À Port-au-Prince, la violence des gangs a des conséquences extrêmement lourdes sur l'accès aux soins. Près des 3/4 des structures médicales ne fonctionnent plus et ce sont essentiellement les organisations humanitaires qui font tourner les quelques centres de santé et hôpitaux toujours ouverts. Nos envoyés spéciaux Justine Fontaine et Achim Lippold ont pu se rendre à l'Hôpital de Drouillard géré par Médecins sans frontières, situé à Cité Soleil, quartier sous la coupe d'un groupe criminel. À l'entrée, une lourde porte métallique et l'obligation de déposer ses armes. La violence n'a pas sa place dans l'hôpital. « Les différentes unités de soin sont protégées contre les tirs. Si l'établissement n'est pas directement visé par les gangs, il peut toujours être touché par des balles perdues » des gangs ou des policiers, raconte notre journaliste Achim Lippold. Ici, bourreaux et victimes sont soignés sans distinction. « La vie coincée entre les gangs et la police qui commet, elle aussi des exactions a profondément traumatisé les habitants », explique encore Achim Lippold. En plus du stress quotidien, le chômage, le manque de moyens et la malnutrition pèsent sur l'état de santé des Haïtiens. « Les gens se sentent vulnérables quand ils ne peuvent pas aider ou prendre soin de leur famille », explique le docteur Darena Diomeri. Anne Cantener prolonge la discussion avec Mumuza Muhindo, chef de mission sortant de Médecins sans frontières pour Haïti, en ligne depuis Port-au-Prince. « Travailler en Haïti, c'est accepter de prendre beaucoup de risques », explique d'emblée l'humanitaire. « Notre priorité, c'est la sécurité des malades et de notre personnel ». À plusieurs reprises, MSF a dû fermer certaines de ses structures. « Nous adaptons nos activités en fonction du contexte sécuritaire », précise Mumuza Muhindo qui prend la température chaque matin, auprès de ses contacts. « Il faut dialoguer avec tous les acteurs de la zone : chefs de gangs mais aussi chefs traditionnels, prêtres, pasteurs... C'est un travail quotidien. » Les besoins sont énormes car seules 25% environ des structures médicales fonctionnent à Port-au-Prince « et encore, pas de façon optimale », précise le responsable. Elles manquent notamment de personnels car les gens ont peur de venir travailler. « Pour eux, c'est s'exposer aux risques de kidnappings, de viols ou de balles perdues ». Voilà ce qui a conduit José Jeri vers la sortie. « Quand un président perd la confiance des gens aussi vite, la chute politique est inévitable », écrit El Comercio. « José Jeri le savait mais il a continué à justifier l'injustifiable, changeant de version au fur et à mesure qu'apparaissaient des preuves compromettantes », déplore le journal. Le président a fini par être lâché, y compris par la droite péruvienne. Cette nouvelle crise politique au Pérou pourrait éloigner encore un plus les électeurs des urnes. « Les responsables politiques péruviens, qu'ils soient de droite ou de gauche, trouvent toujours le moyen de te décevoir », commente avec amertume l'écrivain et célèbre animateur de télévision Jaime Baily interrogé par La Republica. « Ils s'arrangent toujours pour que tu finisses par avoir honte d'avoir voté pour eux ». Ce serait vous mettre vous-même en danger car « face à des faits violents, les forces de l'ordre interviendront », menace le ministère de la Sécurité dans un communiqué publié sur les réseaux sociaux. Le gouvernement dit s'inquiéter pour la sécurité des journalistes alors que la semaine dernière, lors du vote du texte par le Sénat, une manifestation avait dégénéré. Mais l'Association des correspondants étrangers en Argentine dénonce une entrave à la liberté de la presse, rapporte Pagina 12. L'Acera estime que le gouvernement cherche à intimider les journalistes et méconnaît sa propre responsabilité : l'État doit protéger les journalistes quand ils travaillent sur la voie publique. C'est un devoir reconnu internationalement, précise encore l'association. Le passage de Rafah, entre la bande de Gaza et l'Égypte, a rouvert le 2 février, dans les deux sens, sous contrôle israélien et égyptien. Dans ce texte, cosigné par plus d'un millier de dirigeants d'entreprise de l'Union européenne, ils demandent un “made in Europe” afin d'être plus compétitifs face aux États-Unis et la Chine et réduire leur dépendance. Mario Draghi, ancien président de la Banque centrale européenne, avançait une série de 170 propositions pour accroître la production européenne via une politique d'investissements massifs dans les secteurs clés dans un rapport remis en septembre 2024. Rientrano donne, bambini, anziani. I malati gravi, quelli con i nomi già trasmessi all'OMS per evacuazioni urgenti, passano da un altro varco, Kerem Shalom. La “riapertura” esiste, ma resta controllata, fragile, facilmente reversibile. Più utile alla narrazione che alla sopravvivenza quotidiana. Nello stesso giorno, un'altra porta si chiude del tutto. Medici Senza Frontiere viene costretta a lasciare Gaza. Israele parla di requisiti amministrativi e di elenchi del personale locale non consegnati. MSF risponde definendo la misura un pretesto e denunciando l'assenza di garanzie di sicurezza per il proprio staff. L'effetto pratico è immediato: meno medici, meno cure, meno presenza internazionale nei luoghi dove si muore di più. La contraddizione è evidente. Si annuncia un corridoio umanitario e si stringe il rubinetto dell'assistenza. Si invoca la sicurezza e si colpiscono le organizzazioni che tengono in piedi ospedali di fortuna e cliniche improvvisate. I valichi diventano strumenti politici: liste, autorizzazioni, attese che decidono chi passa e chi resta. Fuori dal linguaggio ovattato delle diplomazie, le