According to Fox News, the Biden Administration's FBI decided not to investigate—and he was immediately released by police. Dr. Sharon Kolasinski is a professor of clinical medicine at the University of Pennsylvania and chief of the Division of Rheumatology at Penn Presbyterian Medical Center. Part 1 Her objectives in the presentation are to help listeners by learning about evidence-based treatment for patients with osteoarthritis (OA), to understand the process by which guidelines are developed that might help us figure out our evidence-based approach, to review the recommended treatments for OA, and to review some treatments that are not recommended for OA. Her basic evidence-based reference is a University of Pennsylvania guideline that was published in 2020. Numerous other guidelines are available, which she described. She discussed her work with a case involving a 55-year-old male patient. He arrived for routine follow-up care for immunosuppressive medication monitoring. He described pains that he was experiencing and was diagnosed with OA. She indicated the impacts OA has on patients. Clinicians find it a daunting challenge to provide satisfactory treatment. For example, for some clinicians, the guidelines do not appear to be clear and provide a roadmap. Also, they do not always believe in the recommendations contained in the guidelines. In this presentation, she wants to see if she can change some minds about guidelines. She provided an example based on investigations conducted at her institution. The outcome was a series of recommendations that she described. Part 2 Dr. Kolasinski began Part 2 of her presentation by continuing to focus on the importance of having patients with OA engage in physical activity. She stated that “they are worn out and the implication is that a doctor is needed to fix them.” Perhaps a perspective should be taken of a more participatory discourse where we encourage physical therapy and emphasize that physical exercise is safe when you have arthritis and focus on what the patient can do, empowering them to exercise. A starting point is to give patients a physical therapy prescription. She discussed the extent to which physical therapy is useful, along with indicating how much and how frequently exercise is beneficial (e.g., for 20 minutes, three times a week). Losing weight is an effective way of reducing symptoms. Food choices can affect OA symptoms. Diet and exercise used together can produce effective results. References were made to several studies that involve the status of steroid injections on improving patient health status. Acupuncture also was mentioned. She indicated conditional recommendations on the use of pharmacological interventions. She concluded by describing how to treat the 55-year-old patient she mentioned in Part 1 of her presentation.

And primarily we've seen this not only from reductions in smoking and earlier cancer detection, but advancements in some of our treatments, most notably in the realm of immunotherapy and targeted therapy.” Timestamp (TS) 02:07 “With the discovery of the biomarkers, it has brought around the discovery of genomic-driven therapies that are specific to these biomarkers. That's really changed the landscape of oncology for people that have one of these driver variants.” TS 07:55 “I've definitely seen in my practice where therapy has been completed and, especially for some of these immunotherapy drugs, a couple of months later the patient develops a toxicity that is from the prior immunotherapy. Even chemotherapy can have some long-term toxicities, but we do have to even keep it in mind for immunotherapy that once these drugs are finished, there still could be some long-term side effects. Since they are newer drugs, we still are learning about what some of these long-term toxicities look like.” TS 26:56 “There haven't been a ton of new FDA approvals specific for chemotherapy; however, we have seen chemotherapy still used in practice, particularly in combination with some of these novel therapies. Particularly, we see a lot of chemotherapy and immunotherapy combinations.” TS 27:47

However, Dr. Sloane says that porn is addicting. Men have to mimic the porn in their real sexual life, but they can't. So, they have a hard time getting turned on. Plus, porn leads to desensitization. Even with Instagram, it's an unattainable goal and a real issue. [22:50] All About GAINSWave And The Benefits of Using It GAINSWave is the premium brand of extracorporeal shockwave therapy for ED, performed by our network of highly-trained medical providers. Todd Barton, MD, is a Professor of Clinical Medicine and the Program Director of the Internal Medicine Residency Program at the University of Pennsylvania. He is an attending physician at the Hospital of University of Pennsylvania and Penn Presbyterian Medical Center where his clinical practice focuses on transplant-related infectious diseases and clinical infectious diseases. Dr. Barton completed his medical school from the University of Rochester School of Medicine and his residency from the University of Pennsylvania where he also completed a fellowship in infectious diseases. He's passionate about medical education, and is an active member of a number of local and national organizations influencing resident education. Give yourself permission not to be the best. Today, Dr. Todd Barton shares how being open and honest with our own struggles will ultimately lead us to the right path. He explains that we can take the pressure off trying to be outstanding in everything in our lives. But at the same time, it will serve us well to do the best we can—and try to get as much as possible out of each experience. In his own career, Dr. Todd Barton is forthcoming about what he struggled with, what he decided was not right for him, and figuring out what his true passions were. He assures us that the environments we are in, the people we meet, and the experiences we have will be our greatest guides toward our own passions in medicine. Pearls of Wisdom: 1. Embrace kindness: something you observed in your parents who were a symbol for kindness for you early in your life in the early parts of the AIDS epidemic. 2. We have to give ourselves permission not to be the best. 3. Key trait of a mentee is flexibility - the flexibility to adapt to different challenges, opportunities, rotations, experiences that we go through in order to find the one we will really focus on. 4. We have to be intentional, only only about our work in the hospital, but our life outside of the hospital.

Dr. Benoit Dube is an Associate Professor of Clinical Psychiatry at the Perelman School of Medicine, Associate Vice Provost, and the inaugural Chief Wellness Officer at the University of Pennsylvania. Indeed, he’s the first in the Ivy League to hold such a position, which is, in itself, a response to the urgent mental health issues facing college campuses. Dr. Dube is a core member of the University Life team, directing wellness initiatives across the University and overseeing a new division of Student Wellness Services that includes the Offices of Alcohol and Other Drug Program Initiatives, Campus Health, Counseling and Psychological Services (CAPS), Penn Violence Prevention, and the Student Health Service. He began at Penn in 1997 as a Resident and is now an Attending Physician at the Hospital of the U. of Pennsylvania, Penn Presbyterian Medical Center, and Pennsylvania Hospital. He received a Provost’s Award for Teaching Excellence in 2011, earned an M.D. from the University of Montreal in 1997, and a BSc in Psychology from Concordia University in 1992. Stew and Benoit discuss the importance of creating a culture of wellness, decreasing the stigma associated with mental illness, and making it easier for people to ask for help. They discuss some of the changes he is making in the short-term, since he began as Chief Wellness Officer just a few months ago, as well as his vision for long-term change. For starters, students now have no wait time to speak with a counselor, who are available 24/7 -- that’s a significant change. Benoit would like to see mental health and physical health all housed in and addressed in the same physical space. He’d like to create an environment where students learn the life skills to manage stressful situations, to modulate their emotions, to see stress as an opportunity for growth, and to know that it’s OK to ask for help. He envisions a new academic department in which research, science, theory, and practice related to wellness -- whether in nursing, psychology, medicine, philosophy, or elsewhere throughout the university -- are brought into one hub of innovation. Ultimately, he’d like Penn students to bring evidence-based practices for producing wellness into the world when they graduate. See acast.com/privacy for privacy and opt-out information.
