Medicare Advantage. It's everywhere you turn. From upcoding to denials, the huge and growing program—which now covers more lives than traditional Medicare, but is increasingly the target of local and federal scrutiny and a stream of endless OIG audit reports—seems to be in the news daily. Earning your RHIA (Registered Health Information Administrator) is a huge accomplishment, and for some marks the end of a four-year academic journey and an entry into HIM. “Why are you not giving yourself a solution?” Rhia Gibbs, Black Teachers Connect Educational consultant, Former teacher sociology, criminology https://blackteachersconnect.co.uk/ "Become a teacher and conquer the world." In this week's insightful episode, we sat down with Rhia Gibbs, an accomplished educational consultant with a background in sociology and criminology. Rhia's educational journey was guided by a powerful piece of advice from one of her Black teachers: "Become a teacher and conquer the world." Despite experiencing a rebellious phase, she was reminded that she could do better. Throughout her schooling, Rhia encountered just two Black teachers which underscored her mission to elevate educators. "Creating a community for Black teachers." Rhia's organization, Black Teachers Connect, embodies her unwavering goal to impact and empower teachers while fostering a vibrant community. Rhia emphasizes the need to address traumatic incidents in schools, like the unjust strip-search of a teenage girl. Reflecting on George Floyd's global impact, Rhia underscores the importance of acknowledging issues within the UK's own borders. Despite some progress, she recognizes the ongoing struggle for students within the system to envision themselves reaching the pinnacle of success. With only 2% of UK teachers being Black/African/Black Caribbean, Rhia's Black Teachers Connect offers a much-needed network for Black educators—a place to excel, share wisdom, connect, and have fun. "Hello! My name is Abigail Eliza. I wear all the hats for Back Again, Back Again – if there's something happening in the show, that's me doing it! Back Again, Back Again is a narrative portal fantasy about ex-prophecy children, the end of adventures, and queer girls with swords. It follows the story of Ilyaas, who, on the night of her seventeenth birthday, gets portal-magicked away to a world called Rhysea – where she is told that she is part of a prophecy to save the country. The podcast picks up at the traditional end of all ""portal fantasy"" stories: prophecy completed, Ilyaas must readjust to the ""real world"" while still remembering everything she's seen. This is season two, episode four – Nonbeliever. In this episode, the king's palace has just been raided by the rebels (called the fretim in the show), rescuing Ilyaas and Rhia. However, Ilyaas has spent the entirety of the first season on the side of the kings due to a lack of information and an inability to understand the common language, Rhysean, something that also comes up a lot in this episode. Ilyaas, who mostly speaks English, is having to work all of her thoughts into a language she is, objectively, pretty shit at. Callia, who speaks not only Rhysean but a few of its dialects, has very little patience for this, especially as she mostly still views Ilyaas as a traitor. Ilyaas thinks Callia is a part of the big prophecy in Rhysea. Callia doesn't really think of Ilyaas at all, if she can help it. The Rhysean that you hear happening in the show – especially in this episode – is a conlang that I created for the sake of the show. As someone who works in foreign language learning and teaching, the politics of language are everywhere, and it's really cool to be able to explore them in a fantasy world. Justin Kelly is a bedside nurse who is serious about creating change through advocacy and innovation in nursing. Through his fellowship at Ohio State University, Justin has not only managed to put himself at the forefront of nurse-led change through organizing events for nurse innovators, but has also been given the chance to talk to legislators about the problems nurses are facing as well. If you want to make change and don't know where to start, Justin can put you on the right path.
HIM professionals and nurses can get along, after all. There has long been a divide in the clinical documentation integrity (CDI) profession over who can/should serve in the CDI role. Though it has lessened with the passing of years, that divide still exists today. Recent ACDIS survey data shows that 72% of CDI professionals are nurses. Nothing wrong with that; nurses make wonderful CDI professionals. But I know of many fine CDI professionals with HIM/coding backgrounds who have also succeeded in the role. As well as MDs, even the occasional respiratory tech or CPHQ. With this episode of Off the Record I wanted to prove that an RN and an RHIA can not only exist side-by-side, but get along, quite amicably. As friends, even. Turns out I did not have to look far or dig too deeply. At a reception at the ACDIS national conference earlier this year I ran into Andrea Smith and Rebecca McDade. I made my way over to their table, and a half hour of conversation (and a bit of wine) later, invited them on to a podcast that did not yet exist, except in my mind. What struck me was that they were friends, despite their very different backgrounds. Five months later I had them on Off the Record. And can now bring you this show, and revive a part of the conversation we had that evening at ACDIS. Andrea is the CDI Director at Washington Regional Medical Center in Fayetteville, Arkansas, a nurse who moved into CDI eight years ago. Rebecca meanwhile is Director of Revenue Cycle Integrity for the same organization, and an RHIA. Despite their differences they work very well together, complimenting each other's unique strengths and skills in adjacent roles (note: Rebecca is not directly in CDI but works closely with that department). We cover the lay of the land at Washington Regional Medical Center, including the staffing of its CDI and coding programs, metrics for success, and assistive software. Then a day in a life of each, their roles and responsibilities, and respective origin stories. Things get interesting when we talk about the historical RN/HIM divide, how they've bridged that gap, and the unique strengths each profession brings to the table (this starts at about the 37-minute mark of the show). Hosting this conversion reinforced that we can get along, at least some and perhaps most of the time, if we're willing to acknowledge differences and be generous. Andrea and Rebecca are proof.