parole sono più nette. Francesca Albanese ricorda che Israele non ha alcuna autorità legale per bloccare l'ingresso nel territorio che occupa e richiama il rispetto delle decisioni della Corte internazionale di giustizia. Qualcuno osserva che è forse la prima volta che MSF viene espulsa con l'accusa di essere una “infrastruttura militare”, lo stesso schema usato per colpire l'intero sistema sanitario di Gaza. Intanto gli inviati americani incontrano Netanyahu e parlano di tregua. Sul terreno resta una costante: una porta socchiusa, molte sprangate. MSF has helped to keep the battered healthcare system at a minimal functioning level. On Sunday, Tel Aviv announced that MSF would no longer be allowed to operate in Gaza. Israeli agencies claimed the humanitarian aid organization failed to provide Tel Aviv with sufficient documentation on its staff in the Strip. MSF said it attempted to negotiate with Israel to share information about its staff, with safeguards to protect them, but those talks were unsuccessful. “Following many months of unsuccessful engagement with Israeli authorities, and in the absence of securing assurances to ensure the safety of our staff or the independent management of our operations,” the group's statement explained. “MSF has concluded that it will not share a list of its Palestinian and international staff with Israeli authorities in the current circumstances.” MSF supports about a fifth of all hospital beds in Gaza and a third of births. When Israel announced the ban last year, the UN's human rights chief, Volker Türk, condemned the ban as “outrageous” and explained it was part of an Israeli policy to prevent aid from entering Gaza. During Israel's onslaught in Gaza, nearly all of the Strip's hospitals were damaged or destroyed. Tel Aviv has also barred medical supplies from entering Gaza and injured Palestinians from leaving. The shortage of medical supplies has led to preventable deaths.

Mientras se espera la segunda fase del plan de paz en Gaza, impulsado por el mandatario estadounidense Donald Trump, la situación humanitaria en el enclave sigue siendo catastrófica, alertan las ONG´s. Hay escasez de comida, enfermedades y muertes ligadas a ataques y al invierno, además de la preocupación por la posible partida de 37 organizaciones humanitarias. En octubre pasado inició la tregua en Gaza, pero una verdadera paz aún se ve lejana y las necesidades de la población de alimentos, vivienda digna o atención médica no dan espera, confirman diferentes organizaciones como Médicos Sin Fronteras (MSF) y Oxfam. Esta semana Unicef informó que, desde el inicio de la tregua, al menos 100 niños han muerto en ataques aéreos y actos de violencia en la Franja y el New York Times publicó que también en este periodo Israel ha demolido más de 2.500 edificios en el enclave, complicando aún más el acceso a una vivienda digna para la población, pues muchos se encuentran bajo tiendas de campaña. Las muertes en Gaza continúan y a ello se suman los fallecimientos por hipotermia o enfermedades ligadas a un invierno bajo condiciones catastróficas, cuenta Claire Nicolet, Coordinadora de urgencias de Médicos Sin Fronteras en Gaza. “Todos los días, sigue habiendo muertos en Gaza, ya sea porque la gente se acerca a la zona controlada por el ejército israelí o por los ataques que siguen produciéndose en zonas que normalmente deberían ser seguras. Lamentablemente, siguen produciéndose ataques a ese nivel”, agregó Nicolet. El invierno y las malas condiciones de vivienda también han generado fallecimientos. “Hemos tenido casos de hipotermia entre los niños y también hemos tenido esos famosos edificios que se derrumban sobre la gente. Así que digamos que, aún hoy, seguimos viendo muertes relacionadas con toda esta situación que vemos en Gaza. Por desgracia, todos los días seguimos oyendo bombardeos, demoliciones de edificios y aún no hemos llegado a algo que se pueda llamar realmente paz”, señaló. El trabajo de las ONG's no es suficiente para cubrir las necesidades del enclave, cuenta la trabajadora humanitaria y esto podría agravarse con la decisión de Israel de prohibir el acceso a 37 ONG. El Estado hebreo argumenta que es una respuesta por no comunicar los nombres de sus empleados, pero las organizaciones temen por la seguridad de sus miembros. Por su parte, la comunidad internacional se muestra preocupada por una escalada de la crisis humanitaria que ya es catastrófica, pues explican que sería muy difícil crear rápidamente una alternativa que pueda cubrir el trabajo que ya se está haciendo y que es precario. “Hoy en día, como Médicos Sin Fronteras, contamos con más de 20 centros de salud a los que prestamos apoyo y 5 hospitales. En la actualidad, no hay nadie capaz de sustituirnos con un volumen de trabajo tan grande. Contamos con más de 1.000 empleados nacionales, por lo que el volumen de actividad es enorme. También somos el segundo distribuidor de agua en la Franja de Gaza. Si hoy tuviéramos que dejar de trabajar, no habría ninguna solución alternativa”, aseguró. Además de la presencia física, están las especialidades que aporta cada organización. “En el caso de MSF, somos especialistas en determinadas patologías o enfermedades, como por ejemplo las quemaduras. No habría ninguna solución alternativa, porque no habría ninguna organización que pueda venir de repente a sustituir el volumen de actividad que estamos realizando como Médicos Sin Fronteras”, agregó Nicolet. Varios países entre los que se encuentran los de la Unión Europea, que enviaron una declaración conjunta, insisten a Israel en dar marcha atrás en su decisión de sacar a estas 37 organizaciones, pero de momento la presión no parece dar resultados.

