Atchison County (KS) EMS captain and retired Fire Chief Rob Brader. There is safety in numbers: both the math and statistics — and the herd, where one stay anonymous by choice without worry that exorcizing the demons of Mobile Medicine will result in persecution for the dark thoughts that inevitably spawn of this emotional work.   “Let the numbers do the talking” is a core tenet of the approach first revealed to Jonathon Feit, chief executive of Beyond Lucid Technologies by Ron Nichols, director of Chambers County Emergency Medical Services in Texas. Ron uses a questionnaire to see sparks of mental health stress among his personnel in a manner similar to how others might identify infections (say, COVID-19) before they spiral out of control. His crew members have to write an ePCR after every call; and any member of the crew can catch a bad call. He recognized that this offers two opportunities for insight: The ePCR not only offers details about what happened on the call, it also offers details about what the crew experiencedduring the call; If every member of every crew completes a wellness survey after every call—or even after every call involving specific sentinel events—then the requirement to complete a mental wellness or trauma impact survey becomes, by definition, randomly distributed.  There is no longer a need for anyone to “step forward” because everyone has the same requirement. Anyone can catch an unfortunate call. The result is compelling: The data in ePCRs become vital to crews' mental wellness and health.  No longer are ePCRs mere busywork; no longer should they be despised.  Instead, they become something akin to a recurrent Rorschach test for each member of the crew, revealing how they are doing call-by-call, and in aggregate — inviting management to pay attention.  At the end of this discussion, we'll ask: Can agencies really afford not to step in to help, especially amid a talent crunch? If the analytics can be automated and tuned to examine ePCRs over time, then they can reveal who needs help without a crew member saying anything.  This empowers watchful management to step ahead of a health crisis in-the-making, bolstering agency morale and resiliency, satisfaction and safety.  ePCR data—maximized to their potential and strung together over time—therefore go beyond the collection of legal, billing, and NEMSIS data.  They become a tool to safeguard Mobile Medicine's most precious resource: its professionals on the line.

No topic is off-limits in this multi-party podcast featuring three renowned leaders of Mobile Medicine -- Scott Moore (Moore EMS Consulting, LLC / Workforce Dynamics, Inc), Art Groux (Bennington Rescue Squad, Vermont) and Sean Kukauskas (Spaulding Rehabilitation Hospital EMS, Boston). The discussion goes far beneath bumper-sticker messages, seeking root causes to develop practical solutions in highly impactful areas of Mobile Medical education, economics, provider mental health and wellness, and management of people and processes. We even discuss issues related to the churn of personnel in and out of the industry — and the associated shortage of qualified new people.   All against the backdrop of the power of data to inform decision-making that elucidates the “invisible contracts” that operate all around us: assumptions made but infrequently clarified, which can lead to strained relationships in an industry that is built on trust. TRUST is perhaps more critical now than ever as Mobile Medicine seeks to navigate changes from all directions, forcing an evolution of the key skills required of both field crews and managers alike.

I never thought I would find myself taking a tour of a mobile health clinic, which is more of a visual tour, yet only recording the audio. But that is how this just so happened to transpire. After arriving to see this large vehicle, larger than the biggest Ryder truck, Dr. Paula Watt just wanted to show me around. So I hit record. In part one of this Mobile Medicine series, Episode 017, I sat down inside the mobile health clinic and talked about the background and vision for the future. Sitting inside felt like more a small doctor’s office with lots of examination rooms side-by-side. By the way...if you have not listed to part one, I hope you go back and listen. What does it mean to have the world’s first 100 percent solar-powered, mobile health clinic? Well, that is just as good as being in the Guinness Book of World Records, I think? But what does that mean for South Carolina, the state where this mobile health clinic is based...inside the land-grant institution Clemson University whose primary purpose is educating undergraduate and graduate students to think deeply about and engage in the social, scientific, economic, and professional challenges of our times. Well, improving the health of the underserved communities while providing a teaching experience for public health students...I think it fits rather nicely. Dr. Paula Watt, Clemson University Sullivan Center director has been dreaming of the day when they could have a four wheel drive, solar powered clinic;  so they could serve rural communities primarily in Greenville, Oconee, Pickens and Anderson counties. All of this while demonstrating to Clemson students the challenges in caring for the most vulnerable patients. I had a chance to hear Paula's passion and vision. I was able to talk with her about serving the underserved, the uninsured, the people who desperately need access to quality care. "The funniest weight loss and fitness radio show on the internet." Please click on the POD button to listen to the latest Dr. Fitness and the Fat Guy show.   On tonight's show, Doctor Fitness and the Fat Guy get the skinny about all kinds of tips. Our listeners have been emailing us about having a tip show and we listened to them. There are more weight loss tips on the show then we have ever had. First we had our very own Dr. Fitness give some awesome exercise and fitness tips. Then we brought on Carolyn O'Neil, the best selling author of the Best Health and Nutriton Cookbook (according to the Gourmand people and the World Food Media) titled "The Dish on Eating Healthy and Being Fabulous." Shape magazine called this book "one of the top 5 diet books you've gotta have." She couldn't stop talking about all kinds of tips on eating, exercising while talking on the phone, eating out at restaurants, even what adult beverages you should never order at the pool. Carolyn is the AOL Diet Coach and you can check that out here http://coaches.aol.com/diet/carolyn-oneil/main She is also a weekly columnist for the Atlanta Journal Constitution and is a recurring nutrition expert on Alton Brown's show "Good Eats" on the Food Network which you can check out here  http://www.foodnetwork.com/food/show_ea/text/0,1976,FOOD_9956_50120,00.html Or you can go to her website www.CarolynONeil.com and find out all the things she is up to. Then we had the pleasure of hearing from Dr. Frenesa Hall, a Harvard educated M.D. who left the traditional doctor office behind and goes directly to her patients at their home or their office. That's right she is bringing back the house call. Her company Mobile-Medicine.Net is an innovator in improving the healthcare experience for her patients. Imagine a doctor who will spend as much time as you need going over your illness with you, instead of the 6 minutes they typically give you. Dr. Hall is an amazing doctor and she is truly doing great things. She had some great tips on how to avoid getting the flu or a cold this winter. You can learn more about her and her company at her website www.mobile-medicine.net. On her website you can get information about her personal services as well as information on the AmeriScience line of supplements that she recommends.   Dr Fitness also shared his tips on how to jump start your weight loss program now instead of waiting until New Years. Number 1, make a goal, write it down. Number 2, tell 10 people your goal in order to create some accountability. And Number 3, get a partner to watch your back. You will slip up, everyone does, you just need someone to get you back on track as soon as possible. You will have to listen to the show to find out all the rest of Dr Fitness' great exercise tips. The Fat Guy forgot to give his Sarah Reinersten on the Amazing Race 10 on CBS update. She is the triathlete with the fake leg. She and Peter finished 3th in the fourth round and we will be tracking her progress each week on our show. Sarah is having some major rrealtionship problems with Peter. Check out the Amazing Race website here http://www.cbs.com/primetime/amazing_race10/ and Sarah's website here www.iamsarah.org Another highlight of this show was the Fat Guy sharing his recent dinner at Seasons 52. A kind of healthy restaurant that just opened in Atlanta. Check them out at www.seasons52.com. The Fat Guy and his wife both loved it.