Depuis avril 2023, se déroule au Soudan l'un des conflits les plus meurtriers, à l'origine de « la plus grande crise humanitaire au monde » selon l'ONU. Selon des témoins ayant fui El-Fasher, la capitale du du Darfour-Nord est le lieu de massacres, violences à caractère ethnique, viols et agressions sexuelles depuis qu'elle est tombée entre les mains des paramilitaires FSR du général Mohamed Hamdan Dogolo « Hemedti », le 26 octobre dernier. Plusieurs organisations humanitaires font état de crime de guerre, de crimes contre l'humanité, les Nations unies parlent d'une « spirale d'atrocité ». Après un siège de 18 mois, après la faim, la soif, la violence et la peur, certains Soudanais ont réussi à quitter El-Fasher pour se réfugier au Tchad voisin. Le Tchad et le Soudan sont séparés par un wadi, un oued sec, depuis la fin de la saison des pluies. De part et d'autre de cette frontière se trouvent deux villes : Tina, au Soudan, et Tiné, au Tchad. C'est là que les réfugiés soudanais traversent à pied, en charrette ou à l'arrière de camionnettes. De notre envoyée spéciale à Tiné, La frontière est gardée par des militaires tchadiens, mais aucune entrave n'a lieu au passage des réfugiés qui traversent tous librement. « Voilà les nouvelles familles qui viennent d'arriver. Elles sont orientées pour faire le circuit : d'abord l'enregistrement des réfugiés avec la Commission nationale d'accueil de réinsertion des réfugiés et des rapatriées du Tchad (Cenar), après avoir obtenu les informations de la personne. Puis un screening médical se fait avec Médecins sans frontière (MSF) avant leur transfert vers le site de transit », explique Georges, membre de l'équipe du Haut-commissariat des Nations unies pour les réfugiés (HCR). Le visage fatigué, en partie dissimulé derrière son foulard bariolé, Souad Ibrahim Abdou s'installe à quelques mètres de la frontière, sous la cabane de MSF. Elle attend désormais d'être enregistrée. « On a laissé les enfants à Tawila. On remercie le seigneur d'être arrivé ici en bonne santé. Dieu merci, tout va bien. On nous a conseillé de venir ici, on nous a dit que la ville était bien. Le seul problème ici, c'est la nourriture. Aussi, il n'y a pas de travail », raconte-t-elle. Une fois le premier enregistrement effectué, les réfugiés doivent se rendre par leurs propres moyens au site de transit à la sortie de la ville. Parmi les infrastructures sur place, des pompes à eau, mais peu de latrines. Et les températures chutent fortement le matin et le soir. La jeune Daralnayim,19 ans et enceinte de sept mois, a fait le voyage seule depuis El-Fasher. Elle raconte sa vie dans le site de transit : « Il n'a pas de nourriture, pas de couvertures, pas de lit. Je n'ai pas eu d'autre choix que de rester avec des femmes ici. On a fait les démarches d'enregistrement ensemble. » Les profils de ces derniers arrivants sont majoritairement des femmes et des enfants. Mais il y a beaucoup plus d'hommes comparé au début du conflit en 2023. « Il y a de plus en plus de vieillards blessés, de gens inaptes et beaucoup d'enfants non accompagnés. Ils avaient encore l'espoir en 2023 qu'ils peuvent défendre leur terre, résister. Maintenant, c'est le désespoir total », détaille Jean-Paul Habamungu, responsable du HCR à Iriba. In 1971, 13 men sat down in a Paris office to launch what would become one of the world's best known humanitarian organisations: Médecins Sans Frontières, or Doctors without Borders.The men were among hundreds of volunteers responding to an appeal by the French medical journal, Tonus, after a major cyclone devastated East Pakistan.The campaign sparked the idea for the charity. The 13 founders – two journalists and 11 doctors – drew up a charter aiming to provide medical care regardless of race, religion or politics. MSF's first missions included helping victims of a Nicaraguan earthquake and a hurricane in Honduras. And in 1999, the charity won the Nobel Peace Prize in recognition of their work.
